Relationship between Psychological Factors and Obesity before and after Obesity Treatment in Korean Obese Women (한국 비만 여성에서 비만 치료 전 후의 심리적 상관관계 분석)

  • Objectives : Psychological comorbidities are high in patients with obesity and are associated with a variety of medical and dietary problems. This study aims to examine the association between psychological factors and obesity. Methods : This study was performed in pre-menopausal obese($BMI{\geq}25kg/m^2$, waist circumference ${\geq}85cm$) women in Seoul, in 2008 (n=35). Every patient underwent the obesity treatment program. The program included dietary and exercise education, and abdominal mesotherapy for 6 weeks. Simple anthropometry including weight, BMI and Computed Tomography (CT) including Subcutaneous adipose tissue (SAT), Visceral adipose tissue (VAT) and Visceral adipose tissue/Subcutaneous adipose tissue ratio (VSR) were done. To assess psychological factors, the Rosenberg self-esteem scale (SES) questionnaire, Beck depression inventory (BDI) questionnaire and stress response inventory(SRI) questionnaire were administered. Results : 1. All of the obesity indicators (except VSR) decreased significantly after the obesity treatment program. 2. There was a significant relationship between self esteem (SES score) and visceral obesity (VAT and VSR) measured at the end of the program. 3. During the intervention, the more weight, BMI, and subcutaneous adipose tissue decreased, the more self-esteem (SES) increased. There was no relationship between depression (BDI) and obesity. And the change in stress response (SRI) was associated with the change of deep subcutaneous adipose tissue and total abdominal adipose tissue. Conclusions : This study proves that visceral obesity may contribute to low self-esteem, and there is a possibility that the other psychological factors could also be related with obesity in Korean obese women. Individualised antiobesity therapy may be required depending on the patient's psychological characteristics and weight loss could be helpful in order to treat psychological problem in obese patients.

Selection of oncoplastic surgical technique in Asian breast cancer patients

  • Background Oncoplastic surgery is being increasingly performed in Korean women; however, unlike Westerners, Korean women usually have small to moderate-sized breasts. To achieve better outcomes in reconstructed breasts, several factors should be considered to determine the optimal surgical method. Methods A total of 108 patients who underwent oncoplastic surgery from January 2013 to December 2016 were retrospectively investigated. We used various methods, including glandular tissue reshaping, latissimus dorsi (LD) flap transposition, and reduction oncoplasty, to restore the breast volume and symmetry. Results The mean weight of the tumor specimens was 40.46 g, and the ratio of the tumor specimen weight to breast volume was 0.12 g/mL in the patients who underwent glandular tissue reshaping (n=59). The corresponding values were 101.47 g and 0.14 g/mL, respectively, in the patients who underwent reduction oncoplasty (n=17), and 82.54 g and 0.20 g/mL, respectively, in those treated with an LD flap (n=32). Glandular tissue reshaping was mostly performed in the upper outer quadrant, and LD flap transposition was mostly performed in the lower inner quadrant. No major complications were noted. Most patients were satisfied with the aesthetic results. Conclusions We report satisfactory outcomes of oncoplastic surgical procedures in Korean patients. The results regarding specimen weight and the tumor-to-breast ratio of Asian patients will be a helpful reference point for determining the most appropriate oncoplastic surgical technique.

THE EFFECT OF SOLVENT ACTION OF SODIUM HYPOCHLORITE SOLUTION ON PULP TISSUE (차아염소산(次亞鹽素酸)나트륨용액(溶液)의 치수조직(齒髓組織)에 대(對)한 용해효과(溶解效果))

  • Sodium hypochlorite solution has been widely used as endodontic irrigant due to its ability to dissolve pulp tissue debris and its antimicrobial action. This in vitro study was conducted to evaluate the solvent action of sodium hypochlorite solution on vital pulp tissue under various conditions include concentration, exposure time, and temperature. The percentage of weight loss due to pulp tissue dissolution was calculated with weight difference of lyophilized specimens before and after the exposure to test solutions. The results were as follows; Statistical analysis indicated that the ability of both 5.0% and 2.5% sodium hypochlorite solutions to dissolve pulp tissue was significantly greater than that of distilled water, but no significant difference was found between 5.0% and 2.5% sodium hypochlorite solutions. There was no significant increase in the pulp tissue dissolving ability of sodium hypochlorite solutions; as exposure time increased 2 minutes, 5 minutes, and 10 minutes. Of the given temperatures, no significant difference was found in the solvent aciton of sodium hypochlorite solution on pulp tissue between $20^{\circ}C$ (room temperature) and $37^{\circ}C$(body temperature).

