The Recovery Process of Alcohol Dependent Men Living in a Therapeutic Community (치료공동체에 거주하는 남성 알코올의존자의 회복과정)

  • Purpose: The purpose of this study was to explore the recovering process of men who had abused alcohol and has lived in a therapeutic community. Methods: Individual in-depth interviews were used to collect data from 10 of these men who has lived in the therapeutic community for more than one year. Qualitative data from field notes and transcribed notes were analyzed using the grounded theory methodology developed by Strauss and Corbin. Results: The core category about the recovering process of the men who had abused alcohol and are now in the therapeutic community was identified as "reconstructing a broken life". The recovering process of these men in the therapeutic community consisted of four phases; 'self-awareness stage', 'unfreezing stage', 'readjustment stage', and 'challenging stage'. Conclusion: In this study "reconstructing a broken life", as the core category vividly showed joys and sorrows of men who had abused alcohol and has lived in the recovering process of managing the yoke of life-long disease. In this process of recovery from alcoholic dependence the men gradually adjusted themselves to their given condition. Also they gained coping strategies to care for, and protect themselves. Therefore health care providers can establish supportive programs in the clinical field to empower these men by reflecting their proactive coping strategies.

Research on Therapeutic Factors of Group Counseling for Elementary School Students (초등학교 집단상담의 치료요인에 대한 연구)

  • The purpose of this study is to examine the therapeutic factors of group counseling conducted to the elementary school students. To achieve the purpose, the researchers of this study analysed questionnaire of the 9 therapeutic factors proposed by the Korean Elementary Counseling Education Society(2006) and the survey essay written by the students were added for the research. The reliability (Cronbach's ${\alpha}$) of the whole questions were .753 and the subjects of the survey were 13 paper of elementary group counseling and 213 elementary students. The whole students were classified into two groups which are counseling group and guidance group and the students were classified into 3 types (A, B, C type) by the characteristics they possess. The results of the study are as follow. First, the factors displayed highly in 9 therapeutic factors is the learning of interpersonal relations, the considerations of others, the cohesive power. Second, the 9 therapeutic factors displayed significant difference by the gender and type of the students. Third, the factors displayed highly in analysis of survey essay is the acquisition of informations, the self-understanding, the feeling of intimacy, the other-understanding, the catharsis.

  • The application of recombinant DNA technology to restructure metabolic net-work can change metabolite and protein products by altering the biosynthetic pathways in an organism. Although some success has been achieved, a more detailed and thorough investigation of this approach is certainly warranted since it is clear that such methods hold great potential based on the encouraging results obtained so far. In last decade, there have been tremendous advances in the field of glycobiology and the stage has been set for the biotechnological production of glycoproteins for therapeutic use. Today glycoproteins are one of the most important groups of pharmaceutical products. In this study the attempt was made to focus on identifying technologies that may have general application for modifying glycosylation pathway of the yeast cells in order to produce glycoproteins of therapeutic use. The carbohydrates of therapeutic recombinant glycoproteins play very important roles in determining their pharmacokinetic properties. A number of biological interactions and biological functions mediated by glycans are also being targeted for therapeutic manipulation in vivo. For a commercially viable production of therapeutic glycoproteins a metabolic engineering of a host cell is yet to be established.

Mediating Effects of Hope and Therapeutic Relationship in the Relation between General Social Functions and Mental Health Recovery of Community People with Mental Illness (지역사회 정신장애인의 전반적 사회기능과 정신건강 회복 간의 관계에서 희망과 치료적 관계의 매개효과)

  • Purpose: This study tries to test mediating effects of hope and therapeutic relationship in the relation between general social functions and mental health recovery of community people with mental illness. Methods: This study was carried out in a cross-sectional research design. The participants included 217 people with mental illnesses who were enrolled at eight Mental Health Welfare Centers in the Gyeonggi Province. Data were collected from February to May, 2018. The collected data were analyzed using a regression analysis, and SPSS PROCESS macro was used to test the mediating effects. Results: This study analyzed the direct effects of general social functions on the hope, therapeutic relationship and mental health recovery. And general social functions had indirect effects on their mental health recovery via hope and therapeutic relationship. Both hope and therapeutic relationship had dual mediating effects in the influence of general social functions on mental health recovery. Conclusion: The role of hope and therapeutic relationship in the mental health recovery of community people with mental illness is important, and it is confirmed that hope is a powerful factor influencing mental health recovery.

