Studies on Dose Distribution and Treatment Technique of High Energy Electron (고(高)에너지 전자선(電子線) 치료(治療)를 위(爲)한 선량분포(線量分布) 및 기술적(技術的) 문제(問題)의 연구(硏究))

  • High energy electron beams took effect for tumor radio-therapy, however, had a lot of problems in clinical application because of various conversion factors and complication of physical reactions. Therefor, we had experimentally studied the important properties of high energy electron beams from the linear accelerator, LMR-13, installed in Yonsei Cancer Center. The results of experimental studies on the problems in the 8, 10, 12 Mev electron beam therapy were reported as following. 1. On the measurements of the outputs and absorbed doses, the ionization type dosimeters that had calibrated by $^{90}Sr$ standard source were suitable as under 3% errors for high energy electrons to measure, but measuring doses in small field sizes and the regions of rapid fall off dose with ionization chambers were difficult. 2. The electron energy were measured precisely with energy spectrometer consisted of magnet analyzer and tele-control detector and the practical electron energy was calculated under 5% errors by maximum range of high energy electron beam in the water. 3. The correcting factors of perturbated dose distributions owing to radiation field, energy and material of the treatment cone were checked and described systematically and variation of dose distributions due to inhomogeneous tissues and sloping skin surfaces were completely compensated. 4. The electron beams, using the scatterers; ie., gold, tin, copper, lead, aluminium foils, were adequately diffused and minimizing the bremsstrahlung X-ray induced by the electron energy, irradiation field size and material of scatterers, respectively. 5. Inproving of the dose distribution from the methods of pendulum, slit, grid and focusing irradiations, the therapeutic capacity with limited electron energy could be extended.

  • The authors carried out bovine mastitis test by California Mastitis test (CMT) for the milk from dairy cows in the suburbs of Daejeon. Among the CMT positive milk, Staphylococcus aureus were isolated and, for the proper selection of the commercial antibacterial agents, the strains were tested for the resistant test to the agents. The results obtained were summarized as follows. 1. The resistant test appeared graduate resistance is in such cases continuously repeat with the therapeutic antibacterial agents. 2. The result of resistant test appeared the weakly resistance, untill 4 times subculture in the Rifamastene, Neo-cillin, and 6 times in the Abimasten-100, Neo-masti, Special formular and mastalone. 3. In Tardomycel-L there was rare resistant reactions, untill 6 times subculture. 4. The author recommend that select the antibacterial agents by resistant test and use the selective agents in turn.

Endoscopic transthoracic sympathectomy: An efficient and safe method for the treatment of hyperhidrosis (흉강내시경을 이용한 교감신경절 절제술 -다한증의 치료를 위한 효과적이고 안전한 방법-)

  • Hyperhidrosis of the palms, axillae, and the face has a strong negative impact on the quality of life for many people. Existing nonsurgical therapeutic options are far from ideal. A definitive cure can be obtained by upper thoracic sympathectomy. The traditional open surgical technique is a major procedure; few patients and doctors have found that risk-benefit considerations favor surgery. Endoscopic minimal invasive surgical techniques are now available. We investigated whether endoscopic ablation of upper thoracic sympathetic chain(T2-4) is an efficient and safe treatment of hyperhidrosis. We treated 40 patients with bilateral endoscopic transthoracic sympathectomy. There were no mortalities, life-threatening complication, no treatment failures occurred in 40 patients. After a short-term follow-up, 100% of the patients reported satisfactory results. Endoscopic transthoracic sympathectomy is an efficient, safe, and minimally invasive surgical method for the treatment of palmar, axillary hyperhidrosis.

The Characterization of Anti-HER-2/neu Monoclonal Antibody using Different in vivo Imaging Techniques

  • Recently, specific antibodies have been used extensively to diagnose and treat various diseases. It is essential to assess the efficacy and specificity of antibodies, especially the in vivo environment. Anti-HER-2/neu mAb was evaluated as a possible transporting agent for radioimmunotherapy. The monoclonal antibody was successfully radio-labeled with $^{131}I$. In vitro binding assays were performed to confirm its targeting ability using another radio-iodine, $^{125}I$. Binding percentage of $^{125}I$ labeled anti-HER-2/neu mAb in HER-2/neu expressing CT-26 cells was found to be 4.5%, whereas the binding percentage of $^{125}I$ labeled anti-HER-2/neu mAb in wild-type CT-26 was only 0.45%. In vivo images were obtained and analyzed through $\gamma$-camera and an optical fluorescent modality, IVIS-200. $\gamma$-camera images showed that $^{131}I$ labeled anti-HER-2/neu mAb accumulated in HER-2/neu CT-26 tumors. Optical imaging based on near infrared fluorescence labeled anti-HER-2/neu mAb showed higher fluorescence intensities in HER-2/neu CT-26 tumors than in wild-type CT-26 tumors. Anti-HER-2/neu mAb was found to specifically bind to its receptor expressing tumor. Our study demonstrates that in vivo imaging technique is a useful method for the evaluation of an antibody's therapeutic and diagnostic potentials.

