Association between depression and poor oral health in Korean elderly: the six Korean national health and nutrition examination survey (KNHANES VI-2) (우리나라 노인의 우울과 주관적 구강건강 관련성: 국민건강영양조사 제6기 2차(2014년) 자료 이용)

  • Objectives: The purpose of this study was to examine the association between depression and poor oral health in Korean elderly using Korean version of the Patient Health Questionnaire-9 (PHQ-9) for assessment of depressive symptoms. Methods: This study used the data from Korean National Health and Nutrition Examination Survey (KNHANES VI-2). The study included 1,454 elderly Korean aged over 65. Variables included demographic characteristics (gender, age), socioeconomic factor (income, education), systemic diseases, oral health related factor (tooth brush, dental products), health related factor (alcohol drinking, smoking), and depression. Logistic regression analysis was used as sequential models. Effects were quantified as odds ratios (OR) and 95% confidence intervals (CI). Results: From frequency analysis, being female, primary school or less, non-alcohol drinking, poor oral health were significantly related to depression. In the multiple logistic regression model, depression was significantly associated with poor oral health (OR=1.96, CI=1.15-3.53) after adjustment for other covariates including demographic characteristics, socioeconomic factor, systemic diseases, oral health related factor, and health related factor (OR=1.91, CI=1.13-3.27). Conclusions: Depression had an influence on the poor oral health after adjustment as confounding variable in the elderly. It should be focused on the health promotion for the elderly vulnerable to depression and poor oral health. The development of the mental health and oral health should be established.

Optimum Design of Teeth Shapes of Rotating Serration and Spline-type Torque Converter Parts Operating in a High Temperature Fluids (고온에서 맞물려 회전하는 토크컨버터 부품간 열 및 토크를 고려한 치형상의 최적설계)

  • The tooth shapes of serration-type and spline-type reactors are optimized using finite element methods to improve the working life of the part and to lower the stress concentration during rotation resulting from contact with the outer race for a reactor operating with $170^{\circ}C$ transmission oil. The results of thermal expansion analyses between an Al reactor and the steel outer race indicate that, before optimization, the gap between the two parts increases further as the serration-type reactor expands by 0.1 mm and the spline-type one strains by 0.08 mm. Because of shape optimization, a trapezoidal shape is obtained from the initial triangular serration and the rectangular spline of the two reactors. The maximum von Mises stress of the serration-type convertor decreased by 24.5 %, and by 9.3 % for the spline-type convertor. In addition, there is a 13 % reduction in the axial thickness, as compared to the initially designed model.

  • Cariogenicity of the bacteria is attributed to their binding capacity to the teeth. Bacterial attachment to oral surfaces is an essential step for colonization and subsequently infection. Therefore, it is conceivable that caries prevention can be achieved fundamentally by inhibition of bacterial attachment. The rationale for caries prevention through the use of sugar substitutes or limited use of sugar has been revealed. Among many sugar substitutes, xylitol has been shown to exhibit the most profound cariostatic effect, inhibiting glucose metabolism and possibly binding of mutans streptococci. The purpose of this study was to examine the effect of xylitol on binding of different species of oral bacteria. The effect of xylitol on binding of [$^3H$]-labeled oral bacteria to hydroxyapatite coated with human saliva(SHA) as a model for the pellicle-coated tooth surfaces was investigated. The strains of oral bacteria used in this study were A. viscosus T14V, A. viscosus WVU627, P. gingivaiis 2561, P. gingivalis A7Al-28, S. gordonii G9B, S. gordonii Challis, S. sobrinus 6715, S. mutans UA101, S. mutans KPSK -2, S. mutans T8, and S. mutans UA130. The obtained results were as follows: 1. P. gingivalis A7 Al-28, S. mutans UA130, S. mutans T8 grown with xylitol showed greater binding to SHA than the organism grown without xylitol. Among these, S. mutans T8 showed the greatest rate of increase in its binding to SHA ; 8-fold increase in its binding with xylitol. 2. S. mutans KPSK -2 grown with xylitol showed 2 times lesser binding to SHA than the organism grown without xylitol. 3. Binding ability of the remaining strains grown with xylitol to SHA was almost same as that of the organisms grown without xylitol. The overall results suggest that use of xylitol in the oral cavity may affect the complex oral bacterial ecosystem.

