Development and Evaluation of Tip Pinch Strength Measurement on a Paretic Hand Rehabilitation Device

  • In this study, we described the development of a methodology to measure tip-pinch strength on the paretic hand rehabilitation device and aimed to investigate reliability of the device. FSR sensors were embedded on the device, and tip pinch strength was estimated with data collected from the sensors using a developed equation while participants were demonstrating tip pinch. Reliability tests included inter-rater, test-retest, and inter-instrument reliability. B&L Engineering pinch gauge was utilized for the comparison. Thirty-seven healthy students participated in the experiment. Both inter-rater and test-retest reliability were excellent as Intraclass Correlation Coefficients (ICCs) were greater than 0.9 (p<0.01). There were no statistically significant differences in tip-pinch strengths. Inter-instrument reliability analysis confirmed good correlation between the two instruments (r = 0.88, p < 0.01). The findings of this study suggest that the two instruments are not interchangeable. However, the tip-pinch mechanism used in the paretic hand rehabilitation device is reliable that can be used to evaluate tip pinch strength in clinical environment and can provides a parameter that monitors changes in the hand functions.

  • Purpose : The purpose of this paper is to figure out the effects of the Powerball exercise on grip strength, tip pinch and key pinch. Methods : Twenty subjects participated in this experiment. Spin right wrist counterclockwise for five minutes holding Powerball after making main joint of right hand -predominant hand- into elbow flexion and forearm pronation. Results : 1. Before and after Powerball exercise, when we compared the grip strength of men and women, there was a significant difference. 2. Before and after Powerball exercise, when we compared tip pinch of men and women, there was a significant difference. 3. Before and after Powerball exercise, when we compared key pinch of men and women, there was a significant difference. 4. Before and after Powerball exercise, there was no significant difference between men and women. Conclusion : From what using Powerball exercise influenced grip strength, tip pinch and key pinch positively, we assume that it will be considered a great expectation to improve grip strength, tip pinch and key pinch in the future and it would be better to conduct this experiment in depth to a lot of people.

3D 프린팅 손가락 모형의 동작 제어에 관한 연구 (A Study on the Motion Control of 3D Printed Fingers)

  • This study developed and evaluated the motion control of 3D printed fingers applied to smart gloves. Four motions were programmed by assembling the module using the Arduino program: cylindrical grasping, spherical grasping, tip-to-tip pinch gripping, and three-jaw pinch gripping. Cap and re-entrant (RE) strip types were designed to model the finger. Two types of modeling were printed using filaments of thermoplastic elastomer (TPE) and thermoplastic polyurethane (TPU). The prepared samples were evaluated using three types of pens for cylidrical grasping, three types of balls for spherical grasping, and two types of cards for tip-to-tip pinch gripping and three-jaw pinch gripping. The motion control of fingers was connected using five servo motors to the number of each control board. Cylindrical and spherical grasping were moved by controlling the fingers at 180° and 150°, respectively. Pinch gripping was controlled using a tip-to-tip pinch motion controlled by the thumb at 30° and index-middle at 0° besides a three-jaw pinch motion controlled by the thumb-index finger-middle at 30°, 0°, and 0°, respectively. As a result of the functional evaluation, the TPE of 3D-printed fingers was more flexible than those of TPU. RE strip type of 3D-printed fingers was more suitable for the motion control of fingers than the 3D-printed finger.

두 점 집기 작업 시 손가락 관절토크의 역학적 해석 (Dynamic Analysis of Finger Joint Torque for Tip Pinch Task)

  • This paper presents the dynamic analysis on the joint torque of a finger for the tip pinch task. The dynamic model on finger movement was developed in order to predict the joint torques of an index finger, and the finger was assumed as a three-link planar manipulator. Analysis of the model revealed that the joint stiffness was one of the most important parameters affecting the joint torque. The stiffness of the finger joint was experimentally measured, and it was used in analyzing the finger joint torque required for performing the tip pinch task. The obtained joint torque for the tip pinch task will be used as the design requirements of the finger exoskeletal orthosis actuated by the polymer actuator whose allowable torque limit is relatively low compared to that of a mechanical actuator.

