Objectives : Acne, one of the most common disorders in dermatology clinic, is a chronic inflammatory disease which has the symptoms of comedones, papules, pustules, cysts, nodules and scars mainly on the face. Although some of pathologic findings are suggested, but the exact causes and mechanisms are not yet known in Western Medicine. Hwangryeonhaedok-Tang(HRHDT) is an antiinflammatory, antipyretic and detoxifying herb decoction. In this report, we would like to share our experience of acne treatment with HRHDT. Methods : We had treated several patients with acne, facial flushing and uprising febrile sensation on face using oral administration and external application of HRHDT. HRHDT is basically made up with Scutellariae Radix, Coptidis Rhizoma, Phellodendri Cortex and Gardeniae Fructus. In this trial, several other herbs were added according to the individual patient's accompanying symptoms. After the decoction of herbs in amount for 10 days, $Yogourmet^{(R)}$ Kefir Starter 10 g was added to the herbal solution that was then fermented in an incubator at $25^{\circ}C$ for 72 hours, and divided into 30 doses. Results : Photographies were taken before and after the treatment. The severities of acne were evaluated with these photos according to the Korean Acne Grading System(KAGS). As results, we observed dramatic clinical improvements and the decreases of KAGS grades after the average treatment period of 8 weeks. The medication was orally administered 3 times a day and the external treatment was applied average twice a week during the whole treatment period. Conclusions : From these results, HRHDT may be considered as a good prescription for the febrile and toxic type of acne patients.
Pediatric home-based physical therapy (PHBPT) provides professional rehabilitation programs at the patient's home, where the activities of daily life are actually performed. PHBPT also allows to avoid the difficulties of transporting children with disabilities to the clinic. Despite these advantages, PHBPT is not yet widely practiced in Korea. There is little objective information regarding the opinions of the main stakeholders on PHBPT. To investigate the awareness and demand of PHBPT among the main stakeholders, 41 pediatric physical therapists (PT) (of 60 contacted) were recruited from different regions of Korea on the basis of the regional population distribution. The recruited PTs completed their questionnaires and also participated in collecting questionnaires from 35 medical doctors (MD) with whom they worked and from randomly selected 201 parents of children with disabilities recruited. The overall response rate was 85.5%. The awareness of PHBPT differed between PTs (95.1%) and parents (67.2%) (p<.001). The survey showed that 82.9% of MDs had at least heard about PHBPT. Significantly more parents (83.5%) than MDs (57.1%), and 70.0% of PTs, wanted to start PHBPT service immediately (p<.001). Significantly more parents (90.0%) than PTs (73.2%) were willing to participate in PHBPT (p<.001). Opinions on the details of policies and procedures (i.e., necessity for prescription, treatment cost, and treatment frequency) differed among the respondent groups, but all favored a minimal qualification of 6~10 years of pediatric experience and a treatment session duration of 1 hour. These findings provide objective information to support health service administrators to understand the current demand and develop feasible policies and procedures of PHBPT in Korea.
The purpose of this study is to make clear the management of Environmental Education in Elementary school in Korea. To accomplish this purpose, The research for the actual condition of management of Environmental Education according to school, grade, and class was progressed. The researcher was analyzed Hindrance Factors of Sound Environmental Education in School and presented some suggestion on the basis of above-mentioned findings. On one side, the method of research is a case study on qualitative based way. A case study was conducted with it's focus on public elementary school in Seoul. As for major research methods, observation by participating in school field and in-depth interview were respectively employed; subsidiary methods included literature study. The findings of this study are as belows: First, Environmental Education was not managed practically with specific way in school. Second, Environmental Education by direct and actual outdoor experience was unusual. Third, most of class school work about Environmental Education was focused on not only the type of prescription after the environmental problem but also, the treatment of articles in newspaper and TV. Forth, educational compulsion such as “you have to do that.” was mainly used as Environmental guidance for sound attitude in school without clear reasons. In the last analysis, hindrance factors of sound management for environmental education in elementary schools were ‘passive adjustment’, ‘discriminative reception’, ‘absence of information resource’. these factors came from atmosphere of negligence environmental education in schools. Therefore, all of teachers have to affirmative belief and attitude as personal dimension and all of elementary schools improve environmental education system.

