물리적자극이 좌골신경 절단 마우스의 골격근에 미치는 영향 (Effect of Physical Irritation on The Skeletal Muscles of Sciatic Nerve Neurectomized ddY Mice)

  • To demonstrate the effect of physical irritancy(massages) on the skeletal muscles of immobilization ddY mice models induced by right side sciatic nerve neurectomy, the cross sectional histological profiles of the muscularis (M) gastrocnemius, M. tibialis cranialis and M. tibialis caudalis were observed after 28 days of treatment of physical irritancy with the changes of body weight thickness of hind limb and individual muscle weights. In addition, changes of demonstrated with diameter of individual muscle fiber and muscle fasciculata, and number of muscle fiber in each of three types of muscles located in the calf. The massages were used in this study as physical irritancy. The experimental groups were divided into five groups, 1) Sham-operated group(Sham), 2) Neurectomized but not physical irritated control group(Control), 3) Neurectomized and physical irritated at knee pint regions(T1), 4) Neurectomized and physical irritated at calf regions(T2), and 5) Neurectomized and physical irritated at achilles tendon regions(T3). The experimental animals were used 5 per groups. The changes of number or diameter of muscle fiber in each muscles were calculated using automated image analyzer. The results are as follow : Thickness(diameter) of muscle fiber of M. gastrocnemius, M. tibialis cranialis and M. tibialis cranialis in right side of hind limb were remarkedly decreased in Control, T1, T2 and T3 groups compared to that of Sham group.   • To demonstrate the effect of massages on the skeletal muscles of immobilization ddY mice models induced by right side sciatic nerve neurectomy, the cross sectional histological profiles of the muscularis (M) gastrocnemius, M. tibialis cranialis and M. tibialis caudalis were observed after 28 days of treatment of massages with the changes of body weight, thickness of hind limb and individual muscle weights. In addition, changes of serum aspartate amino- transferase (AST) and creatine phosphokinase (CK) levels were also demonstrated with diameter of individual muscle fiber and muscle fasciculata, and number of muscle fiber in each of three types of muscles located in the calf. The experimental groups were divided into five groups, 1) Sham-operated group (Sham), 2) Neurectomized but not massage control group (Control), 3) Neurectomized and massage at knee joint regions (T1), 4) Neurectomized and massage at calf regions (T2), and 5) Neurectomized and massages at achilles tendon regions (T3). The experimental animals were used 5 per groups. The results are as follow: 1. No significant changes of body weight were demonstrated in this groups during whole experimental periods compared to that of Sham group. In addition, no significant changes of body weight gains during experimental periods (Day 0 - Day 27: 28 days of treatment) were also observed. 2. The thickness of intact left side hind limb was not changes in all tested groups compared to that of Sham group. However, the thickness of right side hind limb were remarkedly decreased in Control, T1, T2 and T3 groups compared to that of Sham group. In addition, the differences of thickness of hind limb between right and left sides were also significantly increased in Control and massage groups (T1, T2 and T3) compared to that of Sham group. However, the thickness of right side of hind limb were significantly increased and the differences of thickness between both sides of hind limb were dramatically decreased in massage groups compared to that of Control group in followed order: T1 > T3 > T2. 3. Absolute and relative weight of M. gastrocnemius in the intact left side of hind limb was not changes in all tested groups compared to that of Sham group. However, the weight of right side M. gastrocnemius were remarkedly decreased in Control, T1, T2 and T3 groups compared to that of Sham group. In addition, the differences of weight of M. gastrocnemius between right and left sides were also significantly increased in Control and massage groups compared to that of Sham group. However, the weight of right side M. gastrocnemius were significantly increased and the differences of weight between both sides of M. gastrocnemius were dramatically decreased in massage groups compared to that of Control group in followed order: T3 > T1 >. 4. Absolute and relative weight of M. tibialis cranialis in the intact left side was not changes in all tested groups compared to that of Sham group. In addition, the differences of weight of M. tibialis caudalis between right and left sides were also significantly increased in Control and massage groups compared to that of Sham group. However, the weight of right side M. tibialis caudalis were significantly increased and the differences of weight between both sides of M. tibialis caudalis were dramatically decreased in massage groups compared to that of Control group in followed order: T3 > T1 > T2. 6. A significant increase of serum AST levels were demonstrated in Control group. However, serum AST levels of massage groups were significantly decreased compared to that of Control group in followed order: T3 > T1 > T2. In case of 73 group, the values decreased into similar levels compared to that of Sham group. 7. A significant increase of serum CK levels were demonstrated in Control group. However, serum CK levels of massage groups were significantly decreased compared to that of Control group in followed order: T3 > T1 > T2.

  • Deformation of soft tissue is known inhomogeneous and non-linear in general. In this study, we propose a measurement methodology of local/global strain during soft tissue elongation precisely using laser extensometer which has high accuracy, resolution and is possible to measure global/local strain. The mechanical tensile test are performed on tibialis cranialis, flexor hallucis longus, extensor digitorum longus of swine hindlimb. In order to measure target displacement, reflective marker is attached to detect elongation on specimen using surgical adhesive. The result of this study is to show that laser extensometer is valid to measure longitudinal elongation which is inhomogeneous and non-linear fur soft tissue.

