식도암에 있어서 식도 절제술 및 재건술 후 문합 위치와 방법에 따른 문합 부위 누출과 협착 (Anastomotic Leakage and Stricture Relating to Anastomotic Level and Methods in Esophageal Resection and Reconstruction for Esophageal Cancer)

  • 배경: 식도암 수술에 있어서 문합 부위 합병증의 발생은 문합 방법이나 위치와 관계가 있는 것으로 알려져 있다. 이에 저자들은 식도암으로 식도절제 및 식도재건술을 시행 받은 환자에서 문합 방법 및 위치와 누출 또는 협착 발생과의 연관성에 대해 조사하였다. 대상 및 방법: 1993년 8월부터 2003년 5월까지 식도암으로 식도재건술을 시행 받은 321명의 환자를 대상으로 하여 문합 방법, 문합 위치 ,합병증의 발생에 대해 조사하였다. 환자군의 평균 연령은 64.5$\pm$4.9세(37${\~}$94세)였고, 성비는 남자 3001 명($93.5\%$), 여자 21명($6.5\%$)이었다. 결과: 문합부위의 누출은 7예($2.2\%$)에서 발생하였다. 부위별로는 경부 문합에서 3예($4.1\%$), 흉부 문합에서 4예($1.6\%$)로 문합 위치에 따른 차이는 없었다. 문합 방법은 stapler 봉합을 한 경우에서 4예($1.6\%$), semi-stapler 봉합을 한 경우에서 3예($9.1\%$), 수봉합한 경우는 0예($0.0\%$)로 문합 방법에 따른 누출의 차이가 없었다. 문합 부위의 협착은 52예($16.2\%$)에서 발생하였고, 부위별로는 경부 문합에서 2예($2.7\%$), 흉부 문합에서 50예($20.2\%$)로 문합 위치에 따른 차이가 있었다(p<0.001). 문합 방법별로는 stapler봉합이 49예($20.0\%$), semi-stapler봉합 1예($3.0\%$), 수봉합 2예($4.7\%$)로 stapler 봉합에서 유의하게 높았다(p<0.001). 결론: 식도암 환자의 식도재건술에 있어서 문합방법은 문합 부위 누출에 의미 있는 영향을 미치지 않았다. 그러나, 문합 부위의 협착과 관련하여 stapler 봉합은 semi-stapler 봉합 및 수봉합보다 높은 빈도로 협착이 나타났다. 그러므로 stapler 문합시 협착을 줄이기 위해서는 문합 방법의 개선이 필요하다고 생각한다.

식도암에 있어서 Fascin의 발현과 예후와의 상관관계에 대한 연구 (Independent Prognostic Value of the Fascin Expression in Patients with Esophageal Cancer)

  • 배경: Fascin은 actin-bundling protein으로 세포막 돌출(cell membrane protrusion)을 야기하고 상피세포의 운동성(motility)을 증가시킨다고 알려져 있다. 식도암은 근치적인 수술 후에도 조기에 광범위한 국소침윤이나 국소 임파절의 전이를 자주 일으키는 치명적인 암의 하나이다. 본 연구의 목적은 식도암에 있어서 fascin에 대한 연구가 국내에서는 거의 없는 상황에서 면역조직화학 염색 방법을 이용하여 fascin의 발현 양상을 알아보고 발현여부에 따른 임상 및 병리학적 특성과 예후인자로서의 의의 등을 알아보고자 하였다. 대상 및 방법: 식도 편평 상피암으로 진단받고 수술을 시행 받은 76명의 한자의 암조직에서 fascin의 발현여부를 면역조직화학염색을 통해 조사하였다. Fascin에 대한 면역조직화학 염색에 대한 판독은 반정량적으로 2등급으로 나누었다. 발현양상에 따른 임상 및 병리학적 특성 및 예후분석을 시행하였다. 결과: 전체 76개의 암조직중 39예(51.3%)에서 Fascin의 발현이 양성이었다. Fascia의 발현은 임상병기가 진행될수록(p=0.030), T 인자가 높을수록(p=0.031) 양성률이 높게 나타났으며 추적기간 중 재발이 있었던 군(p=0.005)과 사망을 한 군(p=0.000)에서 양성 발현율이 통계적으로 유의하게 높았다. 각 임상병리학적 인자들의 예후인자로서 단변량 분석 결과 임파절의 전이유무, 림프판 혈관 침범(lymphovascular invasion)여부, 신경주위침범(perineural invasion)여부, 병기, fascin 발현 여부가 통계적으로 유의한 인자들로 나타났다. 다변량 분석에서는 림프관 혈관 침범(lymphovascula. invasion)여부, fascin 발현 여부가 예후인자로 유의하였다. 결론: Fascin은 식도암조직의 51.3%에서 발현되었으며 이의 양성 발현은 보다 진행된 종양 및 재발과 연관성이 있었다. Fascin의 음성발현군이 양성발현군에 비해 유의하게 예후가 좋았으며 Fascin의 발현 여부가 식도암의 예후 인자로서 유용할 수 있음을 알 수 있었다.

