The purpose of this study was to investigate the quality of life of clients who treated and equipped prosthesis in Eulji University Hospital in Dae-Jun City. The results of this study were as follows. The oral status of Female had more carious tooth than male had. Carious tooth(p<0.05) and losed tooth(p<0.001) was more popular in older female. The frequency of treated tooth was higher under 59 years old female than over 60 years old(p<0.01). In education category, carious tooth(p<0.05) and losed tooth(p<0.000) was more popular in low education than high education In the score of relation of OHIP-14 category and general characteristic, female was significantly higher than male in dysfunction( p<0.05), physical disorder(p<0.001) and activity disorder(p<0.05). The score was significantly higher in older than younger in dysfunction(p<0.05), physical disorder(p<0.001) and activity disorder(p<0.001). The score of education was significantly higher in low education group than high education group in physical pain (p<0.001), mental discomfort (p<0.001). The score of marriage status was significantly higher in marriaged group than other group in dysfunction (p<0.05) and mental discomfort (p<0.05). In the score of relation of OHIP-14 category and oral status, the score of carious tooth group was significantly higher in dysfunction(p<0.05), physical disorder(p<0.05) and activity disorder(p<0.05). The score of losed tooth group was significantly higher in dysfunction(p<0.05), physical pain (p<0.05), mental discomfort(p<0.05), physical disorder(p<0.05) and activity disorder(p<0.01). The score of non treated tooth group was significantly higher in dysfunction(p<0.05), mental discomfort(p<0.05), social disorder(p<0.05) and activity disorder(p<0.05). In the score of relation of OHIP-14 category and the sort of prosthesis, the score of being bridge group was significantly higher than being crown group in activity disorder(p<0.05). The OHIP-14 category and the number of prosthesis was not significant relationship. These results suggest that the information and services of the process of dental prosthesis based on subjective evaluation should be provided to clients rather than based on clinical evaluation. The continued system of oral management should be developed and provided.
Purpose: Depression is a condition that weakens psychosocial functioning and reduces quality of life. In Korea, the prevalence of depression among the elderly is 29.2% and depression is a considerable public health concern for the elderly. Depression has a statistically significant relationship with oral conditions such as number of teeth and salivary flow rate. Oral health-related quality of life (OHRQoL) is the concept including subjective evaluations of psychological, physical and social aspects of oral health. Tooth loss and hyposalivation can affect mental health and quality of life. Our study was designed to identify the relationship among the number of teeth, salivation, OHRQoL, and depressive symptoms in the elderly in Korea. Methods: We recruited 100 participants, aged over 65 years, and assessed their number of teeth, unstimulated salivary flow rate. All participants filled out oral health impact profile 14 (OHIP-14) and Zung self-rating depression score (SDS) for checking OHRQoL and depressive symptoms. Statistical analysis was done by R program. Results: We found that the positive relationship between OHIP-14 and SDS after Mann-Whitney test (p=0.03). The lower SDS group had an OHIP-14 median score of 4. On the other hand, the higher SDS group had a median value of 7.5. The other factors (number of teeth and salivary flow rate) did not show correlations with OHIP-14 or SDS. Conclusions: OHRQoL and depressive symptoms have significant correlation.
Navigation of the main root canal and dealing with a dens invaginatus (DI) is a challenging task in clinical practice. Recently, the guided endodontics technique has become an alternative method for accessing root canals, surgical cavities, and calcified root canals without causing iatrogenic damage to tissue. In this case report, the use of the guided endodontics technique for two maxillary lateral incisors with multiple DIs is described. A 16-year-old female patient was referred with the chief complaint of pain and discoloured upper front teeth. Based on clinical and radiographic findings, a diagnosis of pulp necrosis and chronic periapical abscess associated with double DI (Oehler's type II) was established for the upper left lateral maxillary incisor (tooth #22). Root canal treatment and the sealing of double DI with mineral trioxide aggregate was planned for tooth #22. For tooth #12 (Oehler's type II), preventive sealing of the DI was planned. Minimally invasive access to the double DI and the main root canal of tooth #22, and to the DI of tooth #12, was achieved using the guided endodontics technique. This technique can be a valuable tool because it reduces chair-time and, more importantly, the risk of iatrogenic damage to the tooth structure.