Study on the Expression Dehydration by Recycling of Paper Sludge (제지 슬러지 재활용에 의한 압착 탈수)

  • Expression-deliquoring tests using three types of paper sludges (tissue, newsprint, and paperboard) as deliquoring agent were carried out in order to investigate the effects of sludge dosage and pressure on the expression-deliquoring of slurry. The addition of deli­quoring agent causes expression-deliquoring of slurry to be faster than would be the case without the deliquoring agent. In case of the tissue sludge, the highest compression rate was achieved when $5\%$ of deliquoring agent was added, while in cases of the news­print and the paperboard sludge, $7\%$. Compression rate was increased as pressure increases. Porosity was decreased as pressure increases. The lowest porosity was observed when $5\%$ of tissue sludge was added. When compared the weight of cake where deliquoring agent was not added and the weight of cake that was fastest expression-deliquoring, there was about $17.5\%$ of the weight reduction

Weight analysis of mastectomy specimens and abdominal flaps used for breast reconstruction in Koreans

  • Background Slim patients or those with large breasts may be ineligible for breast reconstruction with an abdominal flap, as the volume of the flap may be insufficient. This study aimed to establish that abdominal tissue-based breast reconstruction can be well suited for Korean patients, despite their thin body habitus. Methods A total of 252 patients who underwent postmastectomy breast reconstruction with an abdominal flap from October 2006 to May 2013 were retrospectively reviewed. The patients' age and body mass index were analyzed, and a correlation analysis was performed between the weight of the mastectomy specimen and that of the initial abdominal flap. Results The average weights of the mastectomy specimen and initial abdominal flap were 451.03 g and 644.95 g, respectively. The ratio of the weight of the mastectomy specimen to that of the initial flap was $0.71{\pm}0.23$. There was a strong positive linear relationship between the weight of the mastectomy specimen and that of the initial flap (Pearson correlation coefficient, 0.728). Thirty nulliparous patients had a final-to-initial flap weight ratio of $0.66{\pm}0.11$. The 25 patients who underwent a contralateral procedure had a ratio of $0.96{\pm}0.30$. The adjusted ratio of the final flap weight to the initial flap weight was $0.66{\pm}0.12$. Conclusions Breast weight had a strong positive relationship with abdominal flap weight in Koreans. Abdominal flaps provided sufficient soft tissue for breast reconstruction in most Korean patients, including nulliparous patients. However, when the mastectomy weight is estimated to be >700 g, a contralateral reduction procedure may be considered.

Anti-obesity Effect of Jeoreongchajeonja-tang in a High-fat Diet-induced Obesity Mouse model (고지방식이로 유도된 비만 생쥐모델에서 저령차전자탕의 항비만 효과)

  • Objective This study investigated the effects of Jeoreongchajeonja-tang in a high-fat diet-induced obesity mouse model. Methods The study examined 9-week-old male mice (C57bl/6J) divided into four groups: the normal(C57bl/6J-Nr), control (high-fat diet only; HFD-CTL), positive-control (high-fat diet with Garcinia cambogia), and experimental (high-fat diet with Jeoreongchajeonja-tang; HFD-JCT) groups. After 7 weeks, the body weight, food efficiency ratio, organ weight, and visceral fat weight of the mice were measured. Blood serum tests, mRNA, liver histopathology, and epididymis adipocytes were also examined. Results Compared with the Control(HFD-CTL) group, the Experimental(HFD-JCT) group given Jeoreongchajeonja-tang showed significant reductions in absolute body weight and food efficiency ratio. The serum alanine aminotransferase, total-cholesterol, triglyceride, low-density lipoprotein (LDL)-cholesterol, high-density lipoprotein (HDL)-cholesterol, insulin-like growth factor-1, and leptin levels were significantly lower in the experimental group than in the control group. The serum adiponectin levels were significantly higher in the experimental group than in the control group. Compared with the control group, the experimental group showed significant reductions in absolute abdominal subcutaneous fat, epididymal adipose tissue, kidney adipose tissue, intestine adipose tissue, and liver, kidney and spleen adipose tissue weights. The C/EBP-${\beta}$, leptin, and SREBP1c/ADD1 mRNA expression were significantly lower in the experimental group than in the control group, while the UCP-2 and adiponectin mRNA expression were significantly higher. Compared with the control group, the experimental group showed a significant reduction in the absolute adipocyte area in the liver and epididymal adipose tissue. Conclusion Jeoreongchajeonja-tang has an anti-obesity effect. Additional clinical studies are expected.

