Saxitoxin 검출을 위한 Neuro-2a 시험법 조건 확립 및 실험실 간 변동성 비교 연구 (Establishment of Test Conditions and Interlaboratory Comparison Study of Neuro-2a Assay for Saxitoxin Detection)

  • 마비성 패류 독소(Paralytic shellfish poisoning, PSP)는 유해 조류에 의해 생성되며, 독소에 노출된 수산물을 섭취하였을 때 중독이 발생한다. 수산물 중 PSP를 검출하는 표준 시험법인 Mouse bioassay (MBA)는 낮은 검출한계와 동물 윤리 문제로 대체 시험법의 개발 필요성이 대두되고 있다. 이러한 대체 시험법 중, PSP가 신경 세포막의 Na+ 채널을 차단하는 기전을 이용한 마우스 뇌신경 모세포종 세포 기반 시험법(Neuro-2a assay)의 표준화를 위한 노력이 대두되고 있다. Neuro-2a assay의 원리는 Neuro-2a 세포주에 Na+/K+ ATPase 억제제인 Ouabain(O)과 Na+ 채널 활성화제인 Veratridine (V)을 처리하여 과도한 Na+ 유입으로 인한 세포사멸을 유도한 상태에서, Na+ 채널 억제제인 PSP를 처리하게 되면 Na+ 유입이 차단되어 세포가 생존하는 것을 측정하는 것이다. 본 연구에서는 PSP 검출을 위한 Neuro-2a assay를 국내 연구 환경에 맞게 다양한 매개변수를 개선하여 최적 시험법을 확립하고자 하였다. 고려한 매개변수들은 세포밀도, 배양 조건 및 PSP 처리 조건 등으로, 그 결과는 아래와 같다. 초기 세포밀도는 40,000 cells/well로, 세포 배양시간 및 처리시간은 각각 24시간으로 설정하였다. 또한 최적 O/V 농도는 500/50 μM로 설정하였다. 본 연구에서 PSP 중 Saxitoxin (STX)에 대해서 O/V 처리가 된 상태에서 S자형 용량-반응 그래프가 도출되는 8가지 농도(368~47,056 fg/μl)를 확인하였고, Neuro-2a assay의 실험실 간 변동성 비교를 통해, 실험의 적정성 확인을 위한 5가지 Quality Control Criteria와 실험 데이터의 신뢰가능 범위(Data Criteria) 6가지를 설정하였다. 확 립된 조건으로 Neuro-2a assay를 진행한 결과 반수영향농도(EC50) 값은 약 1,800~3,500 fg/μl로 나타났다. 실험실 간 변동성 비교 결과, Quality Control Criteria 값 및 Data criteria 값의 변동계수(coefficients of variation (CVs))가 1.98~29.15% 범위로 산출되어 실험의 적정성 및 재현성이 확인되었다. 본 연구를 통해 우리나라에서 활용할 수 있는 PSP 검출용 Neuro-2a assay 시험법의 최적 조건 및 5가지 Quality control 기준을 제시하였고, PSP 중 대표적인 독소인 STX을 대상으로 Neuro-2a assay를 실시한 결과 유의한 EC50 값을 산출할 수 있었으며, 향후 국내 수산물을 대상으로 MBA를 대체할 수 있는 PSP 검출법으로 활용될 것으로 기대된다.