The application of the laser to the tooth hard tissue started from the removal of carious dentin with the laser performed by Goldman in 1964. With the development of the laser technology, the laser treatment with less descomfort such as pain, vibration, and noise, etc. has been attempted. Since it is difficult to give a suitable form for inlay restoration to a cavity prepared with laser, it has to be restored with adhesive resinous materials. However, various evaluation of adhesive properties of the resinous materials to lased tooth surface on the various conditions such as adgerent, irradiation condition, procedure of bond test, and adhesive materials used, etc. have been reported.(omitted)

Background: Hyaluronic acid (HA) is well known to exert an anti-inflammatory effect during oral wound healing and is commonly applied after tooth extraction. However, no double-blind randomized controlled study comparing two hyaluronate mouthwash products has been conducted so far. The aim of this study was to comparatively analyze the efficacy of Mucobarrier® and Aloclair® in terms of clinical symptoms. Results: A total of 112 patients were randomly assigned to assess the degree of discomfort, pain reduction, redness, burning sensation, and swelling between two groups on the day of surgery and 7 days later in a double blind test, with a total 56 Aloclair patients and 56 Mucobarrier patients. There was no statistically significant difference in the overall discomfort, degree of pain reduction, redness, burning sensation, and swelling between the Mucobarrier and Aloclair groups. Conclusion: The local application of hyaluronic acid mouth wash after wisdom tooth extraction is beneficial in reducing overall discomfort and pain reduction, and the clinical utility of Mucobarrier® is no different from Aloclair®. Purpose: The purpose of the present study was to investigate the correlation between age and 34 radiomorphometric parameters on panoramic radiographs, and to provide generalized linear models (GLMs) as a non-invasive, inexpensive, and accurate method to the forensic judgement of living individual's age. Methods: The study included 417 digital panoramic radiographs of Korean individuals (178 males and 239 females, mean age: 32.57±17.81 years). Considering the skeletal differences between the sexes, GLMs were obtained separately according to sex, as well as across the total sample. For statistical analysis and to predict the accuracy of the new GLMs, root mean squared error (RMSE) and adjusted R-squared (R2) were calculated. Results: The adjusted R2-values of the developed GLMs in the total sample, and male and female groups were 0.623, 0.637, and 0.660, respectively (p<0.001), while the allowable RMSE values were 8.80, 8.42, and 8.53 years, respectively. In the GLM of the total sample, the most influential predictor of greater age was decreased pulp area in the #36 first molar (beta=-26.52; p<0.01), followed by the presence of periodontitis (beta=10.24; p<0.01). In males, the most influential factor was the presence of periodontitis (beta=9.20; p<0.05), followed by the number of full veneer crowns (beta=2.19; p<0.001). In females, the most influential predictor was the presence of periodontitis (beta=18.10; p<0.001), followed by the tooth area of the #16 first molar (beta=-11.57; p<0.001). Conclusions: We established acceptable GLM for each sex and found out the predictors necessary to age estimation which can be easily found in panoramic radiographs. Our study provides reference that parameters such as the area of tooth and pulp, the number of teeth treated, and the presence of periodontitis should be considered in estimating age.

To properly prepare teeth, dentists require a direct view of the working area and are often placed in a difficult position, moreover, a high proportion of dentists suffer from headache and back pain. Dentists who make use of the dental mirror and position their patients carefully to gain a proper view report less headache, pain and discomfort in the shoulders. It is recommended that dentists learn the "Home Position(H.P.)" which among the various "Random Position(R.P.)" methods, enables dentists to approach their patients in a stable posture. The purpose of this study was to compare tooth preparation in the H.P. and the R.P., and evaluate the clinical efficacy of the H.P.. Tooth preparation for a full cast crown was performed on the maxillary left 1st molar using the H.P. and the R.P., and the shapes of the prepared tooth surfaces at the two different operating positions were compared. The amount of occlusal reduction, marginal width, height, and axial taper were measured and analyzed. A T-test was performed separately to compare the results of the H.P. and the R.P. with respect to the accuracy of the corresponding tooth reduction. The results were as follows. ; 1. The amount of occlusal reduction was excessive on the mesiobuccal cusp(P<0.05), and deficient on the lingual cusp in the H.P.(P<0.01). In the R.P., this was excessive on the buccal cusp and deficient on the fossa and distolingual cusp(P<0.01). 2. The amounts of marginal width were excessive in all areas except on the lingual and mesial surfaces in the H.P. and lingual surface in the R.P.(P<0.01). 3. The marginal heights were achieved more accurately in the R.P. than the H.P.(P<0.01). 4. Axial surface taper was excessive in all areas in the H.P.(P<0.01). But the axial taper of measured areas was even, and tooth is more like to retain its original axis after reduction. In the R.P., axial surface taper was excessive on the mesial and buccal area, and deficient on the distolingual area(P<0.01), and therefore, the axis of the prepared teeth was tipped in the distolingual direction. 5. The times needed for preparation were 12 minutes and 49 seconds in the H.P., and 11 minutes and 35 seconds in the R.P., and the R.P. was statistically faster(P<0.01). The tooth preparation in the H.P. achieved its goal, in that it enabled the operator to make even tooth reduction. In conclusion, the H.P. system offers an improved method that can be used in clinic after specific training.
