A Clinical Case of Liver Injury with Administration of Yeoldahanso-tang plus Rheum rhabarbarum (열다한소탕가대황(熱多寒少湯加大黃) 복용 후 간 손상 수치가 상승된 1례)

  • 1. Objects: This case study reports a liver injury in treatment of constipation with Yeoldahanso-tang plus Rheum rhabarbarum. 2. Methods: The patient's subjective symptoms and the laboratory data including aspartate transaminase(AST), alanine transaminase(ALT), alkaline phosphatase(ALP), and gamma($\gamma$)-glutamyl transpeptidase(GGT) were observed before and after liver injury. We used the Roussel Uclaf Causality Assessment Method(RUCAM) scale in order to find out whether if Yeoldahanso-tang plus Rheum rhabarbarum was related with the liver injury. 3. Results: The patient's AST and ALT were elevated more than twice after a 3week administration period of herbal medicine. After discontinuation of the herbal medicine and administration of hepatic protective medication, AST and ALT decreased. The patient's general condition improved during hospitalization. The RUCAM score was estimated at 8. 4. Conclusions: This case is presented to bring more attention to the toxicity of herbal medicine.

Insertion Mutation in HMG-CoA Lyase Increases the Production Yield of MPA through Agrobacterium tumefaciens-Mediated Transformation

  • Mycophenolic acid (MPA) is an antibiotic produced by Penicillium brevicompactum. MPA has antifungal, antineoplastic, and immunosuppressive functions, among others. ${\beta}-Hydroxy-{\beta}-methylglutaryl-CoA$ (HMG-CoA) lyase is a key enzyme in the bypass metabolic pathway. The inhibitory activity of HMG-CoA lyase increases the MPA biosynthetic flux by reducing the generation of by-products. In this study, we cloned the P. brevicompactum HMG-CoA lyase gene using the thermal asymmetric interlaced polymerase chain reaction and gene walking technology. Agrobacterium tumefaciens-mediated transformation (ATMT) was used to insert a mutated HMG-CoA lyase gene into P. brevicompactum. Successful insertion of the HMG-CoA lyase gene was confirmed by hygromycin screening, PCR, Southern blot analysis, and enzyme content assay. The maximum MPA production by transformants was 2.94 g/l. This was 71% higher than wild-type ATCC 16024. Our results demonstrate that ATMT may be an alternative practical genetic tool for directional transformation of P. brevicompactum.

Algal Consumption and Preference of Sea Urchins, Strongylocentrotus nudus, S. intermedius and Abalone, Haliotis discus hannai (둥근성게, 북쪽말똥성게 및 참전복의 일일 먹이소비량 및 해조류 먹이선택성)

  • In 2006, we studied herbivore consumption rate and preference for algal species using laboratory experiments. In the no-choice feeding experiment, three herbivores (two sea urchins, Stronglyocentrotus nudus and Stronglyocentrotus intermedius and an abalone, Haliotis discus hannai) showed significantly (P<0.05) distinct preferences for four algae (Laminaria japonica, Undaria pinnatifida, Costaria costata and Ecklonia stolonifera). The alga preferred by S. nudus was L. japonica followed by U. pinnatifida, C. costata and E. stolonifera with respective daily feeding rates of $5.7{\pm}:0.85\;g,\;4.4{\pm}0.89\;g,\;3.1{\pm}0.39\;g\;and\;2.1{\pm}0.32\;g(mean{\pm}SE)$. A similar trend was found for S. intermedius. Interestingly, the herbivore consumption rates for the algae differed with or without competition. The competition stimulated the feeding activity for S. nudus, but lowered the activity for S. intermedius. The multi-choice feeding experiment confirmed that L. japonica was significantly preferred by two herbivores: S. nudus and H. discus hannai (Duncan's multiple range test, ANOVA, p<0.05).

Comparative Study of Multimodal Therapy in Facial Palsy Patients

  • Introduction In chronic facial palsy, synkinetic muscle overactivity and shortening causes muscle stiffness resulting in reduced movement and functional activity. This article studies the role of multimodal therapy in improving outcomes. Methods Seventy-five facial palsy patients completed facial rehabilitation before being successfully discharged by the facial therapy team. The cohort was divided into four subgroups depending on the time of initial attendance post-onset. The requirement for facial therapy, chemodenervation, or surgery was assessed with East Grinstead Grade of Stiffness (EGGS). Outcomes were measured using the Facial Grading Scale (FGS), Facial Disability Index, House-Brackmann scores, and the Facial Clinimetric Evaluation scale. Results FGS composite scores significantly improved posttherapy (mean-standard deviation, 60.13 ± 23.24 vs. 79.9 ± 13.01; confidence interval, -24.51 to -14.66, p < 0.0001). Analysis of FGS subsets showed that synkinesis also reduced significantly (p < 0.0001). Increasingly, late clinical presentations were associated with patients requiring longer durations of chemodenervation treatment (p < 0.01), more chemodenervation episodes (p < 0.01), increased doses of botulinum toxin (p < 0.001), and having higher EGGS score (p < 0.001). Conclusions This study shows that multimodal facial rehabilitation in the management of facial palsy is effective, even in patients with chronically neglected synkinesis. In terms of the latency periods between facial palsy onset and treatment initiation, patients presenting later than 2 years were still responsive to multimodal treatment albeit to a lesser extent, which we postulate is due to increasing muscle contracture within their facial muscles.