금강하구 연안역에서 HF radar로 측정한 유속의 정확도 (Accuracy of HF radar-derived surface current data in the coastal waters off the Keum River estuary)

  • 금강하구 연안역에서 HF radar로 측정한 유속의 정확도를 평가하기 위해 HF radar의 마주보는 radial 유속들을 비교하고, HF radar로 측정한 유속을 현장측정 유속과 비교하였다. 비교 자료들에 대한 회귀선과 편차는 주성분 분석(Principal Component Analysis)으로 구하였다. HF radar site를 연결하는 선의 중간지점에서 마주보는 radial vector를 비교하였을 때 RMS 편차는 동계에 4.4 cm/s, 하계에 5.4 cm/s이었다. HF radar와 유속계로 측정한 유속성분을 비교하여 분석된 RMS 편차에서 GDOP(Geometric Dilution of Precision) 효과를 제거하였을 때 HF radar의 합성 속도 측정오차는 GDOP 값이 적절한 정점들에서 5.1 cm/s 이내였다. 서로 다른 두 방법에 의해 구해진 이 결과는 연구해역에서 HF radar로 측정된 유속의 정확도 하한이 5.4 cm/s임을 제시한다. 기존의 연구에서와 같이 RMS 편차는 섬 주변에 있는 관측점에서 크게 되고, 두 radar에서 평균거리가 멀어질수록 신호 대 잡음수준과 radial vector 교차각의 감소로 인해 증가하였다. GDOP 값을 이용한 오차분리 과정에서 속도성분별 GDOP 값이 유사하고 비교 유속의 성분별 RMS 편차도 비슷한 값을 보이는 경우 HF radar 유속의 오차가 불확실한 값으로 도출될 수도 있음이 밝혀졌다. GDOP가 정상적인 radar 관측 범위 내에 있는 정점에서 측정된 유속을 조류와 해류로 분리하였을 때 HF radar 유속에서 구해진 조류타원의 특성은 유속계로 측정된 타원특성과 잘 부합하였고, 해류의 시간적 변화는 바람과 밀도장의 외력에 의한 물리적 과정을 반영하는 반응을 보였다.

지하수 관개에 의한 수도의 멸준양상과 그 방지책에 관한 연구 (Studies on the Rice Yield Decreased by Ground Water Irrigation and Its Preventive Methods)

  • The purposes of this thesis are to clarify experimentally the variation of ground water temperature in tube wells during the irrigation period of paddy rice, and the effect of ground water irrigation on the growth, grain yield and yield components of the rice plant, and, furthermore, when and why the plant is most liable to be damaged by ground water, and also to find out the effective ground water irrigation methods. The results obtained in this experiment are as follows; 1. The temperature of ground water in tube wells varies according to the location, year, and the depth of the well. The average temperatures of ground water in a tubewells, 6.3m, 8.0m deep are $14.5^{\circ}C$ and $13.1^{\circ}C$, respercively, during the irrigation period of paddy rice (From the middle of June to the end of September). In the former the temperature rises continuously from $12.3^{\circ}C$ to 16.4$^{\circ}C$ and in the latter from $12.4^{\circ}C$ to $13.8^{\circ}C$ during the same period. These temperatures are approximately the same value as the estimated temperatures. The temperature difference between the ground water and the surface water is approximately $11^{\circ}C$. 2. The results obtained from the analysis of the water quality of the "Seoho" reservoir and that of water from the tube well show that the pH values of the ground water and the surface water are 6.35 and 6.00, respectively, and inorganic components such as N, PO4, Na, Cl, SiO2 and Ca are contained more in the ground water than in the surface water while K, SO4, Fe and Mg are contained less in the ground water. 3. The response of growth, yield and yield components of paddy rice to ground water irrigation are as follows; (l) Using ground water irrigation during the watered rice nursery period(seeding date: 30 April, 1970), the chracteristics of a young rice plant, such as plant height, number of leaves, and number of tillers are inferior to those of young rice plants irrigated with surface water during the same period. (2) In cases where ground water and surface water are supplied separately by the gravity flow method, it is found that ground water irrigation to the rice plant delays the stage at which there is a maximum increase in the number of tillers by 6 days. (3) At the tillering stage of rice plant just after