Antimicrobial (AM) resistance is a growing threat in human and veterinary medicine, spreading across species and perceived as One-Health issue. Prudent use of AM products is essential in mitigating this risk in both human and veterinary medicine. Farm veterinarians, responsible for prescribing appropriate AM and offering advice to farmers on their proper usage, are regarded as key players in the livestock industry. An online survey of farm veterinarians (n = 1,531) was conducted to assess their educational experience, beliefs, current status of prescription, practical behavior, and self-efficacy regarding the prudent use of antimicrobials (PUA). The data from 170 respondents were analyzed using descriptive statistics, regression and mediation analysis. Participants chose AM based on their experiences, academic resources, and peer veterinarians. Approximately 77% did not routinely conduct antibiotic-susceptibility-test. Most participants believed in the importance of PUA and the role of veterinarians to reduce AM resistance, but they reported a lack of awareness among farmers and insufficient national support interfere with their practical behavior with regard to PUA. Half of the participants had PUA-education, and 78.6% reported that education had a positive impact on their behavior with PUA. Self-efficacy partly mediates the relationship between belief and behavior for PUA, which accounted for 23.77% of the total effect that beliefs have on behavior for PUA. The findings of the study show that enhancing veterinarians' abilities to practice and their self-efficacy through education tailored to the current status of farms can contribute to the reduction of AMs in the livestock sector.
Objectives: Evidence-based medicine(EBM) advocates the use of up-to-date "best" scientific evidence from health care research as the basis for making medical decisions. EBM also has been applied to traditional Korean medicine(TKM), especially in the field of safety. Recently, the standard prescription for TKM by Korea Institute of Oriental Medicine was published based on toxic index from various toxicity tests. However, there are some limitations when the results from the study based on EBM are applied in clinics. To overcome these imitations, the term "evidence-based practical medicine" was developed and defined as clinically applicable results from the study based on EBM. And safety classification for TKM was suggested as an example of evidence-based practical medicine. Methods: For safety classification for TKM, the data for $LD_{50}$(50% lethal dose), which was transformed to theoretical $LD_1$(1% lethal dose), was analyzed as one of tools for EMB study and divided by maximum dose used in clinics. Results and Conclusions: As a result, human equivalent dose(HED)-based MOS(margin of safety) for korean traditional medicine was calculated and used for safety classification with 5 categories. These categories would be helpful for oriental medicine clinicians to decide the increase and decrease of dosage according to various factors such as patient's sensitivity, potential toxicity of herbal medicines, clinician's experience for better cure. Thus, this safety classification provides some evidences enough that evidence-based practical medicine should be not the same with EBM and defined differently from EBM.

Objectives : This study was conducted to identify awareness and satisfaction of herbal medicine preparations in Korean medicine doctor(KMD). Methods : The questionnaire consisted of three professors of Korean medicine based on previous researches and reports. The pilot survey was conducted to five KMDs who work in local clinic to get face validity. Based on the results of the pilot survey, three professors of Korean medicine completed the final version. Online surveys was conducted to member of the association of Korean medicine from march 25 to april 6 in 2016. Results : The proportion of patients who prescribe herbal medicine was not significantly correlated to gender (p=0.346), but significantly correlated to age(p<0.01), specialty(p=0.017), monthly income(0.022), and clinical experience(p<0.001). The most common reason for using the herbal preparations which is covered by NHI (National Health Insurance) was due to patient's payment(39.3%). Likewise, the most common reason for using the herbal preparations which is not covered by NHI was various kind of prescription(34.1%). Conclusions : The utilization of herbal preparations could be enhanced by expansion of NHI cover range of herbal preparations.
Critically ill trauma patients generally show good nutritional status upon initial hospitalization. However, they have a high risk of malnutrition due to hyper-metabolism during the acute phase. Hence, suitable nutritional support is essential for the optimal recovery of these patients; therefore, outcomes such as preservation of fat-free mass, maintenance of immune functions, reduction in infectious complications, and prevention of malnutrition can be expected. In this report, we present the experience of a patient subjected to 40 days of nutritional interventions during postoperative intensive care unit (ICU) care. Although the patient was no malnutrition at ICU admission, enteral nutrition (EN) was delayed for > 2 weeks because of several postoperative complications. Subsequently, while receiving parenteral nutrition (PN), the patient displayed persistent hypertriglyceridemia. As a result, his prescription of PN were converted to lipid-free PN. On postoperative day (POD) #19, the patient underwent jejunostomy and started standard EN. A week later, the patient was switched to a high-protein, immune-modulating formula for postoperative wound recovery. Thereafter, PN was stopped, while EN was increased. In addition, because of defecation issues, a fiber-containing formula was administered with previous formula alternately. Despite continuous nutritional intervention, the patient experienced a significant weight loss and muscle mass depletion and was diagnosed with severe malnutrition upon discharge from the ICU. To conclude, this case report highlights the importance of nutrition interventions in critically ill trauma patients with an increased risk of malnutrition, indicating the need to promptly secure an appropriate route of feeding access for active nutritional support of patients in the ICU.

