Objective : The predictors of cranioplasty infection after decompressive craniectomy have not yet been fully characterized. The objective of the current study was to compare the long-term incidences of surgical site infection according to the graft material and cranioplasty timing after craniectomy, and to determine the associated factors of cranioplasty infection. Methods : A retrospective cohort study was conducted to assess graft infection in patients who underwent cranioplasty after decompressive craniectomy between 2001 and 2011 at a single-center. From a total of 197 eligible patients, 131 patients undergoing 134 cranioplasties were assessed for event-free survival according to graft material and cranioplasty timing after craniectomy. Kaplan-Meier survival analysis and Cox regression methods were employed, with cranioplasty infection identified as the primary outcome. Secondary outcomes were also evaluated, including autogenous bone resorption, epidural hematoma, subdural hematoma and brain contusion. Results : The median follow-up duration was 454 days (range 10 to 3900 days), during which 14 (10.7%) patients suffered cranioplasty infection. There was no significant difference between the two groups for event-free survival rate for cranioplasty infection with either a cryopreserved or artificial bone graft (p=0.074). Intergroup differences according to cranioplasty time after craniectomy were also not observed (p=0.083). Poor neurologic outcome at cranioplasty significantly affected the development of cranioplasty infection (hazard ratio 5.203, 95% CI 1.075 to 25.193, p=0.04). Conclusion : Neurologic status may influence cranioplasty infection after decompressive craniectomy. A further prospective study about predictors of cranioplasty infection including graft material and cranioplasty timing is necessary.
BACKGROUND/OBJECTIVES: Fruit and vegetable consumption of children in the United States falls below recommendations. The U.S. Department of Agriculture Fresh Fruit and Vegetable Program (FFVP) is a national free-fruit and vegetable school distribution program designed to address this problem. This permanent, legislated program provides funding to qualified elementary schools for provision of additional fruit and vegetables outside of school meals. The objective of this study was to understand children's perceptions of FFVP after the intervention and formulate recommendations that may improve success of the intervention. SUBJECTS/METHODS: Secondary data were obtained from 5,265 $4^{th}-6^{th}$ graders at 51 randomly-selected FFVP intervention schools in Indiana. Anonymous questionnaires were completed late in the 2011-2012 academic year. Multilevel logistic regressions were used to determine associations between students' perceptions of program effects (4 close-ended items) and their preference toward the program. Content analysis was applied to a single open-ended item for program comments. RESULTS: Over 47% of students reported greater intake of fruit and vegetables due to FFVP, and over 66% reported liking the program. Student-reported program effects were positively associated with preference for the program (P < 0.01). Themes that emerged during analysis of 3,811 comments, included, students liked: the opportunity to try different kinds of fruit and vegetables, types and flavors of fruits served, and benefits of eating fruit. Fewer students liked the types of vegetables and their benefits. A small group disliked the program citing poor flavor of vegetables and quality of fruits. Important suggestions for the program include serving more dipping sauces for vegetables, cooking vegetables, and providing a greater variety of produce. CONCLUSIONS: The degree that students liked FFVP may predict the program's effects on fruit and vegetable intake. FFVP may become more acceptable to students by incorporating their suggestions. Program planners should consider these options for achieving program goals.

This tutorial paper has been written for biologists, physicians or beginners in fuzzy sets theory and applications. This field is introduced in the framework of medical diagnosis problems. The paper describes and illustrates with practical examples, a general methodology of special interest in the processing of borderline cases, that allows a graded assignment of diagnoses to patients. A pattern of medical knowledge consists of a tableau with linguistic entries or of fuzzy propositions. Relationships between symptoms and diagnoses are interpreted as labels of fuzzy sets. It is shown how possibility measures (soft matching) can be used and combined to derive diagnoses after measurements on collected data. The concepts and methods are illustrated in a biomedical application on inflammatory protein variations. In the case of poor diagnostic classifications, it is introduced appropriate ponderations, acting on the characterizations of proteins, in order to decrease their relative influence. As a consequence, when pattern matching is achieved, the final ranking of inflammatory syndromes assigned to a given patient might change to better fit the actual classification. Defuzzification of results (i.e. diagnostic groups assigned to patients) is performed as a non fuzzy sets partition issued from a "separating power", and not as the center of gravity method commonly employed in fuzzy control. It is then introduced a model of fuzzy connectionist expert system, in which an artificial neural network is designed to build the knowledge base of an expert system, from training examples (this model can also be used for specifications of rules in fuzzy logic control). Two types of weights are associated with the connections: primary linguistic weights, interpreted as labels of fuzzy sets, and secondary numerical weights. Cell activation is computed through MIN-MAX fuzzy equations of the weights. Learning consists in finding the (numerical) weights and the network topology. This feed forward network is described and illustrated in the same biomedical domain as in the first part.