transplanting, the effect of ground water irrigation on the increase in the number of tillers is better, compared with the method of supplying surface water throughout the whole irrigation period. Conversely, the number of tillers is decreased by ground water irrigation at the reproductive stage. Plant height is extremely restrained by ground water irrigation. (4) Heading date is clearly delayed by the ground water irrigation when it is practised during the growth stages or at the reproductive stage only. (5) The heading date of rice plants is slightly delayed by irrigation with the gravity flow method as compared with the standing water method. (6) The response of yield and of yield components of rice to ground water irrigation are as follows: \circled1 When ground water irrigation is practised during the growth stages and the reproductive stage, the culm length of the rice plant is reduced by 11 percent and 8 percent, respectively, when compared with the surface water irrigation used throughout all the growth stages. \circled2 Panicle length is found to be the longest on the test plot in which ground water irrigation is practised at the tillering stage. A similar tendency as that seen in the culm length is observed on other test plots. \circled3 The number of panicles is found to be the least on the plot in which ground water irrigation is practised by the gravity flow method throughout all the growth stages of the rice plant. No significant difference is found between the other plots. \circled4 The number of spikelets per panicle at the various stages of rice growth at which_ surface or ground water is supplied by gravity flow method are as follows; surface water at all growth stages‥‥‥‥‥ 98.5. Ground water at all growth stages‥‥‥‥‥‥62.2 Ground water at the tillering stage‥‥‥‥‥ 82.6. Ground water at the reproductive stage ‥‥‥‥‥ 74.1. \circled5 Ripening percentage is about 70 percent on the test plot in which ground water irrigation is practised during all the growth stages and at the tillering stage only. However, when ground water irrigation is practised, at the reproductive stage, the ripening percentage is reduced to 50 percent. This means that 20 percent reduction in the ripening percentage by using ground water irrigation at the reproductive stage. \circled6 The weight of 1,000 kernels is found to show a similar tendency as in the case of ripening percentage i. e. the ground water irrigation during all the growth stages and at the reproductive stage results in a decreased weight of the 1,000 kernels. \circled7 The yield of brown rice from the various treatments are as follows; Gravity flow; Surface water at all growth stages‥‥‥‥‥‥514kg/10a. Ground water at all growth stages‥‥‥‥‥‥428kg/10a. Ground water at the reproductive stage‥‥‥‥‥‥430kg/10a. Standing water; Surface water at all growh stages‥‥‥‥‥‥556kg/10a. Ground water at all growth stages‥‥‥‥‥‥441kg/10a. Ground water at the reproductive stage‥‥‥‥‥‥450kg/10a. The above figures show that ground water irrigation by the gravity flow and by the standing water method during all the growth stages resulted in an 18 percent and a 21 percent decrease in the yield of brown rice, respectively, when compared with surface water irrigation. Also ground water irrigation by gravity flow and by standing water resulted in respective decreases in yield of 16 percent and 19 percent, compared with the surface irrigation method. 