규칙적인 운동습관이 여대생의 체지방량 및 혈중 지질초성에 미치는 영향 (The Effect of Regular Exercise on Body Fat Contents and Blood Lipid Compositon of College Women)

  • 1. 수축기 혈압은 운동을 하고 있는 군에서 약간 낮았으나 이완기 혈압은 유의성 있는 차이를 보이지 않았다. 운동을 하고 있는 여대생의 경우 체지방량이 낮게 나타난 반면 제지방량은 변화가 없어 운동 여부에 의한 차이가 거의 보이지 않았다. 또 혈중 포도당 함량은 규칙적인 운동을 하고 있는 여대생에서 다소 높은 수준이었다. 2. 체지방율은 지속적인 운동을 하고 있는 여대생에 서 유의성은 없었지만 약간 낮았으며 혈청 총 콜레스테롤과 중성지방의 농도 역시 유의성 있게 낮게 나타났다. HDL 콜레스테롤 농도는 두 군 모두 거의 비슷한 수준으로 군간에 뚜렷한 차이를 보이지 않았던 반면 LDL 콜레스테롤의 경우 운동에 의해서 유의성 있게 낮게 나타났다. 따라서 규칙적인 운동에 의해서 HDL-Chol/Total-Chol 비율은 증가되었으며 LDL-Chol/HDL-Chol의 비율은 낮았다. 3. 혈액중 지단백질의 상대적인 분포는 HDL(%)의 경우 지속적인 운동을 하고 있는 여대생의 경우 일반 여대생에 비해 거의 차이가 없었고 LDL(%)는 유의성 있게 낮았던 반면 VLDL( %)는 오히려 높게 나타났다. Plasma의 지방산 조성중 포화 지방산 함량은 운동을 하고 있는 여대생에서 약간 높았지만 다가 불포화 지방산 함량은 같은 수준으로 두 군간에 차이를 보이지 않았다.

심혈관질환 예측을 위한 저밀도 대 고밀도 지단백 콜레스테롤 비(LDL : HDL-cholesterol ratio)의 적정기준에 관한 연구 (Assessment of the Optimum LDL : HDL-cholesterol(LDL : HDL-C) Ratio for Predicting CHD)

  • Purpose: This study was aimed to determine the optimum low-density lipoprotein : high-density lipoprotein-cholesterol (LDL : HDL-C) ratio for predicting coronary heart disease(CHD) in Korean people. Methods: It was analyzed this data of 5,431 adults who had undergone health examinations in a hospital in Gyeonggi-do between January 2006 and December 2007. The covariation of the coronary risk factors such as age, HbA1C, systolic blood pressure(SBP), and waist-to-stature ratio(WSR) were analyzed by using logistic regression analysis. Results: The LDL : HDL-C ratio in the male and female groups was mostly distributed between 1.5 and 4.0. The LDL : HDL-C ratio was the most significant cholesterol-related parameter influencing CHD (male: B = .306, p = .054, female : B = .940, p = .010), followed by LDL-C and total cholesterol. It was observed a sharp increase in the odds ratios for LDL : HDL-C ratios of 2.25 - 2.50(male) and 2.00 - 2.25(female). A significant difference was observed in both male(2.25 : $x^2$ = 2.494, p = .072) and female(2.00 : $x^2$ = 413.742, p = .000) groups. Conclusion: The risk level of CHD was set to 2.25 for males and 2.00 for females. Therefore, the optimum LDL : HDL-C ratio for Koreans should be far lower than that for the people in western countries.

한국인의 일부 도시인에서 비만, 이상혈당, 이상지질혈증의 집락과 고혈압의 관련성 (Association of Hypertension with Cluster of Obesity, Abnormal glucose and Dyslipidemia in Korean Urban Population)

