Thyroid dysfunction in premature infants (미숙아에서의 갑상선기능 장애)

  • Purpose : Thyroid hormone is essential for development of the brain in early life. Thyroid dysfunction is more common in the first 2-4 postnatal weeks of life in premature infants than in term infants. This study aimed to identify the prevalence and clinical course of thyroid dysfunction in prematurity. Methods : Premature infants admitted to and given neonatal screenings at Dankook University Hospital between April 1999 and March 2008 were included in this study. We retrospectively reviewed medical records and categorized subjects into six groups: normal, hypothyroidism, hyperthyrotropinemia, hypothyroxinemia, delayed onset of hypothyroidism, and delayed onset of hyperthyrotropinemia. Results : Among 599 subjects, 136 (23%) had initially abnormal thyroid function test (TFT); transient hypothyroxinemia was the most frequent condition (118, 20%). In addition, 8 (17%) of 46 subjects with initially normal TFT levels showed delayed onset of hyperthyrotropinemia with or without low free thyroxine ($fT_4$). Thyroxine was prescribed for 10 patients (1.7%) due to low $fT_4$ levels but was discontinued in 9 patients during follow-up. Thyroid scan confirmed ectopic thyroid in one patient. Conclusion : Thyroid dysfunction was frequently seen in premature infants, but most of the conditions were transient. In addition, some infants showed delayed TSH elevation on routine follow-up. Therefore, a recheck of the thyroid function of premature infants at 3-4 weeks is recommended, even if normal thyroid function is initially seen, especially in prematurity of less than 33 weeks of gestational age or birth weight of less than 2,500 grams.

Effects of nitrate on thyroid function in rabbits (Nitrate가 가토 갑상선 기능에 미치는 영향)

  • The present study was carried out to investigate the effects of nitrate ($KNO_3$) on the thyroid glands in rabbits which were administrated $KNO_3$ of 1g/kg/day for 6 weeks. Growth rate, and serum levels for $T_4$, $T_3$ and TSH were observed every weeks. The histological changes and the weights of the thyroid glands were observed in 6 weeks. The results obtained were summarized as follows: 1. The mean growth rates of experiments were decreased significantly in the 1st week, but were increased a little from the 5th to 6th week compared with those of controls. 2. The serum levels for $T_4$ of the experimental group manifested significantly decreased values than those of control through the experimental term consistently and the serum levels for $T_3$ were greatly decreased in the 3rd and 4th weeks. 3. TSH contents of the serum were not changed through the experimental term. 4. The mean weight of the thyroid gland in the experiment was decreased significantly after 6 weeks compared with that of control. 5. In the experimental group, the color of colloid in the thyroid follicles revealed deeper eosinophilic and the heights of the follicular epithelial cells were taller than those of controls. The colloid in the thyroid follicles revealed depletion. As summarized above, the observations suggest that nitrate can be an antithyroid substance in rabbits and it leads the thyroid glands to hypofunctional state.

Comparison of Effects of Yangkyuksanhwa-tang, Palmulgunja-tang and Cheongpyesagan-tang on the Rat Hyperthyroidism Induced by Levothyroxine (Levothyroxine으로 유발된 갑상선기능항진증 랫트에 대한 양격산화탕, 팔물군자탕 및 청폐사간탕의 효능 비교연구)

