How to Avoid Graft-Tunnel Length Mismatch in Modified Transtibial Technique for Anterior Cruciate Ligament Reconstruction Using Bone-Patellar Tendon-Bone Graft

  • Background: We conducted this study to determine the optimal length of patellar and tibial bone blocks for the modified transtibial (TT) technique in anterior cruciate ligament (ACL) reconstruction using the bone-patellar tendon-bone (BPTB) graft. Methods: The current single-center, retrospective study was conducted in a total of 64 patients with an ACL tear who underwent surgery at our medical institution between March 2015 and February 2016. After harvesting the BPTB graft, we measured its length and that of the patellar tendon, patellar bone block, and tibial bone block using the arthroscopic ruler and double-checked measurements using a length gauge. Outcome measures included the length of tibial and femoral tunnels, inter-tunnel distance, length of the BPTB graft, patellar tendon, patellar bone block, and tibial bone block and graft-tunnel length mismatch. The total length of tunnels was defined as the sum of the length of the tibial tunnel, inter-tunnel distance and length of the femoral tunnel. Furthermore, the optimal length of the bone block was calculated as (the total length of tunnels - the length of the patellar tendon) / 2. We analyzed correlations of outcome measures with the height and body mass index of the patients. Results: There were 44 males (68.7%) and 20 females (31.3%) with a mean age of 31.8 years (range, 17 to 65 years). ACL reconstruction was performed on the left knee in 34 patients (53%) and on the right knee in 30 patients (47%). The optimal length of bone block was 21.7 mm (range, 19.5 to 23.5 mm). When the length of femoral tunnel was assumed as 25 mm and 30 mm, the optimal length of bone block was calculated as 19.6 mm (range, 17 to 21.5 mm) and 22.1 mm (range, 19.5 to 24 mm), respectively. On linear regression analysis, patients' height had a significant correlation with the length of tibial tunnel (p = 0.003), inter-tunnel distance (p = 0.014), and length of patellar tendon (p < 0.001). Conclusions: Our results indicate that it would be mandatory to determine the optimal length of tibial tunnel in the modified TT technique for ACL reconstruction using the BPTB graft. Further large-scale, multi-center studies are warranted to establish our results.

  • Purpose : The purpose of this study is to prevent thc graft-tunnel mismatch by measuring the patellar tendon length, intertunnel distance, tibial tunnel length and by obtaining appropriate bone block length. Materials and Methods : Authors analyzed 15 patients who had taken the anterior cruciate ligament reconstruction from March 1997 to June 1999. Tibial guide was always set on the "endo" $40^{\circ}$ position(Acufex, MA, USA). We measured the following indices, intertunnel distance(X), tibial tunnel length(T), patellar tendon length(N), tibia bone plug length(Y). Both of the femoral tunnel length(F) and the patellar bone plug length(P) were made in 25mm. The appropriate tibial bone plug length was simply calculated by subtracting the patellar tendon length from the sum of the intertunnel distance and thc tibial tunnel length(Y=X+T-N). Results : The average indices were as follows ; the intertunnel distance(X) was $23.4{\pm}1.4mm$, the tibial tunnel length(T) was $43.6{\pm}1.7mm$, the patellar tendon length was $40{\pm}2.4mm$, and the tibia bone plug length was $27{\pm}2.4mm$. Conclusion : In authors' endoscopic technique, establishment of individually determined optimal tibial bone plug length, based on total tunnel length and patellar tendon length could prevent the problem of graft-tunnel mismatch.

Impact of litter on femur and tibial morphology, bone biomechanics, and leg health parameters in broiler chickens

  • Objective: In this study effects of three types of beddings on broiler leg health and bone biomechanics were evaluated. Methods: A total of 504 male chicks (Ross 308) were randomly placed on three beddings (4 replicates/group; 42 birds/pen), zeolite-added litter (ZL), plastic-grid flooring (PF), and wood shavings (WS). On day 42, chickens were weighed, slaughtered, and samples (bone, muscle, and drumstick) were collected. Bones were subjected to leg health tests, morphometric measurements, biomechanical testing, and ash analysis. Results: Broilers in PF and WS groups showed higher live weight than the ZL group (p<0.001), and the incidence of tibial dyschondroplasia (TD) and varus valgus deformity due to distal bending was significantly higher in PF (p<0.001). Multinomial logistic regression showed that bedding has a significant (p = 0.038) contribution toward the development of TD. Tibial strength (p = 0.040), drumstick width (p = 0.001), and total femur and epiphyseal ash contents (p = 0.044, 0.016) were higher in the ZL group. Chicken live weight was correlated with tibial length and weight (r = 0.762, 0.725). Conclusion: Flooring and the type of bedding material directly affect broiler bone length, strength and leg health. Plastic bedding improves the slaughter weight of chickens on the expense of leg deformities, and zeolite litter improves leg health and bone strength.

