INFLUENCE OF POST TYPES AND SIZES ON FRACTURE RESISTANCE IN THE IMMATURE TOOTH MODEL (미성숙 치아 모델에서 포스트의 종류와 크기가 치아의 파절 저항성에 미치는 영향에 관한 연구)

  • The purpose of this study was to determine the effect of post types and sizes on fracture resistance in immature tooth model with various restorative techniques. Bovine incisors were sectioned 8 mm above and 12 mm below the cementoenamel junction to simulate immature tooth model. To compare various post-and-core restorations, canals were restored with gutta-percha and resin core, or reinforced dentin wall with dual-cured resin composite, followed by placement of D.T. LIGHT-POST, ParaPost XT, and various sizes of EverStick Post individually. All of specimens were stored in the distilled water for 72 hours and underwent 6,000 thermal cycles. After simulation of periodontal ligament structure with polyether impression material, compressive load was applied at 45 degrees to the long axis of the specimen until fracture was occurred. Experimental groups reinforced with post and composite resin were shown significantly higher fracture strength than gutta-percha group without post placement (p < 0.05). Most specimens fractured limited to cervical third of roots. Post types did not influence on fracture resistance and fracture level significantly when cement space was filled with dual-cured resin composite. In addition, no statistically significant differences were seen between customized and standardized glass fiber posts, which cement spaces were filled with resin cement or composite resin individually. Therefore, root reinforcement procedures as above in immature teeth improved fracture resistance regardless of post types and sizes.

  • In dental trauma, reattachment of the original tooth fragment improves the reproduction of original tooth shape, texture, color, and radiolucency; thus, it provides good aesthetics. A 9-year-old boy was referred due to complicated crown-root fracture of the maxillary right central incisor. Although it had poor prognosis due to severe coronal damage and subcrestal fracture, reattachment of the tooth fragment was chosen due to the patient's age. One-visit apexification with mineral trioxide aggregate (MTA) was performed, followed by osteotomy and reattachment of the tooth fragment with post placement. Regular observation revealed no clinical signs or symptoms and no radiologic complications.

Conservative and esthetic approach in crown fracture of maxillay anterior tooth: tooth fragment reattachment (상악 전치부 치관 파절의 보존적이고 심미적인 접근법: 파절편 재부착)

  • Crown fractures are the most frequent traumatic injuries to permanent teeth and mainly involve the maxillary incisors due to their exposed position in the dental arch. One option for managing crown fractures, when the tooth fragment is present and in good condition, is reattachment of the fragment to its original position. This paper reports on three crown fracture cases in which successful esthetic and functional results were achieved by reattachment of the tooth fragment.

Oral symptom experiences in adolescents among multicultural families according to the parental nationality: The 16th Korean youth health behavior survey (부모의 국적에 따른 다문화가정 청소년의 구강 증상 경험: 제16차 청소년건강행태조사)

  • Objectives: This study aimed to evaluate the oral symptoms experiences of Korean adolescents among multicultural families. Methods: Data on 985 participants from the 16th Korea Youth Risk Behavior Web-based Survey were collected and analyzed using complex sample descriptive statistics. Results: The foreign-born mother, foreign-born parents, and foreign-born father rates were 77.2%, 17.3%, and 5.4%, respectively. The toothache, gingival bleeding, tooth fracture, and tooth sensitivity experience rates were 21.3%, 18.8%, 13.3%, and 30.4%, respectively. The tooth fracture rate was highest in the foreign-born parents group (24.6%). The toothache odds ratio (OR) was 1.71 (95% CI: 1.24-2.37) times higher in girls. The foreign-born mother group was 2.47 (95% CI: 1.09-5.60) times higher than that of the foreign-born father or both parents foreign-born groups. The smoking group was 2.03 (95% CI: 1.20-3.43) times higher than that of the non-smoking group. The gingival bleeding OR was 5.11 (95% CI: 1.80-14.53) times higher in the lowest economic status group. The tooth fracture OR was 3.44 (95% CI: 1.01-11.70) times higher in the lowest economic status group. The tooth sensitivity OR was 1.53 (95% CI: 1.14-2.04) times higher in girls. Conclusions: It is necessary to establish a program to promote oral health of adolescents from multicultural families.

Comparative Evaluation of Infection Rate according to Extraction of Teeth in the Line of Mandibular Angle Fractures Treated with Stable Internal Fixation (안정된 내고정이 시행된 하악각 골절에서 골절선상의 치아 발거에 따른 감염률 비교평가)

  • Purpose: Mandibular angle fractures constitute approximately 30% of mandibular fractures, and the mandibular third molar is usually in line with the fracture. This study evaluated the relationship between the extraction of a tooth in line with a mandibular angle fracture and the infection rate. Methods: One hundred and forty seven patients with mandibular angle fractures containing a tooth in line with the fracture from 2005 to 2008 were enrolled in this study. The patients were divided into two groups based on an extraction, and the infection rates were evaluated in both groups. Results: An extraction was performed in 70 patients (47.6%) and postoperative infections occurred in 14 patients (9.5%). Six (8.6%) of the 70 patients in the extraction group and 8 (10.4%) of the 77 patients in the non-extraction group had an infection. There was no significant difference between the two groups (P=0.708). Conclusion: These results show that there is no increased risk of postoperative complications when a tooth is present

Relation of necessity of the first aid education and attitude about traumatic dental injuries in some military men (일부 군인들의 치아외상에 대한 태도와 응급처치 교육필요성과의 관련성)

  • Objectives : The purpose of this study was to investigate the traumatic injuries by first aid education and mouth protector in some military men. Methods : Among 195 military men, a total of 164 military men filled out the self-administered questionnaire and the return rate was 84.1%. The data were evaluated statistically using chi-square analysis. Results : 81.5% of respondents agreed that first aid education is necessary and 48.2% of respondents agreed that mouth protector of is necessary. In managing tooth fracture, subluxation, and avulsion, the military men answered the necessity of the first aid education revealed 87.2%, 86.8%, and 91.5%, respectively. The questions in managing tooth fracture, subluxation showed that mouth protector is necessary 56.3%, 49.0% respectively. In managing tooth fracture with reattachment, the necessity of the first aid education showed 88.9%. Conclusions : In order to get the knowledge about emergency treatment of traumatic injuries in military men, it is necessary to develop the professional education program for the dental hygiene personnel.

