A Case of Lingual Thyroglossal Duct Cyst Treated by CO2 Laser via Transoral Approach (경구강 CO2 레이저를 이용하여 치료한 설갑상설관 낭종 1예)

  • A lingual thyroglossal duct cyst(LTGDC) is a rare congenital anomaly that account for only 0.5% to 2% of total thyroglossal duct cyst. LTGDC is frequently associated with respiratory problem in infants and pharyngeal foreign body sensation or dysphagia in adults. Because of its location and characteristics, lingual thyroid, dermoid cyst, and vallecular cyst should be included in differential diagnosis. Standard treatment for thyroglossal duct cyst is sistrunk's operation, but in terms of LTGDC, because of its location and cosmetic reasons, different kinds of treatments such as electrical cauterization, $CO_2$ laser, Robort surgery via transoral approach have been introduced. Recently authors encountered 21 years old woman with LTGDC and the mass was removed successfully via transoral approach using $CO_2$ laser. We report the clinical course with review of the literature.

Results of Modified Sistrunk Operation in Thyroglossal Duct cyst : a Review of 81 Cases (소아에서 변형 Sistrunk술식을 적용한 갑상성 설관낭종의 치료성적)

  • Thyroglossal duct cyst is ectodermal remnant, which may develop along the line of descent of the thyroid gland from the foramen cecum of the tongue to the pyramidal lobe of the thyroid gland. Meticulous dissection of the cyst and duct, along with the body of the hyoid bone is necessary to avoid recurrence. Eighty-one patients with thyroglossal duct cyst treated at Hanyang University Hospital between January 1980 and December 2000 were reviewed to determine the incidence and to analyze the result of management. The male-to-female ratio was 1.4:1(47:34) with a male preponderance. They are most commonly present at 3-8years(54.2 %) of age, but rarely present at infancy. The most common symptom was a painless midline neck mass(76.5 %, 62cases). Eighty-one patients underwent modified Sistrunk operation without evidence of recurrence. Eight-nine percent(72 cases) of these lesions were located between thyroid substance and hyoid bone, and 11 %(9 cases) were above the hyoid bone. There were 22 infected cysts(27.2 %). The Sistrunk operation is a gold standard for treating the thyroglossal duct cysts. For best results in thyroglossal duct cyst surgery, one should make every effort to remove the cyst intact in continuity with the body of the hyoid bone. In our institute, Sistrunk operation modified by the authors showed a good result.

Grounded Theory Approach to the Procedure of International Students' Learning Korean (국제 유학생들의 한국어 학습과정에 대한 근거이론적 연구)

  • The purpose of this study was to figure out the procedure of learning Korean for international students. A research question was set up as follows: What is the procedure of leaning Korean for international students in Korean universities? To achieve the research purpose, this study implemented a method of semi-constructed interviews. Nineteen international students participated in the interview. The collected data for this study included transcripts from each interview. The transcripts of 60 minutes of interviews with all the participants was audio-taped recorded. This study investigated the research question based on the grounded theory. The analysis of open coding, axial-coding, and selective coding was used in the study. Results indicated that international students learned Korean in a daily basis, and then they adapted to academic Korean in their majored fields. Both personality and mother tongue influenced Korean language learning positively and negatively. International students' improvement of Korean was related in studying with Korean mass media such as TV soap dramas, talk shows, and songs. International students think that TOPIK(Test Of Proficiency In Korean) is not much related with their Korean language fluency. In conclusion, the researchers suggested to give more emphasis on academic training courses for Korean language and to improve the TOPIK in general academic Korean.

Analysis of Fine Needle Aspiration Cytology and Ultrasonography of Metastatic Tumors to the Thyroid (갑상샘 전이종양에 대한 세침흡인 세포 소견과 초음파 소견의 분석)