  • The radiopacity of glass ionomer cements is quite variable. The use of a poorly radiopaque material as a base under other restorative materials can mislead the dentist to a diagnosis of recurrent decay. This study investigates the radiopacity of these materials and proposes a minimal radiopacity under which a material should not be used as a base or liner. It is important to determine the radiopacity of glass ionomer dental materials so that the clinician can appreciate the type of restorative materials used when radiographically evaluation the possibility of recurrent dental caries. In this study, radiopacity of Vitrement and Chemfil was compared with that of Cavalite, Miracle mix and polycarboxylate cement. Tooth model of artificial cavity preparation for diagnosis of recurrent caries was omitted. Radiopacity of each material was measured using relatives between thickness and radiopacity of Aluminium step wedge. The results were as follows : 1. Radiopacity of Vitrement was some higher than enamel. 2. Chemfil, restorative glass ionomer, was less radiopaque than enamel. 3. In order of higher radiopacity than enamel, Miracle mix was highest and was followed by polycarboxylate cement, Cavalite and Vitremer. 4. Vitremer, the Glass Ionomer Cement, is useful to detection of recurrent caries, because it is slightly higher radiopaque than enamel. So, it is suitable for restorative material and luting cement.

  • Statement of problem : Clinically, maxillary first premolar has a high risk of fracture. This is thought to be caused by the susceptible figure which the maxillary first premolar has In other words, sharp cusp angles of the premolar is thought to influence this situation. Purpose : This study was to know stress distribution of all-ceramic crown according to the cusp angle. Material and Method : It was manufactured a three dimensional finite element model simplified maxillary first premolar, and then analyzed stress distribution when cusp angle was each $80^{\circ}$, $90^{\circ}$, $100^{\circ}$, $110^{\circ}$ and $120^{\circ}$. Results and conclusion : 1. The von Misses stress showed that stress decreases as cusp angle increases in the central groove of the occlusal surface. 2. It showed that maximum principal stress was centered at the region of the central groove of the occlusal surface and a region which the force was inflicted. And also it appeared high on the lingual and buccal side of finish line. 3. The X axis of normal stress was focused in the central groove of the occlusal surface. The Y axis normal stress appeared high in the central groove of the occlusal surface, buccal and lingual side. 4. The Stress near the finish line showed a low value compared with stress in the region of the central groove of the occlusal surface. 5. It shows that the most dangerous angle for tooth fracture was on $80^{\circ}$ of the cusp angle and low on $120^{\circ}$ of its.

Full mouth rehabilitation with vertical dimension elevation in the patient with severely worn dentition: case report (과도한 치아 마모 환자에서 수직고경 증가를 동반한 전악 수복 증례)

  • Excessive teeth abrasion causes pathologic changes of oral environment and masticatory system due to decrease in occlusal vertical dimension. When establishing new occlusal plane and recovering vertical dimension, accurate diagnosis and analysis are essential. In this case, after appropriate centric relation in elevated vertical dimension was taken using Gothic arch tracer, full mouth rehabilitation was performed. A 70-year-old male patient had the chief complaint that he could not eat due to teeth wear. He showed generalized teeth wear and decreased vertical dimension. Elevation of vertical dimension was planned by model analysis. According to increased vertical dimension, centric relation was recorded using Gothic arch tracer and temporary prostheses were applied. Appropriate occlusion was established by temporary prostheses for 4 months. Final prostheses were fabricated using vertical dimension adapted by temporary prostheses. Through this procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

PREVALENCE OF BLACK-PIGMENTED BACTERIA IN INFECTED ROOT CANALS IN KOREA (감염 근관의 흑색세균의 동정)

  • The role of bacteria in root canals and periapical infections is well known and established. In these bacteria, black-pigmented bacteria(BPH) play important role in endodontic infection. BPB are Gram negative anaerobic rods which are closely related 50 clinical symptoms such as pain, percussion, tenderness, foul odor, etc. In America and Europe, many studies on BPB have been done and are continued. But, relatively few studies have been done in Korea, especially its prevalence in Korean population is not yet studied. The purpose of this study is to establish prevalence of BPB in infected root canals and periapical abscesses in Korean people. Microbial samples were collected from the root canals of 34 intact tooth with periapical rarefactions of endodontic origin and 3 periapical abscesses. All samples were incubated in an anaerobic chamber(Coy, Model No. 77. Ann Arbor, Michigan, USA.). Identification of In microorganism was based on its growth in the anaerobic chamber, colonial pigmentation, colonial morphology, Gram stain, and Rapid ID32A(BioMericux SA/69280 Marcy-l'Etoile/France) results. In addition, the polyme ase chain reaction using specific primers for 16S rRNA genes were used differentiate Prevotella nigrescens for Prevotella intermedia. The results were as follows : 1. In this study, thirteen (35%) of thirty seven samples were positive for the growth of BPB. In thirteen samples, sixteen strains of BPR were found. 2. The most frequently identified BPB in root canals and abscesses in Korean were P. nigrescens 5/37(14%) and P. intermedia 5/37(14%). Porphyromonas gingivalis 3/37(8%), Porphyromonas endodontalis 2/37(5%) and Prevotella loecheii 1/37(3%) were also found. 3. In this study, no significant differences were found between the prevalence of BPB in Korean and that of American and European.

