  • This study is designed to reduce worker fatigue, improve efficiency and provide a functional working environment based on previous studies that pain occurs in the shoulder area, especially the upper trapezius muscle, when the keyboard height is not appropriate. In this study, the height of the keyboard is four, the height of the elbow and desk is the same height, the height of the desk is 3cm lower than the elbow, the height of the desk is 6cm high, and the height is 9cm high. When working on the keyboard, the wrist and forerunner were organized into four groups of 10 people so that the height was different for each group. When the height of the keyboard is given in various ways compared to the height of the elbow of the subject, it is verified whether there is a difference in the RMS (Root Mean Square) of the upper trapezius muscle. The results of this study showed that the muscle activity of the upper trapezius muscle cap was significant only in the left and right keyboard height -4cm, 0cm, +4cm, +8cm group, but the difference in muscle activity was not significant in the rest group. The first study will require a study of the control of the factors affecting the tension of the subjects, the measurement of muscle activity against various muscles, and whether the length of the shoulder and fingertips of the subject affect muscle activity according to the keyboard type.

  • The objective of this study is to examine the prevalence of musculoskeletal disorders(MSDs) among nursing personnels. A self-reporting survey adapted from Nordic questionnaires was used as diagnostic tool, and conducted for 162 nurses working for a general hospital. The survey was performed five times every three months during a year. The MSDs were defined by using three criteria, depending upon the frequency, duration and pain intensity of the symptoms. The results showed that the 12-month prevalences of MSDs at any body site by criterion 1, 2, 3 were 56.8%, 53.7%, 45.7%, respectively, and that the shoulder was the most susceptible to MSDs, followed by knee/lower leg, lower back, hand/wrist, neck, ankle/feet, finger, etc. Only one statistically significant risk factor of departments or units was identified through the Chi-square test. The prevalence of MSDs was the highest in intensive care unit, second in surgical ward, while the prevalence was the lowest in emergency room. The MSDs prevalence of Korea at any body part or by body part was much lower than that of foreign countries such as Japan, USA, Sweden, etc. This implies that when considering poor working conditions and load in Korea, the incidence rate of MSDs may rapidly increase in Korea in the future.

  • The purpose of this study is to research the situation and related knowledge and behavior of Korean beauty artists on their musculoskeletal disorders and the factors on them. It was researched by questionnaire from March 1 to May 31, 2003, and the subjects were 984 of beauty artists who were working as hair designers, skin care specialists, and make-up artists over 1 year in big cities including Seoul, Daegu, Busan and Gwangju. The analyzation was done by frequency, percentage, $x^2-test$, t-test. The results can be summarized as follows; 1. As for their degree of pain according to part of their body, the highest was shoulder as 88.3%, and next were neck(cervical region) as 83.2%, waist as 77.6%, and wrist(hand) as 71.8%, in order. 2. The level of knowledge on the muscloskeletal disorder was 65.6 points on the basis 100points. As for their level of protective behavior and knowledge on the disorders of musculoskeletal system according to their educational experience, the higher educational level they had the higher knowledge level they showed, as 42.6% of 'high', 30.4% of 'medium', and 27.0% of low; and as for degree of performing the protective behavior, they showed the higher protective behavior level when they had the higher educational level. 3. As for their level of knowledge on disorders of musculoskeletal system according to their general uniqueness as beauty artists, they showed statistical significance on the field of their marriage status, educational degree, specialized field, and numbers of customers they manage; and as for the degree of their protective behavior, they showed statistical significance in the field of age, marriage status, specialized field, and rank in their jobs. 4. The level of protecting behavior on the muscloskeletal disorder was 57.4 points on the basis 100points. As for their degree of pain on each part of their body according to their protective behavior and knowledge on musculoskeletal system, it had statistical significance on their elbows and hips, and the less pain they showed when the higher knowledge they had; it had statistical significance on their neck(cervical region), shoulders, backs, waists, knees and ankles(feet) according their protective behavior, and the less pain they showed when the higher level of protective behavior they did.

  • Purpose: To compare clinical characteristics of ganglions in hand & Foot. Materials and Methods: Seventeen cases of ganglions located in foot and fifty-five cases in hand. Excised from Mar.1988 to Apr.2003, were included in the study. The clinical characteristics and recurrence ratio were evaluated Results: The mean size of 2.2 cm in hand and 2.5 cm in foot. The most common area of ganglions are dorsum of foot and wrist. The cosmetic problem of palpable mass is the primary chief complaint of ganglions on hand and the pain is that of foot. The recurrence was found in 5 cases in hand and 4 cases in foot. The recurrence was related to incomplete excision of ganglion in foot and the large size of ganglion and incomplete excision of ganglion in hand. Conclusion: recurrence ratio in the cases of foot is higher than that of hand. The ganglions in foot and hand need to treated by meticulous surgical excision to prevent the recurrence.

