Timing of laparoscopic cholecystectomy after endoscopic retrograde cholangiopancreatography in cholelithiasis patients: A systematic review and meta-analysis

  • There are many variations and unclear definitions of the appropriate timing of laparoscopic cholecystectomy (LC) after endoscopic retrograde cholangiopancreatography (ERCP), and there is still a lack of consistency about the appropriate timing. Inappropriate timing can be associated with serious comorbidity and can affect the patients. This meta-analysis was conducted to assess the operative outcomes and morbidity to provide a benefit to the patients based on the best timing of LC after ERCP. Randomized controlled trials (RCTs) and retrospective studies were identified from the PubMed and Scopus databases from inception to July 2021. A meta-analysis was performed to estimate the treatment effects on operative outcomes and morbidity. Four RCTs and four retrospective studies met our inclusion criteria. A meta-analysis indicated that patients who received LC after ERCP on the same day or within 72 hours had about 0.354 days shorter length of hospital stay with a shorter operative time of about 0.111-1.835 minutes and a lower risk of complications around 37%-73%. Our evidence suggests that the appropriate timing of LC after ERCP is either the same day or within 72 hours for treating cholelithiasis patients based on the severity of disease.

  • In this paper, a new symbol timing algorithm for a magnetic recording system as HDD, is proposed. Above all, the Lorentzian channel model applied, the distortion and the attenuation problems of playback signals at high speed and storage density were considered. The structure and the operation of popular data detectors for HDD were understood, and the adopted symbol timing algorithm were profoundly analyzed. On the basis of these understanding and analysis, a new symbol timing algorithm for high density HDD was proposed, and the performance evaluation of this proposed algorithm was done.

Harnessing Integration of Symbol-Rate Equalizer and Timing Recovery for Enhanced Stability

  • This research conducted a comparative analysis of two communication systems. The first system utilizes a conventional series configuration consisting of a symbol-rate least mean square (LMS) equalizer followed by a timing recovery loop. The second system introduces an innovative approach that integrates a symbol-rate LMS equalizer and a timing recovery component within a single loop, allowing mutual feedback between the two blocks. In this integrated system, the equalizer also provides timing error information, thereby eliminating the requirement for a separate threshold error detector. This study examines the performance curves of both system configurations. The simulation results revealed that the integrated system may offer improved stability in terms of multiple transmission challenges, including phase and frequency offsets and intersymbol interference. Further analysis and discussion highlight the significant insights and implications of the proposed architecture. Overall, the present findings provide an alternative perspective on the joint implementation of equalization and timing recovery in communication systems.

Dynamic Analysis of Timing Silent Chain System for a V-type Engine of a Vehicle

  • Based on multi-body dynamic software RecurDyn, this paper proposes a modified form of timing silent chain system combing with the existing problem that vibration and chain tension is too large, which is applied for complicated conditions in a V-type engine, such as high speed, variable loads. The analysis of chain drive meshing characteristics is completed. Using the multi-body dynamic soft-ware RecurDyn, the dynamics characteristics of the improved system is studied, including chain tension, transmission error, chain fluctuations, equivalent spring force in different operating conditions. The study results show that chain tension, transmission error, chain fluctuation and equivalent spring force are within the scope of permission, all of them can meet the design requirement. There-fore, the design of this system is reasonable and practicable. The research results will provide a basis for assessing timing silent chain system in a V-type engine and a theoretical reference for designing and optimizing the timing silent chain system.

Analysis on power penalty due to timing jitters when considering intersymbol interference in the receivers on intensity modulation/direct detection optical communication systems (강도변조/직접검파 광통신 수신기에서 심벌간 간섭을 고려할 경우 타이밍 지터에 의한 잔력 페널티 해석)

  • In this paper, we propose a new method to analyze the performance degradation by timing jitters in the receivers of intensity modulation/direct detection digital optical communication systems where pulse-shaping filters are used to minimize intersymbol interference. The results obtained from the proposed analytical method show that conventional analytical methods underestimate the influence of timing jitters on the receiver performance. Using the proposed anlaytical method, we derive an analytic equation for approximated power penalty due to timing itters and obtain an exact power penalty by numerical analyses. Assuming Gaussian or uniform probability density function for timing jitters, we also show that assumption of Gaussian distribution for timing jitters yields more performance degration than that of uniform distribution.

