Anxiety, Depression and Uncertainty in Cancer Patients Participating in Clinical Trial of Anticancer Drugs (항암제 임상시험에 참여 중인 암 환자의 불안, 우울과 불확실성)

  • Purpose: The purpose of the study was to identify the levels of anxiety, depression and uncertainty of patients who participated in the clinical trials for anticancer drug, and to identify correlations among these variables. Methods: Cross-sectional survey used the Symptom Check List-90-Revision and the Mishel Uncertainty in Illness Scale from 106 subjects in 2011. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients. Results: The mean score of anxiety was 2.06, that of depression 2.35, and that of uncertainty 2.61. Anxiety and depression (r=.70), anxiety and uncertainty (r=.44), depression and uncertainty (r=.60) were significantly correlated each other. The levels of anxiety, depression and uncertainty were different in various characteristics of the subjects, such as education, recurrence, and economic burden. Conclusion: The results of the study indicate that when implementing psychosocial interventions for cancer patients who participate in clinical trial, factors such as education, economic burden, and recurrence should be integrated into the intervention. Further studies applying theoretical model would be helpful to identify directional relationships among the variables that are important in psychosocial well-being of cancer patients undergoing clinical trial.

Residual Echo Suppression Based on Tracking Echo-Presence Uncertainty (Tracking Echo-Presence Uncertainty 기반의 잔여 반향 억제)

  • In this paper, we propose a novel approach to residual echo suppression (RES) algorithm based on tracking echo-presence uncertainty (TEPU) to improve the performance of acoustic echo suppression (AES) in the frequency domain. In the proposed method, the ratio of the microphone input and the echo-suppressed output signal power is employed as the threshold value for the decision rule to estimate the echo-presence uncertainty applied to the RES filter. The proposed RES scheme estimates the echo presence uncertainty in each frequency bin and effectively reduces residual echo signal in a simple fashion. The performance of the proposed algorithm is evaluated by the objective test and yields better results compared with the conventional schemes.

Correlations among Total Symptom Distress, Uncertainty and Quality of Life among Women With Endometriosis (자궁내막증을 가진 여성의 증상 스트레스, 불확실성, 삶의 질 관계 연구)

  • Purpose: The purpose of this study was to explore the relationships among women's total symptom distress, uncertainty and quality of life in women with endometriosis. Method: A total of 121 women who had been diagnosed with endometriosis was given a questionnaire. Data was collected from August 2004 to January 2005. The data was analyzed by frequency, mean, t-test, ANOVA, Pearson correlation coefficient, and multiple linear regression analysis using SPSS WIN 10.0 software. Result: The mean score of TSD was 3.23. 'Menstrual cramping' showed the highest score. The mean score of uncertainty, and quality of life of the subject was 56.6, and 38.3 respectively. TSD was significantly related with coffee intake and uncertainty was significantly related with marital status, economic status, alcohol intake, diagnosis after period, and family history. Quality of life was significantly related with age, and dysmenorrhea. There were significant relationships between total symptom distress and uncertainty, and quality of life. There were significant relationship between uncertainty and quality of life. Conclusion: Endometriosis symptoms showed a significant relationship with uncertainty and quality of life. This study will help to manage women with endometriosis.

Effect of critical flow model in MARS-KS code on uncertainty quantification of large break Loss of coolant accident (LBLOCA)

  • The critical flow phenomenon has been studied because of its significant effect for design basis accidents in nuclear power plants. Transition points from thermal non-equilibrium to equilibrium are different according to the geometric effect on the critical flow. This study evaluates the uncertainty parameters of the critical flow model for analysis of DBA (Design Basis Accident) with the MARS-KS (Multi-dimensional Analysis for Reactor Safety-KINS Standard) code used as an independent regulatory assessment. The uncertainty of the critical flow model is represented by three parameters including the thermal non-equilibrium factor, discharge coefficient, and length to diameter (L/D) ratio, and their ranges are determined using large-scale Marviken test data. The uncertainty range of the thermal non-equilibrium factor is updated by the MCDA (Model Calibration through Data Assimilation) method. The updated uncertainty range is confirmed using an LBLOCA (Large Break Loss of Coolant Accident) experiment in the LOFT (Loss of Fluid Test) facility. The uncertainty ranges are also used to calculate an LBLOCA of the APR (Advanced Power Reactor) 1400 NPP (Nuclear Power Plants), focusing on the effect of the PCT (Peak Cladding Temperature). The results reveal that break flow is strongly dependent on the degree of the thermal non-equilibrium state in a ruptured pipe with a small L/D ratio. Moreover, this study provides the method to handle the thermal non-equilibrium factor, discharge coefficient, and length to diameter (L/D) ratio in the system code.

