The use of autogenous tooth bone graft material has been commercialized since 2008. Autogenous tooth bone grafts always require that the tooth of the patient be extracted, and thus, the use of graft material are limited in many cases. For solution of limitation in quantity and concurrent use of autogenous tooth bone graft material, the grafting of familial teeth has been suggested. It has the following advantages: the teeth of family members are used as bone graft materials, the genetic composition is identical, and potential genetic and infectious risks can be minimized. Because the teeth of family members are used, a good tissue affinity is obtained, and thus, superior bone generation rates compared to those observed for allogenic or xenogenic bones can be anticipated. We used familial tooth bone-graft materials for alveolar ridge augmentation, socket preservation, and maxillary sinus graft in some cases. In most cases, the impacted third molars of their children were prepared as bone graft material and were used for surgery. In one case, the impacted third molar from the patient's brother was used as bone graft material. We obtained satisfactory result and these cases are reported herein.
One hundred and thirteen human mandibular first molars were injected with china ink, decalcified, cleared and used in study, in vitro, to determine the number of root, the number of root canals, canals per root, frequency and location of transverse anastomoses, frequency and location of lateral canals and frequency of the apical deltas. The results were as follows; 1. Most of the teeth showed three canals, but 21. 25% of the teeth were found to have two canals and 21. 25% of them four canals. 2. In so far as observing two canals per root, 77.0% of teeth were found to have two canals in mesial root and 25.7% of them in distal root. 3. In roots with two canals, the separated apical foramen appeared 59.8% in mesial side and 40.0% in distal side, and the common apical foramen 40.2% in mesial side and 60.0% in distal side. 4. Of the two root canals in one root, 37.3% of the canals were found to have transverse anastomoses and were usually located in the apical third of the root. 5. 25.7% of 113 teeth were found to have lateral canals, and ramifications were mainly located in the apical third of the root.

In roots with, two canals, the seperated apical foramen appeared in 59.5% in mesial side and 40.0% in distal side, and the common apical foramen appeared in 40.5% in mesial side and 60.0% in distal side. 4. Of the two root canals in one root, 36.2% of the canals were found to have transverse anastomoses and were usually located in the apical third of the root. 5. 23.1% of 52 teeth were found to have lateral canals, and ramifications were mainly located in the apical third of the root.

Background: Hyaluronic acid (HA) is well known to exert an anti-inflammatory effect during oral wound healing and is commonly applied after tooth extraction. However, no double-blind randomized controlled study comparing two hyaluronate mouthwash products has been conducted so far. The aim of this study was to comparatively analyze the efficacy of Mucobarrier® and Aloclair® in terms of clinical symptoms. Results: A total of 112 patients were randomly assigned to assess the degree of discomfort, pain reduction, redness, burning sensation, and swelling between two groups on the day of surgery and 7 days later in a double blind test, with a total 56 Aloclair patients and 56 Mucobarrier patients. There was no statistically significant difference in the overall discomfort, degree of pain reduction, redness, burning sensation, and swelling between the Mucobarrier and Aloclair groups. Conclusion: The local application of hyaluronic acid mouth wash after wisdom tooth extraction is beneficial in reducing overall discomfort and pain reduction, and the clinical utility of Mucobarrier® is no different from Aloclair®. Dental sedation plays a pivotal role in alleviating patient anxiety during various procedures. Remimazolam, a benzodiazepine derivative, stands out for its distinctive attributes, particularly its rapid onset of sedation coupled with a brief duration, making it an invaluable option for dental applications. The patient was admitted for the extraction of impacted third molars via patient-controlled sedation and not only demonstrated stable vital signs but also expressed a high level of satisfaction with the procedure. An in-depth analysis of plasma remimazolam concentrations and changes in the Patient State Index revealed negative correlation patterns, highlighting the inherent potential of remimazolam in achieving effective sedation. This expanded research scope aims to provide a more nuanced understanding of the pharmacological responses to remimazolam in dental sedation scenarios. This case report offers valuable insights into the evolving landscape of dental sedation methodologies and paves the way for a more informed and evidence-based approach to the use of remimazolam in patient-controlled sedation.