The purpose of this study was to determine the relationship with the Y-Balance Test, which evaluates dynamic balance ability according to the ankle joint dorsiflexion range of motion in a non-weight bearing posture. This study involved 27 male soccer players who regularly participate in the K3 League with more than 10 years of soccer experience. The Pearson Rank Correlation Coefficient was used to verify the relationship between ankle joint dorsiflexion and dynamic balance ability. The results of this study showed significant differences in PLRD (P<.05) and CS (P<.01) in the HADR group. A significant correlation between ankle joint dorsi-flexion range of motion and severity was found only in PMRD, PLRD, and CS. Therefore, a decrease in ankle joint dorsi-flexion is associated with a decrease in balance ability. In order to prevent injuries in soccer players, it is believed that regular evaluation of ankle joint range of motion as well as training to improve knee and hip joint strength and proprioception are necessary.
This study was carried out to investigate the micro-agribusiness realities of dairy farms by surveying both farmers' opinions and consumers' recognition of dairy farm products for in a value-chain analysis. In most cases, dairy farms produced fermented milk for sales and to prepare cheese both for sales and for providing the necessary material for farm visitors. Value-chain analysis did not seem to be a viable strategy for dairy farmers, except when assessing the quality of farmstead milk products in comparisons of dairy farm milks and commercial milk products. Consumers had a wide range of experience with milk products and had no willingness to buy products from dairy farms because of the high cost and the lack of product diversity. Our findings established three strategies for improving the economic feasibility of dairy farms. First, the price should be reduced by adopting a production balance to establish a farm-gate price, i.e., not as quota milk but as surplus milk. Second, consumers should be educated on the prescription of cheese at home. Third, a value-chain based on cooperative partnerships should be established among dairy farms. In addition, to improve the consumers' satisfaction on the diversity and quality of farm dairy products, developments of new value-added milk products made from the milk of cows allowed to graze on mountain pastures are needed.

Objectives: Case report is a description of medical experience that contains a patient's information and treatments. It has been used to determine the effectiveness and side effect of the treatments. Case report can be used for medical, scientific, or educational purposes. The objective of this study was to evaluate the quality of case reports published in Journal of Oriental Neuropsychiatry from 2013 to 2017 using CARE and CARES guideline. Methods: Case reports were selected from Journal of Oriental Neuropsychiatry and their quality assessed using CARE guideline and CARES guideline. The items of CARE and CARES guideline were assessed as 'Sufficient', 'Not sufficient', 'Not reported', 'Not applicable'. The outcome was analyzed using case reports and items. Results: 26 case reports were selected. For CARE guideline, 'Sufficient' percentage was min at 22.2%, max at 46.4% with a median of 39.3%. Sum of 'Sufficient', 'Not sufficient' percentage was min at 59.3%, max at 85.7% with a median of 75.0%. Items 'Timeline', 'Diagnostic assessment', 'Follow up and outcomes', 'Informed consent' were not reported in more than 50% of all case reports. Items 'Keywords', 'Abstract', 'Introduction', 'patient information', 'Diagnostic assessment', 'Therapeutic interventions', 'Follow up and outcomes' were not sufficiently reported in more than 50% of all case reports. For CARES guideline, 'Sufficient' percentage was min at 0%, max at 50% with a median of 0%. Sum of 'Sufficient', 'Not sufficient' percentage was min at 33.3%, max at 100% with a median of 92.9%. Items of herbal prescription 'herb', 'decoction', 'use', items of acupuncture treatment 'materials', 'techniques', items of moxibustion treatment 'materials', 'techniques' were not sufficiently reported in more than 50% of all case reports. Conclusions: The reporting rate of items was good. But the quality of reporting was low. Also, particular items were not reported frequently. Therefore, there is a need to improve the quality of case reports in Journal of Oriental Neuropsychiatry.
