Clinical Experiences and Usefulness of Cervical Posterior Stabilization with Polyaxial Screw-Rod System

  • Objective : The objective of this study is to investigate the safety, surgical efficacy, and advantages of a polyaxial screw-rod system for posterior occipitocervicothoracic arthrodesis. Methods : Charts and radiographs of 32 patients who underwent posterior cervical fixation between October 2004 and February 2006 were retrospectively reviewed. Posterior cervical polyaxial screw-rod fixation was applied on the cervical spine and/or upper thoracic spine. The surgical indication was fracture or dislocation in 18, C1-2 ligamentous injury with trauma in 5, atlantoaxial instability by rheumatoid arthritis (RA) or diffuse idiopathic skeletal hyperostosis (DISH) in 4, cervical spondylosis with myelopathy in 4, and spinal metastatic tumor in 1. The patients were followed up and evaluated based on their clinical status and radiographs at 1, 3, 6 months and 1 year after surgery. Results : A total of 189 screws were implanted in 32 patients. Fixation was carried out over an average of 3.3 spinal segment (range, 2 to 7). The mean follow-up interval was 20.2 months. This system allowed for screw placement in the occiput, C1 lateral mass, C2 pars, C3-7 lateral masses, as well as the lower cervical and upper thoracic pedicles. Satisfactory bony fusion and reduction were achieved and confirmed in postoperative flexion-extension lateral radiographs and computed tomography (CT) scans in all cases. Revision surgery was required in two cases due to deep wound infection. One case needed a skin graft due to necrotic change. There was one case of kyphotic change due to adjacent segmental degeneration. There were no other complications, such as cord or vertebral artery injury, cerebrospinal fluid leak, screw malposition or back-out, or implant failure, and there were no cases of postoperative radiculopathy due to foraminal stenosis. Conclusion : Posterior cervical stabilization with a polyaxial screw-rod system is a safe and reliable technique that appears to offer several advantages over existing methods. Further biomechanical testings and clinical experiences are needed in order to determine the true benefits of this procedure.

둔상에 의한 심타박상과 심좌상의 임상적 고찰 (A Clinical Analysis of 24 cases of Cardiac Contusion and Cardiac Concussion)

  • 배경: 저자들은 흉부 손상환자에서 심손상이 예상되는 경우 심전도, 혈중내 CK, CK-MB분획 변화를 선별하여 이상이 있는 경우 이면성 심초음파검사를 실시하여 이상유무로 심타박상 및 심좌상으로 분류하여 임상적 고찰을 해보기로 하였다. 대상 및 방법: 1997년 1월부터 1998년 3월까지 15개월간 심손상이 의심되어 본교실에 입원치료한 24명을 대상으로 병력, 내원당시 심전도 및 혈중내 CK, CK-MB 분획을 연속적 선별검사를 실시하여 이상이 있는 경우 심초음파검사를 하였다. 결과: 연령분포는 20∼40대가 58.3%로 가장 많았으며, 남여비는 3 : 1 이었다. 원인은 교통사고가 15례(62.5%)로 가장 많았다. 동반손상의 경우 다발성 늑골골절, 흉골골절 순이었다. 심전도 검사상 심타박상의 경우 정상이 가장 많았으며, 심좌상에서는 ST-T변화가 가장 많았다. CK-MB분획은 심좌상에서 높았으며, 내원 당일은 통계학적 유의성은 없었으나 내원 1, 2, 3일에는 통계학적 유의성이 있었다. 재원기간은 심타박상에서 평균 9.22일이었고, 심좌상에서 26.18일이었다(p=0.0075). 합병증은 급성폐부전이 7례로 가장 많았으며, 이중 5례에서는 인공호흡기 치료를 하였으며 사망한 경우는 없었다. 결론: 저자들은 흉부손상환자에서 심손상이 의심되는 경우 심전도, CK, CK-MB분획을 연속 선별검사하여 이상이 있는 경우 심초음파를 실시하여 심좌상의 유무를 진단하는 것이 좋을 것으로 사료된다.