Objectives: Preoperative nervousness and anxiety are frequently encountered by individuals who undergo extractions of impacted wisdom teeth. The aim of the present study is to evaluate salivary alpha amylase (sAA) level in patients for assessment of stress during third molar surgery while listening to piano music and to determine its co-relation with pain catastrophizing scale (PCS). Materials and Methods: Seven patients (four males and three females) indicated for surgical extraction of bilaterally impacted mandibular third molars were included. Pre-surgical patient assessments were completed, and three samples of saliva were collected during surgery-one at baseline, one 30 minutes after commencement of surgery, and one after suturing. Assessment was performed on both sides separately with and without piano music, and the samples were assessed for sAA level and correlated with the patient's self-reported PCS. Results: Statistically significant results were obtained in patients who underwent surgical extraction while listening to piano music (P=0.046). The correlation of sAA level with PCS was not significant. Conclusion: Music demonstrated a beneficial effect on lowering the levels of stress and anxiety that a patient exhibits during any surgical procedure, and sAA can be a useful biomarker for similar assessments.
The purpose of this study was to investigate the fittness of Golden relation items was advocated by Ricketts whether or not it is applicable to the young adult Korean dentitions and also to evaluate the several new Golden relation items conducted by the author. The material was consisted of 81 dental casts (34 male, 47 female) with ideal occlusion, which never undergone orthodontic, prosthodontic procedures. Measurements were made on the arch dimensions using sliding caliper (Mitutoyo. Co) and data were computerized and analyzed. The findings of this study were as follows, 1. The Golden proportion advocated by Ricketts dose not seem to directly applicable to the Korean normal dentition, however, the modification from the Ricketts' original shows the Golden proportion as follow: A first series of progressive Golden relations was found on the Golden ratio among the lower central incisors width, the inter mesioincisal width of the upper lateral incisors, and the upper first premolars width. A second series was found on the Golden ratio among the lower lateral incisors width, lower inter canine tips width and theupper first molar distal cusp tips width. A third series was found on the Golden ratio between the inter distal aspect width of e lower canine and the mesial cusp tips width of the lower second molars or inter cusp tips width of upper second premolars. 2. In addition to Ricketts' original, 4 new Golden proportions were found in young adult Korean dentition, these are as follows; The tips of lower canine width had Golden relation with the width of the upper first premolar buccal cusp tips or the width of the lower first molar central fossae. The distal aspect of the lower first premolars had Golden relation with the buccal surface widths of the lower or upper second molars. The width of upper lateral incisors had Golden relation with the upper second molar height. The width of the lower canine tips had Golden relation with the lower second molar height.
Objective: The purpose of this study was to clarify the effects of continuous force application for extrusive tipping movement and occlusal interference on periapical root resorption in the rat mandibular first molar. Methods: We constructed an appliance comprising a titanium screw implant with a cobalt-chromium post as the anchorage unit and a nickel-titanium closed coil spring (50 cN) as the active unit. Force was applied on the mandibular left first molar of rats for 8 (n = 10) and 15 days (n = 10; experimental groups), with the tooth in occlusion. Five rats were included as a non-treated control group to examine the body effect of the appliance. Active root resorption lacunae, identified using tartrate-resistant acid phosphatase, were evaluated in terms of the length, depth, and area. Results: The rat mandibular first molars were mesially tipped and extruded in the occlusal direction. This mesio-occlusal tipping movement and occlusion resulted in the formation of a compression zone and active root resorption lacunae in the distoapical third of the distal roots. However, there was no significant difference in the amount of root resorption between the two experimental groups. The control group did not exhibit any active root resorption lacunae. Conclusions: Periapical root resorption was induced by continuous extrusive tipping force and occlusal interference in rat mandibular molars. These data suggest that we orthodontists had better take care not to induce occlusal interference during our orthodontic treatment.
매복치는 유치열기보다 영구치열기에서 더 높은 빈도로 발생한다. 가장 흔하게 이환되는 치아는 상하악 제3대구치들이며, 하악 제1대구치의 매복은 비교적 드문 편이다. 매복치의 치료방법은 자발적 맹출을 위한 지속적인 검사, 외과적 노출술, 외과적 노출 후 아탈구, 교정적 견인, 그리고 외과적 재위치술 등이 있으며 이 모든 치료가 실패한다면 치아를 발거하는 것을 고려할 수 있다. 첫 번째 증례는 8세 남아로서 매복된 하악 제1대구치에 폐쇄장치를 이용한 감압술을 시행하였으며, 정기적인 검진을 통해 치아의 맹출이 관찰되었다. 두 번째 증례는 12세 남아로서 매복된 하악 제1대구치의 외과적 노출술을 시행 후 치아의 맹출이 관찰되지 않아 가철성 장치를 이용한 교정적 정출술을 시행하였으며 이 후 정상적인 치아의 맹출이 관찰되었다.
One bottle system was recently developed in order to simplify the clinical skills and save chair time after continuous improvements on dentin bonding agents. There has been many studies to measure the bond strength of one bottle systems but no actual work has been done on micromorphologic study of resin-dentin interdiffusion zone after one bottle system application. To evaluate the bonding patterns of various commercially available one bottle systems to dentin, observation of resin-dentin interdiffusion zone under TEM was performed. Caries-free human third molars within one month of extractions were chosen for the experiments. The molars were sectioned 1mm above the cementoenamel junction and got rid of the root portions. Crown portions of the teeth were sectioned parallel to occlusal surface so that dentin discs of 1mm in thickness were remained. 7 one bottle systems and 1 two bottle system were applied according to manufacturer's instructions and followings were the results. 1. In every experimental groups, cross bandings of collagen fiber were distinguishable and tight bon dings between the bonding agents and dentin were observed. 2. Hybrid layer was clearly observed in ONE-STEP$^{(R)}$, Prime & Bond$^{(R)}$ 2.1, Syntac$^{(R)}$ SC, MAC-BOND II groups but it was not clear in Single Bond, D-Liner Dual PLUS, ONE COAT BOND groups. 3. Electron-density of hybrid layer was uniform in pattern in MAC-BOND II, Prime & Bond$^{(R)}$ 2.1 groups but not so uniform in ONE-STEP$^{(R)}$ group. 4. Electron-dense amorphous phase in most superior layer of the resin-dentin interdiffusion zone was characteristically observed in Single Bond, Syntac$^{(R)}$ SC, ONE COAT BOND groups. It can be concluded that bondings between the dentin bonding agents and dentin can be various in pattern according to their chemical compositions and the condition during applications.