피부미용사의 손 부위 악력 및 파지력 수준과 관련 요인 (A Study on Factors Related to Grip and Pinch Strength among Estheticians)

  • Objectives: The purpose of this study was to measure estheticians' grip and pinch strength in the hand parts and identify related factors according to general characteristics. Methods: A Jamar dynamometer was used to measure grip strength and a Jamar pinch gauge was used to measure tip, palmar and lateral pinch strength. General data were collected through a questionnaire survey among 116 estheticians from 31 skin care centers and spas. Results: The subjects' mean grip strength was 23.95 kgf for the right hand and 23.42 kgf for the left hand with the dominant right hand recording slightly higher mean grip strength. Their mean pinch strength was the highest with lateral pinch(right 4.89 kgf, left 4.69 kgf), which was followed by palmar pinch(right 3.77 kgf, left 3.88 kgf) and tip pinch(right 2.29 kgf, left 2.25 kgf), in that order. Conclusions: The grip and pinch strength of the hand parts was high when estheticians' commonly used both hands for massage instead of just one. The use of both hands is recommended to protect the hand parts during massage.

방아쇠수지가 손목터널증후군 환자의 악력, 통증 및 상지기능에 미치는 영향: 단면적 연구 (Effect of Trigger Finger on Pain, Grip Strength and Function of Upper Limb of Patients with Carpal Tunnel Syndrome: A Cross-sectional Study)

  • PURPOSE: The purpose of this study was to investigate the effects of trigger finger on pain, muscle strength and function in carpal tunnel syndrome (CTS) patients. METHODS: A total of 60 subjects (30 carpal tunnel syndrome with trigger finger and 30 carpal tunnel syndrome without trigger finger) were assessment for pain, muscle strength (power grip, key pinch , tip to tip pinch, three jaw pinch) and function. The effect sizes of the two groups were compared, and the correlation between the trigger finger and each variable was analyzed. RESULTS: The results showed that there were significantly difference in the pain, muscle strength excluding three jaw pinch and function (p < .05). The results also showed correlation between trigger finger and pain (r = .552), muscle strength excluding three jaw pinch (power grip r = -.296, key pinch r = -.260, tip to tip pinch r = -.285), and function (r = .375). The function of carpal tunnel syndrome patients was related to pain (r = .550) and power grips (r = -.324) of muscle strength. CONCLUSION: In carpal tunnel syndrome patients with trigger finger compared to carpal tunnel syndrome, muscle weakness, pain increase, and function reduction were shown. In addition, trigger finger are correlated with muscle strength, pain and function, and muscle weakness and increased pain affect the daily living of carpal tunnel syndrome patients with triggers finger. Therefore, physical therapy interventions of carpal tunnel syndrome patients with trigger finger should be combined with treatment for muscle strength enhancement as well as pain reduction.

스케일링 실습수업 전후 수부의 악력 및 파지력 비교분석 (Comparison analysis of before and after scaling training on grip and pinch strength)

  • Objectives : The purpose of the research is to measure Dental Hygiene students' level of musculoskeletal pain before and after receiving scaling training which involves repetitive operations and examine effects the operations have on the students' grip and pinch strength. Methods : 15 subjects in Dental Hygiene at Y university located in Chungbuk were tested on November 2, 2011. Results : 12 students had a musculoskeletal pain before receiving scaling training. However, all the 15 students had a musculoskeletal pain after the training. Body part the largest number of the students complained of the pain on was shoulders(37.9%) followed by neck(20.8%), waist(13.9%), wrists(10.3%), then back(6.9%) before receiving the training. Then the result showed that shoulders are where the largest number of students(24.1%) felt the pain, followed by neck(17.2%), wrists and fingers(13.8%), waist(12.1%), back and lower arms(6.9%), then palms(5.2%) after the training. Visual Analogue Scale(VAS) increased to $6.35{\pm}2.09$ after receiving the training from $3.31{\pm}3.38$ and this shows statistically significant difference(p<.001). Upon inspection, average grip strength decreased to $23.23{\pm}5.79kg$ from $25.00{\pm}4.51kg$ after receiving the training and this shows statistically significant difference(p<.05). Key pinch was the strongest among the three pinch strengths followed by tripod pinch then tip pinch. Average of all the three pinch strengths showed to have statistically significant difference before and after the training(p<.05); Tip pinch from $6.53{\pm}1.36$ to $5.87{\pm}1.3$, key pinch from $13.47{\pm}2.5$ to $12.53{\pm}2.30$, and tripod pinch from $9.27{\pm}3.04$ to $8.03{\pm}2.72$. Conclusions : The result showed that scaling training has effects on Dental Hygiene students' musculoskeletal subjective symptoms and their grip and pinch strength changes after receiving the training.

