전완유리피판 공여부 재건에서 전층 피부이식술과 부분층 피부이식술의 비교 (Comparison Study Between Coverage Choices for Radial Forearm Free Flap Donor Site)

  • Backgrounds: The purpose of this study was to compare full thickness skin graft with inguinal skin to split thickness skin graft for coverage of the radial forearm free flap donor site. Patients and Methods: 25 patients who was reconstructed with radial forearm free flap for head and neck cancer were reviewed retrospectively. Results: The graft loss rates of full thickness skin graft were less than split thickness skin graft or split thickness skin graft with dermal substitutes. The recovery times of donor site and skin graft donor site of full thickness skin graft were also shorter than split thickness skin graft or split thickness skin graft with dermal substitutes. Skin texture and aesthetic results of donor site were improved and complications as itching and pain sensation were decreased. Conclusion: Full thickness skin grafts with inguinal skin should be considered for patients requiring a radial forearm free flap.

Iliac Bone Graft for Recurrent Posterior Shoulder Instability with Glenoid Bone Defect

  • Recurrent posterior shoulder instability is a debilitating condition that is relatively uncommon, but its diagnosis in young adults is increasing in frequency. Several predisposing factors for this condition have been identified, such as the presence of an abnormal joint surface orientation, an osteochondral fracture of the humeral head or glenoid cavity, and a postero-inferior capsuloligamentary deficit, but their relative importance remains poorly understood. Whilst, conservative treatment is effective in cases of hyperlaxity or in the absence of bone abnormality, failure of conservative treatment means that open or arthroscopic surgery is required. In general, soft-tissue reconstructions are carried out in cases of capsulolabral lesions in which bone anatomy is normal, whereas bone grafts have been required in cases where posterior bony Bankart lesions, glenoid defects, or posterior glenoid dysplasia are present. However, a consensus on the exact management of posterior shoulder instability is yet to be reached, and published studies are few with weak evidence. In our study, we report the reconstruction of the glenoid using iliac bone graft in a patient suffering recurrent posterior shoulder instability with severe glenoid bone defect.

  • 이상적인 도관은 직경이 커야 하고, 조직 독성이 적으며 감염에 강하고 장기 성적도 좋아야 한다. 대복재정맥은 실제에서 많이 사용되고 있지만 만족할 만한 혈류를 위해서는 때때로 시간을 기다려야 할 필요가 있다. 저자들은 단시간 내 적절한 혈류확보를 위한 생체도관으로서 표재성 대퇴정맥을 사용한 2예를 보고한다.

Biophysical therapy and biostimulation in unfavorable bony circumstances: adjunctive therapies for osseointegration

  • Dental implants using titanium have greatly advanced through the improvement of designs and surface treatments. Nonetheless, the anatomical limits and physiological changes of the patient are still regarded as obstacles in increasing the success rate of implants further, even with the enhancement of implant products. So there have been many efforts to overcome these limits. The intrinsic potential for bone regeneration can be stimulated through adjuvant treatments with the continuous improvement of implant properties, and this can play an important role in achieving optimum osseointegration toward peripheral bone tissue and securing ultimate long-term implant stability in standard surgical procedures. For this purpose, various chemical, biological, or biophysical measures were developed such as bone grafts, materials, pharmacological agents, growth factors, and bone formation proteins. The biophysical stimulation of bone union includes non-invasive and safe methods. In the beginning, it was developed as a method to enhance the healing of fractures, but later evolved into Pulsed Electromagnetic Field, Low-Intensity Pulsed Ultrasound, and Low-Level Laser Therapy. Their beneficial effects were confirmed in many studies. This study sought to examine bone-implant union and its latest trend as well as the biophysical stimulation method to enhance the union. In particular, this study suggested the enhancement of the function of cells and tissues under a disadvantageous bone metabolism environment through such adjunctive stimulation. This study is expected to serve as a treatment guideline for implant-bone union under unfavorable circumstances caused by systemic diseases hampering bone metabolism or the host environment.

Recycling of flap pedicle in complex lower extremity reconstruction: A proof of free muscle flap neovascularization

  • This article portrays the authors' clinical experience of a complex case of lower extremity reconstruction using a recycled pedicle from 10 years old free latissimus dorsi musculocutaneous flap to supply a new free anterolateral thigh flap for proximal tibia wound defect reconstruction. It provides clinical evidence that muscle neovascularization occurs and supports the dogma peripheral tissue neovascularization. This case stipulates that recycling of pedicle is feasible, when used with appropriate strategy and safety and also provides evidence for the long-term survival of greater saphenous vein grafts in lower extremity reconstruction.

Two-Blade Guillotine Technique for Nipple Graft Harvest

  • The nipple-sharing technique for nipple reconstruction offers excellent tissue matching. The method used for nipple graft harvesting determines the quality of the graft and hence, the success of nipple sharing. Here, we described a guillotine technique wherein the nipple is first transfixed with 2 straight needles to stabilise it. Two No. 11 blades are then inserted in the center and simultaneously swept outwards to amputate the distal portion of the nipple. This technique provides good control, resulting in a very evenly cut base. The recipient bed is deepithelialized thinly, and the nipple graft is inset with interrupted 8-0 nylon sutures under magnification. Being a composite graft, it is protected with splint dressings for 6 weeks, and the dressing is regularly changed by the surgeon. The height of the nipple grafts ranges from 4 to 8 mm. This technique was performed in 9 patients with an average follow-up of 2.9 years (range, 1-4.5 years). Apposition between the nipple graft and its bed is crucial for the success of this technique. When correctly applied, we observed rapid revascularization of the graft.

