Public Health Challenges of Electronic Cigarettes in South Korea

  • Electronic cigarettes (e-cigarrettes) were recently introduced and advertised as a smoking cession device in South Korea. As the social norm to quit smoking has gained hold in the country, the number of e-cigarette users is growing rapidly. This phenomenon should be urgently considered, because of the lack of research that has been conducted to examine the safety of e-cigarettes and its efficacy as a smoking cessation aid. This paper raises several public health concerns on e-cigarettes in South Korea. Uncertain regulations of the government on e-cigarettes are contributing to an increase of e-cigarette users and allowing the e-cigarette industry to circumvent existing regulations. The aggressive marketing activity of this industry is also a core factor that is responsible for the rapid increase of e-cigarette use, in particular among the youth. Following the enforcement of tobacco control, some cigarette smokers may be encouraged to purchase e-cigarettes in order to circumvent the regulations, even though the dual use of e-cigarette and cigarette may be more harmful. Until there is clear evidence of the e-cigarette’s safety, it is recommended that the industry’s marketing and promotional activities be banned and closely monitored, and public campaigns be initiated to educate the public regarding e-cigarettes.

  • Transfer of nicotine into the mails stream of smote in Korean commercial cigarettes was investigated. The mass transfer of nicotine into tile main stream of smote and accumulation of nicotine in cigarette filler got higher as the puffing number increased. Only about 14oyo of nicotine ill a cigarette was puffed into mouth end. and the rest of the nicotine went to side stream of smote or converted to other volatile bases.

멘톨담배의 $\ell$-Menthol 이행에 관한 연구 (Studies on the §¤-menthol delivery in mentholated cigarette smoke)

  • A study was made of the puff-by-puff delivery of menthol and tar for several commercial brands. The purpose of this study is for the brands constantly to maintain fresh aroma of menthol till the last puff in smoking. The results from this study could be used for development and evaluation of a mentholated cigarette. Prior to the purpose, a straightforward method from extraction through quantitative analysis of 9-menthol contained in cigarettes has been developed.

오리엔트종 잎담배의 화학성분에 의한 관능 특성 예측 (Prediction of Sensory Characteristics from Leaf Chemical Compounds in Oriental Tobacco)

  • This study was conducted to evaluate the prediction of sensory characteristics of smoke from the leaf chemical compounds and characterize leaf chemical components for the best tobacco taste's leaves in oriental tobacco. For analytical and sensory evaluations, seventy two grades were used. Sensory evaluation of tobacco smoke for six attributes were scored on fifteen-point scale by $10{\sim}14$ expert panels trained to estimate smoking quality quantitatively. The major leaf chemical compounds to predict the sensory characteristics of smoke were ether extract for tobacco-like, nicotine for impact and total nitrogen/nicotine ratio for irritation, and total sugar for off taste & odor. Within ${\pm}20%$ range of difference, the predictable probabilities of sensory characteristics of smoke from the leaf chemical compounds were 87.5 % for off taste & odor and $94.4{\sim}98.6\;%$ for tobacco-like, impact and irritation. As a result of K-means cluster analysis on the basis of tobacco taste, the desirable leaf chemical compound contents were $5.9{\sim}8.3\;%$ in ether extract, $1.35{\sim}2.27\;%$ in nicotine and $1.17{\sim}2.24$ in total nitrogen/nicotine ratio. This study suggest that the some regression equations may be useful to predict the sensory characteristics of tobacco smoke with a few selected leaf chemical compounds in oriental tobacco and to select the oriental tobacco leaves by means of enhancing the tobacco taste of cigarette.

흡연예방을 위한 청소년 교육방향 -교육용 비디오 효과분석율 중심으로- (Development of a Smoking Prevention Video for Youth)

  • The National Health Promotion Law passed in 1995 was a milestone for initiating a national health promotion program in Korea, and local governments and health related organizations are developing and providing health promotion programs for the public. The purpose of this study was to explore the effectiveness of a smoking prevention video for adolescents. There was no educational video on smoking prevention for school students in Korea. Based on funding from the National Health Promotion Fund, an educational video on smoking prevention has been developed and tested for the effectiveness. The primary subject of the video was middle school students. The result of the study has shown that there was a statistically significant difference between pre-and post test among middle school students and elementary school students. Students' knowledge on the harm of smoking has been increased after the video education. In addition, more than half of the students responded that the educational video was interesting and helpful. To reduce initiation of smoking among adolescents, both policies and school health education programs should be reinforced. Policies on tobacco should focus on demand of smokers and potential smokers. So government officials should keep monitor obedience of the law on prohibition of smoking for those under age 19. School teachers should focus on social influence of smoking for adolescents.

