Variation in root canal morphology, especially in maxillary first molar presents a constant challenge for a clinician in their detection and management. This case report describes the successful root canal treatment of a three rooted right maxillary first molar presenting with three canals each in the mesiobuccal and distobuccal roots and one canal in the palatal root. The clinical detection of this morphologic aberration was made using a dental operating microscope, and the canal configuration was established after correlating and computing the clinical, radiographic and cone-beam computed tomography (CBCT) scan findings. CBCT images confirmed the configuration of the canals in the mesiobuccal and distobuccal roots to be Al-Qudah and Awawdeh type (3-2) and type (3-2-1), respectively, whereas the palatal root had a Vertucci type I canal pattern. This report reaffirms the importance of careful examination of the floor of the pulp chamber with a dental operating microscope and the use of multiangled preoperative radiographs along with advanced diagnostic aids such as CBCT in identification and successful management of aberrant canal morphologies.
The mesiobuccal root of the maxillary molars is well known to pose a hindrance during endodontic therapy. Presented here is a case of a maxillary left second molar where three canals were located in its mesiobuccal root with the use of visual and diagnostic aids. Difficulties encountered during the process of unveiling the tooth's internal anatomy were discussed. The dilemmas encountered pertained to the root canal configuration, the nomenclature of the extra canals, and the justification for the presence of a third canal. The root canal configuration of 3-2-1 was confirmed for the mesiobuccal root using information gained from clinical, radiographic, and multidetector computed tomography (MDCT) scan findings. This case demonstrates the need for efforts to locate extra canals in the mesiobuccal root of the maxillary molars as their internal anatomy remains a mystery.
The purpose of this study was to examine the sealing ability of the vitapex, when used with gutta-percha cone, as a root canal filling material. Fourty five canals from extracted human maxillary and mandibular teeth were randomly selected and instrumented in a conventional method with k-file. After instrumentation and dry the canal with paper points, the canals were divided into three groups and fifteen canals in each group were filled with the following materials; Vitapex, Vitapex in combination with gutta-percha cone, and Gutta-percha cone and Zinc-oxide Eugenol Cement. All the specimens were immersed in 2% methylenblue dye solution and the depth of dye penetration into the canals were evaluated by macroscope at the intervals of 1 day, 2days and 7days. The following results were obtained; 1. All the materials experimented showed varying degrees of dye penetration. 2. The canals filled with Vitapex and Vitapex in combination with gutta perch a cone revealed sudden increase of dye penetration with time passage compared to the canals obturated with Gutta-percha cone and Zinc-oxide eugenol cement. 3. In the canals filled with Vitapex, the mean dye penetration was 1.6mm at 1day, but the specimen exposed to the dye for 7days showed mean dye penetration of 9.2mm. 4. In the canals obturated with Vitapex and gutta-percha cone, the mean dye penetration was 2mm at 1day, 2.2mm at 2days, and 8mm at 7days.
Purpose: The aim of this in vitro study was to determine the sensitivity and specificity of cone-beam computed tomography (CBCT) and digital periapical radiography in the detection of mesial root perforations of mandibular molars. Materials and Methods: In this in vitro study, 48 mandibular molars were divided into 4 groups. First, the mesial canals of all the 48 teeth were endodontically prepared. In 2 groups (24 teeth each), the roots were axially perforated in the mesiolingual canal 1-3 mm below the furcation region, penetrating the root surface ("root perforation"). Then, in one of these 2 groups, the mesial canals were filled with gutta-percha and AH26 sealer. Mesial canals in one of the other 2 groups without perforation (control groups) were filled with the same materials. The CBCT and periapical radiographs with 3 different angulations were evaluated by 2 oral and maxillofacial radiologists. The specificity and sensitivity of the two methods were calculated, and P<0.05 was considered significant. Results: The sensitivity and specificity of CBCT scans in the detection of obturated root canal perforations were 79% and 96%, respectively, and in the case of three-angled periapical radiographs, they were 92% and 100%, respectively. In non-obturated root canals, the sensitivity and specificity of CBCT scans in perforation detection were 92% and 100%, respectively, and for three-angled periapical radiographs, they were 50% and 96%, respectively. Conclusion: For perforation detection in filled-root canals, periapical radiography with three different horizontal angulations would be trustworthy, but it is recommended that CBCT be used for perforation detection before obturating root canals.
The purpose of this study was to evaluate the canal preparation time, deviation of the apex (Zipping), and debridement effect of the canal of three canal preparation techniques. Thirty extracted 1st and 2nd molars were divided into 3 groups and each group was enlarged by ultrasonic, sonic or hand instrument. The specimens were examined under scanning electron microscope and light microscope. The results were as follows: 1. In the canal preparation time, ultrasonic group was the shortest, followed by sonic, hand group. All pairs of groups were significantly different at the 0.05 level. (P < 0.05) 2. In the deviation of the canal, no two groups were significantly different at the 0.05 level. (P > 0.05) 3. In the evaluation of debris scores, ultrasonic and sonic groups were better debridement effect than hand group in the fine canals. In the large canals, ultrasonic group was the best results, followed by sonic, hand group. 4. In the evaluation of smear layer scores, ultrasonic group was the best and sonic and hand group were same effect in the fine canals. In the large canals, three groups were same effect. 5. In the effect of the removal of predentin and pulpal debris, in the regardless of canal size, three groups were same and pulpal debris was not completely removed by either technique.