  • To examine the association of hypertension with cluster of obesity, abnormal glucose and dyslipidemia in Korean urban population, we conducted this cross-sectional study among 3027 men and 2127 women age 20-85 years who visited a prevention center between May 1991 and June 1995 for a multiphasic health check at St. Mary's Hospital, Seoul. By the self-administered questionnaire, the informations of educational attainments, monthly income, alcohol consumption, cigarette smoking, and physical excercise level were obtained. Height, weight, and blood pressure were measured by a trained nurse. The fasting blood sugar (FBS), total cholesterol, high density lipoprotein (HDL) cholesterol and triglyceride were tested by enzyme method. Low density lipoprotein (LDL) cholesterol was calculated by 'total cholesterol - HDL cholesterol - triglyceride/5'. For testing the differences of cardiovascular risk factors between hypertension and normotension group, 1-test and $\chi^2$-test were performed. For the age adjusted odds ratios of hypertension in persons with obesity, abnormal glucose, and dyslipidemia compared with normal, logistic regression was performed by using SAS pakageprograme. The results obtained were as follows: 1. Age, weight, body mass index(BMI), blood glucose, total cholesterol, LDL cholesterol, and triglyceride of hypertension group in men and women were significantly higher than normotension group, but height and HDL cholesterol of hypertension group only in women significantly lower than normotension group. The frequency of obesity $(BMI\geq25kg/m^2)$, abnormal glucose $(\geq\;120mg/dl)$, hypercholesterolemia $(\geq\;240mg/dl)$, lower HDL cholesterol (<45 mg/dl in women only), higher LDL cholesterol $(\geq\;160mg/dl)$, and hyper hypertriglyceridemia $(\geq\;250mg/dl)$ in hypertension group of men and women were significantly higher than normotension group. 2. Systolic and diastolic blood pressure were negatively correlated with hight, but positively with age, weight, BMI, total cholesterol, LDL cholesterol, and triglyceride in men and women. BMI was positively correlated with fasting blood sugar, total cholesterol, LDL cholesterol and triglyceride but negatively with HDL cholesterol. 3. The age adjusted odds ratios of hypertension were as follows in men and women : among persons who were obese compared with those nonobese, 2.53 (95% Confidence Intervals [C.I.] 2.08-3.07) and 2.22 (95%C.I. 1.71-2.87): among persons who were abnormal glucose compared with those normoglycemic, 1.43 (95%C.I 1.13-1.82) and 2.01 (95%C.I 1.36-2.94): and among persons who were dyslipidemia (hypercholesterolemia or lower HDL cholesterol or higher LDL cholesterol or hypertriglyceridemia) compared with those normal lipid, 1.59 (95%C.I 1.30-1.95) and 1.51 (95%C.I 1.16-1.96). After combined more than one risk factor, the odds ratios were increased. Among persons with cluster of obesity, abnormal glucose, and dyslipidemia, the odds ratio of hypertension was 2.25 (95%C.I 1.47-3.37) in men and 3.02 (95%C.I 1.71-5.30) in women. In conclusion, it was suggested that hypertension was associated with cluster of obesity, abnormal glucose, dyslipidemia in this Korean urban population.

Effects of Soy Isoflavones on Lipid Profiles and Hepatic LDL Receptor mRNA Level in Growing Female Rats

  • The present study examined the effect of soy isoflavones on lipid metabolism in growing female rats. Rats were randomly assigned to three different groups and provided experimental diets for 9 weeks. The experimental groups were classified into 1) a control group, 2) a soy protein isolate group: soy (+)) group and 3) a soy protein concentration group: soy (-)) group. Diets contained either casein or one of two soy proteins with (soy (+)) or without isoflavones (soy (-)). Serum triglyceride concentration showed no significant differences among the experimental groups. Serum total cholesterol concentration was significantly lower in both the soy (+) and soy (-) groups than in the control group and LDL-cholesterol concentration was significantly lower in the soy (+). Serum HDL-cholesterol concentration was significantly higher in the control group than in the soy protein groups but the HDL-cholesterol share rate in total cholesterol tended to be lower in the control group than in the soy protein groups, insignificant as it was. Hepatic IDL receptor mRNA level was significantly increased in the soy (+) group when compared to the other two groups to be 20% higher than the control group. In conclusion, soy protein isolate, soy protein rich with isoflavones reduced serum total cholesterol and LDL-cholesterol concentration and increased hepatic IDL receptor mRNA expression in growing female rats. Therefore, it is considered that the intake of soy isoflvones during puberty can be advantageous in terms of the long-tenn control of serum lipid.

