Objective: This study aimed to determine the safety and clinical effect of artificial shrinkage (AS) in terms of assisted hatching of fresh blastocysts. Also, we evaluated the correlation between patient age and the effect of AS on clinical outcome. Methods: Two AS methods, using a 29-gauge needle and laser pulse, were compared. Seventy-three blastocysts were shrunk using a 29-gauge needle and the same number of other blastocysts were shrunk by a laser pulse. We evaluated the shrunken blastocysts hourly and considered them viable if they re-expanded >70%. Blastocyst transfer cycles (n=134) were divided into two groups: a control group consisted of the cycles whose intact embryos were transferred (n=100), while the AS group consisted of the cycles whose embryos were replaced following AS (n=34). The implantation and pregnancy rates of the control group and AS group were compared ($p$ <0.05). Results: The re-expansion rates of the 29-gauge needle and laser pulse AS groups were similar (56 [76.7%] vs. 62 [84.9%], respectively). All of the remaining shrunken blastocysts were re-expanded within 2 hours. There was no degeneration of shrunken blastocysts. The total and clinical pregnancy rate of the AS group (23 [67.6%]; 20 [58.8%], respectively) was significantly higher than that of the control group (47 [47.0%]; 39 [39.0%], respectively). In the older patient group, there was no difference in the clinical outcomes between the AS and control groups. Conclusion: These results suggest that AS of blastocoele cavity, followed by the transfer, would be a useful approach to improve the clinical outcome in cycles in which fresh blastocyst stage embryos are transferred.
Purpose: To evaluate the additive value of prenatal chromosomal microarray analysis (CMA) in assessing increased nuchal translucency (NT) (≥3.5 mm) with normal karyotype and the possibility of detecting clinically significant genomic imbalance, based on specific indications. Materials and Methods: Invasive samples from 494 pregnancies with NT ≥3.5 mm, obtained from the Research Center of Fertility & Genetics of Hamchoon Women's Clinic between January 2019 and February 2020, were included in this study and CMA was performed in addition to a standard karyotype. Results: In total, 494 cases were subjected to both karyotype and CMA analyses. Among these, 199 cases of aneuploidy were excluded. CMA was performed on the remaining 295 cases (59.7%), which showed normal (231/295, 78.3%) or non-significant copy number variation (CNV), such as benign CNV or variants of uncertain clinical significance likely benign (53/295, 18.0%). Clinically significant CNVs were detected in 11 cases (11/295, 3.7%). Conclusion: Prenatal CMA resulted in a 3% to 4% higher CNV diagnosis rate in fetuses exhibiting increased NT (≥3.5 mm) without other ultrasound detected anomalies and normal karyotype. Therefore, we suggest using high resolution, non- targeting CMA to provide valuable additional information for prenatal diagnosis. Further, we recommend that a genetics specialist should be consulted to interpret the information appropriately and provide counseling and follow-up services after prenatal CMA.
The sharp decline of fertility in industrialized countries since the 19th century constitutes a major problem for evolutionary approaches to human behavior. Why would people voluntarily reduce their total number of offspring, despite the fact that resources are so abundant in modern times? Here I review three evolutionary hypotheses for low fertility in modern societies, and discuss how the evolutionary perspective could shed new light on solving the problem of low fertility in Korea. Low fertility may be 1) a maladaptive outcome from the mismatch between our ancestral environments and evolutionarily novel environments, 2) a consequence of gene-culture coevolution where traits that reduce genetic fitness can still spread through a population as a result of imitation, especially if the traits are expressed by high-status people, or 3) an adaptation that maximize parents' long-term genetic fitness in knowledge-based industrialized societies where high parental investment is required for rearing competitive offspring. Based on these considerations, I suggest how the evolutionary explanations of low fertility can be applied to increasing the birth rate in Korea.
Among OECD countries, Korea is the only country which has continuously recorded total birthrate below 1.3 person for over 10 years. Since 2006, the Korean government has promoted the population policy in full scale. But, the fertility rate which became low has not shown any sign to go up again. Thus, Korea can be understood as having fallen into 'low fertility trap'. Such a low fertility can cause serious problems such as weakening of national competitive power and even survival of the country. In Korea, due to studies and finding jobs among young women, their childbirths are kept being postponed. In some cases, poor working conditions where women work can cause physical conditions not appropriate for pregnancy. Thus, it is very important to let childbearing women, pregnant women take care of their health. Accordingly, conscious of this low fertility era, this study examined major international organizations and countries' health promoting strategies-with focus on motherhood and suggested some methods to effectively improve health for motherhood.