  • Taping techniques have been usually used by physical therapists long time ago, which have been considered a useful adjunct to treatment programs for a variety of musculoskeletal disorders. Also, taping techniques may be employed as effective prophylactic methods in clinical setting because of ease application and cost effectiveness. The aims of this study were to describe background information for the management of some chronic low back pain patients with/without leg pain that don't respond to conservative treatment, to demonstrate McConnell taping as successful therapeutic strategies for treating these patients, and to provide detailed application methods of McConnell taping in order that physical therapists can readily use the taping in clinical setting. This study emphasized to illustrate biomechanical benefits of McConnell taping in controlling undesirable muscle activation by decreasing mechanical loads on specific muscles. McConnell taping may be helpful for the inhibition of overactive synergist or antagonists, the facilitation of inactive synergists, the promotion of proprioception, the optimization of joint alignment, pain reduction, and unloading of irritable neural tissue. This study provides taping examples of low back pain, sacroiliac joint dysfunction and lower extremity symptoms associated with these conditions, and discusses the possible mechanisms for their successful application.

Genotoxicity of Taxol and 10-Deacetyl Baccatin III Using Single Cell Gel Electrophoresis (Comet Assay) in Chinese Hamster Lung Fibroblast

  • Taxol is used as cancer therapeutic agent. It has been known as weak posotive of chromosome aberration assay in vitro in our previous results (Ryu et al., 1996) and potent clastogens in the mouse bone marrow micronucleus (Tinwell and Ashby, 1994). We performed microgel electrophoresis to determine the effect of taxol and it's precursor 10-deacetyl baccatin III(DAB) on DNA. Microgel electrophoresis is useful, rapid, simple, visual, and sensitive technique for measuring DNA breakage and repair mechanisms in mammalian 근ells. The range of concentration used for taxol were 854, 427, 213.5, 106.8, 53.4 Ug/ml, for DAB 910 ,455, 227.5 U9/ml, Cell viability always exceed 85%. We analyzed the results by using the special software of image analyzer for this comet assay (Komet 3.0). By using this image analyzer software , we can get the result as the tail moment ((mean of tail length - mean of head lengh) x tail%DNA/100). A slight increase in DNA migration was observed for taxol at the concentration of 854 Ug/m4 in the absence of S9 mixture. No increased DNA migration was observed after treatment with DAB.

The Effectiveness of Evjenth-Hamberg Stretching with regards to Muscle Tone and Flexibility in Adults with Hamstring Tightness (에비안스-함베르크 스트레칭이 뒤넙다리근 단축이 있는 성인의 근긴장도 및 유연성에 미치는 영향)

  • PURPOSE: In adults with hamstring tightness, stretching is considered a therapeutic exercise that improves flexibility. However, previous studies have reported efficacy of numerous methods, and are inconclusive in determining the most effective stretching techniques. This study aims to compare the outcomes of Evjenth-Hamberg stretching (E-HS), proprioceptive neuromuscular facilitation stretching (PNFS), and static stretching (SS), on muscle tone and flexibility. METHODS: A total of 30 subjects were assigned to each of the three stretching groups:; E-HS (n1 = 10), PNFS (n2 = 10), and SS (n3 = 10). Muscle tone of the hamstring muscle, active knee extension (AKE), and passive knee extension (PKE) were assessed by range of motion. RESULTS: After the intervention, statistically significant differences were obtained between groups for muscle tone (p < .05). The post hoc test, showed statistically significant differences in muscle tone between the E-HS and PNFS groups (p < .05), and PNFS and SS groups (p < .05). Post hoc test after intervention, also revealed statistically significant differences in flexibility (p < .05) between the E-HS and PNFS groups (p < .05), E-HS and SS groups (p < .05), and PNFS and SS groups (p < .05). Moreover, a statistically significant difference was observed in PKE scores between the E-HS and SS groups (p < .05), and PNFS and SS groups (p < .05). CONCLUSION: This study indicates that E-HS may be the most effective stretching technique for muscle tone and flexibility, in adults with hamstring tightness.