We analyzed 121 age estimation clients who had visited oral medicine clinic, Chonbuk National University Hospital, from January 2000 to December 2007 to evaluate its characters in Jeonllabuk-do. The obtained results were as follows : 1. There was distinct difference in sex distribution(Male : 58.7%, Female : 41.3%) and more than half percent(55.4%) was over 50's. 2. The difference between registered and alleged age was the most in more than 5 years, and most of them were over 50's. 3. There were more clients who wanted to increase their age than to decrease and was no prominent difference in sex. Age estimation clients who wanted to decrease their age were predominant under 10 years old and in 20's, and those wanted to increase were predominant in 10's and over 30's. 4. The most reasons to correct age were related to welfare benefit. The purpose of age estimation was different according to each age groups; welfare benefit was the most over 50's, occupation in 40's and 50's, friendship in 30's and 40's, sibling-related in 50's, employment in 10's and 20's. 5. Age was estimated by the attrition of permanent tooth and pulp/tooth ratio from 20 years, root apex closure for 10's and calcification of permanent tooth under 10 years old. In cases that were difficult to estimate by use of the attrition of permanent tooth and pulp/tooth ratio, age was estimated by missing time of permanent tooth and the change of mandibular angle with age. 6. The estimated ages were close to alleged age in 77.7% of clients but the rest(22.3%) was close to registered age.
Background: Intraosseous anesthesia (IO) allows the anesthetic solution to be injected directly into the cancellous bone. The anesthetic solution immediately reaches the periapical region, and thus the axonal area of the nerve, where it can temporarily disable the sodium pump. The effect is felt almost without any time delay, and only a small amount of anesthetic solution is required. Methods: This study aims to investigate the efficacy of IO using the AnestoⓇ device after infiltration anesthesia (IA) and/or inferior alveolar nerve block anesthesia (IANB) failed to work in symptomatic irreversible pulpitis (hot tooth). The 33 patients included in the study were treated additionally with 1.7 ml articaine hydrochloride with 1:100,000 epinephrine hydrochloride (UltracainⓇ D-S, Sanofi-Aventis, Frankfurt, Germany) IO. Results: The electrical pulp test showed that 95.76% of the volunteers reacted positively to the combination of IANB or IA with the IO. In women, the additive IO was effective at 97.22%. In men, the IO led to pain elimination in 94.00% of cases. The duration of the IO was less than a quarter of an hour (13.03 min). The IO worked longer in women than in men (13.61 min vs. 12.33 min). Overall, more than every third tooth that needed trepanation was located in the posterior area of the mandible (36.4%). Treatment of hot teeth in this area was associated with an increased pulse rate and increased residual pain. There was a moderate correlation (Spearman-Rho [IRI] = 0.280) between the Visual Analog Scale (VAS) score and bone density, and a significant correlation (IRI = 0.612) between subjective residual pain and bone width. The IO resulted in a moderate, transient increase in the pulse rate by approximately 20 bpm. This is similar to the temporary increase in heart rate after conventional anesthesia techniques in non-preloaded patients and can be considered clinically irrelevant. Conclusion: IO with the AnestoⓇ device as an extension and deepening of local pain elimination is recommended for the treatment of hot teeth.
Background: Consumers and patients in the last two decades have increasingly turned to various internet search engines including Google for information. Google Trends records searches done using the Google search engine. Google Trends is free and provides data on search terms and related queries. One recent study found a large public interest in "dental anesthesia". In this paper, we further explore this interest in "dental anesthesia" and assess if any patterns emerge. Methods: In this study, Google Trends and the search term "dental pain" was used to record the consumer's interest over a five-year period. Additionally, using the search term "Dental anesthesia," a top ten related query list was generated. Queries are grouped into two sections, a "top" category and a "rising" category. We then added additional search term such as: wisdom tooth anesthesia, wisdom tooth general anesthesia, dental anesthetics, local anesthetic, dental numbing, anesthesia dentist, and dental pain. From the related queries generated from each search term, repeated themes were grouped together and ranked according to the total sum of their relative search frequency (RSF) values. Results: Over the five-year time period, Google Trends data show that there was a 1.5% increase in the search term "dental pain". Results of the related queries for dental anesthesia show that there seems to be a large public interest in how long local anesthetics last (Total RSF = 231) - even more so than potential side effects or toxicities (Total RSF = 83). Conclusion: Based on these results it is recommended that clinicians clearly advice their patients on how long local anesthetics last to better manage patient expectations.