Effect of Streptozotocin-Induced Diabetes on Bone and Heart Development in Juvenile Rats

  • Our objective of current study was to investigate the development of bone and heart in association with diabetes mellitus (DM). DM was induced by administering an intraperitoneal injection of streptozotocin (STZ; 60 mg/kg) to 4-week-old Sprague-Dawley rats. Body weight and blood glucose were monitored, and rats were sacrificed after 2 or 5 weeks. The left ventricle (LV), including the interventricular septum, was weighed, and body weight and tibial bone length were assessed. Young diabetic rats showed reduced growth in terms of tibial length and body weight compared to controls. Moreover, diabetic males showed more significant growth suppression and reduced LV size than diabetic females. Morphometric analysis of tibiae from diabetic rats revealed suppressed bone growth at 2 and 5 weeks, with no difference between genders. STZ-induced diabetes decreased bone growth and retarded pre-pubertal heart development. As a result, diabetes may increase cardiovascular risk factors and lead to eventual heart failure. Therefore, new therapeutic approaches are required for diabetic children exhibiting growth retardation. Heart growth factor, exercise, and cardiopulmonary physical therapy may be required to promote heart development and physiological function.

Effects of the Herbal-Acupuncture with Acanthopanacis cortex extract at KI10 (Eumgok) on osteoporosis in ovariectomized DDY mice (음곡(陰谷)(KI10)에 시술한 오가피(五加皮) 약침(藥鍼)이 난소적출 생쥐의 골다공증(骨多孔症)에 미치는 영향)

  • Objective : The purpose of this study is to observe the effects of herbal-acupuncture with Acanthopanacis cortex extract (AC-HA) at KI10 (Eumgok) on osteoporosis in ovariectomized (OVX) ddy mice. Methods : We carried out several experimental items to analyze the changes in body weight, uterine weight, uterus index, tibial length, the ash bone weight, tibial BMD, serum ALP, serum creatinine, serum osteocalcin, serum Ca, and the levels of Ca, P, Ca/P ratio in tibia, and we performed histological and histomorphological analysis as well. Results : Tibial BMD was significantly decreased by OVX, while significantly increased by AC-HA at KI10. Serum creatinine level was significantly increased by OVX, while significantly decreased by AC-HA at KI10. Serum osteocalcin level was significantly increased by OVX, while significantly decreased by AC-HA at KI10. Tibial TBV(trabecular bone volume) was significantly decreased by OVX, while significantly increased by AC-HA at KI10. AC-HA at KI10 significantly suppressed the OVX-induced increase of GPL(growth plate length) of tibia in mice. Conclusion : These results suggest that AC-HA at KI10 has a therapeutic effect on osteoporosis in OVX mice.

  • Purpose: The purpose of this study was to evaluate the effectiveness of internal transport using Ilizarov apparatus with free flap surgery for infected tibial nonunion. Materials and Methods: We reviewed 8 patients of infected tibial nonunion treated with internal transport using Ilizarov apparatus and free flap surgery. Seven of eight patients were available for at least 1 year follow-up. All patients were male. The mean age at the time of the surgery was All fractures were Gustilo's type III B open fracture. The mean length of the bone defect was 8.5 cm. All used flaps for covering the soft tissue defect were free rectus abdominis muscle flap. We evaluated bone and functional results with use of the Paley and Catagni's classification. And we classified the complication with use of the Paley's classification. Results: Acceptable length and solid union of bone was achieved in all cases. The mean size of the bone length was 7.2 cm. The mean healing index was 69.5 days/cm. All but one case needed bone graft at docking site. All flaps were survived. There was no recurrence of infection. According to Paley and Catagni's classification, all cases showed excellent or good results. Complications were pin tract infection in 3 cases, persistent pain in 2 cases and limitation of joint motion in 2 cases. Conclusion: Simultaneous free-tissue transfer and Ilizarov distraction osteogenesis was thought to be an attractive treatment modality for infected nonunion of the tibia.