흉골 골절 환자에서 심혈관계 동반 손상의 예측 인자의 분석 (Analysis of Predicting Factors for Cardiovascular Injuries in Sternal Fractures)

  • 배경 : 본 연구의 목적은 흉골골절에서 심혈관계 동반 손상의 예측인자를 알아내고 응급 심장 초음파검사의 적응증을 제시하고자 하였다. 대상 및 방법 : 1994년 3월부터 5년간 이화여자대학교 부속 목동병원 응급실을 통해 입원한 흉골골절로 입원한 40명의 환자들을 대상으로 후향적 분석을 시행하였다. 예측인자로서는 1) 안전띠 착용의 유무, 2) 동반 손상의 유무, 3) 심혈관계 질환의 병력 유무, 4) 내원시 환자의 RTS(Revised Trauma Score) 4가지를 대상으로 분석하였다. 또한 심혈관계 손상의 동반 유무를 파악하기 위하여 일반적으로 사용하는 심전도와 흉부 단순 촬영 및 심근 효소의 분획을 조사하였으며, 그 결과에서, 흉골골절에서 응급 심장 초음파 검사의 적응증을 추론하였다. 결과 : 흉골골절에서 심혈관계 손상의 동반 유무를 나타내는데 통계적으로 의미가 있다고 나온 인자로는 심혈관계 과거 병력이 있는 경우와 입원시 RTS의 이상을 들 수 있다. 응급 심장 초음파검사의 시행은 예측인자들과 심전도와 흉부 단순 촬영 및 심근 효소의 분획 같은 일반적으로 사용하는 검사 결과에 따라 시행하였다. 흉골골절 환자에서 응급 심장 초음파검사의 적응으로 1) 의미 있는 예측 인자의 변화가 없더라도 두 개 이상의 검사에서 이상소견을 보이던가, 2) 예측인자의 의미있는 변화가 하나 이상 나오는 경우를 들 수 있다. 결론 : 흉골골절 환자에서 심혈관계의 과거 병력과 초기활력 징후가 심혈관 동반 손상 유무를 중요하게 나타날 수 있으며, 필요한 경우, 응급 심장 초음파 검사를 시행하여 조기에 확진을 내리는 것이 중요하다고 생각한다.

Concise Bedside Surgical Management of Profound Reperfusion Injury after Vascular Reconstruction in Severe Trauma Patient: Case Report

  • For an orthopaedic surgeon, the critical decisions to either amputate or salvage a limb with severe crushing injury with progressive ischemic change due to arterial rupture or occlusion can become a clinical dilemma at the Emergency Department (ED). And reperfusion injury is one of the fetal complications after vascular reconstruction. The authors present a case which was able to save patient's life by rapid vessel ligation at bedside to prevent severe reperfusion injury. A 43-year-old male patient with no pre-existing medical conditions was transported by helicopter to Level I trauma center from incident scene. Initial result of extended focused assessment with sonography for trauma (eFAST) was negative. The trauma series X-rays at the trauma bay of ED showed a multiple contiguous rib fractures with hemothorax and his pelvic radiograph revealed a complex pelvic trauma of an Anterior Posterior Compression (APC) Type II. Lower extremity computed tomography showed a discontinuity in common femoral artery at the fracture site and no distal run off. Surgical finding revealed a complete rupture of common femoral artery and vein around the fracture site. But due to the age aspect of the patient, the operating team decided a vascular repair rather than amputation even if the anticipated reperfusion time was 7 hours from the onset of trauma. Only two hours after the reperfusion, the patient was in a state of shock when his arterial blood gas analysis (ABGA) showed a drop of pH from 7.32 to 7.18. An imminent bedside procedure of aseptic opening the surgical site and clamping the anastomosis site was taken place rather than undergoing a surgery of amputation because of ultimately unstable vital sign. The authors would like to emphasize the importance of rapid decision making and prompt vessel ligation which supply blood flow to the ischemic limb to increase the survival rate in case of profound reperfusion injury.

Is the Agricultural Work a Risk Factor for Koreans Elderly Spinal Sagittal Imbalance?

  • Objective : A primary degenerative sagittal imbalance has been considered because of unique lifestyles such as the prolonged crouched posture during agricultural work and performing activities of daily living on the floor. Previous papers have reported that sagittal imbalance disease is often seen distinctly in the farming districts of "oriental" countries such as Korea and Japan. However, this finding was only evaluated with the use of X-ray, and other factors such as magnetic resonance imaging (MRI), muscle volume, compression fracture, and laboratory results were not considered. Thus, using these, we evaluate the agricultural work-associated factors for Korean elderly spinal sagittal imbalance. Methods : We recruited 103 Korean participants who had a sagittal vertical axis (SVA) of >5 cm in this Korean Elderly Sagittal Imbalance Cohort Study. The following were evaluated : radiological parameters, MRI, compression fracture, vitamin D, parathyroid hormone, C-terminal telopeptide, osteocalcin, bone mineral density and muscle fatty change, muscle volume, and health-related quality of life from patients' survey. Moreover, in this survey, the farmers' annual working hours were investigated. Subsequently, we analyzed the associated factors for spinal sagittal imbalance depending on occupation. Results : A total of 46 participants were farmers, and the others were housewives, sellers, and office workers. The farmer group had more SVA (141 vs. 99 mm, p=0.001) and pelvic tilt (31° vs. 24°, p=0.004) and lesser lumbar lordosis (20° vs. 30°, p=0.009) and thoracic kyphosis (24° vs. 33°, p=0.03) than non-farmer group. A significantly positive correlation was noted between the working hour and SVA in the farmer group (p=0.014). The visual analogue scale score for back pain (8.26 vs. 6.96, p=0.008) and Oswestry Disability Index (23.5 vs. 19.1, p=0.003) in the farmer group were higher than that in the non-farmer group, but the Short Form-36 score was not significantly different between the two groups. The Mini-Mental State Exam score was significantly lower in the farmer group than in the non-farmer group (24.85 vs. 26.98, p=0.002). Conclusion : The farmer group had more sagittal imbalance and back pain in proportion to the working hours even though the muscle and bone factors and general laboratory condition were not significantly different between the two groups. These results supported that the long hours spent in the crouched posture while performing agricultural work were a risk factor for severe sagittal imbalance.

