Trends in the incidence of tooth extraction due to periodontal disease: results of a 12-year longitudinal cohort study in South Korea

  • Purpose: This study evaluated trends in tooth extraction due to acute and chronic periodontal disease (PD) using data from the National Health Insurance Service-National Sample Cohort for 2002-2013. Methods: A random sample of 1,025,340 individuals was selected as a representative sample of the population, and a database (DB) of diagnostic and prescription codes was followed up for 12 years. We used multivariate logistic regression analysis to assess the incidence of total extraction (TE), extraction due to periodontal disease (EPD), and immediate extraction due to periodontal disease (IEPD) according to sociodemographic factors (sex, age, household income, health status, and area of residence). Results: The incidence of tooth extraction was found to be increasing, and at a higher rate for TE in PD patients. In 2002, 50.6% of cases of TE were caused by PD, and this increased to 70.8% in 2013, while the number of cases of IEPD increased from 42.8% to 54.9% over the same period. The incidence rates of extraction due to acute and chronic PD increased monotonically. We found that the incidence rates of TE, EPD, and IEPD were all 2-fold higher among patients with high income levels and those who were not beneficiaries of health insurance. Conclusions: The rates of TE, EPD, and IEPD have been steadily increasing despite dental healthcare policies to expand public health insurance coverage, increasing the accessibility of dental clinics. Moreover, the effects of these policies were found to vary with both income and education levels. Consistent patient follow-up is required to observe changes in trends regarding tooth extraction according to changes in dental healthcare policies, and meticulous studies of such changes will ensure optimal policy reviews and revisions.

THE EFFECTS OF ATELO-COLLAGEN SPONGE INSERTION ON THE PERIODONTAL HEALING OF SECOND MOLARS AFTER IMPACTED MANDIBULAR THIRD MOLAR EXTRACTION (매복 하악 제3대구치 발치와에 Atelo-collagen Sponge 삽입이 제2대구치 예후에 미치는 영향)

  • Extracellular matrix(ECM) is known to function as a reservoir of endogenous growth factors, can be an effective delivery system of growth factor that easily lost bioactivity in solution. Fibrillar collagens like type I collagen, are the major constituent of the ECM and structural protein of bone. Also, it can be a scaffold for osteoblast migration. The purpose of this study was to compare the effects of absorbable Atelo-collagen Sponge($Teruplug^{(R)}$) insertion in tooth extraction sites on periodontal healing of the mandibular second molar after the extraction of the impacted third molar. The study population comprised 31 cases who had been scheduled for surgical removal of impacted mandibular third molars. All patients were in good general health and were not using any medication that would influence wound healing after surgery. In 15 cases control group, none was inserted into the tooth extraction site. In 16 cases experimental groups, $Teruplug^{(R)}$ was inserted into the tooth extraction site. We evaluated tooth mobility, pocket depth, gingival margin level preoperatively and 1 week, 2 weeks, 4 weeks, and 3 months postoperatively. The change was compared with two groups using Mann-Whitney test. The results were as follows. 1. There was no significant change of tooth mobility on both groups. 2. There was tendency of decreasing of previous pocket depth causing tooth extraction on both groups. 3. On gingival margin level, there was various change according to initial swelling and loss of attachment on both groups. 4. There was tendency of decreasing of gingival margin level on both groups because of removal of inflammation and decreasing of previous pocket depth. 5. There was large change of pocket depth on buccal middle, distal, lingual distal area because of tooth extraction and bone reduction. Compared with the control group and experimental group, we observed significant difference during some periods. The results of this study suggest that absorbable atelo-collagen sponge($Teruplug^{(R)}$) is relatively favorable bone void filler with prevention of tissue collapse, food packing and enhance periodontal healing.