Modular steel scaffolds are commonly used as supporting scaffolds in building construction, and traditionally, the load carrying capacities of these scaffolds are obtained from limited full-scale tests with little rational design. Structural failure of these scaffolds occurs from time to time due to inadequate design, poor installation and over-loads on sites. In general, multi-storey modular steel scaffolds are very slender structures which exhibit significant non-linear behaviour. Hence, secondary moments due to both $P-{\delta}$ and $P-{\Delta}$ effects should be properly accounted for in the non-linear analyses. Moreover, while the structural behaviour of these scaffolds is known to be very sensitive to the types and the magnitudes of restraints provided from attached members and supports, yet it is always difficult to quantify these restraints in either test or practical conditions. The problem is further complicated due to the presence of initial geometrical imperfections in the scaffolds, including both member out-of-straightness and storey out-of-plumbness, and hence, initial geometrical imperfections should be carefully incorporated. This paper presents an extensive numerical study on three different approaches in analyzing and designing multi-storey modular steel scaffolds, namely, a) Eigenmode Imperfection Approach, b) Notional Load Approach, and c) Critical Load Approach. It should be noted that the three approaches adopt different ways to allow for the non-linear behaviour of the scaffolds in the presence of initial geometrical imperfections. Moreover, their suitability and accuracy in predicting the structural behaviour of modular steel scaffolds are discussed and compared thoroughly. The study aims to develop a simplified and yet reliable design approach for safe prediction on the load carrying capacities of multi-storey modular steel scaffolds, so that engineers can ensure safe and effective use of these scaffolds in building construction.
For the purpose of developing the non-stoichiometric Zr-based Laves phase alloy with higher capacity and better performance for electrochemical application, extensive work has been carried out in KAIST. After careful alloy design of $ZrMn_2-based$ hydrogen storage alloys through varing their stoichiometry while susbstituting or adding some alloying elements, the $Zr-Ti-(Lh-V-Ni)_{2.2},\;Zr-Ti-(Mn-V-Cr-Ni)_{1.8\pm0.1}$ with high capacity and better performance was developed. Consequently the $Zr-Ti-(Mn-V-Ni)_{2.2}$ alloy has a high discharge capacity of 394mAh/g and shows a high rate capability equaling to that of commercialized $AB_5$ type alloys. On the other hand, in order to develop the hydrogen storage alloy with higher discharge capacity, the hypo-stoichiometric $Zr(Mn-V-Ni)_{2-\alpha}$ alloys substituted by Ti are under developing. As the result of competitive roles of Ti and $stocihiometry({\alpha})$, the discharge capacity of $Zr-Ti-(Mn-V-Cr-Ni)_{l.8\pm0.1}$ alloys is about 400mAh/g(410 mAh/g, which shows the highest level of performance in the Zr-based alloy developed. Our sequential endeavor is improving the shortcoming of Zr-based Laves phase alloy for commercialization, i.e., poor activation property and low rate capability, etc. It is therefore believed that the commercialization of Zr-based Laves phase hydrogen storage alloy for Ni-MH rechargeable battery is in near future.

The ecological background of a restoration project is complex and difficult to betermine without experimentation. A useful context for experiments is the well-studied process of natural succession, because the factors that drive or inhibit succession are also at work during reclamation (a form of primary succession) and restoration (which often resembles secondary succession). Using experimental studies on urban wasteland reclamation, we have tested for factors that stimulate or inhibit succession during early phases of woodland development in the Northeastern United states. The emphasis has been on mutualisms (seed dispersal, pollination, and mycorrhizae) and microsite limitations in the recruitment, growth, and reproduction of woody plants. Using plantings of seeds, seedlings, and clusters of reproductively mature plants on abandoned landfills, we have observed that (1) soil microsite deficiencies lead to very poor germination (<0.1$\%$) and seedling survival (<0.01$\%$) of most native species; (2) seed dispersal by birds is a significant and reliable source of woody plant recruitment; however (3) proximity effects are strong, with most (up to 95$\%$) of seed rain falling in the vicinity of planted clusters that are closest to putative seed sources; and (4) remnant natural woodlands are critical components of the recruitment process. To emphasize the last point, in one case, we found that the destruction of approximately 50$\%$ of nearby natural woodland vegetation led to a commensurate decline in seed rain. In another case, we found that the species richness of recruits was strictly limited by the species composition of nearby source plant communities, with no evidence of community enrichment by long distance dispersal over 5 years. We conclude from these results that the size and proximity of remnant natural populations are critical considerations when planning reclamation and restoration programs that rely on natural successional processes.
