The Difference of Efficacy for Oral Hypoglysemic Pharmacotherapy Based on Sasang Constitutional Medicine Among Type II Diabetes Mellitus Patients in Korea (제 2형 당뇨병 환자에서 사상체질에 따른 경구 혈당강하요법의 치료 반응성 및 사용 패턴 평가)

  • Although Korean patients with type 2 diabetes mellitus (T2DM) are generally treated by western medicine, many of them strongly believe in the traditional oriental Sasang constitutional classification and depend on it for food, health supplements, and oriental medicines decision making. Sasang constitutional classification is a part of traditional Korean medicine that divides people into four constitutional types (Tae-Yang: TY, Tae-Eum: TE, So-Yang: SY, and So-Eum: SE), which differ in inherited characteristics such as appearance, personality traits, susceptibility to diseases, and drug responses. It is recommended for T2DM patients to control their blood glucose very well from early stages with drugs and diet. However, many T2DM patients respond differently to their drugs, even though they receive the same medicine. Therefore, the present study investigated whether Sasang constitutional type can explain the therapeutic differences between oral hypoglycemic agents (OHAs) therapy (mono, dual and triple drug therapy). Patients of 618 with T2DM diagnosis and Sasang constitutional type known who received both western and oriental medicine treatment in a hospital between April 2006 and April 2013 retrospectively studied. HbA1c (%) and blood glucose (mg/dl) levels before OHAs therapy and 3 month after were collected for metformin (MET) or sulfonylurea (SU) monotherapy, MET+SU dual therapy, MET+except SU (where was either alpha-glucosidase inhibitor, dipeptidyl peptidase-4 inhibitor, meglitinide or thiazolidinedione) dual therapy, and triple therapy, according to Sasang constitutional type. For statistical analysis, ANOVA was used and paired t-test by SPSS 19.0 where P values less than 0.05 were considered statistically significant. Pattern was similar levels of HbA1c and blood glucose and which was decreased in order of mono, MET+SU dual, MET+except SU dual and triple therapy. In all patients comparison, for the So-yang (SY) constitutional type, either monotherapy was less effective; for Te-eum (TE) type, MET+SU dual therapy was less effective while MET+except SU dual therapy was more effective and the triple therapy was less effective; and for So-eum (SE) type, the triple therapy was more effective. For the management of TE type it is recommended to use drugs except SU when dual therapy is needed, restrict triple therapy and consider dual and insulin therapy; for SY type it is recommended to follow current guidelines; and for SE type it is advisable to skip dual therapy and start the triple therapy early. Finally, the therapeutic response to OHAs is different among Korean T2DM patients with different Sasang constitutional types. Taken together, the choice of effective OHAs therapy for each type is necessary in order to minimize the poor control of blood glucose level, the risk of complications, and the costs from a failure of therapy.

Comparison of Bone Mineral Density in Type II Diabetic Patient's and Healthy Elderly Individuals (제2형 당뇨환자와 정상성인의 골밀도 비교)

  • Purpose: This study was aimed for service to clinical data of physical therapy necessity of bone complication through analysis method of BMD (bone mineral density) used DEXA (dual energy X-ray absorptiometry) to their skeletal system for physical therapy assesement and intervention program in type II diabetic patients. Methods: Experimental group of 75 subjects and comparison group of 62 subjects were participated in type II diabetic patients(40-80 ages). BMD was measured by DEXA. BMD change of BMI score and BMD comparison of age and sex would be known. Results: This study was found that decreased BMD and increased osteopenea in type II diabetes. In particular, women were lower BMD and higher incidence of osteopenea than men. Men showed significant difference in normal group. Influence of type II diabetes was great on change of BMD in men. however, it showed no significant difference from normal group. Conclusion: it was found that skeletal system complication by type II diabetes had some relations. Because reduction of BMD had a great danger to induce trauma by fall or degenerative disease of system, evaluation of proper physical therapy for its prevention and improvement and intervention program are needed. In addition, it would be important to divide type II diabetic patients into osteopenea and osteoporosis changes of skeletal system at comprehensive aspect of physical therapy.