4. Results obtained from the experiments on the improvement of ground water irrigation efficiency to paddy rice are as follows; (1) When the standing water irrigation with surface water is practised, the daily average water temperature in a paddy field is 25.2$^{\circ}C$, but, when the gravity flow method is practised with the same irrigation water, the daily average water temperature is 24.5$^{\circ}C$. This means that the former is 0.7$^{\circ}C$ higher than the latter. On the other hand, when ground water is used, the daily water temperatures in a paddy field are respectively 21.$0^{\circ}C$ and 19.3$^{\circ}C$ by practising standing water and the gravity flow method. It can be seen that the former is approximately 1.$0^{\circ}C$ higher than the latter. (2) When the non-water-logged cultivation is practised, the yield of brown rice is 516.3kg/10a, while the yield of brown rice from ground water irrigation plot throughout the whole irrigation period and surface water irrigation plot are 446.3kg/10a and 556.4kg/10a, respectivelely. This means that there is no significant difference in yields between surface water irrigation practice and non-water-logged cultivation, and also means that non-water-logged cultivation results in a 12.6 percent increase in yield compared with the yield from the ground water irrigation plot. (3) The black and white coloring on the inside surface of the water warming ponds has no substantial effect on the temperature of the water. The average daily water temperatures of the various water warming ponds, having different depths, are expressed as Y=aX+b, while the daily average water temperatures at various depths in a water warming pond are expressed as Y=a(b)x (where Y: the daily average water temperature, a,b: constants depending on the type of water warming pond, X; water depth). As the depth of water warning pond is increased, the diurnal difference of the highest and the lowest water temperature is decreased, and also, the time at which the highest water temperature occurs, is delayed. (4) The degree of warming by using a polyethylene tube, 100m in length and 10cm in diameter, is 4~9$^{\circ}C$. Heat exchange rate of a polyethylene tube is 1.5 times higher than that or a water warming channel. 오피니언 마이닝을 이용한 지능형 VOC 분석시스템 (Intelligent VOC Analyzing System Using Opinion Mining)

  • 기업 경영에 있어서 고객의 소리(VOC)는 고객 만족도 향상 및 기업의사결정에 매우 중요한 정보이다. 이는 비단 기업뿐만 아니라 대고객, 대민원 업무를 처리하는 모든 조직에 있어서도 동일하다. 때문에 최근에는 기업뿐만 아니라 공공, 의료, 금융, 교육기관 등 거의 모든 조직이 VOC를 수집하여 활용하고 있다. 이러한 VOC는 방문, 전화, 우편, 인터넷게시판, SNS 등 다양한 채널을 통해 전달되지만, 막상 이를 제대로 활용하기는 쉽지 않다. 왜냐하면, 고객이 매우 감정적인 상태에서 고객의 주관적 의사를 음성 또는 문자로 표출하기 때문에 그 형식이나 내용이 정형화되어 있지 않고 저장하기도 어려우며 또한 저장하더라도 매우 방대한 분량의 비정형 데이터로 남기 때문이다. 본 연구는 이러한 비정형 VOC 데이터를 자동으로 분류하고 VOC의 유형과 극성을 판별할 수 있는 오피니언 마이닝 기반의 지능형 VOC 분석 시스템을 제안하였다. 또한 VOC 오피니언 분석의 기준이 되는 주제지향 감성사전 개발 프로세스와 각 단계를 구체적으로 제시하였다. 그리고 본 연구에서 제시한 시스템의 효용성을 검증하기 위하여 의료기관 홈페이지에서 수집한 4,300여건의 VOC 데이터를 이용하여 병원에 특화된 감성어휘와 감성극성값을 도출하여 감성사전을 구축하고 이를 통해 구현된 VOC분류 모형의 정확도를 비교하는 실험을 수행하였다. 그 결과 "칭찬, 친절함, 감사, 무사히, 잘해, 감동, 미소" 등의 어휘는 매우 높은 긍정 오피니언 값을 가지며, "퉁명, 뭡니까, 말하더군요, 무시하는" 등의 어휘들은 강한 부정의 극성값을 가지고 있음을 확인하였다. 또한 VOC의 오피니언 분류 임계값이 -0.50일 때 가장 높은 분류 예측정확도 77.8%를 검증함으로써 오피니언 마이닝 기반의 지능형 VOC 분석시스템의 유효성을 확인하였다. 그러므로 지능형 VOC 분석시스템을 통해 VOC의 실시간 자동 분류 및 대응 우선순위를 도출하여 고객 민원에 대해 신속히 대응한다면, VOC 전담 인력을 효율적으로 운용하면서도 고객 불만을 초기에 해소할 수 있는 긍정적 효과를 기대해 볼 수 있을 것이다. 또한 VOC 텍스트를 분석하고 활용할 수 있는 오피니언 마이닝 모형이라는 새로운 시도를 통해 향후 다양한 분석과 실용 프레임워크의 기틀을 제공할 수 있을 것으로 기대된다.