흉·요추 불안정성 척추 손상 환자에서 전방 감압술과 전방기기 및 Surgical Titanium Mesh를 이용한 내고정술 (장기적 추적 검사 결과) (Anterior Decompression and Internal Fixation with Anterior Instrument and Surgical Titanium Mesh in Thoracolumbar Unstable Spine Injuries (Long-term Follow-up Results))

  • Objective : Thoracolumbar junction is second most common level of injury next to cervical spine. The object of this study is to study the usefulness of surgical titanium mesh instead of bone graft, as well as to evaluate the correction of spinal deformity and safety of early ambulation in patients with injury at thoracolumbar junction. Patients and Methods : This review included 51 patients who were operated from July 1994 to December 1997. The injured spine is considered to be unstable, if it shows involvement of two or more columns, translatory displacement more than 3.5mm, decrease more than 35% in height of vertebral body and progression of malalignment in serial X-ray. The decision to operate was determined by (1) compression of spinal cord or cauda eguina, (2) unstable fracture, (3) malalignment and (4) fracture dislocation. The procedure consisted of anterior decompression through corpectomy and internal fixation with anterior instrument and surgical titanium mesh which was impacted with gathered bone chip from corpectomy. Results : Fifty-one patients were followed up for at least 12 months. The main causes of injury were fall and vehicle accident. The twelfth thoracic and the first and the second lumbar vertebrae were frequently involved. Complete neural decompression was possible under direct vision in all cases. Kyphotic angulation occurred in a patient. Radiologic evaluation showed correction of deformity and no distortion or loosening of surgical titanium mesh with satisfactory fixation postoperatively. Conclusions : We could obtain neurological improvement, relief of pain, immediate stabilization and early return to normal activities postoperatively. Based on these results, authors recommend anterior decompression and internal fixation with surgical titanium mesh in thoracolumbar unstable spine injuries.

Analysis of the Risk Factors for Unfavorable Radiologic Outcomes after Fusion Surgery in Thoracolumbar Burst Fracture : What Amount of Postoperative Thoracolumbar Kyphosis Correction is Reasonable?

  • Objective : The aims in the management of thoracolumbar spinal fractures are not only to restore vertebral column stability, but also to obtain acceptable alignment of the thoracolumbar junction (T-L junction) to prevent complications. However, insufficient surgical correction of the thoracolumbar spine would be likely to cause late progression of abnormal kyphosis. Therefore, we identified the surgical factors that affected unfavorable radiologic outcomes of the thoracolumbar spine after surgery. Methods : This study was conducted in a single institution from January 2007 to December 2013. A total of 98 patients with unstable thoracolumbar spine fracture were included. In these patients, fixation was done through transpedicular screws with rods by three surgical patterns. We reviewed digital radiographs and analyzed the images preoperatively and postoperatively during follow-up visits to compare the change of the thoracolumbar Cobb angle with radiologic parameters and clinical outcomes. The unfavorable radiologic group was defined as the patients who were measured as having greater than 20 degrees of thoracolumbar Cobb angle on the last follow-up, or who underwent kyphotic progression of thoracolumbar Cobb angle greater than 10 degrees from the immediate postoperative state to final follow-up, or who had overt instrument failure with/without additional surgery. We assessed the risk factors that affected the unfavorable radiologic outcomes. Results : We had 43 patients with unfavorable radiologic outcomes, including 35 abnormal thoracolumbar alignments and 14 instrumental failures with/without additional surgery. The multivariate logistic regression test showed that immediate postoperative T-L junction Cobb angle less than 10.5 degrees was a statistically significant risk factor, as well as the presence of osteoporosis (p=0.017 and 0.049, respectively). Conclusion : Insufficient correction of thoracolumbar kyphosis was considered to be a major factor of an unfavorable radiological outcome. The spinal surgeon should consider that having a T-L junction Cobb angle larger than 10.5 degrees immediately after surgery could result in an unfavorable radiological outcome, which is related to a poor clinical outcome.

