Status and related factors of private health insurance for severely ill patients (중증질환자의 민간의료보험 가입 현황 및 관련요인)

  • This study was conducted to investigate the characteristics of private health insurance subscribers and non-subscribers as they relate to severely ill patients, and to identify the factors of participation. The study was conducted using the National Health and Nutrition Examination Survey for 2015, and data were analyzed using SPSS ver. 23.0. The subjects were 417 patients with severe disease (cancer, heart disease, cerebrovascular disease) over 19 years of age. Crossover analysis was employed to identify differences between the state of private health insurance participation, while binary logistic regression analysis was used to confirm the factors affecting private health insurance subscription. Analysis of the effects of the subjects on the private health insurance participation rate revealed that the social and demographic characteristics were higher in younger individuals regardless of sex, residence, or marital status. Moreover, higher household income, regardless of the education level, was associated with a higher participation rate of health insurance target individuals compared to medical benefit target individuals. The private health insurance participation rate was low and the explaining power was 51.7%, regardless of subjective health awareness and walking practice. Therefore, efforts should be made to improve the living environment and support local governmental programs for the elderly, low income households, socially vulnerable groups with limited activities and groups with limited health behavior. It is also necessary to consider various health policies, such as providing government health education or programs to prevent severe illness.

The Effects of Depression and Oral Symptoms on Oral Health Quality of Life in Self-sufficiency program participating workers (자활프로그램 참가 근로자들의 우울 수준과 구강 증상이 구강건강 삶의 질에 미치는 영향)

  • This study examined the effects of the levels of depression and oral health on the quality of life in workers participating in a self-sufficiency program in the Seoul metropolitan area. As for the study method, a questionnaire survey was conducted from July 26 to September 5, 2020, targeting 569 workers participating in a self-sufficiency program. Depression, Perio-Quotient Index, self-diagnosed oral symptoms, and oral health quality of life were the survey items investigated. The results were as follows. In the workers with systemic diseases, depression, Perio-Quotient Index, oral symptoms, and oral health quality of life were all negative. The level of depression was highly negative in both Perio-Quotient Index, oral symptoms, and oral health quality of life, showing statistically significant differences. In addition, depression showed a strong negative correlation with the Perio-Quotient Index, oral symptoms, and oral health quality of life. Depression, Perio-Quotient Index, and particularly oral symptoms negatively affected oral health and the quality of life. As workers participating in a self-sufficiency program are vulnerable to oral health problems, oral health education is required for the correct awareness and management of oral health, and oral health policy improvement for socially vulnerable groups is essential to ensure continued oral health management.

Gender and healthcare issues related to the Protected Birth Act in Korea (보호출산제 시행과 젠더 및 보건의료 이슈)

  • This paper discusses the implications of the birth notification system and the Protected Birth Act in Korea. Aiming to prevent infanticide and abandonment of infants, the law will enter into force on July 19, 2024 in South Korea. The birth notification system mandates that both parents and the head of the medical institution where the birth occurred must report the event. In parallel, the Protected Birth Act will be implemented, allowing pregnant women in crisis who wish to remain anonymous, the option to give birth outside of a hospital setting in a way that safeguards the life and health of the child. However, many issues are being raised in Korean society in advance of the implementation of the Protected Birth Act. There is widespread concern that the Protected Birth Act fails to protect either women or children, especially as it raises issues regarding the need for legislation to protect children with disabilities and to address gaps for migrant women and children. This paper examines the gender and healthcare issues relating to the Protected Birth Act, focusing on women's health and human rights. The Act continues to perpetuate discrimination against out-of-wedlock pregnancies and upholds the ideology of the traditional family model. Furthermore, the legislative process did not address protective measures for the various reasons behind child abandonment. Critical issues such as women's autonomy, safe pregnancy termination, and paternal responsibility in childbirth are also notably absent. However, with the Act set to take effect soon, it is crucial for healthcare providers to comprehend the rationale and procedures associated with birth notification and the Protected Birth Act, and to prepare for its nationwide implementation. The law defines the socially vulnerable as its main beneficiaries, and it is necessary to strengthen social safety nets to improve their access to healthcare, eliminate prejudice and discrimination against out-of-wedlock pregnancies, and embrace the diversity of our society. We eagerly anticipate future discussions on gender and healthcare issues, as well as amendments to the law that reflect real-world circumstances to provide genuine protection for pregnant women in crisis and their infants.

