  • The purpose of this literature review was to identify the physiological changes with aging. The results of this renew were to follows : We have seen a wide variety of such changes in the cells and in all organ systems. 1. At least 40 percent of people over 65 will die of cardiac disease, 15 percent of cerebrovascular disease, and possibly another 5 percent of other types of vascular impairment. 2. The increase rigidity of the thoracic wall and the decreased strength of the expiratory muscles decrease the propulsive effectiveness of the cough. 3. The density of capillaries per motor units is decreased. 4. Starting before age 40 in both sexes there is a shift from an increase in bone mass to a progressive decrease. 5. Histologic studies show a lim age-correlated decrease in the number of Pacini's. Merkel's, and Meissner's corpuscles.

기관 평활근종의 수술적 절제;1례 보고 (Surgical Resection of Tracheal Leiomyoma; A cas report)

  • Primary benign tracheal tumors are exceedingly rare and leiomyoma has the least frequency among them. It is important to recognize early without misdiagnosis such as asthma because it is curable. A 41 year-old female was admitted for dyspnea and choking since November 1991. Under the impression of asthma, she received medication. Symptom was not improved and thus chest CT scan was performed. There was endotracheal tumor mass which was located just above the carina and arose from the right lateral tracheal wall with broad base. We successfully resected the tumor mass including trachea and the defected area of trachea was reconstructed with autologous graft using pericrdium & rib cartilage. During the follow up period, no complication was developed.

심장내 악성 섬유성 조직구종 (Malignant Fibrous Histiocytoma of the Heart A Case Report and Review of the Literature)

  • A 45-year old woman with congestive heart failure due to left atrial tumor was operated on. Three tumor masses arising from posterior wall, atrial septum, anterior portion of mitral valve were resected. Pathological diagnosis was malignant fibrous histiocytoma [MFH]. Above one case and sixteen previous reports are reviewed. Eleven cases out of 17 were females. The tumors all originated primarily in the left atrium and 8 had distant metastases. The metastatic sites are lung [4 cases], brain [2 cases], liver, jejunum, cervix and pleura etc. Careful pathologic study is necessary to differentiate the uniformly fatal MFH of the heart from the more common benign atrial myxoma. The treatment modalities are surgical resection, chemotherapy, and radiation therapy & the prognosis of intracardiac MFH is poor. We underwent partial resection of left atrial MFH and obtained symptom relief and patient still alive 7 months post-operatively in state of NYHA class II.

선천성 횡격막 무발육증 수술치험 1례 (Congenital Agenesis of Left Diaphragm: Surgical Repair - Report of a Case -)

  • Agenesis of the hemidiaphragm is unusual congenital anomaly associated with a high mortality. A case of congenital agenesis of left diaphragm was experienced in 22-day old male patient who was dyspneic and cyanotic on admission. Emergency exploration through the left eight interspace thoracotomy showed complete agenesis of the left diaphragm. The stomach and transverse colon covered with peritoneal sac was partially herniated into left hemithorax. The left lung was slightly hypoplastic. This neonate had no intestinal malrotation. The defect was reconstructed using Dacron graft. Dacron patch was sutured with interrupted Ethibond to chest wall anteriorly, esophagus aorta and costomediastinal sinus medially, and the tenth rib posterolaterally. Postoperatively, Extubation was performed at 1st day, but some respiratory difficulty was noted. Severe dyspnea was occurred at postoperative 11th day and so reintubation was done. Intermittently ventilatory support and intravenous alimentation were continued for 9 days after that. Thereafter he had no respiratory problems at discharge.

Bentall 씨 수술 치험 -1례 보고- (Bentall`s Operation of Ascending Aorta Aneurysm with Aortic Regurgitation - Report of One Case -)

  • We have experienced one case of ascending aorta aneurysm with aortic regurgitation due to atherosclerosis. The 45 year old man had been suffered from palpitation and precordial chest pain. 2-D echocardiogram and aortogram confirmed aneurysm of ascending aorta with aortic regurgitation. Atherosclerotic change was noted in the aortic wall and there was marked dilatation of the sinuses of Valsalva as well as the aortic annulus with upward displacement of coronary ostia in the operative field. The patient underwent complete replacement of the aneurysmal ascending aorta and the aortic valve with 27mm Bjork-Shiley aortic valve composite graft. We got preclotting with heparin free blood including thrombin and then autoclave at 132` for 3 minutes. The postoperative course was uneventful and the patient was discharged with good clinical result.

