갑상선기능항진증(甲狀腺機能亢進症)의 임상적(臨床的) 관찰(觀察) (Clinical Observation on Hyperthyroidism)

  • A clinical analysis was made on 161 cases of hyperthyroidism seen at the Radioisotope Laboratory of Kyungpook National University Hospital. This series consisted of 144 cases of diffuse goiter and 17 cases of nodular goiter. 1) Hyperthyroidism was most prevalent in the 4th decade and male to female ratio was 1:4.6. 2) Cardinal symptoms in the order of frequency were weakness, easy fatigability, palpitation, weight loss, nervousness, perspiration, heat intolerance, increased appetite, insomnia and dysmenorrhea. 3) Major physical findings in the order of frequency were goiter, fine tremor, tachycardia, wide pulse pressure, emaciation, warm moist skin, exophthalmos, systolic hypertension and atrial fibrillation. 4) The complications were ophthalmopathy (34.2%), thyrotoxic heart disease (5.6%), thyroid crisis (1 case), pretibial myxedema (1 case) and thyrotoxic myopathy (1 case). 5) Mean values of the six hour and twenty-four hour $^{131}I$ uptakes by the thyroid glands were 67.5% and 71.6%, respectively, in diffuse goiter and 64.5% and 65.0%, respectively, in nodular goiter. 6) Mean values of twenty-four hour $PB^{131}I$ conversion ratio were 76.3% in diffuse goiter and 70.2% in nodular goiter and those of the basal metabolic rate was +51% in the former and +41% in the latter. Mean serum cholesterol level was 152mg% in diffuse goiter and that in nodular goiter was 175mg%. 7) Among the 134 cases treated with $^{131}I$, 66 cases (49.3%) were successful1y controlled with single dose and in the majority of the cases the initial therapeutic dose required was $4.1{\sim}5.0mC$ in diffuse goiter and $5.1{\sim}6.0mC$ in nodular goiter. 8) With $^{131}I$ treatment the symptoms improved in the following order: heat intolerance, emaciation, nervousness, insomnia, easy fatigability, weakness, fine tremor, goiter, perspiration, exertional dyspnea and palpitation. And in a few cases improvement of even exophthalmos was seen. 9) Following $^{131}I$ treatment myxedema occurred in 4 cases (3%) and reccurrence in 9 cases (6.8%).

  • A summary of the clinical data of the $^{131}I$-thyroid function tests and the therapeutic results of $^{131}I$ among the 2,658 patients of various thyroid diseases treated over the past 10 years from May 1960 to Oct. 1969 at the Radioisotope Clinic and Laboratory, SNUH were presented and dscussed. 1. The patients examined consisted of: 929 cases (34.9%) of diffuse toxic goiter, 762 cases (28.7%) of diffuse nontoxic goiter, 699 cases (26.3%) of nodular nontoxic goiter, 58 cases (2.2%) of nodular toxic goiter and 210. cases (7.9%) of hypothyroidism. 2. There were 300 (11.4%) male and 2358 (88.6%) female, showing a ratio of 1:8. 3. The majority of patients (79.1%) were in the 3rd-5th decades of their lives. 4. The normal ranges, diagonstic values of $^{131}I$ uptake test, 48 hrs, serum activity, BMR and main subjective symptoms of various thyroid diseases were discussed. 5. In the 579 patients among 867 cases with hyperthyroidism treated with $^{131}I$, 47.8% were confirmed to be cured completely after single therapeutic doses. 6. The complications of $^{131}I$ therapy were discussed and myxedema had developed in 6.75% of our patients. 7. The results of $^{131}I$ thyroid function tests were analysed among the 160 cases of thyroid diseases which were confirmed the diagnosis with histopathological measures.

$^{166}Ho$-chitosan 복합체를 이용한 낭성뇌종양 치료를 위한 베타선의 흡수선량 평가 : 구형 모델을 이용한 Monte Cairo 모사계산 (Beta Dosimetry for Applying $^{166}Ho$-chitosan Complex to Cystic Brain Tumor Treatment : Monte Carlo Simulations Using a Spherical Model)

