Abdominal aortic occlusion (AAO) caused by detachment of cardiac myxoma (CM) is a very rare complication in patients with CM. Although the nature of CMs has been well established, detachment of CM may cause unexpected serious complications such as vicious embolic events. Actually, in several cases of AAO caused by detachment of CM, it has been reported that CM fragments easily migrated to the brain, heart, and lungs, and caused lifelong neurological complications despite appropriate surgical therapy. Herein, we report a case of a patient with AAO caused by detachment of CM who underwent CM excision and abdominal aortic thromboembolectomy. Additionally, we have presented the preoperative and postoperative images using 64-multidetector computed tomography.
Two patients with chronic Budd-Chiari syndrome resulting from segmental obstruction of the inferior vena cava underwent operation. There were 1 man and 1 woman. The obstructed segment was directly visualized by a transthoracic, transdiaphragmatic, retroperitoneal approach. In these two cases, severe segmental obstruction of the inferior vena cava was observed just above the right hepatic vein. These patients underwent successful retrohepatic cavoatrial bypass with a polytetrafloroethylene [PTFE] graft [ 16mm plain and 16mm ringed graft ]. There were no operative mortality and postoperative complication. These patients have been followed up for 6months and 36months without evidence of re-obstruction. When there is a severe stricture of the IVC with hepatic veins draining freely into the obstructed segment of the IVC, a dorsal cavoatrial bypass with a PTFE graft, preferably ringed, is the method of choice.
Intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derives its arterial blood supply through aberrant vessel directly of systemic circulation. Two forms of pulmonary sequestration occur; intralobar sequestration is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We presented on case of intralobar pulmonary sequestration with A-V fistula. The patient was 8 years old male and chief complaints are mild fever and exertional dyspnea. Aortogram revealed an aberrant artery originated from thoracic aorta just above the diaphragm and its vessel directly communicated pulmonary vein in sequestrated segment. At time of operation, sequestrated lobe measuring 6x5x5cm, well demarcated at the lower lobe was noted. Aberrant, measuring 1.0cm in diameter and 1cm in length and directly connected pulmonary vein. After division and ligation of the aberrant artery, only excision of sequestrated lobe was performed and complication is not during postoperative course.
We have experienced a case of right side chylothorax following closed chest injury. A 35-year-old man in his car was accidentally collided against obstacles on September 19, 1986 resulting in a contusion on right anterior chest wall. The only complaint noted on admission was right chest pain. Chest X-ray showed near total radiopaque density of right thorax. Conservative treatment of closed tube thoracostomy at right pleural cavity through midaxillary 7th intercostal space had been continued for 25 days without improvement. Chyle outflow through the chest tube was averaging 1,700cc per day. Oversewing of the thoracic duct and pleura by silk and pledgetted prolene sutures were done. There was no complication and recurrence till postoperative 20 days. Chylothorax following closed chest injury was never reported in this country, and will be a interesting clinical case report.
The dilated haustra of colon after esophagocolostomy was a rare complications, but it may be extremely dangerous because of the potential for perforation of the thin- walled esophageal substitute. Therefore a prompt surgical correction of the lesion should be done. The conventional surgical exploration and resection of the lesion site was very difficult. The use of the ends GIA to eliminate the dilated haustra of colon was simple and safe method and the postoperative results were good. We report a case of experience and methods.
Phlegmasia cerulea dolens (PCD) is one of the most critical disorders of acute deep vein thrombosis in that it can cause permanent disability secondary to the compartment syndrome. Although several etiological factors have been proposed, PCD after coronary artery bypass surgery is extremely rare and its definitive pathophysiology is still under debate. We herein present a case of PCD that resulted in the compartment syndrome after coronary artery bypass surgery. Early recognition and decompression of PCD are crucial for saving the affected limbs.
The development of chylothorax and chylopericardium are serious and often life-threatening clinical entity. The ideal treatment of these problems is not well established to date. We reviewed our experiences with chylothorax or chylopericardium in 16 patients [17 cases] from July 1979 to May 1989. Ages ranged from 20 days to 41 years. The etiologies were traumatic in 10, congenital or idiopathic in 5, and tuberculous lymphadenopathy in one. In 8 patients, the chylothorax or chylopericardium occurred as a complication of cardiothoracic surgery. Eleven patients were treated nonoperatively with either repeated thoracenteses or chest tube drainage. Five patients underwent operative treatment: transthoracic thoracic duct ligation [three patients], thoracic duct ligation combined with decortication [one], and oversewn the defect of mediastinal pleura[one]. Duration of preoperative therapy ranged from 18 to 38 days. One of eleven [9.1 %] patients treated nonoperatively died. Of the surgically treated group, there were no deaths. All patients except one dead cured or improved either treated nonoperatively or operatively. Our experience suggests that surgical management of the chylothorax or chylopericardium is not always required, and each patient must be judged individually according to disease process.
The Bronchial stump disruption in bronchopleural fistula with empyema thoracis after pneumonectomy has remained one of the most dreaded complications of thoracic surgery. Management of chronic bronchopleural fistula still poses a therapeutic dilemma in spite of various surgical techniques that have been attempted to control this complication. Only recently, transsternal transpericardial approach for repair of the postpneumonectomy bronchopleural fistula has been utilized in some cases. The patient was a 31 year-old woman who was admitted to our hospital on August 18th, 1989 due to right postpneumonectomy bronchopleural fistula with empyema thoracis for 5 years since she had undergone right pneumonectomy due to pulmonary tuberculosis at E-hospital in 1984. Transsternal transpericardial closure of the fistula was employed and then the thoracic catheter was removed two months later, after the empyema cavity was sterilized by the Clagett method. So, we think this surgical technique is a relatively simple and effective method to the control of chronic postpneumonectomy bronchopleural fistula with empyema thoracis.
A Case of thoraco-abdominal aortic aneurysm involving from the lower descending- thoracic aorta to bifurcation of abdominal aorta into both common iliac artery is presented in special view-point of its surgical technic and postoperative complication. Operative technic is the most popular method of Dr. De Bakey's shunt of Dacron which is bridging thoracic aorta to terminal abdominal aorta primarily as temporary shunt but after anastomosing the individual hranch of left Renal, Celiac, Sup. meseateric and right Renal artery to corresponding part of the Dacron tube, the Dacroa graft is fixed as permanent graft in stead of excised thoraco-abdominal aorta. The patient died of acute renal failure and increasing evidence of CNS damage due to respiratory acidosis on 6th postoperative day hut this report will he a good experience in respect of further progress of aortic surgery in Korea.
The stomach has become the most commonly used site for grafts to replace the esophagus in esophageal cancer surgery because of its good blood supply and ability to enable single-reconstruction anastomosis. However, anastomotic failure is a serious complication after esophageal cancer surgery. Unlike anastomotic leakage due to local ischemia, gastric tube necrosis is a life-threatening condition with a high mortality rate. Gastric tube necrosis involves extensive ischemia due to a decreased blood supply, and an urgent operation is mandatory in most cases. Endoscopic vacuum therapy (EVT) has been used for anastomotic leakage after esophageal surgery. In recent years, it has been successfully used for more extensive disease, including large esophageal perforation as an indication for reoperation. Hence, we report a case of extensive gastric tube necrosis treated by EVT after an Ivor Lewis operation.