We carried out several experimental items to analyze the changes in body weight, uterine weight, uterus index, tibial length, the ash bone weight, tibial BMD, serum ALP, serum osteocalcin, serum Ca, and the levels of Ca, P, Ca/P ratio in tibia, and we performed histological and histomorphological analysis as well. Results : 1. Herbal-acupuncture with Rosae Laevigatae Fructus at KI10(Umgok) and saline injection at KI10(Umgok) significantly inhibited the reduction of tibial calcium level in ovariectomized mice. 2. Herbal-acupuncture with Rosae Laevigatae Fructus at KI10(Umgok) significantly inhibited the increase of tibial osteoclast cells in ovariectomized mice. 3. Herbal-acupuncture with Rosae Laevigatae Fructus at KI10(Umgok) significantly inhibited the reduction of tibial trabecular bone thickness(TBT) in ovariectomized mice. 4. Herbal-acupuncture with Rosae Laevigatae Fructus at KI10(Umgok) significantly inhibited the overgrowth of tibial growth plate length(GPL) in ovariectomized mice. Conclusion : These results suggested that herbal-acupuncture with Rosae Laevigatae Fructus at KI10(Umgok) has a therapeutic effect on osteoporosis, and to be put to practical use in the future osteoporosis clinic.

Herbal-acupuncture with AaH-HA at $KI_{10}$ significantly inhibited the reduction creatinine level in serum in ovariectomized mice. 3. Herbal-acupuncture with AaH-HA at $KI_{10}$ significantly inhibited the increase of tibial osteoclast cells in ovariectomized mice. 4. Herbal-acupuncture with AaH-HA at $KI_{10}$ significantly inhibited the reduction of TBV(trabecular bone volume) and TBT(trabecular bone thickness) in ovariectomized mice. 5. Herbal-acupuncture with AaH-HA at $KI_{10}$ significantly inhibited the overgrowth of tibial growth plate length(GPL) in ovariectomized mice. Conclusions : In conclusion, our study suggested that Herbal-acupuncturing with AaH-HA at $KI_{10}$ can be effective treatment for osteoporosis.

  • Objective & Method: The purpose of this study is to investigate the effects of herbal-acupuncture with Corni Fructus extract (CF-HA) at Eumgok(KI10) on osteoporosis in ovariectomized (OVX) ddy mice. I carried out several experimental items to analyze the changes in body weight, uterine weight, uterus index, tibial length, the ash bone weight, tibial BMD, the levels of serum ALP, osteocalcin, Ca, P and the levels of tibial Ca, P, Ca/P ratio, and we performed histological and histomorphological analysis as well. Result: 1. CF-HA at Eumgok(KI10) significantly did not increase the level of bone mineral density in overiectomized mice. 2. CF-HA at Eumgok(KI10) significantly decreased the level of serum phosphorus in ovariectomized mice and sinificantly increased the level of serum calcium in ovariectomized mice. 3. CF-HA at Eumgok(KI10) significantly increased the levels of tibial calcium and phosphorus in ovariectomized mice. 4. CF-HA at Eumgok(KI10) significantly decreased the number of tibial osteoclast like cell in ovariectomized mice. 5. In the histomorphometric analysis of tibia, GPL(growth plate length) was significantly decreased by CF-HA at Eumgok(KI10) in ovariectomized mice. Conclusions: These results suggest that CF-HA at Eumgok(KI10) may have useful therapy effects on osteoporosis in ovariectomized mice.

Effects of Herbal-Acupuncture with Cistanches Herba Extract at KI10 (Eumgok) on Osteoporosis in Ovariectomized Mice (음곡 육종용 약침이 난소적출 생쥐의 골다공증에 미치는 영향)

  • Objective & Methods : This study aimed to evaluate the effects of Cistanches Herba herbal-acupuncture (CH-HA) at KI10 (Umgok) on osteoporosis induced by ovariectomy in mice. The results were as follows; Results : CH-HA at KI10 significantly inhibited the overgrowth of tibia, the elevation of serum osteocalcin level, the reduction of tibial Ca and P level, the increase of tibial osteoclast like cells, the reduction of tibial TBV (trabecular bone volume), the overgrowth of tibial GPL (growth plate length) in ovariectomized mice. NP at KI10 significantly inhibited the reduction of BMD (bone mineral density) in ovariectomized mice, but CH-HA did not. Conclusion : Taken together, CH-HA at KI10 has a protective or therapeutic effect for osteoporosis in ovariectomized mice. Therefore, it is suggested that CH-HA may be an useful therapeutics in clinical filed after further researches.