Analysis of factors that affect drainage volume after expander-based breast reconstruction

  • Background Closed-suction drains are widely used in expander-based breast reconstruction. These drains are typically removed using a volume-based criterion. The drainage volume affects the hospital stay length and the recovery time. However, few studies have analyzed the factors that influence drainage volume after expander-based breast reconstruction. Methods We retrospectively analyzed data regarding daily drainage from patients who underwent expander-based breast reconstruction between April 2014 and January 2018 (159 patients, 176 expanders). Patient and operative factors were analyzed regarding their influence on total drainage volume and drain placement duration using univariate and multivariate analyses and analysis of variance. Results The mean total drainage volume was 1,210.77±611.44 mL. Univariate analysis showed correlations between total drainage volume and age (B=19.825, P<0.001), body weight (B=17.758, P<0.001), body mass index (B=51.817, P<0.001), and specimen weight (B=1.590, P<0.001). Diabetes history (P<0.001), expander type (P<0.001), and the surgical instrument used (P<0.001) also strongly influenced total drainage. The acellular dermal matrix type used did not affect total drainage (P=0.626). In the multivariate analysis, age (B=11.907, P=0.004), specimen weight (B=0.927, P<0.001), and expander type (B=593.728, P<0.001) were significant predictors of total drainage. Conclusions Our findings suggest that the total drainage and the duration of drain placement needed after expander-based breast reconstruction can be predicted using preoperative and intraoperative data. Patient age, specimen weight, and expander type are important predictors of drainage volume. Older patients, heavier specimens, and use of the Mentor rather than the Allergan expander corresponded to a greater total drainage volume and a longer duration of drain placement.

Insulin-Like Growth Factors-1 Receptor (IGF-1R) Expression and the Phosphorylation of Endogenous Substrates Lead to Maturation of the Pacific oyster, Crassostrea gigas

  • This study investigated the IGF-1 signal in specific tissues using Pacific oysters artificially matured via water temperature elevation. Pacific oysters were subjected to water temperature elevation from March to June, and 20 were randomly sampled each month. The condition index (CI) and tissue weight rate (TWR) were examined by measuring shell length, shell height, shell width, and soft tissue weight. The IGF-1 signal in tissues (adductor muscle, digestive glands, gills, labial palps, mantle edges, and gonads) was analyzed by sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and Western blotting. From April to June, the TWR of females and males increased from 19.1±2.9 to 21.0±3.6 and 18.2±2.0 to 19.2±2.5, respectively, while the CI remained the same. The IGF-1 signal in each tissue differed. IGF-1 was expressed in the adductor muscle, while tyrosine was expressed in all tissues. The phosphor (p)-ERK and p-AKT activities were high in the adductor muscle, mantle edge, and gonads. IGF-1 signaling affected the growth and maturity of the Pacific oysters examined.

Evaluation of Methods Used in the Calculation of Condition Index Using the Mussel, Mytilus coruscus (Gould, 1861) Collected from Chuja Island, Cheju, Korea (추자도산 홍합, Mytilus coruscus (Gould, 1861) 의 Condition Index 산정에 대한 연구)

  • Condition indices (Cl) are considered to be useful measurements of the nutritive and health status of bivalves although studies on Cl of bivalves are limited due to the lack of a standard formula for calculating Cl. This study attempts to generate Cl of the mussel, Mytilus coruscus inhabiting along the coastal area of Chuja Island in Cheju using three primary formulas. the formulas used in this study: (1) Cl-vol= [tissue dry weight (g) 1000] /shell cavity volume (ml). (2) Cl-wt= [tissue dry weight (g) 1000] / internal shell cavity capacity (g). (3) Cl-size= tissue wet weight (g)/[shell length (mm)]$\^$3/. Monthly condition indices calculated with the three formulas are compared using ANOVA, Duncan's multiple range test and Pearson correlation coefficient. In Chuja Island M. coruscus collected ranged from 50 to 180 mm in shell length. Monthly ranges of Cl-values were 67.48 to 140.61 (Cl-vol), 74.67 to 118.02 (Cl-wt) and 1.4 10$\^$-5) to 1.6 10$\^$-5/ (Cl-size). Cl-vol values in August were higher than two Cl-values in the other months. Monthly Cl-vol was significantly different from Cl-wt and Cl-size. (p<0.05). The results of this study suggest that volumetric condition index (Cl-vol) used in this study is acceptable as a standard measure to evaluate conditions of M. coruscus.