  • Objectives This study's object was to observe the comparative effects of Yangkyuksanhwa-tang, Palmulgunja-tang and Cheongpyesagan-tang on the chronic LT4(levothyroxine) induced hyperthyroidism in rats.Methods Six groups, each of 8 rats in group, were used in this study. Saline and distilled water treated rats are intact control group. Hyperthyroidism was induced by daily subcutaneous LT4 300 μg/kg treatment for 27 days(LT4 control). Since 12th LT4 treatment PTU(propylthiouracil) 10 mg/kg was intraperitoneal injected(PTU group) and aqueous extracts of Yangkyuksanhwa-tang, Palmulgunja-tang and Cheongpyesagan-tang(YS, PG and CS) 500 mg/kg were orally administrated(YS, PG, CS group), once a day for 15 days. The differences in the body, thyroid gland and epididymal fat pad weights, serum T3(tri-iodothyronine), T4(thyroxine), TSH(thyroid-stimulating hormone), thyroid gland and epididymal fat pad histopathology, liver weight, AST(asparte aminotransferase), ALT(alanine aminotransferase) concentrations, hepatic lipid peroxidation, GSH(glutathione), SOD(superoxide dismutase), CAT(catalase) activities, liver histopathology were observed to evaluate effects on hyperthyroidism, liver damages and antioxidant effects.Results As results of LT4 treatment, hyperthyroidism and related liver damages such as lower body, thyroid weights, higher serum T3, T4, AST, ALT levels, thinner follicular lining epithelium in thyroid glands were observed. However, these symptoms were inhibited by oral treatment of YS, PG and CS. As compared with PTU treatment, these herbal prescriptions showed lower overall efficacy on the hyperthyroidism, but YS showed more favorable effects on the hepatic antioxidant defense systems.Conclusions This results suggest that YS, PG and CS favorably control the LT4 induced hyperthyroidism and related liver damages in rats through modulation of the hepatic antioxidative defense systems.

The Effects of Zingiberis rhizoma on Hypothyroidism Rat induced by PTU (건동(乾童)이 PTU로 유발된 Rat의 갑상선 기능저하증에 미치는 영향)

  • Objective : Hypothyroidism is a common disease of the endocrinal system. characterized by fatigue, cold intolerance, bradycardia, and so on. Clinically, Levothyroxine(L-T4) has been usually used for replacement therapy, but it often has side effects. so many hypothyroidism patients wants oriental medical therapy. Zingiberis rhizoma, traditionally has been used in treatment of coldness, fatigue, and bradycardia. In this study. I investigated the therapeutic effects of Zingiberis rhizoma on PTU induced hypothyroidism in rats. Methods : I used two-month-old rats administered PTU and induced with hypothyroidism. After 2 weeks. Zingiberis rhizoma and thyroxine were daily administered, respectively. Body weights was measured every weeks. After 4 weeks, blood samples were taken and analyzed biochemically and T4 and TSH were measured by ELISA kits. Results : In comparison with normal groups, control groups showed hypothyroidism with low T4 and high TSH level. In Zingibris rhizoma administration groups were observed T4 level elevation, this elevation was dependent on the dose of Zingibris rhizoma. Between experimental groups and control groups, there was no difference in TSH level, statistically. Changes of biochemistry were not observed in any experimental groups. Conclusions : These findings suggest that Zingiberis rhizoma makes thyroid cells producing thyroid hormones. There is also a non-toxic effect on the cardiovascular system, liver and kidney function. So, Zingiberis rhizoma should be an effective agents for treating hypothyroidism.

Comparison of Batch Assay and Random Assay Using Automatic Dispenser in Radioimmunoassay (핵의학 체외 검사에서 자동분주기를 이용한 Random Assay 가능성평가)

  • Purpose: Radioimmunoassay (RIA) was usually performed by the batch assay. To improve the efficiency of RIA without increase of the cost and time, random assay could be a choice. We investigated the possibility of the random assay using automatic dispenser by assessing the agreement between batch assay and random assay. Materials and Methods: The experiments were performed with four items; Triiodothyronine (T3), free thyroxine (fT4), Prostate specific antigen (PSA), Carcinoembryonic antigen (CEA). In each item, the sera of twenty patients, the standard, and the control samples were used. The measurements were done 4 times with 3 hour time intervals by random assay and batch assay. The coefficient of variation (CV) of the standard samples and patients' data in T3, fT4, PSA, and CEA were assessed. ICC (Intraclass correlation coefficient) and coefficient of correlation were measured to assessing the agreement between two methods. Results: The CVs (%) of T3, fT4, PSA, and CEA measured by batch assay were 3.2$\pm$1.7%, 3.9$\pm$2.1%, 7.1$\pm$6.2%, 11.2$\pm$7.2%. The CVs by random assay were 2.1$\pm$1.7%, 4.8$\pm$3.1%, 3.6$\pm$4.8%, and 7.4$\pm$6.2%. The ICC between the batch assay and random assay were 0.9968 (T3), 0.9973 (fT4), 0.9996 (PSA), and 0.9901 (CEA). The coefficient of correlation between the batch assay and random assay were 0.9924(T3), 0.9974 (fT4), 0.9994 (PSA), and 0.9989 (CEA) (p<0.05). Conclusion: The results of random assay showed strong agreement with the batch assay in a day. These results suggest that random assay using automatic dispenser could be used in radioimmunoassay.

