The purpose of this study was to investigate how maximum-effort eccentric exercise over different contraction ranges affects the characteristics of torque-angle relationship of human ankle plantarflexor in-vivo. Subjects were randomly assigned in two groups. One group (n=6) performed 120 maximum-effort eccentric ankle dorsiflexion contractions at short muscle length (ankle range of motion from -5 to 15 deg) and the other group (n=6) at long (ankle range of motion from 10 to 30 deg) muscle length. Eccentric exercise decreased the maximum isometric ankle plantarflexion torque ${\sim}40%$. It was found that the optimum ankle joint angle changed from 7.5 deg to 11.1 deg and 10.1 deg, shifted toward the longer muscle length, regardless of the exercise range. The results of this study suggest that eccentric exercise alters the characteristics of torqueangle relationship of the muscle but there is no differential effect of the eccentric contraction range.

The purpose of this study was to investigate the muscle activity of the lower extremity during driver swing in three-foot positions (Feet Open Stance, Feet Straight Stance, Lead Foot Open Stance). The electromyograms of gastrocnemius, tibialis anterior, and vastus lateralis during swing were measured and analyzed in three sections (take away - back swing, back swing - down swing, and down swing - follow swing). There was no significant difference in muscle activity according to foot position. Muscle activity according to phase was significantly higher in right gastrocnemius and tibialis anterior, and the left and right vastus lateralis in down swing - follow swing. In conclusion, the difference in muscle activity according to foot position is insignificant, and it is considered that the muscle activity to maintain the balance of the body increases toward the end of swing.
Tibialis anterior (TA) muscle originates from the lateral surface of tibia and its tendon attaches to the medial cuneiform and base of the first metatarsal. The TA muscle is responsible for both dorsiflexion and inversion of the foot. We present a case of bilateral TA muscle variations that diverge slightly from the current classification systems of this muscle. Recognizing variations such as these may be important for anatomists, surgeons, podiatrists, and physicians. Following routine dissection, an accessory tendon of the TA muscle was found on both sides. Accessory tendons of the extensor hallucis longus and extensor hallucis brevis joined to form a common tendon on both sides. We believe that this unique case will help further the classification systems for the tendons of the TA and also be informative for clinical anatomists as well as physicians treating patients with pathology in this region.
The purpose of this study were to analysis on quality properties of lower extremity of patient with stroke by the existence of the independent walking. We recruited fourteen adults after stroke(7 male, 7 female; mean age, 64y) for our study. The subjects were divided into two groups; independent walking group(3 male, 3 female) and non-independent walking group(4 male, 4 female). We used in this study included a B-mode ultrasonography scanner with a 7.5 MHz linea probe. During the experiment, the subject was seated in the chair. The echogenicity(density, white area index; WAI) of the tibialis anterior and rectus femoris muscles were measured on both sides of the leg. In both of the density and WAI, the tibialis anterior muscle were significant differences between paratic side and non-paratic side, the independent walking and non-independent walking group. However, in both of the density and WAI of the rectus femoris muscle were only significant differences between paratic side and non-paratic side. This study showed that stroke and the independent walking have influenced on quality properties of lower leg. Especially, the independent walking has influenced on the tibialis anterior muscle.
Purpose: This study investigated the effect of core and abdominal muscle-strengthening exercises on muscle activity in the lower extremity on unexpected perturbation. Methods: Twenty subjects were randomly divided into the core exercise group (n=10) or the abdominal muscles strengthening group (n=10). The two groups performed their exercises during three sessions a week for a total of four weeks. The muscle activity in the lower muscles (rectus femoris, biceps femoris, tibialis anterior, gastrocnemius) was assessed using surface electromyography (EMG) and normalized maximal voluntary isometric contraction (MVIC) before and after the exercises. Results: An increase in the tibialis anterior activity after the core and abdominal muscles strengthening exercises was found after four weeks. A significant difference in the pre- and post-exercise was found. The gastrocnemius muscle activity increased in the core exercise group more than the abdominal muscles strengthening group. However, the difference was not significant. Conclusion: The results of this study suggest that the core and abdominal muscles strengthening exercises increased the tibialis anterior muscle activity. It is expected to help make more balance ability that affect who has abdominal muscles weakness.
Purpose: This study aims to examine the power changes in eletrocenphalogram (EEG) detected from the tibialis anterior muscle, during repetitive contraction exercise in normal female adults. Methods: The subjects of this study were 24 normal adult females, with no musculoskeletal or nervous system disorders. The 24 female subjects were divided into two groups: 12 subjects comprised a voluntary stimulation training group, and the other 12 subjects comprised an electrical stimulation training group. A total of thirty contractions were made repetitively by each woman, with maximal voluntary contraction exercise for six seconds, and a resting time of three seconds. During the experiment, their EEG was measured at eight positions. The eight positions were Fpz, Fz, Cz, CPz, C3, C4, P3, and P4, in accordance with the international 10~20 system. Results: The relative alpha power and beta power showed no statistically significant differences between the two groups. But the relative gamma power of the CPz, C3, C4, P3, and P4 areas showed statistically significant differences between the two groups (p<0.05). The relative theta power of the C4 area showed statistically significant differences between the two groups (p<0.05). Conclusion: Our findings show that tibialis anterior muscle contraction by electrical stimulation and by voluntary repeated contraction differentially affected brain activation. In particular, the CPz, C3, C4, P3 positions of relative gamma power showed brain activation in voluntary contraction. The C4 position of relative theta power showed different brain activation between the two groups.

