A case of de novo duplication of 15q24-q26.3

  • Distal duplication, or trisomy 15q, is an extremely rare chromosomal disorder characterized by prenatal and postnatal overgrowth, mental retardation, and craniofacial malformations. Additional abnormalities typically include an unusually short neck, malformations of the fingers and toes, scoliosis and skeletal malformations, genital abnormalities, particularly in affected males, and, in some cases, cardiac defects. The range and severity of symptoms and physical findings may vary from case to case, depending upon the length and location of the duplicated portion of chromosome 15q. Most reported cases of duplication of the long arm of chromosome 15 frequently have more than one segmental imbalance resulting from unbalanced translocations involving chromosome 15 and deletions in another chromosome, as well as other structural chromosomal abnormalities. We report a female newborn with a de novo duplication, 15q24- q26.3, showing intrauterine overgrowth, a narrow asymmetric face with down-slanting palpebral fissures, a large, prominent nose, and micrognathia, arachnodactyly, camptodactyly, congenital heart disease, hydronephrosis, and hydroureter. Chromosomal analysis showed a 46,XX,inv(9)(p12q13),dup(15)(q24q26.3). Array comparative genomic hybridization analysis revealed a gain of 42 clones on 15q24-q26.3. This case represents the only reported patient with a de novo 15q24-q26.3 duplication that did not result from an unbalanced translocation and did not have a concomitant monosomic component in Korea.

무지 외반증을 동반한 중족골통에 대한 Weil씨 절골술 (Weil osteotomy for metatarsalgia of lesser toes with hallux valgus)

  • Introduction: Painful plantar callosities under the second and third metatarsal heads are not uncommonly associated with hallux valgus and there have been controversies concerning its treatment modalities. We performed the Weil osteotomy in patients with painful callosities under the second and third metatarsal head associated with hallux valgus, and evaluated the outcome clinically & radiologically. Materials and Methods: From November 2000 through November 2001, out of 51 patients diagnosed with hallux valgus, the Weil osteotomy was performed in 5 women associated with painful callosities under the second and third metatarsal heads. The mean age was 53.4 years (range, 31-66) with a minimum follow-up period of 12 months(range, 12-16). Results: The mean AOFAS lessor metatarso-phalangeal-interphalngeal Scale(LMIS) improved from 61.2 points preoperatively to 88.8 points postopertively. Mean decreases in length of the second and third metatarsal after surgery were 3.2mm and 3mm. Conclusion: We experienced improvement of metatarsalgia in 80% of patients by Weil osteotomy and all patients were symptom-free at last follow-up, suggesting that the Weil osteotomy is an effective procedure in treating painful plantar callosities under the second and third metatarsal heads associated with hallux valgus.

V-M 성형술을 이용한 지간구축 및 합지증의 교정 임상례 (Clinical Experiences about Correction of Web Space Contracture and Syndactyly using V-M Plasty)

  • Purpose: The loss of web space is caused by congenital syndactyly or acquired burn injury, trauma or surgery. Numerous surgical procedures have been described for restoration of the web space. Local flaps are usually preferred because of the easiness to perform and tolerable postoperative outcome. Among the various local flaps, the authors introduce V-M plasty for correction of web space contracture and syndactyly. Method: From March 2007 to Jun 2008, 4 patients underwent V-M plasty for correction of web space contracture and syndactyly. V-M plasty consists of 3 distinct triangular flaps. One triangular flap is designed next to the web region on the dorsal site of the hand, whereas the remaining 2 triangular flaps are placed on the volar site. The dorsal triangular flap is then placed between the volar adjacent triangular flaps. At the end of the operation, the involved fingers or toes are positioned in abduction to avoid kinking of the triangular flaps. Result: All the patients gained web functions with good esthetic appearance without any recurrence or complications. Mean follow-up was 8 months. Conclusion: V-M plasty is a safe, easy and rapid procedure to design and apply by using local tissues without the needs for a skin graft or risk of linear scarring and recurrence. The authors advise this versatile technique both in primary and recurrent cases of web space contracture and syndactyly.