인공지능을 활용한 경관 지각반응 예측모델 개발 가능성 기초연구 - 머신러닝 기법을 중심으로 - (Basic Research on the Possibility of Developing a Landscape Perceptual Response Prediction Model Using Artificial Intelligence - Focusing on Machine Learning Techniques -)

  • 최근 IT 기술과 데이터의 범람으로 생활 전반적인 부분의 패러다임이 전환되고 있다. 이러한 기술의 발전과 변화는 학술영역에도 영향을 미치고 있다. 학문적 교류와 연계를 통해 연구주제나 연구 방법의 개선이 이루어지고 있다. 특히, 데이터 기반의 연구 방법이 다양한 학문분야에서 진행되고 있으며 조경학에서도 지속적인 연구가 필요한 시점이다. 따라서 본 연구에서는 이러한 시대적 상황을 반영하여 인공지능의 한 분야인 머신러닝을 활용한 경관 선호 평가 및 예측모델의 개발 가능성을 알아보는 것을 목표로 한다. 본 연구의 목표를 달성하기 위하여 경관 분야에 머신러닝 기법을 적용하여 경관 선호 평가 및 예측 모델을 구축하고, 구축된 모형의 모의정도를 검증하였다. 이를 위해 본 연구에서는 최근 신재생에너지 사업으로 주목받는 풍력발전시설 경관 이미지를 연구대상으로 선정하였다. 분석을 위하여 풍력발전시설 경관 이미지를 웹크롤링 기법을 활용하여 수집하고 분석 테이터셋을 구축하였다. 우수한 성능의 예측모델 도출을 위하여 머신러닝 분석에 활용되는 University of Ljubljana의 프로그램인 오렌지 버전 3.33을 활용하였다. 또, 머신러닝 학습데이터의 평가기준을 통합한 모델과 평가기준 별도 모델 구조를 활용하였으며, 머신러닝 분류모델에 적합한 kNN. SVM, Random Forest, Logistic Regression, Neural Network 알고리즘을 사용해 모델을 생성하였다. 생성된 모델을 성능 평가를 실시하여 본 연구에 가장 적합한 예측모델을 도출하였다. 본 연구에서 도출된 예측모델은 경관의 유형에 따른 분류, 경관과 대상의 시거리에 따른 분류, 선호에 따른 분류 등 3가지 평가기준을 별도로 평가 후 종합해 예측하여 결과를 도출하였다. 연구 결과 경관 유형에 따른 평가 기준 정확도 0.986, 시거리에 따른 평가 기준 정확도 0.973, 선호에 따른 평가 기준 정확도 0.952에 달하는 높은 정확도를 가진 예측모델을 개발하였으며, 평가데이터 예측 결과를 통한 검증과정을 보아도 모델의 성능 치를 상회하는 성과를 도출했음을 알 수 있다. 경관 관련 연구에서 머신러닝을 활용한 예측모델 개발 가능성을 알아본 실험적 시도로 이미지 데이터의 수집 및 정제를 통해 데이터 세트를 구축하여 높은 성능의 예측모델이 생성 가능하며, 이후 경관 관련 연구 분야에 활용될 수 있다는 가능성을 확인할 수 있었다. 본 연구의 결과와 시사점, 한계점을 반영한다면 풍력발전시설의 경관뿐만 아니라 자연경관이나 문화경관 등 다양한 형태의 경관 예측모델 개발이 가능할 것으로 생각되며, 경관 유형에 따라 이미지를 분류하는 모델의 연구를 통해 데이터 분류의 시간을 단축하거나 머신러닝을 활용한 경관예측 인자분석을 통해 경관계획 요소의 중요도 분석 등의 주제에 맞는 연구 방법을 탐색하고 적용하여 후속 연구를 진행한다면 조경학 분야에서도 머신러닝 기법을 보다 유용하고 가치 있게 활용할 수 있을 것으로 생각된다.