Anti-tumor Effect of 4-1BBL Modified Tumor Cells as Preventive and Therapeutic Vaccine

  • We have previously reported that genetically modified tumor cells with 4-1BBL have anti-cancer effects in a CT26 mouse colorectal tumor model. In this study, genetically modified tumor cells with 4-1BBL were evaluated for their potential as candidates for preventive and therapeutic cancer vaccine. To identify the effect of preventive and therapeutic vaccine of genetically modified tumor cells with 4-1BBL, tumor growth pattern of CT26-4-1BBL as a cancer vaccine was examined compared to CT26-beta-gal. In therapeutic vaccination, CT26-WT was inoculated into mice and then vaccinated mice with doxorubicin (Dox)-treated CT26-beta-gal and CT26-4-1BBL (single or three times). Triple vaccination with Dox-treated tumor cell inhibited tumor growth compared to single vaccination. Vaccination with CT26-4-1BBL showed an efficient tumor growth inhibition compared to vaccination with CT26-beta-gal. For preventive vaccination, Dox-treated CT26-beta-gal and CT26-4-1BBL was vaccinated into mice with three times and then administered mice with CT26-WT. Preventive vaccination with CT26-4-1BBL showed no tumor growth. Preventive vaccination with CT26-beta-gal also led to tumor-free mice. These results suggest that genetically modified tumor cells with 4-1BBL can be used as therapeutic or preventive cancer vaccine.

Multiple Myeloma and Epidural Spinal Cord Compression : Case Presentation and a Spine Surgeon's Perspective

  • Multiple myeloma, a multicentric hematological malignancy, is the most common primary tumor of the spine. As epidural myeloma causing spinal cord compression is a rare condition, its therapeutic approach and clinical results have been reported to be diverse, and no clear guidelines for therapeutic decision have been established. Three patients presented with progressive paraplegia and sensory disturbance. Image and serological studies revealed multiple myeloma and spinal cord compression caused by epidural myeloma. Emergency radiotherapy and steroid therapy were performed in all three cases. However, their clinical courses and results were distinctly different. Following review of our cases and the related literature, we suggest a systematic therapeutic approach for these patients to achieve better clinical results.

Therapeutic Effect of Combined Radiotherapy and Hyperthermia in Primary Hepatocellular Carcinoma (원발성 간암의 방사선치료및 온열요법의 병용치료 효과)

  • Purpose : This study was undertaken to show the clinical results of combined radiotherapy and hyperthermia in primary hepatoma Materials and Methods : Between December 1989 and March 1993, 50 patients with hepatomas were treated by combined radiotherapy and hyperthermia. Among them, we analyzed retrospectively 33 patients who received the complete course of treatment. The ages of the patients ranged from 36 to 75(mean age: 55.5 years). Twenty-six patients ($78.8\%$) were men, and 7 ($21.2\%$ were women. According to Child's classification, nine patients ($27.3{\%}$) were A group, 9 ($27.3\%$) were B group, 15 ($45.4\%$) were C group. Radiation therapy was done by a 6 MV and 15 MV linear accelerator. Patients were treated with daily fractions of 150-180 cCy to doses of 2550 cGy -4950 cGy (median : 3000 cGy). Local hyperthermia was done by 8 MHZ RF capacitive heating device (Cancermia. Green Cross Co., Korea), 50-60 min/session, 1-2 sessions/wk, and 8.5 sessions (median number)/patient. We analyzed the prognostic factors including age, sex, tumor type, Child's classification, $\alpha$-fetoprotein, liver cirrhosis, ascites, portal vein invasion, esophageal varix, number of hyperthermia, chemotherapy, total bilirubin level, Karnofsky perfomance status. Results : The overall 1-year survival was $24.2\%$, with a mean survival of 10months. Of 33 patients, tumor regression (PR+MR) was seen in $30.4\%$, no response was seen in $52.2\%,\;17.4\%$ patient was progressed. In patients who had tumor regression, the overall 1-year survival was $42.1\%$ with a mean survival of 14 months. Factors influencing the survival were sex (p=0.05), tumor type (p=0.0248), Child's classification (p=0.0001), liver cirrhosis (p=0.0108), ascites (p=0.0009), and Karnofsky perfomance status (p=0.0028). Complications developed in 28 patients, including 18 hot pain,5 fat necrosis, 3 transient fever, 2 nausea and vomiting. Conclusion : In this study, the results suggests that combined radiotherauy and hyperthermia may improve the survival rate of hepatoma.