  • Cytologic diagnosis of the metastatic tumors to the thyroid is important in the management of the patients. There have been rare reports analyzing fine-needle aspiration (FNA) cytology of metastatic tumors to the thyroid. This study examines comprehensive cytologic findings of metastatic tumors to the thyroid with radiologic findings. The FNA cytology slides obtained from 12 cases with metastatic tumors of the thyroid; lung cancer (n=5), tongue and tonsil cancer (n=3), esophageal cancer (n=2), and breast cancer (n=2) were reviewed. Radiological study showed single mass with heterogeneous texture or multiple masses without calcification. Metastatic tumor was easily considered in a differential diagnosis of FNA cytology because they had peculiar cytological features which were not seen in primary thyroid tumor. The smear background varied from predominantly necrotic, bloody, and inflammatory to colloid. The aspirates exhibited a mixture of benign follicular cells and malignant cells in 6 cases. The characteristic cytoplasmic features of the tumor cells, such as keratin, mucin and melanin, were found in 9 cases. Although some cases mimic primary thyroid neoplasm, a careful examination of the cytological characteristics may help cytopathologists to recognize a metastatic tumor in the thyroid by FNA, and may help the clinicians to establish a proper treatment plan.

Effects of Facial Exercise for Facial Muscle Strengthening and Rejuvenation: Systematic Review

  • Purpose: The mass of facial muscles can be increased through exercise, as is also the case for muscles in the extremities. This systematic review was conducted to investigate the effect of facial exercises on facial muscle strengthening and facial rejuvenation, focusing on recent studies. Methods: A literature search was performed using the PubMed, ScienceDirect, and Web of Science databases. The quality of the trials was evaluated according to the PEDro scale. In total, 11 studies were included in this review: four studies on facial exercise for facial rejuvenation and seven studies on strengthening the muscles of the face. Results: Facial exercises for facial rejuvenation increased the mechanical properties and elasticity of the skin of the face and neck, the thickness and cross-sectional area of the facial muscles, and the fullness of the upper and lower cheeks. Conclusion: A study aimed at strengthening facial muscles showed improvements in labial closure strength and tongue elevation strength. Despite the positive results for facial rejuvenation and muscle strengthening, the level of evidence was low. Therefore, in future research, it will be necessary to investigate the effects of facial exercise in a thoroughly controlled experiment with a sufficient sample size to increase the level of evidence.

Prevalence of Human Papillomavirus Infection in the Korean Oral Cancer Patients (한국인 구강암 환자에서 인유두종 바이러스의 발현율 분석)

  • Purpose: High risk human papillomavirus (HPV) is the main source of cervical cancer, nearly up to 100%. Because there are no affordable data regarding the incidence of HPV in different sites of oral cavity, the purpose of this study is to review the previous article and to find out about the incidence and types of HPV in specific areas of oral cavity. Methods: We examined a total of 106 patients with oral cavity cancer for HPV detection. Available DNA chip (MY-HPV chip kit$^{(R)}$, Mygene Co., Korea) was used for the detection of low risk HPV types (6, 11, 34, 40, 42, 43, 44) and high risk HPV types (16, 18, 31, 33, 35, 39, 45, 51, 52, 54, 56, 58). Patients were previously diagnosed as invasive cancer of oral cavity. Specimen used for HPV detection was extracted from the main mass during resection and were tested for HPV type, performed by a DNA chip method. Results: Histologic results showed that there were squamous cell carcinomas, mucoepidermoid carcinomas, cystadenocarcinomas, leiomyosarcoma and etc. Among HPV positive cancer, 2 patients showed positive for high risk HPV type 16, 3 patients showed positive for low risk HPV type 6 and the other 2 patients were positive for other types of HPV. Of the 7 patients with HPV positive, 4 patients were on the tongue, 3 on the lower gingiva. Conclusion: Most of the HPV positive cancers were on the tongue in our study. Cervical cancer of HPV infection is reported to be increasing in our society and because HPV infection largely spreads by sexual contact, the incidence of oral cancer with HPV infection is greatly assumed to be on the increase, simultaneously. Because the incidence of HPV on specific areas of oral cavity among oral cancer patients is not identified, more studies are ongoing.

  • This study was analyzed the effects of blood-activating and stasis-eliminating herbs on anti-tumor and hematogenic metastasis. The metastasis and recurrence of tumor was the basis of yudok(yudu) on remained tumor cell and stagnation of blood, thermotoxo, phlegm, asthenia of healthy enerngy, stagnation of vital energy. Malignant tumor is caused by carcinogen and go through the progress of initiation, promotion, progression, it is closely related with Eohyul$(y{\grave{u}}xi {\breve{e}})$. Symptoms of blood stasis disease are purplish tongue, mass, fixed stabbing pain, ecchymosis of nail, hypodermic petechia, dermal thesaurismosis, melena, ecchymoma, disturbance of circulation. Effects on the therapy of activating blood circulation and congestion are anti-tumor, anti-coagulation, anti-hemolysis, anti-solution, anti-inflammation, anti-infection, control of blood circulations, control of connective tissue metabolism and control of immunity. They can directly kill the cancer cells entering the blood circulation, inhibit the formation of tumor embody and reduce the blood hyperviscosity. It is suggested that these herbs can be used to prevent and treat blood metastasis of cancer under the guidance of syndrome differentiation.