청년층과 노년층 여성의 신발 착용 실태 및 선호도에 관한 비교 연구 (A Comparative Study on the Footwear Wearing Practices and Preferences of Young and Elderly Women)

  • This study is to help produce highly comfortable and wearable and footwear based on the establishment of basic data for manufacturing elderly footwear patterns and products. For this, a survey of 207 women in their 20s and 60-70s was conducted to identify the wearing of shoes, inconvenience areas, purchase practices and footwear preferences by age group. The results are as follows. First, the average age of the young female group was 22.5 years old, the elderly female group was 68.8 years old, and the average shoe size was 236.3 mm for the young female group and 238.3 mm for the elderly female group. Second, young women wore shoes for a longer time than elderly women, resulting in swollen and numb legs, hardened soles, uncomfortable toes and sore heels. However, elderly women felt more uncomfortable wearing footwear than young women. They suffered pain in the joint area, heal and toe areas as well as had more toe deformations due to aging. Third, all age groups preferred athletic shoes and loafers as well as purchases from footwear brand stores and department stores. The elderly spent more money on shoes, longer time to buy and valued the feeling of wearing, whereas the young considered design first. Fourth, both age groups prefer low-heels and natural leather, the young preferred to wear various types of shoes; however, the elderly are found to desire shoes that are comfortable and stable in both form and wearing.

기능성 신발 쿨핀(Coolfin) 착용이 발등 및 엄지발가락 혈액순환에 미치는 영향 (The Effect of Functional Shoes (Coolfin) on Top of Foot and Great Toe Blood Circulation)

  • Purpose: This study aims to investigate the impact of a short walk putting on Coolfin shoes on the foot temperature rise and blood circulation. Methods: Twenty healthy adults participated in experiments in this study. All subjects walked respectively barefoot and putting on Coolfin shoes. Infrared imaging cameras were used to collect and analyze the data on the foot temperature. Results: As a result of this study, there were significant differences between the temperatures of the great toe and top of the foot after 20-minute walks with bare feet and in Coolfin shoes. Both the temperatures of the great toe and top of the foot decreased after a 20-minute walk in the barefoot condition, and there was a significant difference between them (p<0.05). Both the temperatures of the great toe and top of the foot increased after a 20-minute walk in the condition of putting on Coolfin shoes, and there was a significant difference between them(p<0.05). Conclusion: A short walk putting on Coolfin products induces the movement of the toes to increase the blood flow and accordingly, has a direct impact on the increase of the surface temperature of the feet, so they are effective products for helping blood circulation in the feet.

블라스트 표면처리가 하중비전달형 십자필렛 용접이음의 피로거동에 미치는 영향 (Effect of Blast Cleaning on Fatigue Behavior of Non-load-carrying Fillet Welded Cruciform Joints)

  • 강교 제작에는 강재 표면의 이물질 제거와 피복방식재료의 부착성 증대를 위하여 블라스트 표면처리가 실시되고 있다. 블라스트 표면처리는 쇼트나 그릿 등의 연마재를 압축공기로 분사하여 강재 표면에 충격을 가하는 표면처리법으로, 블라스트 처리에 의해 표면형상이 개선되고 압축잔류응력이 도입되어 용접이음의 피로수명이 향상될 것으로 예상된다. 본 연구에서는 하중비전달형 십자필렛 용접이음의 피로실험을 실시하여, 블라스트 표면처리가 용접이음의 피로거동에 미치는 영향을 실험적으로 검토하였다. 피로실험에는 용접 후 무처리 시험편(용접 그대로), 용접 후 블라스트 표면처리 한 시험편과 블라스트 처리 후 열처리에 의해 잔류응력을 제거한 시험편의 합계 3종류의 용접시험편을 대상으로 하였다. 그 결과 블라스트 표면처리에 의해 용접지단부의 곡률반경은 약 29% 증가하였으며, 용접지단부의 인장잔류응력이 제거되고 압축잔류응력이 도입되었다. 그리고 블라스트 처리에 의해 피로수명과 피로한계가 증가하였다. 피로수명은 응력범위가 낮을수록 더 크게 증가하였고, 피로한계는 약 1.5배 증가하였다.