Dysesthesia after Tooth Extraction and Implant Surgery Reported by Dentists (치과의사에 의해 보고된 발치 및 임프란트 수술 후 지각이상에 대한 분석)

  • The purpose of this study was to analyze the nerve damage after tooth extraction and implant surgery, and to establish a predictive model for assessment and management of dysesthesia. In this questionnaire study, the subjects chosen for this study were 276 dentists who answered the questionnaire about dysesthesia after tooth extraction and implant surgery. The analysis of the results consist of the sex and age distribution, affected site, associated symptoms, rate and duration of the recovery. The results are summarized as follows. : 1. There were no significant difference between the sex and the dysesthesia. 2. The most common affected site was the mandibular region. In the group of the implant surgery, 100% affected the mandibular site. The tooth extraction group was 93.2% affected. 3. Pain was one of the most associated symptom with dysesthesia-46.5% of the tooth extraction and 44.8% of the implant surgery. 4. The recovery ratio was 72.3% in the tooth extraction, 71.8% in the implant surgery. Most of them, they recovered in $1{\sim}6$ months. In conclusion, most of dysesthesia may be recovered within 1 year. However, the possibility of persistent dysesthesia should not be neglected. Therefore, practitioners must discuss the possibility of nerve injury with their patients, and include this possibility in the consent forms. Various methods of monitoring recovery of sensation should be considered for objective assessment of prognosis. In addition, immediate referral to orofacial pain specialists can offer the patients an opportunity for more effective and noninvasive treatments.

Horizontal alteration of anterior alveolar ridge after immediate implant placement: A retrospective cone beam computed tomography analysis

  • PURPSE. The aim of this study was to evaluate the labio-lingual alterations of the alveolar bone where the implant was placed immediately after tooth extraction. MATERIALS AND METHODS. Implants were placed immediately after tooth extraction on anterior alveolar ridges in the maxilla and mandible. The pinguide system was used to help determine the location and path of implants during the surgical process. The horizontal distance from implants to the outer border of alveolar bone was measured at the rim and middle of the implants in the cone beam computed tomography images. The alteration of alveolar bone was evaluated comparing the horizontal distances measured immediately after surgery and 3 months after surgery. RESULTS. The results show that more resorption occurred towards the labial bone than the lingual bone in the maxilla. A similar amount of labial and lingual bone resorption was observed in the mandible. CONCLUSION. Considering the horizontal alteration of alveolar bone, labio-lingual positioning of the implant towards the lingual bone in the maxilla and at the center of the alveolar ridge in the mandible is recommended when it is placed immediately after tooth extraction.

Comparative Evaluation of Infection Rate according to Extraction of Teeth in the Line of Mandibular Angle Fractures Treated with Stable Internal Fixation (안정된 내고정이 시행된 하악각 골절에서 골절선상의 치아 발거에 따른 감염률 비교평가)

  • Purpose: Mandibular angle fractures constitute approximately 30% of mandibular fractures, and the mandibular third molar is usually in line with the fracture. This study evaluated the relationship between the extraction of a tooth in line with a mandibular angle fracture and the infection rate. Methods: One hundred and forty seven patients with mandibular angle fractures containing a tooth in line with the fracture from 2005 to 2008 were enrolled in this study. The patients were divided into two groups based on an extraction, and the infection rates were evaluated in both groups. Results: An extraction was performed in 70 patients (47.6%) and postoperative infections occurred in 14 patients (9.5%). Six (8.6%) of the 70 patients in the extraction group and 8 (10.4%) of the 77 patients in the non-extraction group had an infection. There was no significant difference between the two groups (P=0.708). Conclusion: These results show that there is no increased risk of postoperative complications when a tooth is present

Reasons for primary teeth extraction in children : Retrospective study (유치 발거 원인에 대한 후향적 연구)

  • The purpose of this study was to investigate the principal reasons for primary teeth extractions and the tooth type extracted in children. 1159 patients were selected in this study. Dental records and radiographs were reviewed and age, gender, medical history, type of tooth extracted and the reasons for extraction were collected. The data were statistically analyzed using Chi-square test. Total 2078 primary teeth were extracted. Central incisors(34.1%) were most frequently extracted. Extractions due to physiological mobility(77.5%) were the most frequent followed by caries(13.8%), orthodontic(3.9%), trauma(1.7%). Reason for the extraction was different according to age (p = 0.000), but there was no difference according to gender (p = 0.109). While extractions due to physiological mobility predominated overall, reason for extraction was different according to the tooth type. There was no previous treatment in 54.6% of primary teeth extracted due to caries. Excluded physiological mobility, caries are the most common reason for extraction of primary teeth. The importance of preventive care should be emphasized in order to preserve primary teeth and improve children's oral health.