  • 폐의 카르시노이드 종양은 기관지 상피의 Kulchitsky 세포에서 유래하는 신경내분비 종양이다. 폐의 카르시노이드는 대개는 중심부에 위치한다. 그러나, 비전형적 카르시노이드는 주변부에 위치하기도 하며 조직학적으로 악성도가 높다 흉막에서 기원한 카르시노이드에 대한 보고는 거의 없는 상태로 저자들은 벽측 흉막에 위치하며 폐실질내로의 침윤이 없이 흉벽을 침윤하는 전형적인 카르시노이드를 보고하는 바이다.

  • Within the group of congenital cardiac anomalies manifesting dextrocardia and double-outlet right ventricle, ventricular non-inversion [S.D.L] is extremely rare. Recently, a 5 year-old boy underwent a successful surgical correction of dextrocardia associated with double-outlet right ventricle, ventricular non-inversion [S.D], ventricular septal defect, pulmonary stenosis, and patent foramen ovale. The operation consisted of construction of an internal baffle connecting the left ventricle to the aorta through the large ventricular septal defect [subaortic]. The pulmonary stenosis was managed by infundibulectomy and patch enlargement of the right ventricular wall. The patient`s postoperative recovery has been uneventful, and 2 months after the operation, he is doing well.

기계적판막의 임상적 고찰 (Postoperative Clinical Evaluation of Mechanical Valve Replacement)

  • In the department of chest surgery of WonKwang university hospital, mechanical valve replacement was performed in 51 cases from June 1985 to September 1987. Among these, 32 cases were mitral valve replacement, 4 cases were aortic valve replacement, and 15 cases were double valve replacement. 26 cases were male and 25 cases were female and age distribution ranged from 16 years old to 63 years old. Early death within 30 days after operation was 2 cases [3.9%] and causes of death were right heart failure [1] and right ventricular wall rupture [1]. Among 49 early survivors, 2 cases of late death were developed and the causes of death were cardiomyopathy [1] and ventricular arrhythmia [1] Anticoagulant therapy was done with warfarin sodium to the level of 1.5-2 times of normal prothrombin time [20-40%] in 47 survivors. Symptomatically, 93.6% of preoperative NYHA functional class III or IV were converted to the NYHA functional class I or II during follow up.

개에서 유방절제술에 의한 피부결손의 Z-plasty응용 (Application of Z-plasty for Skin Defects by Mastectomy in a Dog)

  • A fourteen-year-old mongrel female dog that was 22 kg and had two large mammary tumor lesions was admitted to the Animal Teaching Hospital of Chonbuk National University. Two large mammary tumor lesions were observed in the right caudal thoracic and the left inguinal mammary glands, which were sized 6 cm and 5 cm in diameter, respectively. These tumor lesions were removed by block mastectomy, then large skin defect of 7 cm diameter in caudal abdominal wall were recoverd by classic Z-plasty in both medial thigh. Classic Z-plasty was designed with 60 in angle and, with 6 cm in length of central limb in right medial thigh and 4 cm in left, respectively. This patient had been recoverd to normal gait and behavior at 20 days after the plastic operation.

기관지성 낭종 2례 치험 보고 (Bronchogenic Cyst, A Report of 2 cases [An Intraesophageal cyst and A Multiloculated cyst])

  • Bronchogenic cysts are a congenital cystic lesion which are usually found within the lung parenchyme or mediastinum. Two cases of bronchogenic cysts were presented and related literatures were reviewed. The first case of bronchogenic cyst was located in the wall of the esophagus. Preoperatively, this case was thought duplication cyst of esophagus, but postoperative microscopic examination showed the tumor was a bronchogenic cyst with respiratory epithelium. The second case had double cysts; one in the superior and posterior mediastinum, the other in the lung parenchyme. The cyst in the mediastinum was extirpated and the other cyst in the lung was removed by right upper lobectomy. Postoperative course were uneventful in both patients.