늑골골절 환자에서 지속적 경막외 신경차단에 의한 진통효과 (Effects Of Continuous Epidural Analgesia For Fractured Ribs)

  • 늑골골절을 입은 환자들은 심한 흉통으로 괴로워하며 이 통증은 기침, 심호흡과 기도세척을 방해하여 결국 무기폐와 호흡부전등을 초래할 수 있다. 통증의 완화는 환자를 편하게 해주고 효과적인 물리요법으로 객담배출을 용이하게한다. 늑골골절 환자에서 경막외 신경차단의 효과를 측정하기 위하여 20명의 환자를 대상으로 경막외 진통제을 투여한 10명은 실험군, 진통제를 근육주사한 10명은 대조군으로 정하 여 통증호소와 운동장애의 정도, 말초동맥혈 산소분압 및 폐기능(FRC, FEVI)의 변화를 입원직후와 경막외 진통제투여 시작후 12, 24시간 및 3일, 5일 그리고 7일째에 각각측정조사 하였다. 신경차단군에서 통증호소와 운동장애의 정도는 감소하였고 동맥혈 산소치는 약간 증가하였으나 의의가 없었으며 FRC와 FEVI는 유의하게 증가하였다. 경막외 신경차단의 부작용은가벼웠으며 쉽게 치료되었다. 위의결과로 저자들은 늑골골절 환자에서 경막외 신경차단에 의한진통법이 동통완화효과와 폐기능 향상에 괄목할만한 효과을 나타내므로 이의 임상적 이용이 바람직하다고 생각한다.

우측 쇄골 및 늑골골절과 동반된 폐경식 1례 (Pulmonary Infarction Associated with Fracture of Right Clavicle and Multiple Ribs)

  • 폐경색은 임상적으로나 방사선학적으로 폐암, 폐출혈, 울혈성 무기폐, 단순괴사와 감별이 힘든 질환으로 흉부외상과 관련되어 보고된 예는 없다. 환자는 45세 남자로 약 3주전 흉부외상에 의한 우측의 다발성 늑골골절과 쇄골골절로 치료를 받아오 다가 증상이 점점 악화되어 본원에 전원된 후 우상엽 절제술을 받았으며, 조직검사 결과 폐경 색으로 진 단되었다. 전원 당시 흉부의 컴퓨터 단층활영상 우상엽에 거대한 종괴음영이 있었고, 양측 폐야에 5개 정도의 작 은 결절상 음영이 있었으나 폐 생검에서 암세포는 발견되지 않았고, 우상엽 절제술 당시 대부분의 작은 결절들은 자연 소실되어 있었다 절제된 우상엽에서 육안적으로 인지되는 혈전은 보이지 않았고, 조직검사상 주로 폐동맥의 분지인 0.6 ∼2.0 mm 정도의 근형 동맥들이 혈전에 의해 폐쇄되어 있었으며 일부는 더 진행하여 기질화를 보이거나 혈류의 재소통이 이루어진 곳도 관찰되었다.

The Role of Surgery in the Treatment of Spinal Myeloma

  • Objective: Spinal myeloma has been treated with radiation therapy and chemotherapy. However, the role of surgery was not fully evaluated. This study is performed to evaluate the efficacy of surgery in the treatment of spinal myeloma. Methods: 22 patients who were treated with surgery for spinal myeloma from August 1999 to April 2003 were analyzed. Radiological finding, surgical methods and result were reviewed in retrospective study. For compression fracture due to myeloma infiltration, percutaneous vertebroplasy(PVP) was done. Decompression surgery with or without fixation was performed for patients with neurologic deficit. The modalities of surgery consist of PVP (14 cases), corpectomy and fixation (7 cases), and laminectomy and epidural mass removal (3 cases). To evaluate clinical outcome, visual analogue pain score and Frankel neurological scale were used. Results: In 14 cases of PVP, total 57 vertebral segments were treated including 21 thoracic vertebral bodies and 36 lumbar vertebral bodies. Pain relief was achieved in all cases. The pain score changed from 7.7 (preoperatively) to 2.5 (postoperatively). And pain relief effect was maintained over than one year. Frankel grade improved in decompression cases. Conclusion: Surgical treatment can alleviate pain and improve neurologic deficit immediately in spinal myeloma patients.