Effects of Ginseng Radix on the Rat Hypothyroidism Induced by PTU (6-n-propyl-2-thiouracil) (인삼이 PTU (6-n-propyl-2-thiouracil)로 유발된 Rat 갑상선 기능저하증에 미치는 영향)

  • Object : This study was to evaluate the effect of Ginseng Radix, aqueous extracts of the root part of Panax ginseng on the 6-n-propyl-2-thiouracil(PTU)-induced rat hypothyroidism. Methods : Aqueous extracts of Ginseng Radix(GR; yield = 11.70%) were administered, once day for 42 days from 2 weeks before start of PTU treatment as an oral dose of 300 and 150 mg/kg(body weight), and hypothyroidism was induced by daily subcutaneous treatment of PTU 10mg/kg for 28 days. The changes in the body weight, thyroid gland weights, serum levels of thyroid hormone-thyroid stimulating hormone(TSH), tri-iodothyronine(T3) and thyroxine(T4), total cholesterol, low density lipoprotein(LDL), high density lipoprotein(HDL) and triglyceride, liver antioxidant defense system-lipid peroxidation, $H_2O_2$, superoxide dismutase(SOD) and catalase(CAT) were observed with histopathology of thyroid glands. Results : Results were compared with LevoT4 0.5mg/kg treated rats. GR extracts suppressed the decreases in the body weight, thyroid gland weights, T3 and T4, TG, liver $H_2O_2$ and SOD activities as results of PTU treatment. And GR extracts suppressed the increases of HDL contents, liver CAT activities, thyroid gland weight as results of PTU treatment. In addition, marked hyperplasia of follicular cells with decreases of follicular colloid contentsand sizes were demonstrated at histopathological inspections. However, these PTU-induced histopathological changes related to hypothyroidism were dramatically decreased by treatment of both different dosages of GR extract, respectively Conclusions : This study suggest that GR extracts have favorable effects on the thyroid hormone productions with beneficial effects on the hypothyroidism mediated by the modulatory effects on the antioxidant defense system.

Serial Changes of Serum Thyroid-Stimulating Hormone after Total Thyroidectomy or Withdrawal of Suppressive Thyroxine Therapy in Patients with Differentiated Thyroid Cancer (분화성 갑상선 암 환자에서 갑상선 전절제술후 또는 갑상선 호르몬 억제 요법 중단에 따른 갑상선 자극호르몬의 변화)

  • Background: Radioactive iodine (RAI) therapy and whole-body scanning are the fundamentals of treatment and follow-up of patients with differentiated thyroid cancer. It is generally accepted that a Thyroid-Stimulating Hormone (TSH) level of at least 30 ${\mu}U/ml$ is a prerequisite for the effective use of RAI, and that it requires 4-6 weeks of off-thyroxine to attain these levels. Because thyroxine withdrawal and the consequent hypothyroidism are often poorly tolerated, and occasionally might be hazardous, it is important to be certain that these assumptions are correct. We have measured serial changes in serum TSH after total thyroidectomy or withdrawl of thyroxine in patients with thyroid cancer. Subjects and Methods: Serum TSH levels were measured weekly after thyroidectomy in 10 patients (group A) and after the discontinuation of thyroxine in 12 patients (group B). Symptoms and signs of hypothyroidism were also evaluated weekly by modified Billewicz diagnostic index. Results: By the second week, 78% of group A patients and 17% of group B patients had serum TSH levels ${\geq}30{\mu}U/ml$. By the third week, 89% of group A patients and 90% of group B patients had serum TSH levels ${\geq}30{\mu}U/ml$. By the fourth week, all patients in two groups achieved target TSH levels and there were no overt hypothyroidism. Conclusion: in all patients, serum TSH elevated to the target concentration (${\geq}30{\mu}U/ml$) within 4 weeks without significant manifestation of hypothyroidism. The schedule of RAI administration could be adjusted to fit the needs and circumstances of individual patients with a shorter preparation period than the conventional.