  • 낭성뇌종양은 낭 내부에 베타선을 방출하는 방사성동위원소를 주입하여 낭 내부 및 낭벽에 존재하는 암세포에 일정량의 방사선 에너지를 전달함으로써 그 치료 효과를 기대할 수 있다. 본 연구에서는 $^{166}Ho$-chitosan 복합체를 낭성뇌종양 치료에 이용하고자 할 때 낭의 크기와 주입되는 방사능의 변화에 따라 낭벽에 전달되는 방사선 흡수선량이 어떻게 변화하는가를 평가하고자 한다. 구형의 종양성 낭 모델에 대하여 Monte Carlo code인 EGS4를 이용하여 $^{166}Ho$ 베타선의 에너지 전달 현상에 대한 모사계산을 수행한다. 종양성 낭 내부에 주입된 $^{166}Ho$-chitosan 복합체의 낭내 분포는 낭 내부액과 섞여있거나 낭벽 표면에 부착되는 두 가지 경우를 고려한다. 방사선 조사의 표적 영역으로서, 낭벽의 표면으로부터 매 1mm 깊이의 체적을 설정하여 4mm 깊이까지 고려한다. 직경이 각 1cm, 2cm, 그리고 3cm 인 종양성낭을 평가 대상으로 설정한다. 직경이 3cm인 종양성 낭에 10mCi의 $^{166}Ho$-chitosan 복합체가 주입되어 낭 내부에 균일하게 분포하였다고 가정하였을 경우에 1mm 두께의 낭벽에 전달되는 방사선 흡수선량은 매 1mm 깊이의 낭벽 체적에서 각각 40.06Gy, 14.96Gy, 5.315Gy, 1.660Gy으로 계산되었다. 한편, 낭 내부에 주입된 10mCi의 $^{166}Ho$-chitosan 복합체가 낭벽에 균일하게 분포하였다고 가정하였을 경우에는 매 1mm 두께의 낭벽 체적에 전달되는 방사선 흡수선량이 601.7Gy, 188.7Gy, 73.87Gy, 27.80Gy로 평가되었다. 낭 내부에 주입된 $^{166}Ho$-chitosan 복합체가 낭벽에 부착될 가능성이 있음이 한 임상 적용 예에서 시사된 바, 정확한 $^{166}Ho$-chitosan 복합체의 낭 내부벽 부착률을 확인함으로써 낭벽에 대한 흡수선량을 예시하고 이를 근거로 주입할 $^{166}Ho$-chitosan 복합체의 양을 결정해야 할 것이다.

  • Several recent advances in our knowledge of thyroid physiology have broad application to the diagnosis and management of thyroid disorders. For in the thyroid, more than other endocrine organs, pathophysiology can be translated directly into the diagnosis and management of thyroid disease. Graves' disease is a syndrome including goiter with hyperthyroidism, exophthalmos and dermopathy. The pathogenesis of Graves' disease is not yet clearly identified, but various autoantibodies to the thyroid gland and immunopathologic studies indicate that autoimmune processes are involved in the pathogenesis of the disease. The diagnosis and management of Graves' disease are largely dependent on radionuclide techniques as radioimmunoassay, radioactive iodine therapy and so on. Several laboratory tests are also developed to determine the remission of this disase including TRH stimulation test, $T_3$ suppression test and detection of thyroid stimulating immunoglobulins. Autoimmune thyroiditis is almost certainly a primary immunologic disease and the incidence tends to increase recently, mainly due to the application of biopsy technique in thyroid diseases. Thyroid nodules have been a great challenge to physicians because of the possibility of malignancy. But recently, cytologic examination of thyroid aspirate provides a very simple and also reliable diagnostic method in patients with thyroid nodules. In 163 patients with thyroid nodules, only 19.3% was revealed to be malignant. Therefore cytologic examination of thyroid aspirate and thyroid biopsy should be included in the diagnosis of nodular patients prior to surgical intervention. In this paper, a comprehensive review is presented on the pathogenesis, clinical features, laboratory findings and therapeutic modalities of various thyroid diseases on the basis of over 80 researches performed during the past 20 years at radioisotope clinic, Seoul National University Hospital.

$Lipiodol-I^{-131}$의 경간동맥 주입에 의한 간암치료의 가능성에 관한 연구 (Therapeutic Feasibility Study and Clinical Trial of Intrahepatic $I^{-131}-Lipiodol$ on Patients with Hepatocellular Carcinoma)

  • An iodized oil such as Ethiodol or Lipiodol was selectively retained in the tumor vessels of the large hepatomas as well as in the small daughter hepatomas for long periods following the intra-arterial hepatic injection of such contrast material. The specific aim of the study is to deliver a high internal radiation dose to hepatocellular carcinoma (HCC) in an attempt to control the disease. We were able to replace a small fraction of the stable iodine (I-127) of the 37% iodine in Lipiodol by the $I^{-131}$ with 100% exchange efficiency. $I^{-131}$ labeled Lipiodol was injected through the super-selected tumor feeding artery under superselection or into the proper hepatic arterial level of patients who have malignant hepatomas confirmed by aspiration cytology serum AFP and various imaging modalities. Clinical traial was performed on 43 cases during recent 6 months and follow-up observation was carried out. No severe complications or other adverse reactions were encountered until nowdays. $I^{-131}-Lipiodol$ was stable in vivo and no significant activity was noted in the thyroid, stomach, blood and urine after the injection. Only small fraction of radioisotope activity was noticed in the both side of lungs. Tumor to normal liver radio was very high. Therefore, $I^{-131}-Lipiodol$ (or P-32-Lipiodol) will be effective delivering high internal radiation dose to the tumor while delivering small radiation doses to normal tissues. Labeling, tumor dose calculation and preliminary findings will be presented.

