밀라드 구순성형술 (Millard's Cheiloplasty)

  • For many years a wide variety of surgical techniques for closure of cleft lip has been used. Still many surgeons prefer the Millard's rotation-advancement lip repair because the surgical scar is masked in the philtral crest and the nostril floor, and it improves the relationship of the alar base of the cleft side, producing harmonious symmetry of the nostril and the nostril sill. In addition, it uses and preserves the lip anatomy, returning lip tissue into its normal position, minimizing the amount of tissue that is discarded, and reconstructing the orbicular oris muscle. One of the major disadvantages of this procedure is the lack of accurate measurements. The object of this study is to help in the cleft lip surgery with investing its features and design.

The Effect of a Pulsed Electromagnetic Field with Time on Pain in Muscle Crushed Rat Model

  • Acute injuries to skeletal muscles can lead to significant pain and disability. Muscle pain results in muscle weakness and range of motion (ROM) decreases. Pulsed electromagnetic fields (PEMF) promote tissue repair, healing rates and reduce musculoskeletal pain. The results of many previous studies suggest that PEMF can contribute to chronic pain reduction, particularly in musculoskeletal injurys. However, we do not have enough information of its effects compared to a placebo. The principal objective of this study was to investigate differences in acute pain induced by the direct destruction of muscle tissue (extensor digitorum) with varying times of the application of PEMF, measured through the expression of c-fos on the spinal cord. Significant reduction of pain was found in groups exposed to PEMF and the group exposed to PEMF immediately after muscle injury showed the most significant differences. In conclusion, PEMF may be a useful strategy in reducing acute pain in muscle injury.

  • The purpose of this study was to evaluate the pulpal response against calcium hydroxide and zinc oxide containing various concentration of formocresol (0.1, 0.5, 1, 5, 10, 20 and 30%). The experiment was performed on dog's teeth (75 teeth from 5 dogs: Table 1.) and the teeth were routinly treated in laboratory procedures. Followings are the results obtained through microscopical examination. 1. In zinc oxide group, intlammatory reaction was severe in low concentrated formocresol and the (higher the concentration of formocresol the milder the inflammatory reaction was more evident. 2. In zinc oxide group, inflammatory change was milder at 3 weeks than 1 week, and proliferation of young connective tissue was seen at 3 weeks. 3. In calcium hydroxide group, inflammatory change in relation to the concentration of formocresol was not noticeable. 4. In calcium hydroxide group, repair process with decreased inflammatory reaction and fibrosis, and dentin bridge like layer was found at 3 weeks.

Thoracic Splenosis after Splenic and Diaphragmatic Injury

  • Thoracic splenosis is a rare disease that develops as a result of autotransplantation of splenic tissue into the thoracic cavity following splenic and diaphragmatic injury. We report the case of a 53-year-old man with a chief complaint of heartburn and cough. He had a history of traumatic diaphragmatic rupture treated with surgical repair and splenectomy 15 years ago. Imaging studies revealed a paraesophageal mass, and surgical resection was performed considering the possibility of Castleman disease or an esophageal submucosal tumor. Pathologic results showed findings of normal splenic tissue. The patient was discharged on postoperative day 5 without any complications.

  • Recently, the barrier membranes have been applied for regenerating bone surrounding peri-implant defects in guided bone regeneration(GBR). GBR membrane should provide mechanical support sufficient to withstand in vivo forces and maintain wound space for bone regeneration. The ability to exclude unwanted tissues of cells(connective tissue and epithelium) is needed. In addition large surface area is conductive to tissue ingrowth. The search for ideal materials that biocompatible, bioresorbable and can support the growth and phenotypic expression of osteoblasts is a major challenge in the biomedical application for the repair of bone defects. Therapeutic application of extracellular vesicles for various kidney diseases: a brief review

  • Extracellular vesicles (EVs) released from different types of kidney cells under physiologic conditions contribute to homeostasis maintenance, immune-modulation, and cell-to-cell communications. EVs can also negatively affect the progression of renal diseases through their pro-inflammatory, pro-fibrotic, and tumorigenic potential. Inhibiting EVs by blocking their production, release, and uptake has been suggested as a potential therapeutic mechanism based on the significant implication of exosomes in various renal diseases. On the other hand, stem cell-derived EVs can ameliorate tissue injury and mediate tissue repair by ameliorating apoptosis, inflammation, and fibrosis while promoting angiogenesis and tubular cell proliferation. Recent advancement in biomedical engineering technique has made it feasible to modulate the composition of exosomes with diverse biologic functions, making EV one of the most popular drug delivery tools. The objective of this review was to provide updates of recent clinical and experimental findings on the therapeutic potential of EVs in renal diseases and discuss the clinical applicability of EVs in various renal diseases.

상장간막동맥류의 수술적 치료 -1례 보고- (Superior Mesentic Artery Aneurysm -A Case Report-)

  • Superior mesenteric artery aneurysm is the third most common lesion and comprises approximately 5.5 % of all visceral artery aneurysms. The first successful repair was performed by DeBakey and Cooley in 1949. Since then, more than 100 cases have been reported. Fifty to sixty percent of these aneurysms are mycotic in origin. Other less frequent causes include arteriosclerosis, trauma, and medial degeneration. The operations are bypass with autologous tissue or with artificial vascular graft and aneurysmorrhaphy. We have experienced a case of superior mesenteric artery aneurysm which had undergone aneurysmectomy and artificial graft interposition. This is the first domestic case which was successful surgical repaired.

상.하둔동맥 천공지피판을 이용한 대전자부 욕창의 치료 (The Treatment of Trochanteric Pressure Sore Using Superior or Inferior Gluteal Artery Perforator Flap)

  • Purpose: Management of pressure sores has been improved, along with development of musculocutaneous flaps and perforator flaps. Nowadays, the treatment of pressure sore with perforator flaps has shown several advantages, including minimal donor site morbidity, relatively versatile flap design not only in primary cases but also in recurred cases and minimized anatomical rearrangement of regional muscle position. In this study, we report our clinical experience of gluteal perforator flap used in the treatment of a greater trochanteric pressure sore. Methods: A clinical study was performed on 7 patients who underwent total 10 operations. 1 superior gluteal artery perforator flap and 9 inferior gluteal artery perforator flaps were used to reconstruct the defect, followed by the mean observation duration of 22 months. Results: There were no total flap loss. We treated 2 cases of partial flap loss with debridement and primary repair. 2 recurred cases were successfully treated using the same method. Donor sites were all primarily repaired. Conclusion: The gluteal perforator flap could be considered as a safe and favorable alternative in the treatment of soft tissue defects in the greater trochanteric area. The advantages of the flap include low donor site morbidity and the possibility of versatile flap design not only in primary cases but also in recurred cases.