손 기능에 관한 고찰 (A Review of Hand Function)

  • The purpose of study to know hand function in order to prevent disability or handicap. The ability to perform precise refined movements of hand is an important human function. Improvement in object manipulation is common goal of therapist. The ability to manipulate an object in the hand is need for many functional tasks, including writing, handling coins, small objects and ADL skills. Therapists have commonly used hand grip and pinch strengths as baseline measures to evaluate hand function. The patterns of grasps are precision grasp, power grasp, hook grasp, spherical grasp, sylindrical grasp, disc grasp, pinch, three point pinch and tip pinch. And the motion of in-manipulation are finger to palm translation, palm to finger, shift, simple rotation and complex rotation. The hand function are include to evaluate of ROM, sensation, muscle strength of hand. It used to evaluate of decision of effect and suppose of disability and acceptance of vocation. Good evaluation is need to pretreatment and baseline of treatment and help to evaluate of effect on treatment.

타악기를 활용한 과제지향운동이 만성 뇌졸중 환자의 상지 기능에 미치는 영향 (Task-oriented Approach using Percussion Instruments in Chronic Stroke Patients Effect on Upper Limb Function: a Randomized Controlled Trial)

  • PURPOSE: This study examined the effects on the upper extremity function, muscle strength, and hand function of a task-oriented training approach using a percussion instrument for patients with chronic stroke. METHODS: Twenty-four chronic stroke patients accompanied with upper extremity hemiplegia were selected for research and were classified randomly into 12 experimental groups and 12 control groups. The experimental group performed a task-oriented approach, and the control group performed upper extremity occupational therapy. Stroke upper extremity test, Jebsen-Taylor Hand Function test, upper extremity muscle strength test, and hand muscle strength test were measured before and after training in the evaluation process. RESULTS: In the upper extremity test and Jebsen-Taylor test, there were no significant differences between the groups. In the upper extremity muscle strength test, there were significant differences in shoulder flexion, internal rotation, and elbow flexion in the experimental group. In the hand muscle strength test, there were significant differences in the grip, tip Pinch, lateral Pinch, and 3-jaw chuck in the experimental group and significant differences in only grip, tip pinch, and lateral pinch in the control group. In addition, there were significant differences in the lateral pinch compared to the amount of change. CONCLUSION: Task-oriented approach using percussion instruments for upper extremity rehabilitation in stroke patients is effective in the upper extremity function and strength, hand function, and strength.

핀치이론의 수정 모델을 이용한 스프레이 모드의 해석 (Analysis of Spray Mode Using Modified Pinch Instability Theory)

  • While the pinch instability theory (PIT) has been widely employed to analyze the spray transfer mode in the gas metal arc welding (GMAW), it cannot predict the detaching drop size accurately. The PIT is modified in this work to increase the accuracy of prediction and to simulate the molten tip geometry to be more physically acceptable. Since the molten tip becomes a cone shape in the spray mode, the effective wire diameter is formulated that the effective diameter is inversely proportional to current square. Modifications are also made to consider the finite length of the liquid column and current leakage through the arc. While the effective diameter influences drop transfer significantly, the current leakage has negligible effects. The effects of modifications on drop transfer are analyzed, and the predicted drop diameters show good agreements with the experimental data of the steel wire.