항 바이러스 치료중인 B형 간염환자에서 HBeAg 및 HBV DNA 검출에 관한 분석 (Analysis of HBeAg and HBV DNA Detection in Hepatitis B Patients Treated with Antiviral Therapy)

  • [목적] B형 간염바이러스(hepatitis B virus, HBV)감염은 전세계적으로 중요한 공중 보건 문제이며 만성간염, 간 경변, 간암의 주요 원인으로 알려져 있으며, 이러한 질환의 진단 및 치료에 B형 간염바이러스의 혈청학적 검사는 필수적이다. 항 바이러스 치료중인 B형 간염 환자를 대상으로 면역방사계수 측정법(IRMA; Immunoradiometric assay)과 화학발광 미세입자 면역분석법(CMIA; Chemiluminescent Micropartical Immunoassay)을 이용하여 HBe-Ag 검사를 시행하였고, 실시간 중합효소 연쇄반응(RT-PCR; Realtime-Polymerase Chain Reaction)법을 이용하여 혈청 내 HBV DNA 검출율 을 비교 분석 하였다. [대상 및 방법] 항 바이러스 치료가 시행중인 B형 간염 환자 270명을 대상으로 HBeAg 혈청 검사와 HBV DNA 정량 검사를 실시하였다. HBeAg 혈청 검사는 검출 원리가 다른 두 가지 혈청학적 검사법(IRMA, CMIA)을 적용 하였고, 혈청 내 HBV DNA는 Abbott m2000 System을 사용하여 실시간 중합효소 연쇄반응(RT-PCR; Realtime-Polymerase Chain Reaction)법으로 정량 측정 하였다. [결과] HBeAg 검출율은 면역방사계수법(IRMA)의 경우 24.1% (65/205), 화학발광 미세입자 면역 분석법(CMIA)에서는 82.2% (222/48)의 결과를 보였다. 혈청학적 검사방법(IRMA, CMIA)에 따른 HBeAg 검사결과의 일치율은 33% (89/270)이다. 실시간 중합효소 연쇄반응(RT-PCR)을 이용한 혈청 내 HBV DNA의 검출율은 29.3% (79/191)를 보였고, 혈청 내 HBV-DNA 농도는 $16IU/mL{\times}1.0{\times}10^9IU/mL$ 이며 검출한계는 <15IU/mL 이다. 면역방사계수법(IRMA)으로 HBeAg 검출결과가 양성일때 55.4%, 그리고 음성일때 20.9%의 HBV DNA 검출율과 $1.1{\times}10^8IU/mL$, $5.7{\times}10^5IU/mL$의 혈청내 HBV DNA농도를 나타냈다. 이에 반해 화학발광 미세입자 면역 분석법(CMIA)의 경우 HBeAg 검출결과가 양성일때 HBV DNA 검출율은 28.4%, 음성일때 33.3%의 결과를 나타냈으며 혈청 내 HBV DNA농도는 $6.0{\times}10^7IU/mL$, $2.4{\times}10^5IU/mL$ 이었다. 면역방사계수법과 화학발광 미세입자 면역 분석법에서 동일하게 HBeAg 검출 결과가 양성인 경우 HBV DNA 검출율은 62.3%의 결과를 보였으며, 혈청 내 HBV DNA 농도는 $1.1{\times}10^8IU/mL$ 이다. [결론] 혈청학적 검사법에 따른 HeAg 검출율은 많은 차이를 보였다. 이러한 차이는 검사kit에 사용된 Ab의 특성과 epitope, HBV의 genotype등 여러 가지 원인으로 생각된다. 혈청학적 검사 결과로 분류 된 그룹별 HBV DNA의 검출율과 농도를 비교한 결과, Group II(IRMA 양성, CMIA 양성, N=53)에서 높은 검출율과 농도를 확인할 수 있었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. 비인강암의 병합요법 (Combined Modality Treatment in Nasopharyngeal Carcinoma)

