Tooth discoloration detracts from one's appearance and influences self-image and it is particularly true in children. Therefore, pediatric dentists are required to treat tooth discoloration manifested in children for the normal development of their psycosocial health. Three treatment modalities are currently availabler for the removal of a variety of intrinsic stains from vital teeth. These are enamel microabrasion technique using hydrochloric acid, office bleaching and home bleaching technique with carbamide. Microabrasion technique has several advantages over bleaching in that it is easy to accomplish and does not require multiple office visits or the expensive instruments and the color change seems to be permanent after treatment. The process relies on decalcification, a softening with HCl and then removal of the enamel containing the stain with rubbing. Due to the mechanism of stain removal, this method is indicated for the removal of superficial enamel stains or disc oloration only. We report four successfully treated cases by enamel microabrasion using 15% HCl and pumice. Entire clinical steps are described in detail with some discussions on the outcome.
The purposes of this study were to examine the effect of different fluoridated bleaching solution on the changes in physical and chemical characteristics of tooth. Forty-eight bovine incisors were divided into four groups to receive bleaching treatments, over a 14days period, as follows: no treatment; 10% carbamide peroxide (CP) bleaching; 10% CP containing 0.05% fluoride; and 10% CP containing 0.1% fluoride. All the specimens were highly polished and discolored with commercial COCK.Color and enamel changes were determined with colorimeter, microhardness tester, scanning electron microscope, atomic force microscopy. All the collected data were analyzed with one-way ANOVA. After the bleaching, bleached groups showed the color change(E*). Microhardness of 10% CP group decreased after tooth bleaching. But microhardness of containing fluoride bleached groups increased after tooth bleaching. Enamel surface of 10% CP bleached group showed any apparent morphology and roughness changes compared to the enamel which was stored in distilled water only. These results demonstrated that Fluoridated 10% Carbamide Peroxide have appreciable bleaching effect on bovine teeth and were not adversely affects enamel. Supporting influence of fluoride-containing bleaching solution on remineralization could be observed and further research must be carried out in various active environments to confirm these results clinically.

The aim of this study was to develop an ultrasonic tooth-whitening apparatus using piezoelectric transducers, which enhance bleaching efficiency by applying ultrasound, while performing a teeth whitening procedure. For this purpose, an ultrasonic transducer was designed and manufactured, and the effects of reduction in the whitening material's concentration and in the whitening treatment time through using the ultrasound cavitation phenomenon were confirmed. Also, the validity of this study was investigated by comparing the whitening performance with a commercialized optical whitener, through color comparison. The results revealed that the ultrasound whitener produced color values that were enhanced by as much as double that of the conventional LED light whitening method. Even when the operational time was reduced by half, the ultrasound method showed superior performance by over 54% compared to the conventional light whitener, revealing that the ultrasound method showed a remarkable treatment reduction effect.

Materials and Methods Twelve exrtacted central incisors were cut into pieces and central four segments were chosen from each tooth and embedded in acrylic resin. Four blocks with 12 tooth segments embedded in acrylic resin were acquired and numbered from one to four. The objective of this in situ study was to evaluate the effects of whitening strip (Claren, LG Household & Health Care Ltd, 2.6% hydrogen peroxide) and gel (Opalescence, Ultradent, 10% carbamide peroxide) on microhardness of enamel in comparison with untreated control. Extracted twenty human upper incisors were disinfected, cleaned, and labial side of each incisor sectioned into 3 fragments by 2 ${\times}$ 2 mm size. After sectioning, labial sides of fragments were flattened and fixed to orthodontic bracket using flowable composite resin. Specimens prepared from each tooth were attached to the labial side of upper incisors of twenty volunteers one by one and treated by three different methods: (1) untreated control (2) treated with whitening strip for 14 days (3) treated with whitening gel for 14 days. Microhardness (Microhardness tester, Zwick) of each specimen was measured at the baseline of pre-treatment, immediate after bleaching treatment, 14 days after bleaching treatment and Knoop Hardness Number was determined. Microhardness changes of experimental groups were compared. The results show that tooth whitening strip and gel used in this study does not effect the microhardness of enamel during bleaching procedure.