Lovastatin과 Simvastatin의 고지혈증 치료 비교 (A Comparison of Lovastatin and Simvastatin in Treatment of Hyperlipidemia)

  • Hypercholesterolemia is one of main causes of coronary heart disease(CHD). Clinical trials demonstrated that lowering serum cholesterol levels would reduce incidence of new cardiovascular events and mortality by primary or secondary preventions. The objective of this retrospective study was to compare efficacy and side effects of lovartatin and simvastatin in treatement of hypercholesterolemia. In Boramae Hospital, patients were included when they have taken lovastatin 20 mg or simvastatin 10 mg for 52 weeks with laboratory monitoring for cholesterol at baseline, 3, 6 and 12 month period. As results, total 128 outpatients were included with their total cholesterol level <240 mg/dl and triglyceride level <400 mg/dl at baseline. Total cholesterol and LDL cholesterol of lovastatin group (n=60) and simvastatin group (n=68) were significantly reduced from baseline (p=0.001). Lovastatin maximally reduced total cholesterol by $23.9\%,\;triglyceride\;by\;12.3\%$, LDL cholesterol by $36.1\;\%$ and increased HDL cholerterol by $7.8\%$ and simvastatin reduced by $24.1\%,\;20.5\%,\;34.3\%\;respectively$ and HDL increased by $11.2\%$. There were no significant differences between lovastatin and simvastatin in mean percent change of lipid levels at 12, 24 and 52 weeks from baseline. Cumulative percentage of patients reaching the target LDL cholesterol concentration by 24 weeks was $61.7\%$ in lovastatin and $64.7\%$ in simvastatin. Average time to reach the target LDL goal was 100.1 days in lovastatin and 99.8 days in simvastatin. Both lovastatin and simvastatin also significantly reduced total cholesterol and LDL cholesterol in all subgroups (diabetes mellitus, hypertension, and coronary heart disease). In this study, treatment efficacy in patients with coronary heart disease was lower than other patients. Considering clinical importance of secondary prevention, more intensive treatment is necessary to decrease LDL cholesterol level of 100 mg/dl or lower in patients with coronary heart disease or other clinical atherosclerotic disease. There were no serious side effects during the study period. Digestive side effects were most frequently reported (lovastatin $8.3\%\;vs\;simvastatin\;8.8\%$). In conclusion, both lovastatin and simvastatin were similar in lipid lowering effects and there was no difference in incidence of side effects.

여자 노인의 체지방량 및 체지방 분포와 혈장 콜레스테롤 대사 (Total Body Fat Content and its Distribution and Plasma Cholesterol Metabolism in Elderly Women)

  • This study was performed to investigate the risk factors for coronary heart disease in elderly women. Seventy five elderly women over 65 years of age participated with 35 elderly men over 65 years of age, 40 middle-aged men and 31 middle-aged women as control subjects. The percentage of body fat(34.1 5.6%) in elderly women found to be significantly higher than in other groups and their waist/hip circumference ratio(WHR) was higher than in middle-aged women. The concentrations of plasma total cholesterol and LDL-cholesterol in elderly women were higher than in other groups, TG concentration higher than in middle aged women and HDL-cholesterol % lower than in other groups. Plasma cholesterol ester transfer protein(CETP) activities of elderly women were significantly higher than in middle-aged subjects, but were not different from those of elderly men. Plasma Apolipoprotein(Apo) A-I level in elderly women was higher than in middle-aged men but not different from the other groups. Differences among groups were not great in Lipoprotein (Lp)(a)levels. CETP activities were significantly correlated with age, body fat % total cholesterol, LDL-cholesterol and Lp(a). Therefore, it appears that the increased risk of coronary heart disease in elderly women is due to the increase of body fat, central adipose distribution, serum total cholesterol, LDL-cholesterol and CETP activities.

Higher levels of serum triglyceride and dietary carbohydrate intake are associated with smaller LDL particle size in healthy Korean women

  • The aim of this study was to investigate the influencing factors that characterize low density lipoprotein (LDL) phenotype and the levels of LDL particle size in healthy Korean women. In 57 healthy Korean women (mean age, $57.4{\pm}13.1$ yrs), anthropometric and biochemical parameters such as lipid profiles and LDL particle size were measured. Dietary intake was estimated by a developed semi-quantitative food frequency questionnaire. The study subjects were divided into two groups: LDL phenotype A (mean size: $269.7{\AA}$, n = 44) and LDL phenotype B (mean size: $248.2{\AA}$, n = 13). Basic characteristics were not significantly different between the two groups. The phenotype B group had a higher body mass index, higher serum levels of triglyceride, total-cholesterol, LDL-cholesterol, apolipoprotein (apo)B, and apoCIII but lower levels of high density lipoprotein (HDL)-cholesterol and LDL particle size than those of the phenotype A group. LDL particle size was negatively correlated with serum levels of triglyceride (r = -0.732, $P$ < 0.001), total-cholesterol, apoB, and apoCIII, as well as carbohydrate intake (%En) and positively correlated with serum levels of HDL-cholesterol and ApoA1 and fat intake (%En). A stepwise multiple linear regression analysis revealed that carbohydrate intake (%En) and serum triglyceride levels were the primary factors influencing LDL particle size ($P$ < 0.001, $R^2$ = 0.577). This result confirmed that LDL particle size was closely correlated with circulating triglycerides and demonstrated that particle size is significantly associated with dietary carbohydrate in Korean women.