A Comparison on the Operative Results of Benign Esophageal Disease by Video-Assisted Thoracic Surgery and Thoracotomy (양성 식도질환에서 개흉술과 비디오 흉강경을 이용한 수술 성적의 비교)

  • Background: Video-assisted thoracic surgery(VATS) is being used as a therapeutic modality in many diseases in which thoracotomy has been used. We studied that the VATS can substitute the thoracotomy in benign esophageal disease. Material and Method: Group I (n=18) underwent video-assisted thoracic surgery, and group II(n=19) thoracotomy. Group I includes 14 leiomyomas and 4 achalasias. Group II includes 16 leiomyomas and 3 achalasias. Operative technique is enucleation in the leiomyoma and modified Heller's myotomy in the achalasia. Analyzing factors of operation-efficacy are anesthetic time, operation time, hospital stay, chest tube drainage amount and chest tube removal day. The degree of the postoperative pain is assessed by the frequency of opioid analgesics injection. Result: There was no death in both groups. There were 5 complications in the group I and 2 in the group II. Prolonged pleural effusion and restenosis of achalasia occurred to 1 patient in each group. In the group I, there were 1 temporaty vocal cord palsy and 2 mucosal tear leading to thoracotomy. There were no differences in anesthesia time, operation time, hospital stay, total chest tube drainage amount, chest tube removal day and frequency of opioid analgesics injection. The amount of the chest tube drainage at POD 1 day was significantly lower in group I(155.6$\pm$77.8cc) than in group II(572.8$\pm$280.1cc)(p<0.05). Conclusion: The results of our data showed that video-assisted thoracic surgery for benign esophageal disease is as effective as thoracotomy and in addition, cosmetic effect is much better. We concluded VATS may be a substitute for thoracotomy in benign esophageal disease.

Effect of the Upper Limb Nerve Mobilization on Functional Recovery in Hemiplegic Patients Following Stroke (상지신경 가동기법이 뇌졸중 후 편마비 환자의 기능회복에 미치는 영향)

  • The purpose of this study was to examine the effect of the upper limb nerve mobilization (ULNM) on functional recovery of upper extremity in hemiplegic patients following stroke. Twenty patients who had functional impairment on upper extremity were participated. Subjects were randomly divided into two groups: Control group (n=10) received traditional physical therapy only for 4 weeks; Experimental group (n=10) received ULNM treatment along with traditional physical therapy for the same period. Upper extremity functions were assessed by manual muscle test (MMT), modified Ashworth scale (MAS), and Fugl-Meyer assessment (FMA) before and after the treatment. In both experimental and control group, upper extremity functions were significantly improved in MMT (p<.01) and FMA (p<.01), however only experimental group showed significant improvement in FMA after the treatment (p<.05). Moreover, experimental group showed significantly greater improvement than control group in MMT (p<.05), MAS (p<.05), and FMA (p<.05). We conclude that the upper extremity functions is a useful additional therapeutic technique for the effective treatment of upper extremity deficits in hemiplegic patients.

Male Infertility in the Era of ICSI (ICSI시대에서의 남성불임)

  • As a result of the technological advance provided by intracytoplasmic sperm injection (ICSI) in 1992, the evaluation and treatment of the infertile male has changed significantly. Many men who were previously thought to be irreversibly infertile have the potential to initiate their own biologic pregnancy. However, not all men having impaired semen parameter are ideal candidates for ICSI for numerous reasons including a lack of addressing the underlying problem causing the male infertility, unknown genetic consequences, and cost-effectiveness issues. In this era of ICSI, the fundamental approach to the male with suspected subfertility is unchanged and is based on a history, physical examination, and focused laboratory testing. The urologist should approach the patient with an intent to identify remediable causes of subfertility given the specific clinical situation. For instance, should a gentleman have his varicocele repaired or vasectomy reversed, or should he proceed directly with ICSI? If no factors can be improved in a timely manner, then ICSI should be considered using the available sperm. Examples of recent advances include the diagnosis and treatment of ejaculatory duct obstruction, indications and techniques for performing testis biopsy, and technique for sperm harvesting. In addition, potential genetic causes of male subfertility should be diagnosed and discussed with the patient. Cystic fibrosis gene mutation, karyotype abnormallities, and Y-chromosome microdeletions all have recently been identified as causative for male infertility in otherwise phenotypically normal men. With recently evolved diagnostic and therapeutic techniques now available for the infertile couple, even the most severe male factor problems in patients previously considered irreversibly infertile are now potentially treatable. The physician should be aware of the availability and limitations of these new and exciting reproductive technologies because they will allow him to provide timely and more effective therapy for the infertile couple. An understanding of these advances by all physicians is important as we progress into the $21^{st}$ century