The impact of heat stress (HS) on reproductive performance and its problems in cows remains to be investigated in Bangladesh. The study was, therefore, aimed to evaluate the effect of HS on fertility and reproductive health problems of dairy cows in a selected area of Bangladesh. A total of 1,095 cows from 500 farms were included in this study. The climate-related data were recorded daily basis for every month in a year and temperature-humidity index (THI) values were calculated to determine the level of HS. Concurrently, data on fertility parameters [number of services per conception (NSC), conception rate (CR) and calving to the first service interval (CFSI)], and reproductive problems were collected through a pre-defined questionnaire. The results show that crossbred [Native x Holstein-Friesian, (HF)] cows were more vulnerable (p < 0.05) to a magnitude of HS effects considering physiological parameters of cows [age, body condition score (BCS), parity and milk yield]. Regarding fertility, HS had no effect on NSC, CR and CFSI in both native and crossbred cows (p > 0.05). The findings on the reproductive problems indicate that HS had significant influence on the prevalence of anestrus (χ2 = 21.814, p < 0.05) and retained placenta (χ2 = 24.632, p < 0.05) in cows. Of note, the prevalence of repeat breeding syndrome was 2.5 folds higher in stress condition than in no stress condition. Abortion and dystocia were not influenced by HS. In conclusion, HS does not influence the fertility parameters of cows studied; however, anestrus and retained placenta are likely to occur under HS conditions in cows.
Objectives : The purpose of this study is to analyze the factors affecting the enactment of local municipalities' ordinance for supporting Korean Medicine infertility treatment in the metropolitan area. Methods : For Seoul, Incheon, and Gyeonggi Province, data on the enactment of Korean Medicine infertility treatment and socio-demographic data of the local municipalities were collected through the Enhanced Local laws and regulations Information System and on-line statistical database. Logistic regression analysis was performed to analyze the factors affecting the enactment of the ordinance, The enactment of the ordinance was dependent variable, and the budgeting of local municipalities, the number of population, the number of births, the number of Korean Medicine doctors, and the total fertility rate were used as independent variables. Results : Up to 2000, the ordinances for supporting Korean Medicine infertility treatment were enacted in 16 local municipalities and the budget for that was supported by 18 local municipalities. Regarding the effect on the enactment of the ordinances, it was found that budgeting of Korean Medicine infertility treatment had a positive (+) effect, and the metropolitan government's budgeting and the total fertility rate had a negative (-) effect. Conclusions : This study suggests that the ordinance for Korean Medicine infertility treatment is a policy tool derived based on the problem of low fertility faced by each local municipality rather than the political influence of health provider's groups.
This paper examines the interplay among changes in socioeconomic conditions, population policies, and fertility decline during $1960{\sim}2000$ in South Korea. Period-parity-progression-ratios (PPPR) are calculated using 2% samples of Population Census with the implied distribution of completed family size. The total fertility rate based on PPPR declined from 5.21 in 1960 to 2.76 in 1980, and 1.69 in 2000. The family planning programs were successful in meeting the needs of families that wanted to prevent further childbearing after having achieved the desired family size during $1960{\sim}1970$. The 1980s mark two important landmarks: achievement of replacement fertility and emergence of high sex ratio at birth. In the 1990s the "quality and welfare" approach was emphasized. In 2000s, South Korea's fertility is one of the lowest in the world. In response to this, in 2005, the Government adopted a new population policy with the goal of increasing the total fertility level to 1.6 by 2010. The results of this study indicate that proportion of women who would have no children at all throughout their reproductive life span increased substantially from 10% in 1995 to 16% in 2000, with a sharp increase since 1997. Thus, pro-natal programs need to address the problems associated with marriage and having the first child. Towards that, it is important that work and family life become more compatible.