가족치아를 이용한 골이식술: 증례보고 (Familial Tooth Bone Graft: Case Reports)

  • The use of autogenous tooth bone graft material has been commercialized since 2008. Autogenous tooth bone grafts always require that the tooth of the patient be extracted, and thus, the use of graft material are limited in many cases. For solution of limitation in quantity and concurrent use of autogenous tooth bone graft material, the grafting of familial teeth has been suggested. It has the following advantages: the teeth of family members are used as bone graft materials, the genetic composition is identical, and potential genetic and infectious risks can be minimized. Because the teeth of family members are used, a good tissue affinity is obtained, and thus, superior bone generation rates compared to those observed for allogenic or xenogenic bones can be anticipated. We used familial tooth bone-graft materials for alveolar ridge augmentation, socket preservation, and maxillary sinus graft in some cases. In most cases, the impacted third molars of their children were prepared as bone graft material and were used for surgery. In one case, the impacted third molar from the patient's brother was used as bone graft material. We obtained satisfactory result and these cases are reported herein.

조직 접착제를 이용한 유리 피부 이식술 - 임상적, 조직병리학적 고찰 - (FREE SKIN GRAFTING WITH FIBRIN ABHESIVE - CLINICAL AND HISTOPATHOLOGIC REVIEWS -)

  • A fibrin adhesive have been widely used in oral and maxillofacial surgery for microvascular anastomosis, autogenous chip bone grafts, many kinds of soft tissue surgery (vestibuloplasty, bleeding control after extraction, primary healing by covering of suture of a gum after the extirpation of large cysts). There are two principal components in adhesive systems biologically: lyophilized human fibrinogen and bovine thrombin. The fibrinogen component contains coagulation factor XIII and enhance the initial wound healing, which polymerizes soluble fibrin monomers into an insoluble clot. The thrombin is dissolved in a solution of calcium chloride to provide the second component. We applied fibrin adhesive, Beriplast (Behring, Behringwerke AG, D-3350, Marburg, FRD), to 4 patients for fixation of free skin grafting donors who had facial scar around eye, nose, mouth corner which received from accidents, or burn. We have experienced initial accelerated graft fixation between donor and recipient sites with no additional fixation. And It's made easy bleeding control and easy manipulation during operation. But two cases showed partial hypertrophic scar engrowth in above 3 months follow up, but no significant. Histopathological reviews in general were showed similar scar findings such as abundant collagen bundles in H&E, M/T stain, but slight positive signs in elastic and collagen antibody immunopathologic findings in hypertrophic scar cases.

탈회동종골을 이용한 신혈류화골판 형성에 관한 연구 (FABRICATION OF NEO-OSSEOUS FLAP USING DEMINERALIZED ALLOGENEIC BONE)

  • Microsurgical vascularized bone transfer has the disadvantages of limitation of available donor sites, loss of donor function, and the possibility of donor site defects or deformity. To overcome these shortage of current microsurgical tissue transfer, the method of creating the neovascularized free flap has been introduced. Potentially, this technique must be an innovation in providing the free vascularized bone grafts that are not limited by natural vascular anatomy. But, as could be imagined technique resulted in unavoidable donor bone defect and additional operation for harvesting the autologous bone. The purpose of this study was to evaluate the efficacy of demineralized allogeneic bone as a possible substitute for autologous bone in fabricating the neo-osseous flap. By histologic, microangiographic and radioisotope method, the viability and vascularity of neo-osseous flap, which has been fabricated using allogeneic bone or autologous bone, was assessed in rat model. After 6 weeks, demineralized allogeneic bone showed consistent bone formation and neovascularization. The clinical and microscopic findings of demineralized allogeneic bone group were inferior to those of autogenous bone with regard to bone regeneration. The amount of bone blood floow per dry weight of demineralized allogeneic bone group was significantly higher than that of autogenous bone, even higher that of control intact iliac bone. In conclusion, findings supported that allogeneic bone could be the potential substitute for autologous bone source in creating a prefabricated neo-osseous flap.

Polymer Nanofibers for Biomedical Engineering

  • Recent advancements in the electrospinning method enable the production of ultrafine solid and continuous fibers with diameters ranging from a few nanometers to a few hundred nanometers with controlled surface and morphological features. A wide range of biopolymers can be electrospun into mats with a specific fiber arrangement and structural integrity. These features of nanofiber mats are morphologically similar to the extracellular matrix of natural tissues, which are characterized by a wide pore diameter distribution, a high porosity, effective mechanical properties, and specific biochemical properties. This has resulted in various kinds of applications for polymer nanofibers in the field of biomedicine and biotechnology. The current emphasis of research is on exploiting these properties and focusing on determining the appropriate conditions for electrospinning various biopolymers for biomedical applications, including scaffolds used in tissue engineering, wound dressing, drug delivery, artificial organs, and vascular grafts, and for protective shields in specialty fabrics. This paper reviews the research on biomedical applications of electrospun nanofibers.