결핵성 식도천공 수술치험 1예 (Tuberculous Esophageal Perforation -Report of A case-)

  • This is a report of a case of tuberculous esophageal perforation, which was surgically treated.. The patient was 32-year-old Korean female patient, who complained swallowing difficulty for` 4 weeks duration. Esophagogram was shown irregular filling defects in the upper one third of esophagus, about 4 cm in length. It was noticed that a small amount of contrast media was leaked out from the involved area of esophagus into the right mediastinum. It was highly suggested that abscess formation was due to perforation of esophageal cancer. Esophagoscopy revealed no definitive evidence of perforation of esophagus, but punch biopsy specimen of esophageal mucosa was shown acute and chronic inflammatory changes. Operation was performed under impression of esophageal cancer. In the right-sided upper mediastinum, a walnut-sized abscess cavity which was connected with esophagus through a fistulous tract was noted. A portion of cavity submitted for frozen section was shown tuberculous inflammation. The abscess cavity, fistula tract, and involved esophageal wall were removed. The perforated esophagus was closed layer by layer. The tuberculous changes were confirmed by histopathologic examination postoperatively. The postoperative course was uneventful.

양성 식도 협착의 외과적 치료 (Surgical Treatment of Benign Esophageal Stricture)

  • During a ten-year period from August, 1978 to September, 1989 45 patients with benign esophageal stricture were surgically evaluated. The results are as follows; l. Out of 45 patients, there were 26 males and 19 females ranging from 2 to 70 years of age with a mean of 31.9 years. 2. The most common cause of benign esophageal stricture was corrosive burn due to caustic agents[40 cases, 88.9 %]. Corrosive agents were 15 cases of lye, 22 cases of acid and 3 cases of other agents. Other causes were two cases of esophageal web and each one case of previous surgical result, inflammation and idiopathic mediastinal fibrosis respectively. 3. The most frequent stricture site was whole esophagus as 21 cases[46.7 %] and the next was lower a third thoracic esophagus[10 cases, 25.0%]. 4. In 33 of total 45 cases, colon interposition with right colon was performed without resection of the strictured esophagus except one case which was complicated esophageal cancer. Other procedures were 4 cases esophagogastrostomy with segmental resection, 2 cases of plastic repair and so on. 5. Major postoperative complications which were needed for secondary operation were 5 cases[11.1 %]. [2 cases of stenosis, ileus and ulcer bleeding respectively] Overall mortality rate was 4.4 %.

대동맥 침습이있었던 식도암의 절제수술 (Exophagectomy Combined with Resectiion of Invaded Aorta for T4 Esophageal Carcinoma.)

  • Advanced esophageal carcinoma which invades into adjacent organs are classified as T4 esophageal cancer,. Its complete resection without residual tumor would be difficult. Preoperative chemoradiotherapy and combined modality therapy are being tried to improve survival in patients with T4 esophageal carcinoma. In a 74-year-old man a 6cm squamous cell carcinoma of the esophagus with invasion of the thoracic aorta was detected (T4). After neoadjuvant chemoradiotherapy the patient was operated on using bio-pump with aorto-femoral cannulation. The invased segment of descending aorta was resected and reconstructed with a graft. The tumor was resected and EG anastomosis was done. The postoperative period was uneventful the patient was discharged after good condition and has been well to now.

Fistulas between the Esophagus and Adjacent Vital Organs in Esophageal Cancer

  • Esophageal fistulas may occur in an advanced stage or as a potentially life-threatening complication of treatment. They can be divided into esophageal-respiratory and esophageal-aorta fistulas. The diagnosis is confirmed with fluoroscopy using dilute barium oral contrast, followed by thin-section computed tomography, which defines the precise location and extent of the fistula. Flexible esophagoscopy and bronchoscopy are required for confirmation and anatomic assessment of the suspected fistula and provide additional information for treatment planning. Contamination is traditionally controlled by surgical exclusion, along with a jejunal feeding tube. Currently, fully covered self-expanding metal stents are the primary treatment option.