Application of extraction chromatographic techniques for separation and purification of emerging radiometals 44/47Sc and 64/67Cu

  • Considerably increasing interest in using the theranostic isotopes/ isotope pairs of radiometals like $^{44/47}Sc$ and $^{64/67}Cu$ for diagnosis and/or therapeutic applications in the nuclear medicine procedures necessitates its reliable production and supply. Separation and purification of no-carrier-added (NCA) isotopes from macro quantitates of the irradiated target matrix along with other impurities is a cardinal procedure amongst several other steps involved in its production. Multitudinous methods including but not limited to liquid-liquid (solvent) extraction, extraction chromatography (EXC), ion exchange, electrodeposition and sublimation are routinely applied either solitarily or in combination for the separation and purification of radioisotopes depending on their production routes, radioisotope of interest and impurities involved. However, application of EXC though has shown promises towards the numerous separation techniques have not received much attention as far as its application prospects in the field of nuclear medicine are concerned. Advances in the recent past for application of the EXC resins in separation and purification of the several medically important radioisotopes at ultra-high purity have shown promising behavior with respect to their operation simplicity, acidic and radiolytic stability, separation efficiencies and speedy procedures with the enhanced and excellent extraction abilities. In this mini review we will be talking about the recent developments in the application and the use of EXC techniques for the separation and purification of $^{44/47}Sc$ and $^{64/67}Cu$ for medical applications. Furthermore, we will also discuss the scientific and practical aspects of EXC in the view of separation of the NCA trace amount of radionuclides.

RI검출 바이오칩의 혈관계 질환 발생 위험인자 검지에 대한 타당성 연구 (Feasibility Study on RI Biochip Application to Detection of Risk Factors of Atherosclerosis)

  • Microarrays can be used to screen thousands of binding events in a parallel and high throughput fashion and are of major importance in disease diagnosis and drug discovery. The use of radioisotope is conventionally regarded as one of the most sensitive detection methods. Atherosclerosis is a common disorder affecting arterial blood vessels. It happens when fat, cholesterol, and other substances made in the arterial blood vessels form a hard substances called plaque. Lipoprotein-associated phospholipase $A_2$ ($Lp-PLA_2$), a phospholipase $A_2$ enzyme, is used as a marker for cardiac disease. The detection of $Lp-PLA_2$ was accomplished by using radioactive [$^3H-acetyl$] PAF as a substrate and a feasibility study on RI biochip application to detection of $Lp-PLA_2$, a risk factors of atherosclerosis was performed. Inhibitive activity of a native plant extract was also determined by using the RI biochip. It was found to be applicable to a high-throughput screening of inhibitors for developing atherosclerosis therapeutic agents.

코코넛 기반 활성탄 필터의 라돈 제거 효율 (Radon Removal Efficiency of Activated Carbon Filter from Coconut)

  • The Korea Institute of Radiological and Medical Sciences plans to produce 225Ac, a therapeutic radio-pharmaceutical for precision oncology, such as prostate cancer. Radon, a radioactive gas, is generated by radium, the target material for producing 225Ac. The radon concentration is expected to be about 2000 Bq·m-3. High-concentration radon-generating facilities must meet radioactive isotope emission standards by lowering the radon concentration. However, most existing studies concerning radon removal using activated carbon filters measured radon levels at concentrations lower than 1000 Bq·m-3. This study measured 222Rn removal of coconut-based activated carbon filter under a high radon concentration of about 2000 Bq·m-3. The 222Rn removal efficiency of activated carbon impregnated with triethylenediamine was also measured. As a result, the 222Rn removal amount of the activated carbon filter showed sufficient removal efficiency in a 222Rn concentration environment of about 2000 Bq·m-3. In addition, despite an expectation of low radon reduction efficiency of Triethylenediamine-impregnated activated carbon, it was difficult to confirm a significant difference in the results. Therefore, it is considered that activated carbon can be used as a radioisotope exhaust filter regardless of whether or not Triethylenediamine is impregnated. The results of this study are expected to be used as primary data when building an air purification system for radiation safety management in facilities with radon concentrations of about 2000 Bq·m-3.