Activities to Improve the Accessibility to clinical social work for Patients from Vulnerable/Disadvantaged/Marginalized Social Groups (취약계층 환자의 의료사회 복지서비스 접근성 증진 활동)

  • Objectives: With the increase in the number of people who are marginalized in receiving medical services, the role of Seoul National University Hospital as a public hospital is being emphasized. However, many patients are either experiencing delays in receiving medical services or simply being left out as a result of the inaccessibility to the department of social work on part of both the patients and the medical staff. Methods: In order to increase consultation from other departments and the accessibility to the department of social work for the socially marginalized group through early consultation from other departments, the following steps were taken. First, an orientation program for novice medical residents led by clinical social worker was introduced/implemented. Second, posters and brochures on various financial aids programs were produced and distributed. Third, a system of early screening was built/constructed, and once a week rounds and early screening meeting were executed/carried out. Result: The department of social work's rate of consultation from other departments increased by 4.4% compared to last year, while it showed 61% increase for those wards that had an early screening meeting. In addition, the average time of consultation from other departments was reduced by 3.1 days, securing sufficient amount of time for clinical social work services, both in terms of quality and quantity. Conclusion: It is believed that the various promotional activities, along with the strengthening of accessibility to clinical social work services and early consultation on part of social disadvantaged/marginalized group, would undoubtedly help provide quality services to patients and increase their level of satisfaction. In this way, the wards can effectively reduce the number of unnecessary hospital stay days while the hospital can prevent the accumulation of outstanding bills/fees as well as contribute to the publicness of hospitals. The promotion of clinical social work programs in various ways are crucial to ensuring the satisfaction of patients and hospital staff.

A Study on the Residential using of Socially Vulnerable in Rural Areas (농촌지역 취약계층의 주거이용에 관한 연구)

  • Due to difficulty of marriage with women in Korea, unmarried men in countryside tend to have an international marriage. A ratio of international marriage was 3.7% in 2004, but increased by over 10% in 2004 due to the change of population composition in countryside. Without social or national preparation on a multicultural family and an increase of elder who lives alone made problems such as changes of population composition in countryside, a high rate of divorce due to differences on culture and society. These problems are becoming critical problems in countryside, not only for a family. This Study has a purpose as below, Firstly, this study will examine living place of elder who live alone and multicultural families. Secondly, characteristics of using each space will be analyzed and then surveyed efficiency and satisfaction. The result of survey will be provided to space planning for improvement of living standard. The results of this study can be summarized as follows: The characteristics of the surveyed households, the elderly ratio of 70 to 80 units high, and when the elderly become less than 10 years, 51.6%, 77.4% of women were overwhelmingly high. The characteristics of multicultural families in the in the middle or early 2000s when immigration is concentrated and national origin in Vietnam was 66.7 %. Housing characteristics of households with elderly housing type, the ratio improved to 61.3% is Majority. Type strain and 29% in traditional houses, traditional housing type is 9.7%. And house of multicultural families improve housing type is 87.5% Majority. The main space of the housing for the elderly, most are usually satisfied. But generally low level of self-determined housing survey confers real. These results have lived a long time because of space adaptation and can be viewed as attachment. For the elderly passive use of space and simplify the Act and heating cooling at the expense of the use of housing space due to the reduction, but multicultural families is the space required for the growth of their children because there are more modifications required.

Factors Affecting the Purchase of Indemnity Private Health Insurance and Related Factors of Outpatient Healthcare Utilization among Chronic Disease Patients (만성질환자에서 실손형 민간의료보험의 가입 관련 요인과 외래 의료이용의 관련 요인)

  • Purpose: The purpose of this study was to investigate determinants of purchasing indemnity private health insurance and its impact on the healthcare utilization among outpatients with chronic disease. Methods: The study analyzed 4,997 chronic ill patients using 2015 Korean Health Panel data. Logistic regression analysis was conducted to analyze the factors affecting the purchase of indemnity private health insurance and multiple regression analysis was conducted to analyze the effect of private health insurance on the number of outpatient visits and outpatient expenditures. Findings: The age, education level, and number of chronic diseases were significant factors affecting the purchasing of indemnity private health insurance among chronic patients. As a result of analyzing the impact of indemnity private health insurance on healthcare utilization, the number of outpatient visits for those who enrolled in the indemnity private health insurance was higher than the number of outpatient visits for those who did not. But there was no statistically significant difference in outpatient medical expenses. Practical Implications: The results of this study shows that indemnity private health insurance may cause inequality in the healthcare utilization of the socially vulnerable. It is necessary to seek ways to strengthen the health security of chronic disease patients and high-risk elderly people who need more healthcare.