Objective: Sperm must be properly prepared in in vitro fertilization (IVF)-embryo transfer (ET) programs in order to control the fertilization rate and ensure that embryos are of high quality and have appropriate developmental abilities. The objective of this study was to determine the most optimal sperm preparation method for IVF. Methods: Patients less than 40 years of age who participated in a fresh IVF-ET cycle from November 2012 to March 2013 were included in this study. Poor responders with less than three mature oocytes were excluded. Ham's F-10 medium or sperm-washing medium (SWM) was used in combination with the density-gradient centrifugation/swim-up (DGC-SUP) or SUP methods for sperm preparation. A total of 429 fresh IVF-ET cycles were grouped according to the media and methods used for sperm preparation and retrospectively analyzed (DGC-SUP/Ham's F-10, n=82; DGC-SUP/SWM, n=43; SUP/Ham's F-10, n=181; SUP/SWM, n=123). Results: There were no significant differences among these four groups with respect to the mean age of the female partners, duration of infertility, number of previous IVF cycles, and retrieved oocytes. We determined that both the DGC-SUP and SUP methods for sperm preparation from whole semen, using either Ham's F-10 or SWM media, result in comparable clinical outcomes, including fertilization and pregnancy rates. Conclusion: We suggest that both media and both methods for sperm preparation can be used for selecting high-quality sperm for assistive reproductive technology programs.
This experiment was conducted to predict the effects of motional characteristics on the fertility of Korean native cattle(KNC) by using CASA technology and in vitro fertilization system. Twenty-six KNC frozen semen straws were obtained from Korean KNC improvement department, livestock improvement main division, national livestock cooperatives federation in Korea. Specimens were allowed to thaw at $37^{\circ}C$ for 30 sec in water bath. Semen analysis was performed on semen image analysis system(SIAS, Medical supply, Korea) adjusted to the gate settings and used the semen droplet ($5{\mu}l$) placed on Makler counting chamber(Sefi medical instrument, Israel) prewarmed at $37^{\circ}C$. The same person used the same micropipette to fill the Makler counting chamber. A total of 150 or more of sperms were analysed in each specimen by a single trained person by scanning at least 5 to 10 fields. The oocytes collection, in vitro maturation, IVF, in vitro culture and determination of the cleavage rate were performed by the technique, as described by Hwang et al (1997). Statistical analysis was done by linear regression with use of the Sigma plot program on a IBM personal computer. The cleavage rate in vitro fertilized oocyte was significantly correlated(P<0.05) with MOT, VCL, VSL, VAP, ALH, BCF and MAD, but not CON, LIN, STR, WOB, DNM, DNC and HYP in regressional analysis. The results show that some kinematic characteristics of frozen-thawed semen by CASA can be predict the fertility in in vitro model system.
Previous research has not evaluated the effects of various rates and frequencies of manure application and frequencies of cutting on yield and stand persistence of cool-season grasses and alfalfa-grass mixtures. The primary objective of this study was to compare the effects of cutting management systems on herbage yield and stand persistence of orchardgrass (Dactylis glomerata L.) and an alfalfa (Medicago sativa L.)-orchardgrass mixture from various rates and frequencies of dairy slurry application. A randomized complete block design with treatments in a sub-subplot arrangement with four replicates was used. The main plot consisted of 2 cutting management systems (4 and 5 annual cuttings). The subplots were 9 fertility treatments: 7 slurry rate and frequency of application treatments, one inorganic fertilizer treatment, and an unfertilized control. The split-split-plots were the two forage species: orchardgrass and alfalfa-orchardgrass mixture. The study was initiated after 1st cutting in 1995. Cumulative yields of the 2nd and subsequent cuttings of both orchardgrass and alfalfa-orchardgrass in 1995 were higher for the 5-cutting system than the 4-cutting system. The 1995 growing season was abnormally dry. In 1996, an abnormally wet year, the reverse was true, total herbage yields being higher for the 4-cutting system than the 5-cutting system. Species response to fertility rate/frequency treatments was different in both years. Higher application rates early in the season and carryover of nutrients from late season applications the previous year appear to be responsible for the yield increases of those fertility treatments having significant yield differences between the cutting management systems. The stand ratings of orchardgrass were not affected by cutting management. In the spring of 1997, however, the stand ratings of alfalfa-orchardgrass in the 4-cutting management system were significantly greater than the 5-cutting management system. The very high manure application rate significantly reduced the stand ratings of alfalfa-orchardgrass in the 5-cutting system.