A Clinical Study on $^{125}I\;T_3$ Resin Uptake Rate and Serum Thyroxin ($T_4$) in Hyperthyroidism (갑상선기능항진증(甲狀腺機能亢進症)에 있어서 $T_3RU$$T_4$에 관(關)한 임상적(臨床的) 연구(硏究))

  • Hyperthyroidism may be defined as those clinical conditions which result from an increase in the circulating levels of one or both thyroid hormones. Hyperthyroidism in broad sense could be classified with toxic diffuse goiter, toxic adenomatous goiter, and toxic multinodular goiter on the basis of the circulating thyroid hormone levels. For this study, the subject included 94 cases with hyperthyroidism were presented in 77 with toxic diffuse goiter, 8 with toxic adenomatous goiter, and 9 with toxic multinodular goiter on the levels of $^{125}IT_3$ resin uptake rate and serum thyroxine ($T_4$). The observed results were as follows: 1) In the cases of hyperthyroidism including toxic diffuse goiter, toxic adenomatous goiter, and toxic multinodular goiter, 20.21% of the patients were male and 79.79% female. The majority of the patients were in 2nd to 4th decades of their lives. 2) There were objective signs clearly manifested in hyperthyroidism including toxic diffuse goiter and toxic adenomatous goiter which were rare in the multinodular goiter. The clinical signs in toxic diffuse and toxic adenomatous goiter included wide pulse pressure, tachycardia, systolic murmur, exophthalmos, tremor and warm skin etc. (Table 3.) 3) The most freauent complaints of the patients with hyperthyroidism were palpitation, weight loss, increased appetite, perspiration, heat intolerance, nervousness, exertional dyspnea, and menstrual disturbance etc. (Table 4.) There was no clear difference in the incidence of symptoms between toxic diffuse goiter and toxic adenomatous goiter, but there was clear difference between toxic multinodular goiter. 4) Considering of results of $^{125}IT_3$ resin uptake rate and serum $T_4$ level in toxic diffuse goiter, toxic adenomatous goiter and toxic multinodular goiter, $^{125}I\;T_3$ resin uptake rate was $49.15{\pm}9.94%$ (mean) and serum $T_4\;21.29{\pm}7.04ug/dl$ (mean) in toxic diffuse goiter. In toxic multinodular goiter, $^{125}I\;T_3$ resin uptake rate was $32.47{\pm}6.74%$ (mean) and serum $T_4$ level $11.03{\pm}5.0ug/dl$, and then there was clear difference in the results of $^{125}I\;T_3$ resin uptake rate and serum $T_4$ between toxic diffuse goiter and toxic multinodular goiter. The levels of $^{125}I\;T_3$ resin uptake rate and serum $T_4$ in toxic adenomatous goiter were $40.32{\pm}13.08%$ (mean), $15.47{\pm}8.25ug/dl$ (mean) respectively, so there was no clear difference between toxic diffuse goiter and toxic adenomatous goiter. 5) There was no significant differnece in length and width performed with thyroid scanning in toxic diffuse goiter, toxic adenomatous goiter, and toxic multinodular goiter.

Hormonal Profiles during Periparturient Period in Single and Twin Fetus Bearing Goats