청소년 흡연자의 전자담배 사용 빈도와 흡연 관련 특성 간의 연관성 (Associations between the Frequency of Electronic Cigarette Use and Smoking-related Characteristics among Adolescent Smokers)

  • Purpose: We aimed to identify associations between the frequency of electronic cigarette (e-cigarette) use and smoking-related characteristics among adolescent smokers. Methods: We used data from the 14th Korea Youth Risk Behavior Survey. The participants were 3,722 smokers who had smoked cigarettes on at least one day during the past 30 days. The frequency of e-cigarette use was defined as the number of days that participants used e-cigarettes during the past 30 days, and was divided into four categories: 0, 1~2, 3~9, and ${\geq}10$ days. We used the $x^2$ test and multinomial logistic regression to examine the associations between the frequency of e-cigarette use and smoking-related characteristics. Results: Those who attempted to quit smoking, had used a heated tobacco product (HTP) at least once, or initiated e-cigarette use before middle school were more likely to use e-cigarettes for 3~9 days in the past 30 days. More frequent e-cigarette use (${\geq}10$ days) was associated with a higher frequency and greater amount of cigarette use, lifetime HTP use, and earlier initiation of e-cigarette use. Conclusion: Smoking-related characteristics differed by the frequency of e-cigarette use. Therefore, intervention programs targeting adolescent smokers should consider differences in smoking-related characteristics based on the frequency of e-cigarette use.

보건학 전공 대학생 대상 금연교육 프로그램 개발 (Development of Smoking Cessation Education Program for University Students Majoring in Health Sciences)

  • Objectives: This study aimed to develop a smoking cessation education program and to provide basic data for evaluating program for university students majoring in health sciences. Methods: The education program was developed based on the instructional system design model(ADDIE), that was based on literature review, brainstorming, and interviews of related experts. Education program was implemented for 2 sessions of 3 hours to the 82 university students majoring in health science. Knowledge, competency, and self-efficacy for performance of smoking cessation were analyzed. One group pre- & post-test design was used for evaluation of this program. Results: In the analysis stage, a total of 5 duties, 16 tasks, and 55 task elements were derived. In the design and development stage, based on job analysis, education program contents were composed of understanding tobacco, planning and implementation of smoking cessation program, smoking cessation counseling and drug treatment. After this education program, students achieved remarkable improvement in increasing knowledge, competency, and self-efficacy for smoking cessation counselling and program. Conclusions: It is needed to be included the smoking cessation education in department related to health science regular course.

Tobacco Use Increases Oxidative DNA Damage in Sperm - Possible Etiology of Childhood Cancer

  • Background: Cigarette smoking and tobacco chewing are common modes of consuming tobacco all over the world. Parents need to be aware that germ cell integrity is vital for birth of healthy offspring as biological parenting begins much before birth of a child and even before conception. The present study was conducted to determine the etiology of non-familial sporadic heritable retinoblastoma (NFSHRb), by evaluating oxidative sperm DNA damage in fathers due to use of tobacco (smoking and chewing). Materials and Methods: We recruited 145 fathers of NFSHRb children and 53 fathers of healthy children (controls) in the study. Tobacco history was obtained by personal interview. Seminal reactive oxygen species (ROS) in semen, sperm DNA fragmentation index (DFI) and 8 hydroxy 2' deoxyguanosine (8-OHdG) levels in sperm were evaluated. The RB1 gene was screened in genomic blood DNA of parents of children with NFSHRb and controls. Odds ratios (ORs) derived from conditional logistic regression models. Results: There was significant difference in the levels of ROS (p<0.05), DFI (p<0.05) and 8-OHdG (p<0.05) between tobacco users and non-users. The OR of NFSHRb for smokers was 7.29 (95%CI 2.9-34.5, p<0.01), for tobacco chewers 4.75 (2.07-10.9, p<0.05) and for both 9.11 (3.79-39.2; p<0.01). Conclusions: This study emphasizes the adverse effect of tobacco on the paternal genome and how accumulation of oxidative damage in sperm DNA may contribute to the etiology of NFSHRb. In an ongoing parallel study in our laboratory, 11 of fathers who smoked underwent. Meditation and yoga interventions, showed significant decline in levels of highly mutagenic oxidised DNA adducts after 6 months. Thus our lifestyle and social habits impact sperm DNA integrity and simple interventions like yoga and meditation are therapeutic for oxidative damage to sperm DNA.