Uncertainty, Anxiety and Coping with Mastectomy for Breast Cancer (유방절제술 환자의 불확실성, 불안 및 대처방식)

  • The purpose of this study was to provide basic information for developing a nursing intervention that helps patients learn how to acquire coping to reduce post operation uncertainty and anxiety by investigating the level of uncertainty and anxiety experienced by mastectomy patients. The subjects were 134 patients selected from St. Mary's Kangnam and St. Mary's hospital, and the data collection period was from October to December of 1998. Uncertainty was measured by using Mishel Uncertainty in Illness Scale (MUIS), anxiety measured by using State-Trait Anxiety Inventory(STAI), and coping by using a questionnaire developed by Kim & Yoo (1996). Data were analyzed with SAS program by t-test, ANOVA, Duncan's multiple range test, Pearson correlation coefficients, and stepwise multiple regression. The results were as follows : 1. The mean uncertainty score was 57.6. The results of the correlation between the compliance of medical regimen and demographic characteristics were as follows ; Those who monthly income over 2,010,000 won had lower than above 1,000,000 won, and those with the experience of chemotherapy had higher than those without, and the patient who has 7∼12 months(1 yr.) post operation period had higher than the one below 6months, 25∼36 months(3 yrs.), 37∼60 months(5 yrs.), and over 61 months. 2. The mean anxiety score was 45.9. Anxiety tended to be increased slightly in subjects with low educational background, poor monthly income, experience of chemotherapy, and 7∼12 months(1 yr.) post operation period, but there was no significant difference by general characteristics 3. The mean value of the coping score was 100.7. The study revealed higher score in problem-focused coping than emotion-focused coping. In regard to coping by demographic characteristics were as follows ; those who had monthly income over 2,010,000 won had higher level of coping than those whose monthly income was between 1,010,000 and 2,000,000 won. In terms of problem- focused coping, those who had 25∼36 months of post operation period showed significantly lower level of coping than those below 6 months or 37∼60 months(5 yrs.) or over 61 months of post operation period. Regarding the emotion-focused coping, those with the christianity had significantly lower level of coping than those without it. Also, those whose monthly income over 2,010,000 won had significantly higher coping level than those with income of between 1,010,000 and 2,000,000 won. 4. A positive relationship was found between uncertainty and anxiety. Patients who experienced more uncertainty also showed more anxiety. Problem-focused coping was inversely related to uncertainty and anxiety. 5. The major variable that affected uncertainty was anxiety, explaining 63.3% of the uncertainty. In addition to this, it would explain 66.4% in total when experience of chemotherapy was added.

  • A PSD system has a lot of characteristic for safety. Trust of performance test is very important. But PSD performance test has an effect on trust by proficiency of test staff because it is difficult to automate. In that specially the effort test, manual opening and shutting test and abrasion measurement, test staff is demanded for proficiency. Therefore the paper repeated measurement in same condition and studied proficient factor of examiner according to increased measurement counter. As a result, measurement uncertainty has improved.

The Effect of Nursing Information on Anxiety and Uncertainty in Patients for Endoscopic Submucosal Dissection before the procedure of the patients (간호정보제공이 내시경 점막하 박리술 환자의 시술 전 불안 및 불확실성에 미치는 효과)

  • This study examined the effects of nursing information provision using a booklet on the patients' anxiety and uncertainty with an endoscopic submucosal dissection before the procedure, which is a non-equivalent controlled pre-post test designed study. Twenty three patients in a control group received the existing intervention, which is the general education prior to the procedure without standardized format as well as an oral explanation, and 23 patients in the experimental group received nursing information with a booklet developed by the investigator of this study. The collected data were analyzed using a ${\chi}^2$ test and Fisher's exact t-test on SPSS 21.0. The experimental group reported significantly lower anxiety (t=3.319, p=.002) and anxiety behavioral responses (t=3.508, p=.001) than those in the control group. There were no significant differences in uncertainty between the groups (t=.745, p=.460). Nursing information using the booklet is a useful nursing intervention to reduce the anxiety of patients with endoscopic submucosal dissection before the procedure.