The purposes of this study were to examine the effect of 35% Carbamide Peroxide(CP) bleaching agent on the changes in physical and chemical characteristics of tooth. The effect of bleaching agent on enamel was analyzed using Hardness test, SEM and EDS. The microhardness between bleached groups after bleaching showed statistically significant difference according to the paired t-test. The bleached enamel surface showed apparent morphological changes compared to the enamel, which was stored in distilled water only. The difference of the total mineral contents for the distilled water and Carbamide Peroxide did not show statistical significance. These results demonstrated that bleaching using 35% Carbamide Peroxide were adversely affects application time of experimental group and may the safety of using these agents for a short time in dentist-monitored bleaching.
This study evaluated whether fluoride treatment can affect recovery of the irregularity of enamel surface after tooth whitening with a high concentration of hydrogen peroxide (HP) activated by plasma arc light. A total of 36 bovine teeth stained with coke were used in this experiment. The specimens were classified into following three groups (two different commercial plasma arc groups and a control group without light curing source): (1) 35% HP gel only, (2): 35% HP gel and Plasma arc A, and (3) 35% HP gel and Plasma arc B. To measure color changes and surface morphologies before and after the bleaching, colorimeter and scanning electron microscopy were used, respectively. When the specimens were bleached with hydrogen peroxide and plasma arc lights, the bleaching effect was greater than when only hydrogen peroxide gels were used (Kruskal-Wallis test, p<0.05). In addition, plasma arc B showed the more color changes than plasma arc A (Bonferroni post-hoc test, p<0.05). The surfaces of the teeth treated with fluoride gel after the whitening treatment came to be smooth. Therefore, the results of this study suggested that the fluoride application for patients who got tooth whitening therapy with a high concentration of hydrogen peroxide gels activated by plasma arc light will be effective to recover rough enamel surfaces.
The purpose of this study was to evaluate tooth color and microhardness after 15% carbamide peroxide(CP) bleaching treatments with/without potassium nitrate and fluoride(PF), which were used home bleaching. Thirty tooth specimens were obtained from thirty premolar and were randomly divided into three groups: 1, untreated controls(Distilled water): 2, treatment with 15% CP bleaching agent; 3, treatment with 15% CP bleaching agent (contained 3% potassium nitrate and 0.11% fluoride). All groups were treated 6h per day for 14 days then immersed in distilled water. Changes in enamel color were evaluated on Baseline and Day 14. Microhardness were evaluated on Baseline, Days 7 and 14. All the bleached enamel specimens revealed increased whiteness without control group. Groups 2 and 3 showed significantly decreased enamel microhardness compared to control group. On Day 7, Groups 2-3 showed significantly decreased enamel microhardness compared to control group and respective baseline data. The percentage microhardness loss(PML) look at Day 7 and 14 for Group 1, respectively, there was little difference between 1.7 and 0.8. However, Group 2 was 21.9 and 3.5, Group 3 was 16.7 and 1.4 as a baseline and Day 7 were significantly different (p<0.05). The PML of group 2 was significantly highest than that of group 3 on Day 7. As a result, the data indicate that the addition of PF did not influence the whitening efficacy of the bleaching agent negatively. PF-containing bleaching agent reduce the percentage microhardness loss. PF-containing tooth bleaching your teeth with a whitening effect can be reduced by decreasing the hardness of enamel.
Among the effects of tooth bleaching on composite resins, degree of color change and dye deposition onto composite resins after office bleaching were investigated in vitro. Seventy two disc-shaped resin samples were fabricated with hybrid type composite resin, Z-100 under 3 different environments(24 samples for each environment) characterized by 3 kinds of light-curing intensity and duration($250mW/cm^2$-20sec., $250mW/cm^2$-40sec., $550mW/cm^2$-20sec.). As control, one-third samples of each group were not treated with bleaching agent. The remaining two-thirds samples of each group were treated with bleaching agent(bleaching group). Then, before thermocycling procedure in coffee bath, the half of the samples treated with bleaching agent were polished(polishing group) with polishing system. SofLex, but the other half(not-polishing group) and control group were not polished. Another 72 samples were also made with microfilled type composite resin. Sillux Plus and treated according to the experimental procedures mentioned above. The color of each resin sample was measured before bleaching, after bleaching, and after thermocycling preceded by bleaching. And color difference was evaluated. It was concluded as follows: 1. The amount of color change of resin samples after office bleaching was not statistically significant(p>0.05). But the samples which were treated with bleaching agent showed more color change than that of control group. 2. After thermocycling in coffee bath, the amount of color change of resin samples between control and bleaching group was not statistically significant(p>0.05). 3. After thermocycling in coffee bath, the polishing procedure of resin samples showed no statistically significant difference(p>0.05) between polishing and not-polishing group in the aspect of color change.