Parasitic leeches could directly (through causing poor growth, anemia and wound in the fish) and indirectly (by predisposition of the fish to secondary bacterial and fungal infections) affects their hosts. In the present study, fishes that were attacked by leeches in natural and experimental environment were studied. Pathologic samples were obtained from damages at the site of leech bite, as well as kidney and liver of the fish. Histopathological examination revealed numerous lesions at the site of leech bite including tissue demolition, detachment at the site of leech bite in the epidermis of epithelial tissue in the skin, destructed nucleus in epithelial cells of the skin plus necrosis in the damaged skin and weak inflammatory penetration to acute necrotic damages along with piercing dermis layer. Pathologic lesions in the kidney included some changes such as proliferation by increasing glomerular cells and membrane cells in capillary vein of the kidney, blood cell necrosis in kidney with infiltration of white blood cells mainly mononuclear and less polymorphonuclear which are the symptoms of anemia due to blood feeding and sucking by leeches. There was also a chronic kidney infection probably originated from another part of body such as skin. Moreover, leeches caused hemorrhagic anemia due to blood consumption of the hosts, which led to observation of immature red blood cells. Also results showed that diseases induced by leeched in fish could be acute or chronic, which depends on size of fish, species of leech and severity of infection.
We present one case of 26-year-old male having saddle block combined with mitral valvular disease [NYHA Class IV] with auricular fibrillation. The most common cause of emboli is atrial fibrillation. The clinical manifestations of saddle emboli are relatively slow due to development of collateral circulation and large size of lumen of the aorta. The 5month duration of saddle emboli in this case led to severe atrophic changes, coldness, peripheral cyanosis on the both lower extremities, and flexion deformity on the knee and ankle joint of the left lower extremity. We planned staged operation for the saddle block and for mitral stenoinsufficiency and tricuspid insufficiency, because of poor general condition of the patient. The thromboembolectomy of aortic bifurcation was performed through the transabdominal approach without trial of Fogarthy catheter embolectomy, because of expectation of the secondary inflammatory changes of the vessel wall and thrombi which was 3 cm X 1 cm X 0.5 cm in size with irregular surfaced solid in consistency. 1 month later, after thromboembolectomy, mitral valve replacement and tricuspid annuloplasty were performed, with successful early operative result. During operation organized thrombi [1 cm X 0.5 cm] in the left auricle was removed. We wonder if simple management using Fogarthy catheter might be possible to remove the thromboemboli instead of thromboembolectomy by aortotomy in this case.
A broiler farm in Jeonbuk province experienced high mortality due to decreased feed intake and poor growth, and pathological evaluation of 11- and 17-day-old broilers was performed, which led to the diagnosis of runting-stunting syndrome. From the start of the rearing period to shipment, 18.4% of the chickens in the three barns experiencing continuous culling and mortality were affected, compared to 7.7% in the other five barns. Gross findings on the 11-day-old broiler chicken revealed proventricular dilatation and hemorrhage, intestinal hemorrhage, urate deposition in the pericardium and renal tubule, nephropathy, and mild hepatic capsulitis. Similar proventricular dilatation and hemorrhage were observed in the 17-day-old broiler chickens. In addition, hepatitis and pericarditis were observed with the progression of secondary bacterial infection, and pathogenic Escherichia coli was isolated from these lesions. As a result of PCR, Newcastle disease virus, fowl adenovirus, chicken anemia virus and Marek's disease virus were not detected in the all tissue samples. In contrast, infectious bronchitis virus was detected in the proventriculus, kidney and cecal tonsil. chicken astrovirus was detected in the intestine, cecal tonsil and bursa of Fabricius, and chicken parvovirus was detected in proventriculus, intestine, cecal tonsil and bursa of Fabricius. By sharing the diagnostic process of a case of malabsorption syndrome through this case report, we hope that it can be widely utilized in the diagnostic process of livestock disease pathognomonic institutions.
While investigation of the fungal diseases on apples collected from Cheongsong-gun and Bonghwa-gun in Gyeongbuk province, Korea, between August and September 2023 isolated five fungal strains from fruits with sooty blotch and flyspeck (SBFS) disease. The strains were designated as KNUF-23-CS02, KNUF-23-CS-06, KNUF-23-CS12, KNUF-23-BH01, and KNUF-23-BH03. When grown on potato dextrose agar and 2% water agar, the cultural characteristics of the strains were similar to those previously reported characteristics of Peltaster fructicola Pf001. The strains produced monoblastic, hyaline conidiogenous cells; the conidia were hyaline, unicellular, cylindrical to ovoidal, and 3.5-7×1.7-3.9 and 4.0-6.6×1.8-3.2 μm in size on synthetic nutrient-poor agar or water agars, respectively. Secondary conidia production by microcyclic conidiation and budding was observed. The KNUF-23-BH03 strain was shown to cause SBFS symptoms similar to those observed on the apples in the pathogenicity test. Molecular phylogenetic analyses were conducted based on the isolated species sequences of the internal transcribed spacer region, nuclear large ribosomal DNA subunit, and mitochondrial small ribosomal RNA subunit gene. The five strains were clustered with Peltaster fructicola Pf001. Based on the cultural and morphological characteristics and phylogenetic analysis, the five strains were identified as Peltaster fructicola, which has not been previously reported in Korea.