Perceived Uncertainty and Rearing Attitude of Mothers with Chronically ILL Children (만성질환아 어머니의 질병에 대한 불확실성 정도와 양육태도)

  • The purposes of this study were to measure the degree of perceived uncertainty and to identify the rearing attitudes of mothers with chronically ill children, and to examine the relationship between the perceived uncertainty and the rearing attitude of these mothers. The subjects of this study consisted of 133 mothers with chronically ill children, registered at 2 university hospitals in Seoul. Data was collected from April 1 to May 31, 1996. The Parents' Perception of Uncertainty Scale(28-item 4 point scale) and the Maternal Behavior Research Instrument (49-item 5 point scale) were used. Data was analyzed by Cluster analysis, ANOVA MANOVA and t-test. Results of this study are summarized as follows : 1. Mothers perceived their uncertainty to be slightly high (Mn 2.48). The degree of perceived uncertainty by the four components slightly differed unpredictability(2.72), lack in clarity(2.58), vagueness (2.52) and lack of information(2.04) . The degree of perceived uncertainty of mothers with ill children revealed to be influenced significantly by the age of the ill children, duration of illness after the diagnosis, and the experience of hospitalization. 2. Among the rearing attitudes : moderatlely high affective(Mn 3.98) and resrictive(Mn 3.58) attitudes of mothers toward their ill children were identified. Mothers tend to give positive evaluations of their childrens' behaviors (Mn 3.38) and less rejection(Mn 2.81). 3. Mothers' rearing attitude were correlated with the degree of perceived uncertainty in illness ; mothers in the Low Perceived Uncertainty Group (Mn 1.99) revealed the highest affective (Mn 4.08), the lowest resrictive(Mn. 2.72) attitudes and tendency to give positive evaluations of their childrens' behaviors (Mn 3.54) compared to the High Perceived Uncertainty Group(Mn 3.26) and Moderate Perceived Uncertaity Group(Mn 2.57). 4. The degree of perceived uncertainty, the duration of illness after the diagnosis and the experience of hospitalization revealed to be significantly influential to the rearing attitude of mothers with chronically ill children. From the above results, it can be concluded that predicting and controlling mothers' uncertainty are necessary for improved, efficient nursing interventions and normal growth & development of the chronically ill children.

  • In this paper, a control system using robust H.inf. controller based on convex parametrization is presented for nonlinear system with uncertainty. accounting for the time delay, noise and linearization error by frequency analysis, the suboptmal controller is designed to meet robust stability and performance for uncertainty. The desinged control system is applied to a nonlimear boiler moderl to test its performances.

Quantifying Uncertainty of Vitamin C Determination in Infant Formula by Indophenol Titration Method (인도페놀 적정법에 의한 성장기용조제식 중 비타민 C 함량분석의 측정불확도 산정)

  • Uncertainty involved during determination of vitamin C content in infant formula was quantified by indophenol titration method. Uncertainty sources in measurand, such as purity, weight, final volume of standard, volume of standard solution used for titration, sample weight, final volume of sample, extraction solution used for titration, titration of extraction solution and standard solution by indophenol solution were identified and used as parameters for combined standard uncertainty based on Guide to the expression of uncertainty in measurement (GUM) and Draft EURACHEM/CITAC Guide. Uncertainty parameters of each source in measurand were identified as resolution, reproducibility and stability of chemical balance, standard material purity, repeatability, reproducibility, end point of titration, 1 mL pipet, 5 mL autopipet, and 100 mL mass flask. Each uncertainty component was evaluated by types A and B and included to calculate combined uncertainty. Analytical test result for traceability under laboratorial conditions using Certified Reference Material (CRM) test was certified as $108.4{\pm}1.7mg/100g$, which was within CRM certification range of $114.6{\pm}6.6mg/100g$. Uncertainty test result of vitamin C content of 5 g sampling was $56.7{\pm}2.44mg/100g$. Uncertainty could be reduced by identification of uncertainty sources and components related with vitamin C determination by indophenol titration method and by decreasing uncertainty sources and components.
