Endoscopic retrograde cholangiopancreatography (ERCP) in patients with Roux-en-Y gastric bypass anatomy is a well-documented challenge. Traditionally, this problem has been overcome with adjunctive techniques, such as device-assisted ERCP, including double-balloon or single-balloon enteroscopy and laparoscopy-assisted transgastric ERCP. Endoscopic ultrasound-directed transgastric ERCP (EDGE) is a novel technique that enables access to the ampulla using a duodenoscope without surgical intervention and has shown high clinical and technical success rates in recent studies. However, this approach is technically demanding, necessitating a thorough understanding of the gastrointestinal anatomy as well as high operator experience. In this review, we provide a technical overview of EDGE in parallel with our personal experience at our center and propose a simple algorithm to select patients for its appropriate application. In conjunction, the outcomes of EDGE compared with those of device-assisted and laparoscopy-assisted transgastric ERCP will be discussed.
Advances in nanobiotechnology have presented numerous possibilities of more effective diagnostic and therapeutic options. In particular, gold nanoparticles have demonstrated the potential for application in molecular imaging and treatment of cancers, including drug delivery system of certain target molecules, enhancement of radiation therapy, and photothermal treatment. This review discusses the properties, mechanism of action, and clinical application of gold nanoparticles. Although the safety of nanoparticles is yet to be ascertained, there is no doubt that in the future, nanotechnology will play an important role in the development and enhancement of a wide range of diagnostic and treatment modalities.
Introduction: Psoriasis is a major health concern around the world. Physicians of the Unani system of medicine have been treating psoriasis for centuries. Aim: The purpose of our study was to assess the effect of Majoon Mundi (a semisolid Unani medication intended for oral intake used as blood purifier) and Qairooti Karnab (a Unani medication in paste form intended for topical application used as emollient) in the treatment $na{\ddot{i}}ve$ psoriasis cases and to collect data to warrant further clinical trials. Material and Methods: Psoriasis cases were diagnosed clinically. Data were collected during treatment of five patients of psoriasis treated with the Majoon Mundi (oral intake of 5 gm twice daily with 200 ml of water for 12 weeks) and Qairooti Karnab (topical application on affected sites twice a day for 12 weeks). Patients were treated for 12 weeks. Treatment response was seen with clinical improvement in skin lesions and measurement of Psoriasis Area and Severity Index (PASI Scoring) before and after treatment. Results: Reduced PASI Score was observed in all five patients after 12 weeks of treatment [PASI before and after treatment was (mean${\pm}$SD) $20.7{\pm}4.6$ vs. $3.2{\pm}1.8$; p-value <0.05.]. Clinical improvement was noticed within an average of 4 weeks of treatment. Conclusion: Preliminary findings indicate the potential therapeutic role of Majoon Mundi and Qairooti Karnab in the treatment of psoriasis. Clinical trials based on this Unani pharmacopeial formulation should be conducted to explore the therapeutic potential of this formulation in psoriasis
Extracellular vesicles (EVs) released from different types of kidney cells under physiologic conditions contribute to homeostasis maintenance, immune-modulation, and cell-to-cell communications. EVs can also negatively affect the progression of renal diseases through their pro-inflammatory, pro-fibrotic, and tumorigenic potential. Inhibiting EVs by blocking their production, release, and uptake has been suggested as a potential therapeutic mechanism based on the significant implication of exosomes in various renal diseases. On the other hand, stem cell-derived EVs can ameliorate tissue injury and mediate tissue repair by ameliorating apoptosis, inflammation, and fibrosis while promoting angiogenesis and tubular cell proliferation. Recent advancement in biomedical engineering technique has made it feasible to modulate the composition of exosomes with diverse biologic functions, making EV one of the most popular drug delivery tools. The objective of this review was to provide updates of recent clinical and experimental findings on the therapeutic potential of EVs in renal diseases and discuss the clinical applicability of EVs in various renal diseases.
Objectives : This study examined the possibility of therapeutic application of natural pigments in the Joseon Period other than aesthetic purposes. Methods : The medicinal properties of natural pigments as written in the texts Sangayorok, Gyuhapchongseo, Imwongyeongjeji were searched and analyzed in the texts Bencaogangmu, Encyclopedia of Eastern Medicine, and the Encyclopedia of Chinese Medicinals focusing on their Qi, flavor, effects and application. Of the conditions they treated, we examined whether they treated skin conditions using external application. The Korean Medical effects of these pigments were examined based on the content of the skin chapter of the Donguibogam. Results : We were able to extract 47 kinds of natural pigments, most of which had cold nature and bitter tasting properties. In addition, most of the pigments had the effect of treating skin conditions, illnesses due to micro-organisms, and of detoxifying. Conclusions : After consideration of medicinal properties and effects of the herbs that were used as pigments in the Joseon period, it could be concluded that as these herbs not only function as pigments but also hold properties that prevent skin conditions, their therapeutic functions could have been considered when using them as pigments.