Compromised extraction sockets: a new classification and prevalence involving both soft and hard tissue loss

  • Purpose: Previous studies have solely focused on fresh extraction sockets, whereas in clinical settings, alveolar sockets are commonly associated with chronic inflammation. Because the extent of tissue destruction varies depending on the origin and the severity of inflammation, infected alveolar sockets may display various configurations of their remaining soft and hard tissues following tooth extraction. The aim of this study was to classify infected alveolar sockets and to provide the appropriate treatment approaches. Methods: A proposed classification of extraction sockets with chronic inflammation was developed based upon the morphology of the bone defect and soft tissue at the time of tooth extraction. The prevalence of each type of the suggested classification was determined retrospectively in a cohort of patients who underwent, between 2011 and 2015, immediate bone grafting procedures (ridge preservation/augmentation) after tooth extractions at Seoul National University Dental Hospital. Results: The extraction sockets were classified into 5 types: type I, type II, type III, type IV (A & B), and type V. In this system, the severity of bone and soft tissue breakdown increases from type I to type V, while the reconstruction potential and treatment predictability decrease according to the same sequence of socket types. The retrospective screening of the included extraction sites revealed that most of the sockets assigned to ridge preservation displayed features of type IV (86.87%). Conclusions: The present article classified different types of commonly observed infected sockets based on diverse levels of ridge destruction. Type IV sockets, featuring an advanced breakdown of alveolar bone, appear to be more frequent than the other socket types.

Improving oral rehabilitation through the preservation of the tissues through alveolar preservation

  • When performing a tooth extraction, imminent collapse of the tissue by resorption and remodeling of the socket is a natural occurrence. The procedure for the preservation of the alveolar ridge has been widely described in the dental literatures and aims to maintain hard and soft tissues in the extraction site for optimal rehabilitation either with conventional fixed or removable prosthetics or implant-supported prosthesis.

  • In treatment of dentigerous cyst, complete enucleation, histopathologic examination and postoperative care are important to prevent the potential complications (mural ameloblastoma, squamous cell carcinoma). On the other hand, a maxillary impacted supernumerary anterior tooth are removed surgically, owing to the possibility of the cyst formation in future. After the cyst enucleation and extraction of the involved tooth, the wound area sutured and removable resin plate is then applied. In this operation, the postoperative bleeding and infection is likely to occur owing to postoperative accumulation of hematoma & seroma, psychologic stress and other contaminated factor. So, the authors established the immediate rubber & iodoform gauze drainage into the sutured wound of cyst enucleation & tooth extraction for the prevention of postoperative bleeding and infection. The removable resin splint are not used because of the poor cooperation and economic factor. The results were more favorable without the postoperative bleeding & wound infection in a cerebral palsy patient.

A Case Report on Facial Nerve Palsy after Tooth Extraction and Korean Medical Treatments (발치 후 병발한 안면마비 환자에 대한 한의학적 치료 사례 보고)

  • Objectives : Facial nerve palsy is a rare but well-known complication that occurs after a tooth extraction. The paralysis follows the injection of a local anesthetic, but patients typically recover after a few hours. However, there are a number of reports of delayed paralysis, and the cause of delayed facial palsy remains uncertain. This study is the first case report detailing how Korean medicine can be used to treat facial nerve palsy following tooth extraction. This study reports our experience of a patient's favorable recovery. Methods : A 25-year-old male patient experienced acute facial palsy after four premolar teeth were extracted. He was hospitalized in the Pusan National University Korean Medical Hospital. We provided complex Korean traditional medical treatments such as acupuncture, cupping, use of a hot water steamer, and herbal medicine for 18 days. Results : Using the Yanagihara Grading Score, we found improvements in the patient's voluntary facial movement as his score increased from 22 to 34. Furthermore, his accompanying symptoms, such as dry eye and facial pain, disappeared. However, the patient reported transient pain around acupoints after the acupuncture intervention. Conclusion : Our study suggests that Korean medical treatments might be effectively used to treat facial nerve palsy after tooth extraction, although further research should be conducted due to the limited number of cases in this area.