A Study on the Right to Housing in International Human Rights Laws and Instruments (국제인권법 및 인권규범의 주거권 규정에 대한 연구)

  • Today human rights are the most complex and prominent issue in the system of international law, and the right to housing(housing right) is also recognized as a basic human right in the international human right instruments including the Universal Declaration of Human Rights. This study targets to comprehensive review of the housing rights provisions with 85 international human rights laws and instruments. And the contents and characteristics of housing rights are analyzed with categorization based on housing rights in general, housing rights of workers, socially vulnerable groups, international regional organizations. Housing right takes also the features of universality, indivisibility, interdependence, and right to adequate housing should be interpreted with holistic view including legal security of tenure, accessibility, affordability, location beyond ensuring just a physical housing space. Approaches to the housing right comprehensively reflect the view of the right to development, the perspective of gender equality, the principle of non-discrimination, the participation rights, and orient the housing right should be seen as the right to live somewhere in security(safety), peace and dignity.

the Association between the Single-Person Household & Beneficiary of National Basic Livelihood and Recommendation to Refrain Drinking Alcohol, Counseling for Drinking Problems (독거가구 및 기초생활수급 여부와 절주권고, 음주문제 상담 간의 연관성 분석)

  • Purpose: The purpose of this study was to analyze the association between single-person households & Beneficiary of National Basic Livelihood and recommendation on alcohol consumption, and counseling on drinking problems for adults 19 years of age or older. Methods: In this study, excluding missing values, the association between the single-person household & Beneficiary of National Basic Livelihood and recommendation to refrain drinking alcohol, counseling for drinking problems was analyzed by using the chi-squre test and logistic regression analysis. Results: In the case of non-single person households, compared to single-person households, the recommendation rate to refrain drinking alcohol was 1.519 OR (Odds Ratio [OR]: 1.159 , p-value <.0001). meanwhile, in the case of Beneficiary of National Basic Livelihood, the recommendation rate to refrain alcohol consumption was higher by 1.414 OR (OR: 1.414, p-value: 0.011), and the drinking problem counseling rate was also higher by 2.257 OR (OR: 2.257, p-value: 0.026) compared to non-beneficiary group. Discussion & Conclusion: Based on the 2016-2019 National Health and Nutrition Survey, this study investigated the associaiton between single households & Beneficiary of National Basic Livelihood and recommendations to refrain alcohol, and counseling on drinking problems. Compared to the Beneficiary of National Basic Livelihood group, single-person household group has recently been classified as a socially vulnerable group, but it is not applicable in the policy category. If policy and institutional measures for treatment are provided, it is expected that the problem of alcohol abuse can be reduced.

Resident Evaluation on Housing and Demand on Residential Service according to Type of Public Rental Housing (공공임대주택의 유형에 따른 거주자 평가와 주거서비스 조사)

  • There is a growing demand for public rental housing since housing price increased rapidly and socially vulnerable people need more stable housing. The purpose of this study is to investigate housing evaluation (satisfaction), moving plan, and residential service according to the type of the public rental housing and the characteristics of the resident, and to suggest the development direction for comprehensive housing services including the community level. Data used for analysis in this study are from 2016 Seoul panel survey of public rental housing residents. A total of 3009 questionnaire from public rental households were collected in 2016 Seoul panel survey. In order to investigate housing requirements, we analyzed household characteristics, housing satisfaction, moving plans, welfare services, residential activities and community facilities, social environment. In this study, the difference of residential satisfaction, moving plan, and residential service according to the type of the public rental housing and the characteristics of the resident is analyzed to identify their housing demands. As a result, there were differences in residential satisfaction, moving plan, and residential service depending on type of public rental housing and characteristics of residents. Residents in public rental housing modified from the multi-family housing after purchase showed lower residential satisfaction, and higher intention for moving plan compared to residents living in other type of public rental housing. In the residential service, residents in permanent lease type have higher experience of housing service as well as demand on housing service. Especially single household, elderly household, female household, or household with handicapped person indicated higher demand for housing services.

A Study on the Establishment of Concept and Selection criteria of Intelligent Security Technology Test-bed based on Spatial Information (공간정보 기반 지능형 방범 실증지구 개념 정립 및 선정기준에 관한 연구)

  • Establishment of safety net for the socially disadvantaged attracts large attention because of the recent crime increasing against vulnerable groups. For the successful establishment of social safety net, the test-bed for evaluation and realization of crime-related research results is required. However, previous R&D test-bed projects such as The Korean Land Specialization Program or U-Eco City project remains only to the stage of verification. Therefore, there are limitedness for realization of result technologies or sustainable operation & management of test-bed after projects finished. So, sustainable operation & management system and guideline of test-bed are necessary. Therefore, this study reviews the strengths and weaknesses of existing test-bed cases and intelligent security researches. After reviewing, the concept of a Intelligent Security Test-bed is established and appropriate test-bed selection criteria is also suggested. Based on objective criteria, selected test-bed can achieve sustainable management even after finishing the project and contribute the construction of standard model for citizen's safety.