A Historical-Geographical Identification of East Asia as a Cultural Region (동아시아 문화지역의 역사-지리적 설정)

  • In East Asia, regional identity can be expected to obtain popular consent more successfully when it is firmly based on historical-geographical reality. This study is an attempt to apply a broadened concept of place to the identification of East Asia as a cultural region. Cultural mixture within places at various scales, rather than cultural integration across those places, would give greater coherence to East Asia as a cultural region. This cultural mixture varies from one place to another, depending on the relative position in power relations. It could appear in the form of either domination or resistance, and even entanglement. The concept of a "mountain as a contested place" is proposed as an experimental effort to search for the basis for cultural identity within East Asia. This concept of place should be extended to the individual studies of such spatial units as houses, gardens, villages and cities. These individual studies, if accumulated, would result in improved theories of East Asia as a region that has a distinct cultural identity in historical-geographical terms.

A Case Report of FOLFOX-induced Adverse Events in a Patient with Recurrent Advanced Gastric Cancer Treated with Korean Medicine (재발 진행성 위암 환자의 FOLFOX 유발 부작용 개선에 대한 한의 치료 1례)

  • Objective: The purpose of this study was to report the clinical effectiveness of Korean medicine treatment on FOLFOX-induced symptoms such as nausea and dizziness in a patient with recurrent advanced gastric adenocarcinoma. Methods: The patient was diagnosed with recurrent advanced gastric cancer and had FOLFOX-induced nausea and dizziness. The patient was treated with Samchulgunbi-tang and Banhasashim-tang for symptom management. The clinical outcomes were measured by National Cancer Institute Common Terminology Criteria for Adverse Event (NCI CTCAE) and a numeral rating scale (NRS). Results: After treatment, the patient's nausea and dizziness were relieved from NRS 9 to 2 and NRS 8 to 2 respectively. During and after treatment, no severe toxicities were detected on laboratory findings. Conclusion: This case study suggests that Samchulgunbi-tang and Banhasashim-tang may relieve symptoms of FOLFOX-induced nausea and dizziness.

Molting and Growth of the Snow Crab Chionoecetes opilio in the East Sea of Korea (한국 동해안 대게, Chionoecetes opilio의 탈피와 성장)

  • Molting and growth of the snow crab, Chionoecetes opilio was investigated using samples captured in the East Sea from July 2002 to June 2004. Individuals over 40 mm carapace width (CW) molted once a year from July to October. Annual molt stage of C. opilio can be divided into four stages; premolt stage, molting stage, postmolt stage and intennolt stage. The relationship between CW and chela height (CH) can be expressed as Y=-82lnCW+73.1129lnCH+166. They were separated into two groups based on the equation, that is, one group having a negative value (below 70 mm in CW) and other group having a positive value (over 130 mm in CW). Carapace width at 50% terminal molt ($CW_{50%}$) of males was estimated to be 105 mm. The Gompertz growth equation estimated from a non-linear method was $CW=118.99e^{-6.296e^{-0.3062t}}$ for females and $CW=156368e^{-6.6619e^{-0.2626t}}$ for males.

Microwave-Accelerated Click Chemistry: Expeditious Synthesis of Novel Triazole-linked Salicylic β-D-O-Glycosides with PTP1B Inhibitory Activity

  • The incorporation of microwave irradiation with the prevalent "click chemistry" is currently of considerable synthetic interest. We describe here the introduction of such laboratorial shortcut into carbohydrate-based drug discovery, resulting in the rapid formation of a series of triazole-linked salicylic $\beta$-D-O-glycosides with biological activities. All "clicked" products were achieved in excellent yields ($\approx$ 90%) within only a quarter. In addition, based on the structural characteristics of the afforded glycomimetics, their inhibitory activities were evaluated toward protein tyrosine phosphatases 1B (PTP1B) and a panel of homologous protein tyrosine phosphatases (PTPs). Docking simulation was also conducted to plausibly propose binding modes of this glycosyl salicylate series with the enzymatic target.

A Case Report of Metastatic Breast Cancer Patient Treated with Integrative Cancer Treatment (전이성 유방암 환자의 통합 암 치료에 대한 증례보고)

  • Objective : The purpose of this study is to report and observe effects of Integrative Cancer Treatment (ICT) on metastatic breast cancer patient. Method : A left breast cancer patient diagnosed with recurrence on liver and bones April 2018. The patient had received paclitaxel chemotherapy for two months and discontinued it because of tumor progression and side effects. The patient has been treated with ICT since March 2018 and has been taking capecitabine since October 2018. The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA15-3). The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA15-3), liver function test (AST, ALT), Eastern Cooperative Oncology Group performance status (ECOG PS), and numeric rating scales (NRS). Results : After the ICT, tumor size was partially decreased accompanying by reducing the levels of tumor markers. Major clinical symptoms induced by paclitaxel chemotherapy were improved. There were no severe adverse events induced by ICT based on National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE) version 5.0. Conclusion : This case suggests that ICT may have synergistic effect for the treatment of metastatic breast cancer patient in conjunction with capecitabine.

Geophysical survey around East Sea Research Institute (KORDI) using multi-beam and shallow seismic survey (다중빔 음향측심기 및 천부탄성파 탐사를 이용한 동해연구소 주변 지구물리조사)

  • Geophysical survey were investigated in the offshore around East Sea Research Institute, Korea Ocean Research and Development Institute (Jukbyeon-myun, Uljin-gu, Gyeongsangbuk-do, Korea). The surveys were conducted aboard the R/V Jangmok in 2008 using a hull-mounted EM 3002 multi-beam echosounder. Precise bathymetry and seabed images were obtained using multi-beam and thicknesses of sedimentary layer were found through seismic survey. Submarine topography deepens parallel to the coastline to -60 m and rock mass distributed in the southeast of study area. By finding the thickness of sedimentary layer through seismic survey, a sedimentary thickness on the study area was established. Futhermore, monitoring data of bathymetry, substructure and sedimentary environment will be secured through successive geophysical investigation.