Effects of treatment of temporomandibular disorders on headache, quality of life, and neck function in patients with tension-type headaches: a randomized controlled study

  • Objective: Tension-type headaches usually occur with temporomandibular disorder, which increases the risk of the chronic tension-type headaches. This study was conducted to investigate the effect of additional temporalmandibular therapy compared to cerivcal joint therapy for tension-type headaches with pericranial tenderness. Design: Randomized controlled trial. Methods: Forty-one patients with chronic tension-type headaches and pericranial tenderness were randomized into the 3 groups, such as the temporomandibular joint therapy group (TMJT group, n=14), cervical manual therapy group (CMT group, n=14) and conservative therapy group (CT group, n=13). All patients were assessed at baseline and after each intervention during the three sessions. The participants in the TMJT group received the temporomadibular joint treatment and cervical manual therapy for 30 minutes, once a week, for 3 weeks. The participants in the CMT group received the cervical manual therapy, and those in the CT group received modalities during same time period. The outcome measurements used were the intensity of headaches measured on the Visual Analogue Scale (VAS), quality of life measured with the Headache Impact Test (HIT-6), and function of the cervical spine using the Neck Diability Index (NDI). Results: The TMJT group that received temporomandibular joint treatment and cervical manual therapy showed a significant decrease in VAS, HIT-6, and NDI compared with the other 2 groups (p<0.05). Conclusions: This study suggected that temporomandibular joint treatment combined with cervical manual therapy was more effective for the chronic tension-type headaches with pericranial tenderness than the usual cervical therapy alone.

A Comparison of Three Constitution Types of Korean Hand Therapy (Seogeum Therapy) for Migraine Care (편두통환자와 건강인의 삼일체형 분류 비고)

  • Purpose: This study examines the relationship between the occurrence of migraine headaches and three constitution types of Korean Hand Therapy (Seogeum Therapy). Methods: A total of 684 subjects comprised the study data: 500 healthy individuals and 184 migraine sufferers. The data were analyzed according to three constitution types of Seogeum Therapy. The subjects were classified according to their constitution types and body sides. Results: There were significant differences between the three types. Among the healthy group, the Yang Type and Shin Type Constitutions were more predominant than the Eum Type; the Yang and Shin Types were also more predominant than Eum Type with respect to both sides of the body. Among the migraine sufferers, the Eum Type Constitution was more predominant than the Eum Type; the Eum Type was also more predominant than the other types with respect to both sides of the body. Conclusions: These results suggest that the Eum Type Constitution might be associated with migraine headaches.

  • The aim of study was compared with the possible role of cranial artery in headache pathogenesis each 2 group (group I : ultrasound therapy alone, group II : myofascial release therapy alone)- divided each 5 tension- type headache patients. Each group were applied ultrasound therapy for 5 minuets and myofascial release therapy for 15 minuets in occipital portion. The study carried out to determine the effects of ultrasound and myofascial release on the cranial arteries velocity from November 11, 2001 to March 29, 2002 the objects were 10 patients who having the tension-type headache at H-hospital. Transcranial doppler ultrasonograpy(TCD) is new non-invasive applicable method to evaluate flow velocities of intrans cranial and extracranial cerebral arteries. TCD was peformed with standard method to measure the Mean Flow Velocity(MFV) of the middle and posterior cerebral arteries, the vertebral arteries.

Novel Therapeutic Approaches to Mucopolysaccharidosis Type III

  • Mucopolysaccharidosis type III (MPS III) or Sanfilippo disease is an orphan-inherited lysosomal storage disease. It is one of the most common MPS subtypes. The classical presentation is an infantile-onset neurodegenerative disease characterized by intellectual regression, behavioral and sleep disturbances, loss of ambulation, and early death. Unlike other MPS, no disease-modifying therapy has been approved. Here, we review the curative therapy developed for MPS III, from historically ineffective hematopoietic stem cell transplantation and substrate reduction therapy to the promising enzyme replacement therapy or adeno-associated/lentiviral vector-mediated gene therapy. Preclinical studies are presented with recent translational first-in-man trials. We also present experimental research with preclinical mRNA and gene-editing strategies. Lessons from animal studies and clinical trials have highlighted the importance of early therapy before extensive neuronal loss. Disease-modifying therapy for MPS III will likely mandate the development of new early diagnosis strategies.