  • 한국재래산양(韓國在來山羊) 12마리의 후지근(後肢筋)을 절개하여 관찰하였던 바 다음과 같은 결과를 얻었다. 1. 한국재래산양(韓國在來山羊)의 후지근(後肢筋)에서는 다음과 같은 근(筋)들을 관찰할 수 있었다 : 소요근(小腰筋) M. psoas minor, 대요근(大腰筋) M. psoas major, 장골근(腸骨筋) M. iliacus, 요방형근(腰方形筋) M. quadratus lumborum, 대퇴근막장근(大腿筋膜張筋) M. tensor fasciae lata, 중둔근(中臀筋) M. gluteus medius, 심둔근(深臀筋) M. gluteus profundus, 둔이두근(臀二頭筋) M. gluteobiceps, 반건양근(半腱樣筋) M. semitendinosus, 반막양근(半膜樣筋) M. semimbranosus, 봉공근(縫工筋) M. sartorius, 박근(薄筋) M. gracilis, 치골근(恥骨筋) M. pectineus, 내전근(內轉筋) M. adductor, 대퇴방형근(大腿方形筋) M. quadratus femoris, 외폐쇄근(外閉鎖筋) M. obturatorius externus, 내폐쇄근(內閉鎖筋) M. obturatorius internus, 쌍자근(雙子筋) M. gemelli, 대퇴사두근(大腿四頭筋) M. quadriceps femoris, 제삼비골근 M. fibularis tertius, 내측지신근(內側趾伸筋) M. extensor digitorum medialis, 장지신근(長趾伸筋) M. extensor digitorum longus, 전경골근(前脛骨筋) M. tibialis cranialis, 장비골근 M. fibularis longes, 외측지신근(外側趾伸筋) M. extensor digitorum lateralis, 비복근 M. gastrocnemius, 가제미근(筋) M. soleus, 천지굴근(淺趾屈筋) M. flexor digitorum superficialis, 심지굴근(深趾屈筋) M. flexor digitorum profundus, 슬와근(膝窩筋) M. popliteus, 골간근(骨間筋) M. interosseus medius. 2. 천둔근(淺臀筋)의 전부(前部)는 대퇴근막장근(大腿筋膜張筋)과 융합된 것 같고, 후부(後部)는 대퇴이두근(大腿二頭筋)과 융합된 것 같다. 그러나 천둔근(淺臀筋)의 후부(後部)와 대퇴이두근(大腿二頭筋)이 결합된 것으로 생각되는 부분에는 완전융합이 일어나지 않고 천둔근(淺臀筋)을 구분(區分)할 수 있을 정도로 표면으로 2근(筋)을 분리(分離)할 수 있었다. 3. 외측지신근(外側趾伸筋)과 내측지신근(內側趾伸筋)의 건(腱)은 부전골의 원위(遠位) 1/3부(部)에서 서로 건막성(腱膜性)띠에 의하여 서로 연결 되었는데, 이 건막성(腱膜性) 띠는 건섬유(腱纖維)의 방향(方向)으로 보아 외측지신근(外側趾伸筋)의 건(腱)에서 분리(分離)되어 나온 한 가지 (branch)가 내측지신근(內側趾伸筋)의 건(腱)으로 이행되고 있었다. 4, 양(羊)에서 볼 수 있는 이상근(梨狀筋) M. piriformis과 장모지신근(長母趾伸筋) M. extensor hallucis longus은 나타나지 않았다.

Lidocaine과 dexamethasone 혼합용액의 근육내 주사 후 조직학적 변화 (Microstructural Changes after Intramuscular Injection of Lidocaine and Dexamethasone)

  • 측두하악장애 및 구강안면통증 환자들 중 근막통증에 의한 기능제한이나 통증을 호소하는 경우를 흔히 볼 수 있다. 근막통증환자를 치료할 때 여러 가지 물리치료가 유용하지만, 즉각적인 통증완화효과를 나타내는 발통점주사요법이 근막통증의 치료로서 널리 적용되고 있다. 발통점주사요법에 흔히 사용하는 화학약제로 생리식염수, 국소마취제, 스테로이드 등이 있으며, 국소마취제는 근육에 대한 부작용이 보고되어 있어 사용상 주의가 필요하다. 이 연구는 상품화된 lidocaine과, dexamethasone 주사제의 혼합용액을 근육내 주사한 후 조직학적 변화를 관찰함으로써 근육에 대한 위해성 여부를 평가하고자 하였다. 또한 용액의 농도에 따라 각각 조직학적 변화를 관찰함으로써 위해성이 없는 적정한 농도를 제시하고자 하였다. 이 연구에서는 생후 9주된 BALB/c 생쥐 (자성) 63마리를 7군으로 분류하여, 앞정강근 (전경골근, tibialis cranialis) 에 각각 생리식염수, dexamethasone, 2.0% lidocaine, 생리식염수와 혼합한 1.0% lidocaine, 생리식염수와 혼합한 1.5% lidocaine, dexamethasone과 혼합한 1.0% lidocaine, dexamethasone과 혼합한 1.5% lidocaine을 주사하였다. 그 후 1일, 7일, 14일째에 희생시켜 실험부위를 절취한 후 조직절편을 만들어 HE염색과 Van-Gieson염색을 거쳐 광학현미경으로 관찰하여 다음의 결론을 얻었다. Lidocaine과 dexamethasone의 혼합용액을 근육내 주사하였을 때, lidocaine의 농도가 1.5% 이하인 경우 조직학적으로 유해하지 않았으며, 통증완화효과와 항염증작용을 동시에 기대할 수 있을 것으로 생각된다. 향후 상기 혼합용액을 발통점주사요법의 약제로 사용할 경우, 인체에서도 동일한 결과가 나올 것인가에 대해서는 추가적인 연구가 필요하다.