  • The effect of fetal number (single or twin) on plasma concentrations of progesterone, estradiol $17{\beta}$, cortisol, prolactin, growth hormone, triiodothyronine, thyroxine and insulin around parturition (periparturient period) were studied on ten $Alpine{\times}Beetle$ crossbred goats in their first to third lactation. The hormone profiles were studied on days -20, -15, -10, -5, -4, -3, -2, -1 prior to kidding and on day 0 and +1, +2, +3, +4, +5, +10, +15, +20 days postkidding. Plasma progesterone levels were significantly (p<0.01) higher in twin bearing goats comparison to single bearing goats during all the days of sampling. The decline in progesterone concentration from day 20 to day 1 before kidding was 56% in twin and 42% in single bearing goats. In single bearing goats plasma estradiol $17{\beta}$ was significantly (p<0.01) higher during prekidding days compared to twin bearing goats. The level of estradiol $17{\beta}$ was highest on the day of kidding in both the groups. The plasma prolactin level in twin bearing goats from day 10 to day 1 prepartum was higher as compared to single fetus bearing goats. However there was abrupt increase in prolactin level on the day of kidding in both the groups. The plasma growth hormone levels were significantly (p<0.01) higher in twin compared to single bearing goats. On the day of kidding growth hormone levels were significantly (p<0.01) higher in twin as compared to single bearing goats (1.40 vs. 0.95 ng/ml). In twin bearing goats plasma cortisol values from day 5 till the day of kidding remained elevated and the levels on the day of kidding was significantly highest in both the groups. The levels of triiodothyronine ($T_3$) were significantly higher (p<0.01) during all the periods of sampling in single compared to twin bearing goats. Plasma thyroxine ($T_4$) was significantly (p<0.01) lower in twin compared to single bearing goats. In single bearing goats plasma insulin levels were significantly (p<0.01) higher than twin bearing goats during prepartum period however during post partum period the levels in both the groups remained similar. It can be concluded that number of fetuses is having significant influence on the hormone profile during periparturient period.

THYROID HORMONES AND TESTOSTERONE IN SHEEP AGE RELATED PROFILES OF SERUM THYROXINE, TRI-IODOTHYRONINE AND TESTOSTERONE IN KAGHANI, RAMBOUILLET AND KAGHANI × RAMBOUILLET SHEEP

  • Secretory patterns of serum tri-iodothyronine, thyroxin and testosterone were analysed by specific radioimmunoassay in Kaghani (n = 42), Rambouilet (n = 42) and Kaghani ${\times}$ Rambouillet (n = 42) male sheep to determine age and breed related changes. Animals were examined at 1-2 wk, 8, 10, 12, 20, 22 and 24 months of age (n = 6/age group). All animals were maintained under standard conditions of feeding and management. $T_3$ and $T_4$ concentrations were maximal (p < 0.01) in 1-2 wk old lambs of all breeds studied ($207.4{\pm}14.9-276.8{\pm}20.4ng/dl$ and $5.76{\pm}0.3-7.23{\pm}0.9{\mu}g/dl$, respectively). For all the breeds, concentrations of $T_3$ declined markedly (p < 0.01) at 8 month of age and then varied in a narrow range ($44.5{\pm}14.0-87.0{\pm}4.8ng/dl$) up to the age of 24 month. While, serum $T_4$ concentrations decreased (p < 0.01) to nadir ($0.97{\pm}0.1-1.43{\pm}0.3{\mu}g/dl$) at 10 months of age in all the 3 breeds. Afterwards, $T_4$ levels increased gradually and attained a small peak at 20 months before declining again at 22 and 24 months of age. Serum $T_3$ and $T_4$ concentrations were correlated significantly (p < 0.05) and their ratio was constant at all ages in the different breeds. Age significantly (p < 0.01) influenced the secretion of testosterone in the breeds examined but the breed differences were comparable. Mean serum concentrations of testosterone were low from 1-2 wk to 10 months of age in the Kaghani, Rambouillet and Kaghani ${\times}$ Rambouillet animals. Peak testosterone levels were noted in the 12 months old Kaghani ($1.75{\pm}0.4ng/ml$), Rambouillet ($1.30{\pm}0.2ng/ml$) and their $T_3$ cross animals ($1.16{\pm}0.08ng/ml$). Mean testosterone levels, thereafter, decreased at 20 months and then at 22 and 24 months remained variable but higher than observed at 1-2 wk -10 months of age. The findings of the present study indicate a significant influence of age but not of breed on the peripheral concentrations of thyroid hormones and testosterone. Serum concentrations of $T_3$ and $T_4$ were not related to testosterone levels. High thyroid secretory activity observed at prenatal phase of development may play an important role in preparing the lambs to extrauterine environments.