Nursing Students' Attitudes towards Complementary Alternative Therapy - Q methodological approach - (간호학생의 보완대체요법에 대한 태도 연구 - Q 방법론적 접근 -)

  • This study aims to identify the types and type-specific characteristics of subjectivity on complementary alternative therapy of nursing students using Q-methodology and to provide them as basic materials for nursing education. Q-methodology provides a scientific method for identifying perception structures that exist within certain individuals or groups. Thirty C college nursing students' classified 35 selected Q-statements into 9 points standard, according to the level of agreement or disagreement by forced normal distribution. The collected data was analyzed by using a QUNAL PC program. Resultingly, the attitude of nursing students regarding complementary alternative therapy was divided into three types. Type 1 is positive recognition. The students who think the therapy contributes to maintaining and improving health, and is a way to protect human body and improve welfare by enhancing immunity. Type 2 is objective acceptance. The students who think the complementary alternative therapy is effective when it is properly used for specific constitutions in discussion with medical teams after its effects are scientifically reviewed. Type 3 is popularity consideration. The students who put focus on its popular use and regard the therapy as very useful for not only patients but also ordinary healthy people because it is based on the natural and human approach. In conclusion, proposed are studies on influence factor of nursing students' awareness about the complementary alternative therapy and reviews on the effects of the complementary alternative therapy that is applied as nursing intervention.

  • Purpose: The purpose of this study was to investigate the effects of cervical mobilization on the tension type headache recovery. : The subjects were consisted of 70 patients with tension type headache. All subjects randomly assigned to cervical mobilization group and massage group. The mobilization group received cervical mobilization with modality treatment and massage group received cervical massage with modality treatment. Visual analogue scala(VAS) was used to daily headache hours and patient's pain level. Results: The results of this study were summarized as follows: 1. The Visual Analogue Scale(VAS) was mobilization group showed significant1y decreased more than massage group(p<.01). 2. The daily headache was mobilization group showed significantly decreased more than massage group(p<.01). Conclusion: mobilization is beneficial treatment for tension type headache.

Musculoskeletal diseases of heavy industrial workers

  • Objective: The purpose of this study was to investigate the musculoskeletal diseases (MSDs) that occur in heavy industrial workers according to the occupational category, prevalence, environment, and number of physical therapy visits. Design: Retrospective cohort study. Methods: For this study, data was collected of workers who were engaged in heavy industry in Korea and who visited the company physicians and received physical therapy in 2016. Data was collected from 855 subjects and was analyzed. With the data collected, analysis of the type and prevalence of MSDs and the number of physical therapy visits that have occurred according to the occupational category and environment was performed. Results: The most common MSDs were lumbar sprains and spasms (31.1%), and shoulder sprain and spasm (19.4%). In addition, the most common type of MSDs according to the occupational category and environment (occupational type) were lumbar sprain and spasms in 11 occupations, including white collar workers, and in milling, inspection, crane operation, and finishing jobs, shoulder muscle sprain and strain were the most common disorders. Also, the prevalence of MSDs according to occupational category and environment (occupational type) was the highest in workers involved with welding, which was 29.7%. Conclusions: Through this study, the type and prevalence of MSDs according to the occupational category and environment of heavy industrial workers have been confirmed. Further studies are necessary to study the future types of the work patterns of industrial workers and to develop a system for preventing and managing MSDs that may occur.

Types of literary therapy's subjective perceptions utilized by Q-methodology (Q-방법론을 활용한 문학치료에 대한 주관적 인식 유형)

  • In Korea, the word, 'Literary Therapy' can be replaced by 'Bibliotherapy', 'Journal Therapy', 'Writing Therapy', 'Poem Therapy', 'Integrated Literary Therapy', or 'Imaginary-Oriented Poetry Therapy', and so on. This study, intended on people related to literary therapy, uses Q-methodology as useful research method of objectifying abstract concepts which can be perceived differently according to individual experience or perception, to investigate types of literary therapy's subjective perceptions. The result of study can be analyzed by four types: The first type is 'Pursuing Generalization', the second type is 'Considering the Therapist', the third type is 'Emphasizing the Integration', and the forth type is 'Maximizing the literary therapy'. Through this result of the study, we can find the responsibility of literary therapy as a leader of cultural, artistic therapy to break down various pathological situations which contemporary society have.