A retrospective in vivo study of endodontically treated mandiblar molars was conducted. The radilographs and clinical records of mandibular molars that had been treated endodontically in the Dental hospital of Yonsei University from 1986 to July 1997 were reviewed and categorized. A total of 1751 cases were studied and following results were obtained. 1. The number of root canals in mandibular first permanent molar in a Korean population revealed three in 53.2 %, four in 23.0 %, two in 18.4 %, C-shaped canal in 5.0 %, and one in 0.4 %. 2. In mandibular second molar, three canals was observed in 53.2 %, four in 22.2 %, two in 14.9%, C-shaped canal in 9.1 %, and one in 0.6%. 3. In the results of the percentage of 4 canals of mandibular first molars at various age, before age 20, 37.7 % was obtained whereas 19.3 % after 30. 4. 7.94 % of mandibular first molars examined were found to have three roots. 5. The mandibular second molar had C-shaped canals in 9.1 %.

This paper compares the estimated total construction and maintenance costs for irrigation canals in six irrigation districts near Pyongtaek. Three conventional canal types were considered: concrete bench flumes, concrete-lined canals, and earth canals. The total expenses for each type were grouped to four catagories: the net construction, land acquisition, maintenance, and other expenses. The construction costs included the assumed costs for conveyance losses from each type of canals. The results from this study support earlier studies that the bench flumes are economically feasible. Initially cheaper conveyance systems like concrete-lined and earth canals demand 'higher maintenance costs and thus, their total expenses are summed to exceed those for the bench flumes within a few years following the construction. Banch flumes are economically justifiable for the main canals of the studying districts. Sensitivity analyses were executed to evaluate the relative importance of each expense to the total cost. The analyses show that total expenses very significantly with the change of the following costs: cost for conveyance losses, net construction, maintenance, and land acquisition, in that order However, other expenses contribute little if not at all to the total. The results indicate that bench flumes should be adopted as main canal structures for most irrigation districts in the Republic.
The aim of this study was to compare shaping ability of nickel-titanium rotary files with different taper in simulated resin canals. The instruments used in this study were GT with .04 taper ProFile with ISO-sized tip, .04 taper ProFile with ISO-sized tip alone, and .02 taper Naviflex file and all canals were prepared by crown-down technique. A total of 30 composite images were made from pre- and post-canal scanned images using Scanjet 4C scanner and Corel photopaint 8.0 and then, prevalence of canal aberrations was measured. The amount of coronal substance the instruments removed was also calculated two-dimensionally on digitized images with the Brain C software to compare the relative enlarging efficiency. Finally, the prepared internal canal form was evaluated three-dimensionally with impression technique. The results were as follows; 1. The incidence of zipping in Naviflex group was significantly high compared to the other two groups(P<.05). 2. The amount of coronal substance the GT combined group removed was significantly larger than the other two groups(P<.05). 3. The GT combined group produced canals with good taper and flow. Under the conditionas of this study, the combined use of larger tapered nickel-titanium rotary files prepared simulated canals safely, efficiently and with good three-dimensional form.
This study was conducted to evaluate the cleansing effect of RC-Prep (10% Urea-peroxide, 15% EDTA) on apical third of root canal. Thirty single rooted human teeth were divided into three groups, and ten canals in each group were all enlarged three sizes greater than their original diameter with K-type files and irrigated with each of three irrigants. The three used irrigants were RC-prep (Premier Co.) in combination with 3.5% Sodium hypochlorite, Normal saline, 3.5% Na OCL. In each group, one of three irrigants were used in conjunction with instrumentation as they would be during clinical condition. After final irrigation, the canals were dried with paper points and the teeth were split longitudinally. The cleanness of canal walls iii the apical region were examined with Scanning Electron Microscope. The following results were drawn. 1. The use of Rc-Prep in combination with 3.5% NaOCl showed more clean canal surface than the use of other two irrigants. 2. The canals used RC-Prep in combination with 3.5% NaOCl revealed remnants of pulp tissue and smeared layer, but the openings of dentinal tubules were relatively clean and wide. 3. There was no significant difference in the debridement effect of 3.5% NaOCl and Saline solution. 4. The use of Saline solution showed great amount of remnants of pulp tissue which couldn't find in the use of 3.5% NaOCl.
Purpose: This study was performed to compare the accuracy of micro-computed tomography (CT) and cone-beam computed tomography (CBCT) in detecting accessory canals in primary molars. Materials and Methods: Forty-one extracted human primary first and second molars were embedded in wax blocks and scanned using micro-CT and CBCT. After the images were taken, the samples were processed using a clearing technique and examined under a stereomicroscope in order to establish the gold standard for this study. The specimens were classified into three groups: maxillary molars, mandibular molars with three canals, and mandibular molars with four canals. Differences between the gold standard and the observations made using the imaging methods were calculated using Spearman's rho correlation coefficient test. Results: The presence of accessory canals in micro-CT images of maxillary and mandibular root canals showed a statistically significant correlation with the stereomicroscopic images used as a gold standard. No statistically significant correlation was found between the CBCT findings and the stereomicroscopic images. Conclusion: Although micro-CT is not suitable for clinical use, it provides more detailed information about minor anatomical structures. However, CBCT is convenient for clinical use but may not be capable of adequately analyzing the internal anatomy of primary teeth.