마금탕(麻芩湯)이 흰쥐의 고지혈증 병태에 미치는 영향 (Effects of Mageum-tang on the Hyperlipidemia in Rats)

  • This study was designed to investigate the hyperlipidemia-healing effect of liquid extract from Mageum-tang(麻芩湯), it was performed on the hyperlipidemia of rats induced by high cholesterol diet and Triton WR-1339. We prepared two types of hyperlipidemia model in rats induced by high cholesterol diet and Triton WR-1339. The amount of total cholesterol, HDL-cholesterol, LDL -cholesterol, triglyceride, phospholipid, free fatty acid, the activity of superoxide dismutase(SOD), and glutathione were measured. The liquid extract from Mageum-tang showed significant decreasing effects on total cholesterol, LDL- cholesterol, triglyceride, phospholipid, free fatty acid in the both model. And it showed significant increasing effects on HDL-cholesterol, the activity of SOD in both model and amount of glutathione in Triton WR-1339 model. These results suggest that liquid extract from Mageum-tang has healing efficacy on hyperlipidemia induced by high cholesterol diet and Triton WR-1339.

Comparison of LDL-Cholesterol direct measurement with the estimate using various formula

  • Low-Density Lipoprotein cholesterol (LDLC) is the most important marker for the treatment of hyperlipidemia in NCEP-ATP III(National Cholesterol Education Program-Adult Treatment Panel III) guideline. Therefore, LDL cholesterol is pathologically meaningful, accurate measurement should be a top priority. Currently, LDLC is directly measured in most cases, but, the estimate is still used in mass health examination or screening test. This study is about the comparison of LDL-Cholesterol direct measurement with the estimate using various formula (Friedewald: [LDL-F=TC-HDL-TG/5], Nakajima: [LDL-N=TC-HDL-TG/4], Hattori: [LDL-H =0.94TC-0.94HDL-0.19TG], Puavilai: [LDL-P=TC-HDL-TG/6], Carvalho: [LDL-C=3(TC-HDL)/4]) for calculating more accurate value. We analyzed total cholesterol (TC), try-glyceride (TG), high-density lipoprotein cholesterol (HDLC), and LDLC levels of 210 subjects between June and November in 2011. Until now, the Friedewald formula is the most commonly used estimate for the LDLC. When Friedewald formula was applied, the correlation coefficient (r) was 0.940, showing high correlation. But, the result of the direct method was significantly different, compared with those of the Friedewald formula in triglyceride levels ${\geq}400mg/dL$(p<0.05). There was the highest correlation when we used LDL-P formula(r=0.947) in triglyceride levels <400 mg/dl. Also there was the lowest mean difference regardless of triglyceride level. Therefore, the study showed that TG/6 is more precise means of calculation than TG/5. On the other hand, the calculation of LDL-Cholesterol was underestimated, compared with direct measurement. It is necessary to have more data and modified Friedewald formula should be used for the accurate calculation.

석류가 뇌혈류 및 비만에 미치는 실험적 효과 (The Experimental Effects of Pomegranate on the Cerebral Blood Flow and Obesity in Rats)

  • To examine the effects of pomegranate on cerebral blood flow and obesity, we observed regional cerebral blood flow in normal rats, as well as body weight, serum total cholesterol, serum HDL-cholesterol, serum LDL-cholesterol, serum triglycerides, serum total lipids, serum phospholipids and serum free fatty acid levels in rats fed a high fat diet. The results were as follows. Pomegranate seed extract significantly increased regional cerebral blood flow and the peel extract increased regional the cerebral blood flow in a dose dependent manner. In addition, the pomegranate extracts(seed, peel, juice) decreased body weight, serum total cholesterol, serum LDL-cholesterol, serum triglycerides, serum total lipids, serum phospholipids and serum free fatty acid levels, and increased serum HDL-cholesterol in the rats fed a high fat diet. More specially the seed extract significantly decreased serum total cholesterol, serum triglycerides and serum phospholipid levels.