  • 목적 : 국소 진행된 비인강암으로 유도 화학요법 혹은 동시 화학요법 및 방사선 치료를 받은 환자들의 부작용 및 단기 생존율을 비교하기 위하여 본 연구를 시행하였다. 대상 및 방법 : 1989년 10월부터 1998년 5월까지, 비인강암으로 진단받고 유도 화학요법 혹은 동시 화학요법 및 방사선 치료를 받은 62명의 환자들을 대상으로 후향적 분석을 시행하였다. 유도 화학요법군은 50명, 동시 화학요법군은 12명이었다. 나이, 성, 전신 수행능력, 조직학적 유형 등은 양군에서 비교적 고르게 분포하였다. 병기 분포는, 유도 화학요법군의 경우 IIB가 $32\%$, III가 $30\%$, 그리고 IV가 $38\%$였으며, 동시 화학요법군은 각각 50, 33.3, $16.7\%$였다. 양군에서 화학요법 약제는 CF (cisplatin and 5-FU)였고, 약제 투여 방법은 동일하였다. Cisplatin $100\;mg/m^2$을 day 1에, 5-FU $1,000\;mg/m^2$을 day $2\~6$에 각각 정맥투여하였으며 3주 간격으로 반복하였다. 방사선 치료 종료시까지 시행된 화학요법의 횟수는 양군 모두 테$1\~3$회(중앙값 2회)였다. 방사선 치료는 양군 모두 일일선량 $1.8\~2.0\;Gy$를 주 5회 조사하였다. 총 방사선량은, 유도 화학요법군의 경우 $69.4\~86\;Gy$ (중앙 선량 73.4 Gy), 동시 화학요법군은 $69.4\~75.4\;Gy$ (중앙 선량 70.8 Gy)였다. 추적기간은, 유도 화학요법군의 경우 $9\~l16$개월(중앙값 40.5개월), 동시 화학요법군은 $14\~29$개월(중앙값 21개월)이었다. 결과 : 전체 환자에서 2년 생존율은 $78.7\%$였다. 치료 방법별 2년 전체 생존율은, 유도 화학요법군이 $77\%$, 동시 화학요법군은 $87\%$였으며(p>0.05), 2년 무병 생존율은 각각 $56\%$$81\%$였다(p>0.05). 최종 치료에 대한 완전 관해율은 $75.5\%$$91.7\%$로 동시 화학요법군에서 높았으나 통계학적 유의성은 없었다. 방사선 치료시 grade $3\~4$의 혈액학적 독성은 양군에서 차이가 없었고, grade 2의 백혈구 감소가 동시 화학요법군에서 많았다 $(18\%\;vs\;66.7\%)$. Grade $3\~4$의 점막염은 동시 화학요법군에서 많았다$(4.0\%\;vs\;33.3\%)$. 전체적으로 grade $3\~4$의 급성 부작용이 동시 화학요법군에서 빈번하였고$(6.0\%\;vs\;41.7\%,\;p=0.005)$, 부작용으로 사망한 경우는 없었다. 결론 : 국소 진행된 비인강암에서 동시 화학요법을 시행함으로써 치료의 반응율과 2년 무병 생존율이 향상되는 경향을 보였으나, 통계학적 의의는 없었다. 따라서, 좀더 많은 대조군과 장기 추적관찰이 요구된다.

광주지역 저소득층 여자노인의 영양상태와 건강위험요인에 관한 연구 (Nutritional Status and Health Risks of Low Income Elderly Women in Gwangju Area)

  • 본 연구는 저소득층 노인을 대상으로 하여 식생활 실태 및 건강상태를 조사하여 노인의 건강과 영양상태의 상호연관성 및 이에 영향을 미치는 위험 요인을 파악하고자 실시되었다. 