  • Mucolipidosis (ML) is a kind of skeletal dysplasia. Characteristic X-ray findings of the bone may contribute to the early diagnosis and treatment of ML II/III. Skeletal radiographs show distinctive patterns at different ages: neonatal hyperparathyroidism, osteodystrophy (similar to chronic osteitis fibrosa cystica), and dysostosis multiplex. Patients with ML II/III show a mixture of osteodystrophic bone changes and atypical changes of dysostosis multiplex: proximal pointing of the metacarpals in the wrist, dysplastic changes in the lower third of the ilia, marked broadening of the ribs becoming oar-shaped, and beaking of the lower thoracic and lumbar vertebrae. In ML II, the osteodystrophy has clinical and radiographic features of neonatal hyperparathyroidism. In some neonatal subjects, chemical hyperparathyroidism is also demonstrated. After transient hyperparathyroidism in newborns, the progressive osteitis fibrosa cystica develops from 3-6 months of age. Patients with ML III show prominent skeletal involvement, particularly the destruction of vertebral bodies and the femoral heads. Intravenous pamidronate treatment is well tolerated, and it can produce clinical effects, with a reduction in bone pain and improvements in mobility in patients with ML III. In this review, the skeletal manifestations of ML II and III are investigated.

  • The authors present an extremely rare case of a pseudoaneurysm of the ulnar artery as a complication of a two-portal endoscopic carpal tunnel release (ECTR). A 70-year-old man with chronic renal failure and on maintenance hemodialysis with a left arteriovenous fistula presented with paresthesia of his right hand. A clinical diagnosis of right carpal tunnel syndrome was confirmed by ultrasonography and an electro physiologic study. He underwent two-portal ECTR, and the paresthesia was much improved. However, he presented to us one month after operation with severe pain, a tender mass distal to the right wrist crease and more aggravation of the paresthesia in the ulnar nerve distribution. Doppler ultrasound was performed and revealed a hypo echoic lesion 20 mm in diameter in the right palm, with arterial Doppler flow inside connected to the palmar segment of the ulnar artery. An ulnar artery pseudoaneurysm was diagnosed and treated by ultrasound-guided percutaneous thrombin injection. Transverse color Doppler ultrasound image showed complete thrombosis of the pseudoaneurysm and flow cessation after a total injection of 500 units of thrombin. The symptoms were also improved.

  • Epicondylitis, as a tendinopathy characterized by fibroblast and microvascular hyperplasia, is a common musculoskeletal problem especially related with repetitive hand and wrist motion. It has a prevalence of between 0.2% and 5% in general population depending on the amount of exposure to manual labor jobs. Although it is known that the pathological lesions lie in the flexor or extensor common tendons, there could be collateral ligament lesions and/or reactive synovitis accompanied, which may make a case unresponsive to the treatment aimed only at the tendinopathy. Epicondylitis is easy to diagnose with typical pain, tenderness, and positive provocation tests. However, many conditions can mimic epicondylitis that further imaging or electrodiagnostic studies should be undertaken to exclude other possible problems. Ultrasonography provides information about the existence and extent of tendinopathy with relatively high specificity. Magnetic resonance imaging is often required to rule out other problems and confirm the diagnosis of the cases intractable to long term treatment. Many options of treatment are available for epicondylitis while numerous conflicting evidences have been noted, debating one treatment method is better than the others. Since it was reported that over 80% of epicondylitis improved within a year no matter what was done as treatment, it is a challenge to make accurate diagnosis and combine effective therapeutic regimens for the 20% of intractable cases.

  • We report a rare case of late-onset brachial artery occlusion caused by subclavian artery stenosis with excessive scar tissue after open reduction and plate fixation for clavicular fracture. When he referred to us, the right hand were pale and the radial and ulnar pulses at the wrist were absent. CT-angiogram showed compression of subclavian artery by excessive scar tissue beneath the fracture site and angiography revealed stenosis of subclavian artery with thrombus and complete obstruction of blood flow in the brachial artery with emboli. Therefore, we performed embolectomy. 2 years after operation, patient was essentially asymptomatic except mild pain after long standing elevation of arm. We recommend that minimal soft tissue dissection should be needed in the operative treatment of clavicular fracture, especially soft tissue beneath the clavicle should be protected maximally.