흡연자와 비흡연자의 권리보호를 위한 공동주택 내의 흡연 공간 계획에 관한 연구 - 동 평면의 샤프트 공간을 활용하여 - (A Study on the Planning of Smoking Space in Apartment for the Rights of Smokers and Nonsmokers - Using the shaft space of the unit plan -)

  • Through various media, damage of both smoking and second-hand smoking has been recognized, and brought global scale of interest in antismoking. In Korea, government has tightened regulations of smoking in non-smoking zone since December, 1980, and after National Health Promotion Act in 1995, non-smoking zone has been gradually expanded. On the other hand, there were law suits to find those regulation towards smokers are either unconstitutional or not for 4 times. In this current state, people need smoking area to prevent second-hand smoking and to consider smokers in multi-unit dwelling. Main purpose of this research is to plan smoking spaces based on various typology of multi-dwelling plan for protection of both smokers and non-smokers' right. The research group collected and analyzed the smoking behaviors in various multi-unit dwelling types such as flat type, tower type, hybrid type and others. Based on those data, the group found three phenomena. First, there are internal regulations in multi-unit dwelling to make non-smoking zone based on National Health Promotion Act and resident representative meeting decision. Second, main smoking activities are occurring at major traffic line and entrances. Third, smoking inside of multi-unit dwelling complex causes second-hand smoking to residents live in $1^{st}$ floor and when they enter. Therefore, one can achieve both smokers' and non-smokers' protection of right by creating a designated smoking space near main entrances of multi-unit dwelling complex to consider smokers' and prevents second-hand smoking by using shaft space, which is in core space, to ventilate tobacco smoke through roof.

실내환경의 환경성담배연기(ETS) 중 카드뮴에 관한 연구 (Cadmium Concentrations in Environmental Tobacco Smoke of Indoor Environments)

  • 실내 사무환경에 대해 흡연환경에 따라 흡연실, 흡연 허용 사무실, 비흡연사무실, 흡연실 인접 지역 등을 대상으로 ETS 중 발암물질인 카드뮴을 포함한 금속류, 니코틴, 3-EP, 호흡성먼지등 지표물질의 농도분포와 이의 분포에 영향을 주는 흡연밀도, 흡연지수 등을 조사하여 그 상관성을 파악하였다. 공기교환횟수는 흡연실과 흡연허용사무실에서 각각 평균 11.1, 4.6 회/시간을 나타냈으며, 흡연밀도는 최고 $2.6cig/m^2{\cdot}hr$에서부터 최저 $0.2cig/m^2{\cdot}hr$로 나타났다. 금속류 중 카드뮴은 기하정규분포를 보였으며 흡연환경별 기하평균 농도가 흡연실에서 $0.045{\mu}g/m^3$, 흡연실 인근 지역에서 $0.018{\mu}g/m^3$, 흡연허용사무실에서 $0.021{\mu}g/m^3$, 비흡연사무실에서는 $0.017{\mu}g/m^3$의 분포를 보였으며 통계적으로는 유의한 차이를 나타냈다(p<0.05). 실내 흡연 환경에 따른 카드뮴의 농도 비를 조사한 결과 비흡연실 기준으로 흡연실에서 2.6배, 흡연허용사무실에서 1.2배의 농도 분포를 보였다. 카드뮴과 지표물질간 상관성을 조사한 결과 니코틴과의 상관계수가 0.53 (p<0.001)으로 가장 높게 나타났으며 호흡성먼지와는 0.43, 3-EP와는 가장 낮은 0.41의 상관계수 값을 보였으며, 흡연 밀도와는 0.54 (P<0.001), 흡연지수와는 0.63(p<0.001)의 상관계수 값을 보였다. 이러한 연구 결과를 토대로 ETS 중 중금속류에서는 카드뮴을 지표물질로 활용할 수 있다고 판단할 수 있다.