  • 수제군 주변의 유동장 및 지형학적 변화에 대한 수리학적 효과를 분석하기 위하여, 본 연구는 영천강 만곡부에 설치된 저수 수제군 주변의 하도지형 특성의 변화에 대하여 모니터링과 수치모의를 수행하였다. 연구 대상지는 연장 190m로서 2008년 3월에 저수 수제군이 설치되었다. 현장 모니터링을 2008년 5월부터 2014년 4월까지 수행하였다. 설계된 수제 높이 보다 홍수시 수위가 더 높아져도 수제는 하안으로부터 흐름을 하도의 중심으로 밀어낸다. 따라서, 만곡부의 하도 안정을 위하여 설치된 수제군은 어느 정도 효과적으로 수리적 기능을 수행하고 있다. 시간의 경과에 따라 주수로가 하도의 중심으로 이동하면서 안정되었으며, 국소세굴에 의하여 수제 선단부 주변에 소가 형성되었다. 또한, 수제 사이에는 유사가 퇴적되었고, 하도에는 여울-소가 형성됨으로써 부수적으로 수제가 하도의 지형적 측면에서 다양한 물리적 특성을 창출함을 확인하였다.

족양명위경 하지부 유주에 관한 고찰 - 『중국침구경락통감』을 중심으로- (A Study on the Courses of Lower Limb Region of Stomach Meridian -Focused on Categorized Collection of Literatures on Chinese Meridians and Collaterals-)

  • Objectives : Most meridians have each well point located at the distal end of fingers or toes, congruous with their meridian courses. Exceptively, ST45(Yeotae), stomach meridian(ST)'s final point, is located at the lateral side of the tip of the second toe, while the tip of the second toe was not mentioned in the course of ST. So, we studied the course of lower limb region of ST. Methods : Courses of ST in Categorized Collection of Literatures on Chinese Meridians and Collaterals(CCM) was examined. The literatures included in CCM was selected as study subjects to be studied if the meridian route was different from "Youngchu" or more detailed notes were attached. There are 3 branches in lower limb region of ST, we analyzed theories of scholars of all time about the 3 branches of ST. Results and Conclusions : Branch 1 descends along the lateral margin of the fibula to the dorsum of foot, entering into the medial side of the middle toe. Branch 2 leaves ST36 separately, terminating at the lateral side of the middle toe. Branch 3 leaves ST42 and descends to the end of the great toe. The medial side of the middle toe equate to the lateral side of the second toe, and it is connected to ST45. So branch 1 is the main stream of ST. Branch 3 drives to the great toe passing between first and second toe, it goes medial side of LR2.

골프 클럽에 따른 타격자세의 변화 (Changes of Impact Variables by the Change of Golf Club Length)

  • To know the proper impact posture and changes for the various clubs, changes of impact variables according to the change of golf club length was investigated. Swing motions of three male low handicappers including a professional were taken using two high-speed video cameras. Four clubs iron 7, iron 5, iron 3 and driver (wood 1) were selected for this experiment. Three dimensional motion analysis techniques were used to get the kinematical variables. Mathcad and Kwon3D motion analysis program were used to analyze the position, distance and angle data in three dimensions. Major findings of this study were as follows. 1. Lateral position of the head remained more right side of the target up to 3.5cm compared to the setup as the length of the club increased. 2. Left shoulder raised up to 5cm and right shoulder lowered up to 2.5cm compared to setup. The shoulder line opened slightly (maximum 11 degrees) to the target line. 3. Forward lean angle of the trunk decreased up to 4 degrees (more erected) compared to setup. 4. Side lean angle of the trunk increased compared to setup and increased up to 16 degrees as the club length increased. 5. The pelvis moved to the target line direction horizontally and opened up to 31 degrees. Right hip moves laterally to the grip position at the setup. 6. Flexion of the left leg maintained almost constantly but the right leg flexed up to 11 degrees compared to setup. 7. Left arm is straightened but the right arm flexed about 20degrees compared to straight. 8. Center of the shoulders were in front of the knees and toes of the feet. 9. Hands moved to the left (8.7cm), forward (5.7cm) and upward (11.6cm) compared to the setup. This is because of the rotation of pelvis and shoulders. 10. Shaft angle to the ground was smaller than the lie angle of the clubs but it increased close to the lie of the clubs at impact.