악관절 동통 환자에서 Bone SPECT의 유용성 (Utility of Bone SPECT in Temporomandibular Joint Pain)

  • 악관절 동통은 여러 가지 원인에 의해 발생하는 것으로서 동통을 호소하는 환자에서 Bone SPECT의 유용성을 알아보았다. 악관절 동통을 호소하여 Bone SPECT를 시행한 34명을 대상으로 하였으며 모든 환자에서 단순 촬영, planar 골스캔, Bone SPECT를 시행하였다. Bone SPECT에서 동위원소의 섭취 증가 정도에 따라 3가지로 분류하였다. Grade 0은 정상, Grade I은 후두골과 비슷한 섭취 증가, Grade II는 상악동과 비슷한 섭취 증가가 있는 것으로 정하여 각각 섭취 증가 정도에 따라 환자의 임상 증상과 치료 방법을 비교 분석했다. 34예중 27예 (80%)에서 악관절 Bone SPECT 시행시 섭취 증가가 있었고, 이중 21예 (78%)에서 하악과두에 섭취 증가가 있었고 나머지 6예 (22%)에서 하악궁과 상악궁에 섭취가 있었는데 이들은 치주질환으로 확인되었다. 총 34예중 Grade 0인 7예 (20%)에서는 관절 잡음이 3예 있었고 4예에서 약물 치료로 증상이 호전되었고 나머지 3예에서 치료 없이 증상이 호전되었다. Grade I 인 4예(12%)에서는 관절잡음이 3예 있었고 모두 약뭍 치료로 증상이 호전되었다. Grade II 23예(68%)중 관절 잡음은 7예 있었고 14예에서 약물치료 및 관절천자와 같은 감압술을 병행하여 증상이 호전되었다. 6예에서는 치주 치료로 증상이 호전되었다. 총 34예중 평면 골스캔에서는 11예(32%)에서 섭취 증가가 있었다. 단순 촬영에서는 16예(47%)에서 관절내강 협소 혹은 팽대, 골리란, 관절운동 제한 등이 관찰되었다. one SPECT는 악관절 동통 환자에서 유용하게 사용할 수 있는 진단적 방법이며, 치료방법을 결정하는 데 도움이 되었다. 동위원소 섭취 증가가 많을 수록 환자 치료에 보다 적극적인 치료를 필요로 하였으나 임상 증상과 동위원소 섭취증가는 밀접한 관계는 없었다.

마우스 CT26 종양에서 Tc-99m 표지 플루란유도체의 저류 특성 (Retention Characteristics of Tc-99m-Pullulan-Derivatives in CT26 Tumor of Mice)

  • 배경: 플루란유도체(PD)로 만든 나노입자는 이온강도에 따라 뭉치는 수용성겔이다. 본 연구에서는 PD가 방사성핵종을 종양 조직 내에 효과적으로 머무르게 할 수 있는지 알아보고자 하였다. 방법: 네 종류의 PD 즉, pullulan acetate (PA), succinylated PA(SPA), PA-DTPA와 SPA-DTPA 결합체를 합성한 후 테크네슘-99m(Tc-99m)으로 표지하고 표지효율을 측정하였다. Balb/c 생쥐에 CT26 대장암세포를 피하주사하고 2주 후에 Tc-99m 표지 PD(Tc-PD)를 종양 내에 주사하였다. 주사직후와 30분, 1, 2, 4, 12시간 후에 감마카메라로 생쥐를 촬영하여 종양 내의 Tc-PD의 저류율을 측정하였다. 종양크기에 따라 각각의 암의 직경이 5 mm와 10 mm일 때 Tc-99mpertechnetate와 Tc-99m 표지 PA를 종양 내 주사하여, 주사 직후 1시간 동적영상을 얻고, 1시간, 2시간, 3시간 그리고 4시간 후에 감마카메라로 생쥐를 촬영하여 종양 내의 저류율을 측정하였다. 결과 PA, SPA, PA-DTPA 및 SPA-DTPA의 Tc-99m 표지율은 각각 $94.5{\pm}5.9%,\;97.8{\pm}3.5%\;94.2{\pm}3.8%,\;92.5{\pm}6.2%$ 로 서로간에 유의한 차이가 없었다 (p>0.05). Tc-99m-PA와 Tc-99m-PA-DTPA의 %RR은 대조군에 비해 의의있게 높았으나(p<0.05), SPA는 4시간까지 그리고 SP-DTPA는 2시간까지만 대조군보다 %RR이 높았을 뿐 그 이후에는 대조군과 유사한 %RR을 보였다. 종양의 크기에 따라 PA군에서 측정한 저류율은 대조군보다는 의의있게 높았으나, 직경이 5 mm와 10 mm일 때의 저류율 간에는 차이가 없었다. 결론: PD가 종양 조직 내에 저류될 수 있음을 알았으며, 종양 크기에 따른 저류율의 차이는 없었다. 향후 PD와 치료용 방사성핵종을 이용한 종양 치료에 활용할 가능성을 기대할 수 있을 것으로 사료되었다.