Reconstruction of the lower extremities is difficult due to a lack of skin laxity and muscular tissues. Here, we present a case of lower extremity reconstruction via the anterior tibial artery perforator based segmental muscle island flap. Our patient was a 75-year-old male with a chronic ulcerative wound on the right lower leg from an old car accident. A $5.0{\times}0.5cm$ size ulcerative wound with tibial bone exposure was noted. We planned to reconstruct the lower extremity defect with a free flap, but the vessel status was severely compromised intraoperatively. Thus, we found the anterior tibial artery perforator using Doppler ultrasound, elevated the tibialis anterior muscle segment flap, and transposed it to cover the defect successfully. The flap presented with a nice contour and the skin graft covering the flap survived completely. There were no complications of the surgical site at three months follow-up and no gait morbidity. This is a meaningful case applying the concept of segmental muscle flap based on a perforator that had advantages including proper bulkiness, vascularization, and preservation of function, which were well applied, leading to great success.
The tibialis anterior tendon functions as a major dorsiflexor of the ankle. A rupture in this tendon can cause serious problems in the ambulatory function. A closed traumatic rupture without open wound or an atraumatic rupture can delay diagnosis and treatment. There are not enough guidelines for an effective surgical treatment on this chronic condition. Herein, we report two cases of chronic tibialis anterior disruption successfully treated by semitendinosus autograft.
The purpose of this study was to investigate muscle activation of lower extremity such as rectus femoris, tibialis anterior and soleus according to 0cm(bare foot), 4cm and 7cm heel height of shoe on the rocking surface in older women. 20 older women who did not have any lower musculoskeletal and neurological disorders in the past were participated in this study. Each subject was standing for 15 seconds on the level 8 of Biodex Stability System (BSS) while wearing 4cm and 7cm heel height shoes including bare foot. Electromyography was used to measure muscle activation of lower extremity, and the muscle activation was expressed as a percentage of maximal voluntary isometric contraction (% MVIC). We measured % MVIC of three muscles during 5 seconds except for the first 5 seconds and last 5 seconds. SPSS 12.0 program was used for this study. Repeated one-way analysis of variance(ANOVA) was performed to compare the significant difference among the muscles of lower extremities according to heel heights of shoe on the rocking plate. % MVIC of each muscle such as rectus femoris, tibialis anterior and soleus regarding heel heights of shoe had statistically significant differences (p<0.05). The results of contrast test were as follows; 1) % MVIC of rectus femoris had significant differences between barefoot and 4cm, and barefoot and 7cm. 2) % MVIC of tibialis anterior had significant differences between barefoot and 4cm, barefoot and 7cm, and 4cm and 7cm. 3) % MVIC of soleus had significant differences between barefoot and 7cm, 4cm and 7cm. The results indicate that all commonly responsive muscle on the conditions of barefoot, 4cm, and 7cm shoe height on the rcoking surface is tibialis anterior muscle. We found out that the more heels of shoe high, the more muscle activation increases. High-heeled shoes above 7cm remarkably increase the muscle activation of lower extremity and may result in muscle fatigue. Thus, these shoes may summate risk factors of falls in older women. We can acknowledge that the heels above 4 cm affect each muscle activation in lower extremity on the rocking surface.
Purpose : The purpose of this study was to research the effects on resistance CKC & OCK exercise method on lower limb muscle activity, in chronic stroke patients. Method : In this study, 18 patients with stroke caused by hemorrhage or infarction were participated. resistance exercise method was conducted in tow different group : one is a close kinetic chain exercise(CKC) group and the other is an open kinetic chain exercise(OKC) group. CKC with physical therapy was applied to 9 patients, and OKC with physical therapy was applied to 9 patients. Under the researcher's guidance, exercise for CKC and OKC group carried out 3 times a week for 30 minutes during 4weeks. Result : There was significantly different for the vastus lateralis, tibialis anterior and gastrocnemius muscle activity within the intervention period both group. The vastus lateralis, tibialis anterior and gastrocnemius muscle activity was significantly increased within the CKC group. The vastus leteralis and tibialis anterior muscle activity was significantly increased within the OKC group. Conclusion : It was confirmed in this study that the CKC exercise was more effective than OKC exercise in improving lower limb muscle activity of chronic stroke patients. This study seggested that CKC may be suitable for individuals with a chronic stroke. furthermore study should be made a lot of researches regarding in other method and varying conditions for many hemiplegic patients.