The development of therapeutic recreation programs for the elderly: implementation, monitoring and evaluation (노인 우울예방 프로그램 개발 및 효과검증)

  • The objectives of this study were to develop the therapeutic recreation programs for relieving depressive symptoms of the elderly and to verify the effect on their psychological condition. The key elements of these programs, which consisted of 9 sessions. were aerobic exercises, group dynamics. and making fun. These programs were developed through a multidisciplinary approach with social workers and the faculties of preventive medicine. The social workers gave these programs to fifteen elderly people at the community center, so called noinjung, for 9 weeks. Before intervention of this program for the experimental group, the baseline was measured by GDS(geriatric depression scale). After intervention for 9 weeks, GDS as an outcome was measured to evaluate the effects of the program. This data was analyzed by $X^2$-test and Wilcoxon signed rank test. and the results were as follows: 1. Based upon the cutting point(GDS = 5) which could distinguish between depression group and normal group, the number for the normal group increased, while the number for the depression group decreased slightly after intervention with the therapeutic recreation programs, however, there was no significant change(p〉0.05). 2. The score of GDS decreased significantly after intervention with the therapeutic recreation programs(p〈0.05). 3. The scores of GDS decreased in widow(or) group and in female group compared to those of the married group and the male group, respectively(p〈0. 05). 4. The score of GDS increased in groups with similar labor and emotional support compared to those of groups with sufficient or less support, respectively(p〈0.05), These findings indicate that the therapeutic recreation programs might be effective for relieving depressive symptoms of the elderly. It is, therefore, suggested that this program be modified and standardized through review of the intervening process, experimental results, and responses of clients for appling in other noinjungs.

Effects of Therapeutic Exercise on Pain, Range of Motion, Strength and Balance Ability in a Patient with Anterior Ankle Impingement: A Case Study (치료적 운동이 앞 발목 충돌 증후군 환자의 통증, 관절가동범위, 근력 및 균형능력에 미치는 영향: 증례보고)

  • PURPOSE: The purpose of this study is to determine the effect of therapeutic exercise on pain, range of motion and strength, and balance ability in a patient with an anterior ankle impingement (AAI). METHODS: A-32-year-old male presented limited ankle motion and pain with forced dorsiflexion at both ankle joints. In response, a therapeutic exercise program consisted of ankle joint mobilization, strength exercises using an elastic band, and proprioceptive exercises including semi-squats and a one-legged standing exercise with open and closed eyes. The program was performed for 40 min/day, twice per week, for 8 weeks. Pain, range of motion (ROM), and muscle strength (ankle dorsiflexion, plantarflexion, inversion, and eversion), as well as a one-legged standing test of both ankles, were measured before and after 4 and 8 weeks of therapeutic exercise. RESULTS: VAS decreased in both ankles after 8 weeks of exercise, respectively, compared to baseline levels. Range of motion and strength increased in both ankles for dorsiflexion, plantarflexion, inversion, and eversion after 8 weeks of exercise compared to baseline levels. In addition, the ability to perform a one-legged standing test with eyes opened and closed improved in both legs after 8 week of exercise compared to baseline levels. CONCLUSION: These results suggest that therapeutic exercise improves pain, ROM, muscle strength, and balancing ability in patients with AAI.

Psychological Needs of Game Addiction: An Exploratory Study Focusing on Therapeutic Catharsis Seeking and Game Self-Efficacy (카타르시스 추구와 자아 효능감에 기반한 게임 과몰입의 심리적 욕구에 관한 탐색적 연구)

  • There are general surveys on factors related to game addiction. However, less is known about the relationships between game addiction, therapeutic catharsis seeking, and game and life self-efficacy in terms of desired needs. This study sought to clarify several of these relationships. Using data from a survey of 489 online game users, we conducted a regression analysis to explore the association between therapeutic aspects (therapeutic catharsis seeking and game and life self-efficacy), psychological problems (loneliness and depression), and demographic variables (age and gender). Results showed that both therapeutic catharsis seeking and game self-efficacy increase game addiction. The implications of these results are discussed.