식도암의 외과적 요법에 관한 연구 (surgical treatment of esophageal cancer)

  • Between September 1973 and December 1983, 61 patients with carcinoma of the esophagus were treated surgically at the Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital. Among 61 patients, male patients were 51 cases, female 10 cases and the age ranged from 21 years old to 72 years old with the average of 54.6 years old. Min symptoms of esophageal cancer were dysphagia (91.5%), weight loss (40.4%), pain(27.6%), and the average symptom duration was 3.85 months. The anatomical locations of esophageal cancer in preoperative esophagogram revealed 41.7% in middle 1/3, 8.3% in esophagograstic junction or cardia. Among 61 cases, 9 cases were managed by feeding gastrostomy due to inoperability, 8 cases by exploratory thoracotomy or lapatotomy only without curative or palliative resection, and 44 cases by curative or palliative resection with reconstruction. Among 52 cases of exploration, 44 cases were managed with curative or palliative resection of cancer and the resectability revealed 84.6% in operated cases. Among palliative or curative resected group, the esophagogastrostomy was performed in 40 cases (90.9%), esophagojejunostomy in 3 cases(6.8%), esophagectomy only in 1 case(2.3%). Postoperative complications were noticed in 12 cases, such as anastomotic leakage in 7 cases(15.6%), empyema in 2 cases (4.4%), respiratory failure in 2 cases (4.4%), anastomotic stricture in 1 case (2.2%). among 7 postoperative anastomotic leakage, 2 patients died as a result of that complication and the operative mortality revealed 4.3%. During follow-up work, the mean survival period was 19.3 months in patients who discharged hospital alive, and the 2 year survival rate was 34.6%.

Overexpression of RUNX3 Inhibits Malignant Behaviour of Eca109 Cells in Vitro and Vivo

  • Runt-related transcription factor 3 (RUNX3) is a tumor suppressor gene whose reduced expression may play an important role in the development and progression of esophageal squamous cell cancer (ESCC). The aim of this study was to investigate the clinical relevance of RUNX3 in ESCC patients and effects of overexpression on biological behaviour of Eca109 cells in vitro and in vivo. Immunohistochemistry was performed to detect the clinical relevance of RUNX3 and lymph node metastasis in 80 ESCC tissues and 40 non-cancerous tissues using the SP method. RT-PCR and Western blotting were applied to assess the RUNX3 level and verify the Eca109 cell line with stable overexpression. Localization of RUNX3 proteins was performed by cell immunofluorescence. CCK-8 and Scrape motility assays were used to determine proliferation and migration and the TUNEL assay to analyze cell apoptosis. Invasive potential was assessed in cell transwell invasion experiments. In nude mice, tumorigenesis in vivo was determined. Results showed decreased expression of RUNX3 in esophageal tissue to be significantly related to lymph node metastasis (LNM) (P<0.01). In addition, construction of a recombinant lentiviral vector and transfection into the human ESCC cell line Eca109 demonstrated that overexpression could inhibit cell proliferation, migration and invasion, and induce apoptosis. The in vivo experiments in mice showed tumorigenicity and invasiveness to be significantly reduced. Taken together, our studies indicate that underexpression of RUNX3 in human ESCC tissue is significantly correlated with progression. Restoration of RUNX3 expression significantly inhibits ESCC cells proliferation, migration, invasion and tumorigenesis.

Treatment Patterns and Outcomes of Anastomotic Leakage after Esophagectomy for Esophageal Cancer

  • Background: Anastomotic leakage (AL) following esophagectomy represents a serious complication that often results in prolonged hospitalization and necessitates repeated interventions, including nothing-by-mouth (NPO) restriction, endoscopic vacuum therapy (EVT), or surgical repair. In this study, we evaluated the patterns and outcomes of AL treatment. Methods: We retrospectively reviewed the medical records of patients who underwent esophagectomy for esophageal cancer at a single center between 2003 and 2020. Of 3,096 examined cases, 181 patients (5.8%) with AL were included in the study: 114 patients (63%) with cervical anastomosis (CA) and 67 (37%) with intrathoracic anastomosis (TA). Results: The incidence of AL was 11.9% in the CA and 3.2% in the TA group (p<0.001). Among patients with CA who developed AL, 87 (76.3%) were managed with NPO, 15 (13.2%) with EVT, and 12 (10.5%) with surgical repair. Over 90% of patients with cervical AL resumed an oral diet by the time of discharge, regardless of treatment method. Among patients with TA and AL, 36 (53.7%) received NPO, 25 (37.7%) underwent EVT, and 6 (9%) required surgery. Of these, 34 patients who were managed with NPO and 19 with EVT could resume an oral diet. However, only 2 patients who underwent surgery resumed an oral diet, and 2 patients required additional EVT. Conclusion: Although patients with CA displayed a higher incidence of AL, their rate of successful oral intake exceeded that of those with TA, regardless of treatment method. Among patients exhibiting AL with TA, EVT was more commonly employed than in CA cases, and it appears effective.
