흉곽내벽에 발생한 거대섬유종 -1예 보고- (A Huge Intrathoracic Fibroma (One case report))

  • Primary intrathoracic neoplasms occur frequently, and the most frequent intrathoracic tumors are of pulmonary or bronchogenic origin. Second in frequency are mediastinal tumors, and least frequency or comparatively rare are the neoplasms arising from the wall of the thorax and extending into the thoracic cavity. Of all intrathoracic tumors, fibroma is extremely rare. Recently, we have experienced a huge rapid growing intrathoracic fibroma originating from thoracic wall, which is removed successfully and confirmed histopathologically. Authors present one case of fibroma and discussion with a brief review of the relevant literatures.

Sternal Resection and Reconstruction for Solitary Plasmacytoma of the Sternum: Case Report

  • A 63-year-old patient was admitted with a sternal fracture and mass. On evaluation, most of the body of the sternum had been destroyed by a tumor. Radical resection of the sternum was performed and part of the major pectoral muscles adherent to the sternal tumor was also resected. The chest wall defect was reconstructed with mesh, bone cement, and a titanium rib plate system. Reconstruction with this method seemed to be an appropriate procedure to prevent instability of the chest wall.

  • Herein, we report a case in which thoracomyoplasty was performed to manage chronic postlobectomy empyema (PLE). A 54-year-old male patient with a surgical history of right upper lobectomy and thymectomy 35 years previously who had undergone adjuvant radiotherapy presented with purulent discharge on the anterior chest wall. The patient was diagnosed with chronic PLE with ascending infection and concurrent osteonecrosis of the parasternum. Proper drainage was performed for local infection control and the dead spaces were successfully closed with muscle flaps. There have been no complications to date.

Preserved Respiratory Function after Reconstruction of a Large Chest Wall Defect

  • A case report of a patient who developed radiation-induced sarcoma in the left chest wall is presented. The patient had partial mastectomy and adjuvant radiation therapy (total dose, 5,220 cGy) and chemotherapy. Five years later, she visited with rapidly growing mass with central ulceration in the irradiated chest wall. The mass was diagnosed as malignant fibrous histiocytoma. The chest wall mass resected en bloc ($23{\times}18cm$) including five consecutive ribs. After the defected thoracic cage was reinforced using a polytetrafluoroethylene patch, omental flap and split thickness skin graft was done for soft tissue coverage. We applied negative pressure wound closer system for effective suction of omeantal exudate. The wound healed without complications. The patient suffered no perioperative pulmonary complications. Pulmonary function tests showed no significant changes. Each of Gore-Tex, omental flap, negative pressure wound therapy and skin graft is widely used method. However, If these methods are used in combination, we can reconstruct the large defect of chest wall including multiple ribs without any repiratory function problems.

소아의 흉벽에서 발생한 윤활막 육종 (Synovial Sarcoma Arising from the Chest Wall in a Child)

  • 윤활막육종은 젊은 성인의 팔, 다리에 호발하는 악성종양이다. 흉막, 폐에 발생하는 윤활막육종은 드물게 보고되고 있으나, 본 증례처럼 흉벽에 생기면서 소아에 발생한 경우는 매우 드물다. 저자들은 소아의 흉벽에 발생한 윤활막종을 수술 치료한 경험을 보고하는 바이다. 현재 환아는 술 후 3년 동안 재발없이 잘 지내고 있는 중이다.

Surgical Treatment of a Tuberculous Abscess of the Chest Wall

  • Background: Tuberculous abscess of the chest wall is a very rare disease. Few articles have reported on it and those that have enrolled few patients. To determine the characteristics of this disease and to suggest an optimal treatment strategy, we reviewed patients treated by surgical management. Materials and Methods: Between October 1981 and December 2009, 68 patients treated by surgical management for a tuberculous abscess of the chest wall were reviewed retrospectively. Results: Of 33 men and 35 women, 31 patients had a current or previous history of tuberculosis. The main complaints were chest pain, a palpable mass, pus discharge, and coughing. A preoperative bacteriologic diagnosis was performed in 12 patients. Abscess excision was performed in 54 cases, abscess cavity excision and partial rib resection in 13, and abscess excision and partial sternum and clavicle excision in 1 case. Postoperative wound infection was noted in 16 patients and a secondary operation was performed in 1 patient. Recurrence occurred in 5 patients (7.35%). Reoperation with abscess excision and partial rib resection was performed in all of the 5 cases. Conclusion: Complete excision of the abscess and primary closure of the wound with obliteration of space would decrease postoperative complications. Anti-tuberculosis medication may reduce the chance of recurrence.

흉벽종양의 임상적 고찰 (Clinical Review of Chest Wall Tumors)

  • The author reviewed 10 patients of chest wall tumors which were operated during 7 years, from 1970 to 1976, in Department of Thoracic and Cardiovascular Surgery in Chonnam University Hospital. Benign tumors were 6 patients, malignant tumors were 2 patients and metastatic tumors were 2 patients. Benign tumors were more than malignant tumors, but total cases were small, so, statistical analysis was not enough. Specific symptoms of chest wall tumor were swelling due to mass and pain. All cases had swelling due to mass and 4 cases [40%] had pain. Dyspnea was noted in metastatic tumors. The review of the literatures was also done.

흉벽종양 21례에 대한 임상적 고찰 (A Clinical Review of Chest Wall Tumors: 21 Cases)

  • 21 cases of the chest wall tumors that were operated and proved by pathologically at the dept. of thoracic & cardlovascular surgery, SNUH during 16 years from 1965 to 1981 were reviewed. The results are as follows: 1.Benign tumors were 11 cases. Primary malignant tumors were 7 cases. Metastatic tumors were 3 cases. 2.Incidence rate of male to female was 1.5:1. 3.Main symptoms were palpable mass [52.4%] and localized chest pain [14.3%]. 4.Locations of tumors were rib [61.9%], soft tissues and muscle [33.3%] and sternum [4.8%]. We excluded the Tbc, rib carles from the chest wall tumors.

Judet's Strut와 Teflon Mesh를 이용한 다발성 늑골골절 및 흉벽결손의 치험 1례 (One Case Tratment of Multiple Ribs Fracture with Chest Wall Defects (by Use of Judet's Struts and Teflon Mesh))

  • Recently, we experienced one case of multiple ribs fracture with large chest wall defects. This patient was treated with internal fixation of ribs by use of Judet`s struts and reconstruction of chest wall defects by use of Teflon mesh. Postoperative outcome was satisfactory result and its advantages were reduced duration of operation, prevention of pulmonary herniation and reduced risk of postoperative infection.

흉부에서 발생한 악성 섬유성 조직구종 (Malignant Fibrous Histocytoma Originating from the Chest Wall)

  • Malignant fibrous histiocytoma(MFH) is a deep-seated pleomorphic sarcoma, which occurs principally as a mass of the extremities, abdominal cavity, or retroperitoneum in adults. However, it only rarely occurs in the chest wall. An 85-year-old man had undeergone excision of a small mass on the right posterior chest wall under local anesthesia 14 months age. However, the lesion did not heal and the mass recurred. He was referred to our hospital after the mass had grown to a size of 10.5$\times$8$\times$4cm with a 3$\times$3cm skin defect. Intraoperative frozen biopsy revealed MFH. An en-bloc wide resection and thin-thickness skin graft from his thigh were performed. Although distant metastasis to the lund developed 14 months later and the patient died 2 months later, there was no local recurrence. Thin-thickness skin graft is a simple method for a wide range skin defect, especially in the old age. He recovered in good condition without any physical disabilities.