Observation of Mechanical Strength of Materials for Dog Dental Prosthesis Production (중형견(犬) 치과 보철물 제작을 위한 소재의 기계적 강도 관찰)

  • Purpose: This study is making a dog dental prosthesis using digital dental technology. The mechanical strength of the prosthetic material was observed in terms of compressive strength and fracture pattern. Methods: The experiment was performed using dog mandibular molars. The teeth were scanned and modeled. The specimens were made of zirconia, PMMA and Ni-Cr. The specimens were subjected to a vertical compression test with an artificial cancellous bone in UTM tester. Vertical compressive strength and fracture behavior of specimen were observed. Results: The result of observing the compressive load between specimen and artificial bone were $184.8{\pm}5.7N$ in the zirconia specimen, $185.6{\pm}8.9N$ in the PMMA specimen, and $184.4{\pm}4.0N$ in the Ni-Cr alloy specimen. Compression marks of artificial bones were observed. The fracture strength of specimen was observed. The fracture strength of the zirconia specimen was an average of 1,381.4N. The fracture strength of the PMMA specimen was an average of 572.2N. Conclusion: The crown made of three kinds(zirconia, PMMA, Ni-Cr alloy) of materials has the strength to chew about the artificial bone. zirconia and PMMA have vertical compressive strength applicable to medium dog dental prosthetic materials.

Difference of visiting time and major cause of disease by operating time changes of a university dental emergency room (일개 치과대학병원 응급실 운영시간 변경에 따른 환자의 방문시기와 주요상병의 차이)

  • Objectives: The purpose of the study is to investigate of visiting time and major cause of disease by operating time changes of a university dental emergency room. Methods: This study was a retrospect study carried out by reviewing 9,172 records visiting the university emergency room from January 1997 to December 2009. Data were analyzed by frequency test, chi-square test, and logistic regression using SAS version 9.3. Results: Twenty four hours emergency room had 19 percent more patients than those in daytime emergency room. Daytime emergency room had more male patients during spring(26.6%) and winter(20.2%) than female patients(p<0.05). Female patients exceeded male patients during summer(24.4%) and autumn(36.2%)(p<0.05). Twenty-four hours emergency room had the maximal patients in autumn and the minimal patients in winter(p<0.05). The most common injury in the daytime was tooth fracture from 0 to 19 years old(p<0.05). Tooth fracture, jaw fracture, and soft tissue injury were the most common injury in dental emergency room and majority of the patients were the male(p<0.05). Conclusions: Twenty-four hours dental emergency room had a variety of causes of dental emergency thant that in the daytime. Twenty four hours dental emergency room must be prepared for the dental emergency patients at any time provided with personnel and facilities.

Fracture Resistance of Incisal Tooth Fragment reattached with different Materials and Preparation (레진재료와 치아형성 방법에 따른 파절편 재부착치아의 파절저항성)

  • The purpose of this study is to analyze the fracture resistance of reattached tooth according to the resin materials and tooth preparation type under physiological conditions. Uncomplicated crown fracture in the oblique direction was reproduced on the extracted 64 anterior teeth. Depending on the composite resin material, reattachment was performed using a flowable resin and a packable resin. Depending on retentive forms, reattachment was performed using simple reattachment, 1.0 mm × 1.0 mm labial chamfer bevel, 1.0 mm × 1.0 mm lingual chamfer bevel and 1.0 mm × 1.0 mm circumferential bevel. A load was applied to the palatal surface of the tooth using a universal testing machine at an angle of 125 degree, which is the interincisal angle of normal children. Under the masticatory pressure condition, fracture resistance of lingual chamfer groups was 28.28 ± 7.41 MPa and 27.54 ± 4.45 MPa, which was significantly higher than those of simple reattachment groups, 17.21 ± 5.87 MPa and 20.10 ± 6.00 MPa, in both flowable and packable resin groups. When considering the lingual force similar to masticatory pressure, the fragment retention was significantly improved when the lingual chamfer was formed compared to the simple reattachment. Clinicians may consider the design of the lingual chamfer in order to improve fracture resistance to masticatory pressure during fragment reattachment.

Surgical Repair of Rostral Hemimandibular Fractures with Canine Tooth Luxation in a Dog (개에서 견치 탈구를 동반한 앞쪽 하악골 골절의 수술적 정복 1례)

  • A sexually intact female Alaskan Malamute, 5-year-old weighing 25.2 kg presented to the Hangang Animal Hospital, following hit-by-car. On initial presentation, the dog had severe salivation and the right canine tooth luxation. Radiographs revealed fractures of the right rostral hemimandible with the right canine tooth luxation and symphyseal separation. Surgical repair of fractures was performed by use of an interdental wiring technique and external fixation technique that included pins, an acrylic connecting bar, and polymethyl methacrylate bone cement. Post-operative radiographs revealed that bridging callus was well formed over cortices of the fracture area. Pins and an acrylic connecting bar were removed 7 weeks post-operatively. The dog exhibited evidence of normal mastication.