  • The responses of human pulp fibroblastic cells to Ga-As Semi-Conductor-Dens-Bio Laser (Frequency: 5 Hz~10,000 Hz Model: SD-101A RCA, U.SA)) were examined in vitro using pulp fibroblastic cells obtained from the pulp tissue of human tooth. The mitogenic effect of soft laser was assessed by measuring the MTT assay. The morphologic effect for soft laser showed under the scanning and transmission electron microscopy. The results as follows; 1. The mitogenic response of the soft laser was not observed until 4th time of radiation, while the mitogenic response at 4th time increased mitogenic effect by as much as 1.7 fold compared to the control value. 2. The mitogenic response of the soft laser on pulp fibroblast differ from the mitogenic response on other fibroblasts. 3. In scanning electron microscopic study, The microvilli of cell surface increased gradually with width and length after laser radiation, it demonstrate that development of microvilli have close connection with differentiation of cells. 4. Under the transmission electron microscope, The laser-treated cells maintained their elongated shape and a high degree of cellular polarization. The large cell body containing a well developed Golgi complex, a large number of profiles of rough endoplasmic reticulum, and great numbers of mitochondria. 5. The laser-treated cells maintained the long straight bundles of closely apposed microfilaments or individual filaments forming a cross-linked network. These findings suggest that the laser may have important roles in promotion of pulp healing and consequently may be useful for clinical application in pulp regenerative procedures.

  • The purpose of this study was to analyze the magnitude and distribution of stress using photoelastic model with the rigid connection using T-block attachment and non-rigid connection using key & keyway attachment. The vertical load of 16 Kg was applied on the central fossa of the tooth, the pontic and the implant, and the pattern and distribution under each condition was analyzed. The following results were obtained : 1. In case of vertical load on the central fossa of the implant, the stress was concentrated at the apex of the implant involving the mesial alveolar bone in both fixed partial denture with the rigid connection and that with the nonrigid connection and the stress concentration at the mesial cervical area of the implant was a little more in the nonrigid connection than in the rigid connection. 2. In case of vertical load on the central fossa of the pontic, the stress was concentrated at the apex of 2nd bicuspid in both 3 unit fixed partial denture with nonrigid connection and that with the rigid connection. The stress was more concentrated at the mesial alveolar bone of the implant, but the stress distribution at the natural teeth more favorable at the rigid connection than at the non-rigid connection in case of 4 unit fixed partial denture. 3. In case of vertical load of the central fossa of the 2nd bicuspid, much stress with 3 fringe order was observed at the apex of the 2nd bicuspid in the 3 unit fixed partial denture, but relatively even stress distribution was observed at the apex of the implant, the 1st and 2nd bicuspid, and the adjacent cuspid in the 4 unit fixed partial denture.

Comparison of two computerized occlusal analysis systems for indicating occlusal contacts

  • PURPOSE. The purpose of this study was to compare the performance of Accura to that of the T-scan for indicating occlusal contacts. MATERIALS AND METHODS. Twenty-four subjects were selected. Their maxillary dental casts were scanned with a model scanner. The Stereolithography files of the casts were positioned to align with the occlusal plane. Occlusal surfaces of every tooth were divided into three to six anatomic regions. T-scan and Accura recordings were made during two masticatory cycles. The T-scan and Accura images were captured at the maximum bite force and overlapped to the cast. Photographs of interocclusal records were used as the reference during overlap. The occlusal contacts were counted to compare the T-scan and Accura. McNemar's test was used for statistical significance and the corresponding P-values were calculated from a chi-square distribution with one degree of freedom. The accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of Accura were calculated relative to the T-scan values as a control. RESULTS. No statistical differences (P>.05) were found between the T-scan and Accura methods. The accuracy of Accura was 75.8%, sensitivity was 82.1%, specificity was 60.1%, PPV was 82.9%, and NPV was 60.1%. CONCLUSION. Accura could be another possible option as a computerized occlusal analysis system for indicating occlusal contacts at maximum intercuspation.