  • Granular cell tumors (GCT) are uncommon benign neoplasms. Their location is mostly in the the skin, tongue, and breast; appearance in other parts of the body is rare, but it has been reported. They have also been reported to occur synchronously in multiple organs and metachronously in a single organ. The incidence of GCTs in the tracheobronchial tree is unknown and pulmonary GCTs are uncommon, with approximately 100 reported cases in the literature. We present the case of a 33-year-old man with a granular cell tumor of the left main bronchus. The tumor was found at bronchoscopy performed to exclude suspected endobronchial mass with symptoms of pneumonia. Biopsies revealed the histological pattern of a benign granular cell tumor. He underwent resection of the left main bronchus followed by end to end anastomosis of left main bronchus. He has not had any recurrence of the tumor during the 1 year follow-up.

The cold water mass along the southeast and east coasts of Korea in 2016-2017

  • The spatial and temporal behaviors and fluctuations of the cold water that appeared in the South East Sea and the East Sea coast from 2016 to 2017 were investigated. The water temperature drop was large in the east coast from April to June and the southeast coast from July to September, and the temperature drop period was longer in the southeast coast. The water temperature fluctuated sensitively to the wind direction, and it gradually decreased in the southwest wind but rose as if jumping in the northeast wind. Wind stress and surface water temperature had an inverse correlation, which was larger in Bukhang-Idukseo, and decreased toward the north of Guryongpo. The cold water appeared mainly in Geojedo-Pohang after 1 to 2 days when the southwest wind was strong, but when the wind became weak, it shrank to the Idukseo (Ulgi-Gampo) and extended into the open sea in a tongue shape. Cold water was distributed only in Samcheok-Toseong in mid-May, Idukseo-Guryongpo and Hupo-Jukbyeon-Samcheok from late May to mid-July, and Bukhang-Idukseo in August-September. The intensity of cold water was greatest in mid-August, and the center of cold water descended from the east coast to the southeast coast from spring to summer. The water temperature fluctuation was dominant at the periods of 1 d and 7-21 d. In wavelet spectrum analysis of water temperature and wind, wind speed increase-water temperature decrease showed phase difference of 12 h in 2 d, 18 h in 3 d, 1.5 d in 4-8 d, and 2-3 d in 8-24 d period. The correlation between the two parameters was large in Geojedo and Namhang, Bukhang-Idukseo, Guryongpo-Jukbyeon, and Samcheok-Toseong. Monitoring stations with high correlation in all periods were generally parallel to the monsoon direction.

TREATMENT OF CAPILLARY HEMANGIOMA ON MAXILLARY BUCCAL MUCOSA : A CASE REPORT (상악 협측 치은에 발생한 모세혈관종 치험례)

  • The hemangioma, a benign proliferation of blood vessel, is the most common tumor of infancy and childhood. In many instances, the lesion probably represents a hamartoma or malformation rather than a true neoplasm. In the oral cavity, common sites are lips, followed by tongue, buccal mucosa and palate. Clinical characteristics appear as a flat or raised reddish-blue lesion and are generally solitary. They are classified on the basis of their histological appearance into capillary, mixed, cavernous or a sclerosing variety. A 6-year-old male of this case was referred to the Department of Pediatric Dentistry, Chonnam National University with a chief complaint of swelling lesion on gingiva. The strawberry appearance mass was detected by clinical examination on attached gingiva at the upper left primary lateral incisor and canine. Surgical excision and biopsy were carried out for histological examination and the lesion was diagnosed with a capillary hemangioma. The risk of recurrence after this therapy is rare, and there is no malignant transformation. Despite their benign origins and behaviour, hemangiomas in the region of oral cavity are always clinically important to the dental profession because of bleeding tendency.