Sphingosine-1-phosphate (S1P) and its receptors have been implicated in atopic dermatitis. S1P2 was found to function as a proallergic receptor, while its antagonist JTE-013 was found to suppress allergic asthma in mice. Topical application of JTE-013 has not been investigated in an in vivo model of atopic dermatitis. Therefore, the therapeutic potential of JTE-013 topical application was evaluated by the use of a 2,4-dinitrochlorobenzene (DNCB)-induced atopic dermatitis mouse model. DNCB-induced inflammation and mast cell accumulation in skin tissues were significantly suppressed by topical JTE-013 treatment in BALB/c mice. DNCB-induced increase of lymph nodes sizes and elevated inflammatory cytokines (IL-4, IL-13, IL-17, and IFN-γ) in lymph nodes were also significantly reduced by the JTE-013 treatment. Elevated serum levels of IgE were significantly suppressed by the topical treatment of JTE-013. In summary, the topical treatment of JTE-013 S1P2 antagonist suppressed DNCB-induced atopic dermatitis symptoms and immune responses. These results suggested JTE-013 as a potential therapeutic agent for atopic dermatitis.
This research was carried out to determine the effect of various therapies on the hyperthermic symptom of the Holstein cattle, The 50 cases were calassified as 3 groups according to the therapies aplied, needle Acupuncture therapy, antipyretic administration therapy, combination of prior 2 therapies. The results are as follows. 1. On the 23 cases applied needle acupuncture therapy only, the body temperature was decreased from 39.9$\pm$0.7$^{\circ}C$ to 38.6$\pm$0.3$^{\circ}C$ after 4 hours from application. It was in the arrange of the normal body temperature. 2. On the 10 cases administered antipyretic drug, the body temperature was decreased from. 39.8$\pm$0.3$^{\circ}C$ to 38.7$\pm$0.1$^{\circ}C$ after 4 hours from application. the therapeutic efficacy of these cases found somewhat inferiopr to the cases acupunctured. 3. On the 9 cases comined both acupuncture and antipyretic administration therapy, the body temperature was decreased from 40.1$\pm$0.4$^{\circ}C$ to 38.5$\pm$0.1$^{\circ}C$ after 4 hours from application. The therapeutic efficacy found the best among the 3 groups.
Embryonic stem cells (ESCs) and induced pluripotent stem cells (iPSCs), which are collectively called pluripotent stem cells (PSCs), have emerged as a promising source for regenerative medicine. Particularly, human pluripotent stem cell-derived cardiomyocytes (hPSC-CMs) have shown robust potential for regenerating injured heart. Over the past two decades, protocols to differentiate hPSCs into CMs at high efficiency have been developed, opening the door for clinical application. Studies further demonstrated therapeutic effects of hPSC-CMs in small and large animal models and the underlying mechanisms of cardiac repair. However, gaps remain in explanations of the therapeutic effects of engrafted hPSC-CMs. In addition, bioengineering technologies improved survival and therapeutic effects of hPSC-CMs in vivo. While most of the original concerns associated with the use of hPSCs have been addressed, several issues remain to be resolved such as immaturity of transplanted cells, lack of electrical integration leading to arrhythmogenic risk, and tumorigenicity. Cell therapy with hPSC-CMs has shown great potential for biological therapy of injured heart; however, more studies are needed to ensure the therapeutic effects, underlying mechanisms, and safety, before this technology can be applied clinically.
Theranostics, a composite word of therapy and diagnosis, is known as personalized medicine and the concept of diagnosis and treatment at the same time. In nuclear medicine, it means performing both therapeutic and diagnostic radioisotope therapy using the same target molecule. The increased production and utilization of 64Cu opens a new era of theranostics. The studies introduced here have shown that 64CuCl2 and various compounds or biomolecules labeled with 64Cu are unique radiopharmaceuticals with physiological properties suitable for use as diagnostic and therapeutic agents. So far, these two abilities have been described only for radioactive iodine. Although 64Cu has complex chemical properties compared to other PET radioisotopes such as 68Ga, it has an appropriate half-life and enables high-quality PET images similar to 18F, which is an advantage in terms of diagnosis. In addition, since it also has therapeutic properties through the release of β- particles and Auger electrons by electron capture, radiopharmaceuticals using 64Cu stand for innovative radiopharmaceuticals for theranostic purposes. Therefore, based on the initial results obtained using 64Cu as a therapeutic agent, it is expected that additional research on the application of 64Cu will lead to a new era in the theranostics field.