조사는 광주광역시에 거주하는 저소득층 여자노인 92명을 대상으로 하였으며, 조사대상자를 연령에 따라 세 그룹 ($65{\sim}74$세, $75{\sim}84$세, 85세 이상)으로 체위, 혈액, 영양상태 등을 비교하였으며, Nutritional Screening Initiative (NSI) 점수에 따라 $NSI{\leq}3$은 영양 양호군, NSI > 3은 영양 위험군으로 구분하여 영양위험정도에 따른 식생활 관련 건강요인을 비교하였으며 연구결과를 요약하면 다음과 같다. 1) 본 조사대상자의 평균 연령은 79.0세이었으며, 사회경제적 수준이 매우 낮았으며, 치아건강상태 (70.7%) 및 경제적인 이유 (76.1%)로 음식섭취가 부실하였으며 혼자거동하기 힘든 여자 노인의 비율이 48.9%인데, 63.0%의 노인이 혼자 식사한다고 응답하여 저소득층 여자 노인의 사회경제적 위험 요인이 영양상태를 더욱 악화시키는 것으로 조사되었다. 2) 본 조사 대상자가 앓고 있는 주요 질병으로는 고혈압(42.9%), 관절염 및 신경통(33.0%), 당뇨병 (15.4%), 심혈관계 질환 (8.8%), 소화기계 질환 (8.8%) 등의 순이었다. 전체 여자 노인의 44.6%가 비만하였으며, 고혈압, 당뇨병, 심혈관계 질환 중 1개 이상의 질병을 보유한 노인이 53.3%, 대사 증후군을 보유한 노인이 49.3%로서 여자 노인의 건강상태가 전반적으로 불량한 것 판단되었다. 3) 조사대상자의 평균 체중은 54.5, 신장은 147.1cm로서 한국인 영양권장량설정을 위한 체위 기준치와 비교할 때 체중은 더 많은 반면 신장은 낮은 것으로 조사되었으며, 연령증가에 따라 체중과 체단백질이 감소한 반면 체지방은 감소되지 않았고, 체지방 비율이 40.0%, 허리둘레가 87.7cm에 달하였으며, BMI가 25.1 $kg/m^2$으로 전체적으로 비만한 경향을 나타내었다. 4) 연령에 따라 혈압에 유의적인 차이가 없었으며, 혈청알부민을 제외하고는 혈액의 지질성분, 면역성분, 항산화 성분에서 연령별로 유의적인 차이가 없었다. 여자 노인의 평균 수축기 혈압 143.0 mmHg, 공복 혈당은 119.7mg/dl로서 노인의 중요한 건강 문제는 고혈압, 당뇨 등임을 확인할 수 있으며, TNF-${\alpha}$수준이 높아 노년기 염증 반응 증가를 시사하였다. 5) 조사대상자의 영양상태는 매우 불량하였으며, 연령증가에 따라 영양섭취 실태가 더 불량해지는 경향을 나타내고 있으나 연령에 따른 유의적인 차이는 없었다. NSI 점수를 기준으로 하여, 영양 위험군 (NSI>3)에 비해 영양 양호군 $(NSI{\leq}3)$으로 구분된 노인의 하루 3회 이상의 식사 비율이 높았으며, 건강 인지도가 더 좋고, 우울증이 적었으며 운동 실시율이 높고, 당뇨 유병률이 낮았다. 6) 영양섭취 실태와 면역, 염증반응, 항산화 능과의 상관관계를 살펴 본 결과, 혈청의 IL-6와 $TNF-{\alpha}$가 영양상태와 유의적인 상관관계를 나타내어 항 염증성 인자보다는 염증유발인자가 노인의 영양상태와 관련이 있었으며, IL-2, TAS, 공복 혈당 등이 체위와 유의적인 관계가 있어 노인의 영양상태나 체위가 염증반응을 일으켜 노인의 건강에 영향을 미칠 수 있는 것으로 생각된다. 결론적으로, 연령이 증가함에 따라 생리적, 심리적, 사회경제적 요인이 노인의 영양상태에 복합적으로 작용하여, 영양불균형을 초래하고, 만성질환이 있는 경우에는 영양상태가 더욱 불량해지는 등의 건강상의 악순환을 초래하고 있다. 노년기에 건강한 삶을 유지하기 위해서는 적절한 영양소 섭취, 운동, 정상체중 유지가 중요하며 영양문제를 초래할 수 있는 위험요인을 줄여야 될 것이다. 특히, 노인이 양질의 식사를 할 수 있는 방안이 필요하며, 노인의 영양개선 및 건강 증진을 위해서 우리나라 노인의 식습관 및 영양상태, 체위, 질병 등에 관한 종합적이고 체계적인 연구방법에 대한 타당성 연구가 선행되어야 할 것이며 타당성 있는 연구결과를 바탕으로 하여 노인의 영양균형 및 건강증진을 위한 적절한 연구와 지원대책이 필요한 것으로 사료된다.