Analysis of the Timing Detector's Characteristics of the Modified BECM(M-BECM) Algorithm (M-BECM의 타이밍 검출기 출력 특성 분석)

  • Previously, we have proposed the M-BECM(Modified-Band Edge Component Maximization), which is a symbol synchronization algorithm based on spectral line method for all-digital high speed digital communications. However, Until now, the characteristics of the timing detector based on the spectral line method including M-BECM was not analyzed, particularly the effect of a timing offset at the optimal convergence pont. In this paper, we analyze the timing dtector's characteristics of the M-BECM and present optimal design value. First, the expression for the timing detector's mean value(often called its S-Curver) as a function of the normalized symbol timing offset is derived. Next, the P $D_{bias}$, the value for compensating the timing offset at an optimal convergence point, and the bandwidth of bandpass filter in the timing detector are calculated. It is also shown and analyzed that the P $D_{bias}$ is affected by varuous factors such as the excess bandwidth of input signal, frequency offsets, noise and particularly, the excess bandwidth of input signal is a major parameter to decide P $D_{bias}$. Finally, analytic resutls are compared to simulation results.

  • This paper represents the occurrence structure of the timing variation of TV horizontal synchronizing pulse(H-sync pulse) in mobile TV reception. Fluttering ghost is caused by timing varations of the H-sync pulse and is due to frequency elective fading in a multipath propagation envoronment. H-sync timing fluctuations, and hence, fluttering ghost are directly correlated with the multipath parameters, i.e., H-sync timing fluctuations reflect well the severity of the multipath environment. The occurrence structure of H-sync timtng fluctuation is analysed theoretically in relation to the multipath environnment. The occurrence structure of H-sync timing fluctuation is analysed theoretically in relation to the multipath parameters, assuming the two-ray propagation model. The H-sync timing fluctuation occurs with the variation in relative phase and /or D/U variation of long-delayed multipath waves.

  • In this paper, we present techniques which perform incremental timing analysis using Timed Boolean Algebra that solves the false path problem and extracts the timing information in combinational circuits. Our algorithm sets histories of internal inputs that are substituted for internal output and extracts maximal delays through checking sensitizability of primary outputs. Once finding the sum of primitive delay terms, then it applies modified delay with referencing histories of primary output and it can extract maximal delays of primary outputs fast and efficiently. When the structure of circuit is changed, there is no need to compute the whole circuit again. We can process partial timing analysis of computing on the gates that are need to compute again. These incremental timing analysis methods are considered both delay changes and structure of circuit, and can reduce the costs of a trial error in the circuit design.

The Optimal Time between Embolization and Surgery for Hypervascular Spinal Metastatic Tumors : A Systematic Review and Meta-Analysis

  • Objective : Preoperative transarterial embolization (TAE) of tumor feeders in hypervascular spine metastasis is known to reduce intraoperative estimated blood loss (EBL) during surgery. The effect of TAE varies for several reasons, and one controllable factor is the timing between embolization and surgery. However, the adequate timing remains unclear. This study aimed to evaluate the timing and other factors that reduce EBL in spinal metastasis surgery through a meta-analysis. Methods : A comprehensive database search was performed to identify direct comparative studies of EBL stratified by the timing of surgery after TAE for spinal metastasis. EBL was analyzed according to the timing of surgery and other factors. Subgroup analyses were also performed. The difference in EBL was calculated as the mean difference (MD) and 95% confidence interval (CI). Results : Among seven studies, 196 and 194 patients underwent early and late surgery after TAE, respectively. The early surgery was defined as within 1-2 days after TAE, while the late surgery group received surgery later. Overall, the MD in EBL was not different according to the timing of surgery (MD, 86.3 mL; 95% CI, -95.5 to 268.1 mL; p=0.35). A subgroup analysis of the complete embolization group demonstrated that patients who underwent early surgery within 24 hours after TAE had significantly less bleeding (MD, 233.3 mL; 95% CI, 76.0 to 390.5 mL; p=0.004). In cases of partial embolization, EBL was not significantly different regardless of the time interval. Conclusion : Complete embolization followed by early spinal surgery within 24 hours may reduce intraoperative bleeding for the patients with hypervascular spinal metastasis.

The Prediction of Remodelling Timing Based on the Cash Flow of Permanent Rental Housing (영구임대주택의 현금흐름을 고려한 리모델링 시기 추정에 관한 연구)

  • The purpose of this study is to predict remodeling timing of Permanent Rental Housing through historic data and cash flow analysis. For this aim, the study has estimated cash flow of Permanent Rental Housing considering initial construction costs, government supporting fund, rental incomes and maintenance expenses. Based on the expected cash flow analysis, reasonable remodeling timing is predicted for Permanent Rental Housing. The results of this study are as follows: (1) it is analyzed that building a8e of about 27 years is the best time for remodeling because cumulative surplus amounts reach maximum level, (2) it is required that remodeling should be made before 34 years of age roughly because cumulative surplus amounts change into minus from this time.
