  • The best estimate plus uncertainty methodologies for loss-of-coolant accident analyses make use of the best-estimate codes and relevant experimental databases. Inherently, best-estimate codes have various uncertainties in the model parameters, which can be quantified by the dedicated experimental database. Therefore, this study was devoted to establishing procedures for identifying the input parameters of predictive models and quantifying their uncertainty ranges. The rod bundle heat transfer experiments were employed as a representative reflood separate effect test, and the TRACE code was utilized as a best-estimate code. In accordance with the present procedure for uncertainty quantification, the integrated list of the influential input parameters and their uncertainty ranges was obtained through local sensitivity calculations and screening criteria. The validity of the procedure was confirmed by applying it to uncertainty analyses, which checks whether the measured data are within computed ranges of the variables of interest. The uncertainty quantification procedure proposed in this study is anticipated to provide comprehensive guidance for the conduct of uncertainty analyses.

Resident Perceptions of Competency-Based Korean Medicine Education: A Qualitative, Content Analysis Study Conducted using Focus Group Interviews

  • Objectives: The new educational system emphasizes acquisition of clinical competency by the time of graduation from Korean medicine colleges that allow Korean medicine doctors to immediately perform clinical tasks. This study investigated awareness of competency-based education in Korean medicine hospital residents who must simultaneously undergo training and assist medical students in clinical practice. Methods: This was a qualitative research that was conducted using focus group interviews (FGIs) to investigate the awareness of demands for improvement in competency-based Korean medicine education in Korean medicine hospital. To apply the principles and procedures of FGIs, a semi-structured questionnaire was developed. Data analysis was conducted using the five steps of framework analysis. Results: According to contents analysis, first competency-based education that reflects actual clinical practice tasks is needed. Second, sufficient basic skill mastery education must be reinforced. Third, an intermediate curriculum that mediates clinical practice and basic education is needed. Fourth, the Objective Structured Clinical Examination and Clinical Performance Examination must be expanded to prepare for the Korean medicine doctor practical test. Conclusions: Korean medicine residents reported the gap between clinical practice and use of knowledge and skills acquired in the curriculum while acting as direct observers and educations of clinical clerkship in hospitals. Based on this exploratory study it is necessary to conduct research on the educational competency of Korean medicine residents who play an important role as educational leaders in Korean medicine clinical practice training.

Application of artificial intelligence for diagnosis of early gastric cancer based on magnifying endoscopy with narrow-band imaging

  • Although magnifying endoscopy with narrow-band imaging is the standard diagnostic test for gastric cancer, diagnosing gastric cancer using this technology requires considerable skill. Artificial intelligence has superior image recognition, and its usefulness in endoscopic image diagnosis has been reported in many cases. The diagnostic performance (accuracy, sensitivity, and specificity) of artificial intelligence using magnifying endoscopy with narrow band still images and videos for gastric cancer was higher than that of expert endoscopists, suggesting the usefulness of artificial intelligence in diagnosing gastric cancer. Histological diagnosis of gastric cancer using artificial intelligence is also promising. However, previous studies on the use of artificial intelligence to diagnose gastric cancer were small-scale; thus, large-scale studies are necessary to examine whether a high diagnostic performance can be achieved. In addition, the diagnosis of gastric cancer using artificial intelligence has not yet become widespread in clinical practice, and further research is necessary. Therefore, in the future, artificial intelligence must be further developed as an instrument, and its diagnostic performance is expected to improve with the accumulation of numerous cases nationwide.

Allosteric Probe-Based Colorimetric Assay for Direct Identification and Sensitive Analysis of Methicillin Resistance of Staphylococcus aureus

  • The accurate and rapid detection of methicillin-resistance of Staphylococcus aureus (SA) holds significant clinical importance. However, the methicillin-resistance detection strategies commonly require complicated cell lysis and gene extraction. Herein, we devised a novel colorimetric approach for the sensitive and accurate identification of methicillin-resistance of SA by combining allosteric probe-based target recognition with self-primer elongation-based target recycling. The PBP2a aptamer in the allosteric probe successfully identified the target MRSA, leading to the initiation of self-primer elongation based-cascade signal amplification. The peroxidase-like hemin/G-quadruplex undergo an isothermal autonomous process that effectively catalyzes the oxidation of ABTS2- and produces a distinct blue color, enabling the visual identification of MRSA at low concentrations. The method offers a shorter duration for bacteria cultivation compared to traditional susceptibility testing methods, as well as simplified manual procedures for gene analysis. The overall amplification time for this test is 60 min, and it has a detection limit of 3 CFU/ml. In addition, the approach has exceptional selectivity and reproducibility, demonstrating commendable performance when tested with real samples. Due to its advantages, this colorimetric assay exhibits considerable potential for integration into a sensor kit, thereby offering a viable and convenient alternative for the prompt and on-site detection of MRSA in patients with skin and soft tissue infections.

  • With the increasing age of motherhood in recent years, attributed to late marriages due to social or environmental factors, the Down's syndrome screening test using biochemical markers has become essential for pregnant women. The process of diagnosing Down's syndrome pregnancy in the high-risk group subjects involves chromosomal analysis, which is performed on samples obtained through invasive procedures such as chorionic biopsy or amniotic fluid. Thus, to reduce unnecessary invasive tests and lower the risk to mother and fetus, it is important to identify a screening test with low risk and high Down's syndrome detection rate. Recently, as the average age of mothers has increased, numerous inspection agencies have classified high-risk mothers as women over the age of 35 years. This study evaluated a total of 36,436 pregnant women aged between 17 to 46 years, and who requested prenatal screening at an inspection agency in Yongin in 2018. Test (13,690 people) Four tests were conducted by applying the time-resolved fluoroimmunoassay method using the direct sandwich and indirect sandwich technology, and the immunoassay method using the sandwich method. We aimed to confirm the difference in positivity rate with increasing age of the subjects. We believe that in future, data obtained from this study will be very useful for the prevention and treatment of Down's syndrome risk at varied inspection institutions, and for prospective mothers.

Sealing Ability of Three Different Materials Used as Retrograde Filling

  • Purpose: To test the apical leakage prevention performance of three different materials through protein leakage procedures using bovine serum albumin (BSA) and Bradford protein reagent. Materials and Methods: A total of 60 human single-rooted teeth were divided into 4 groups, and conventional root canal filling was done. The root was cut 3 mm from the apex, and a cavity was formed. Proroot MTA (MTA), Fuji II LC (GI), Fuji II LC with XP bond (GIA), and Caviton (CA) were used as experimental materials to fill the cavity in a retrograde filling manner. The extent of BSA leakage was then measured with a ultraviolet visible spectrophotometer 24, 48, and 72 hours after filling. Result: After 24 hours, among the 15 teeth of each group, 2 in MTA, 4 in GI, 3 in GIA, and 7 in CA showed leakage. After 48 hours, 3 in MTA, 5 in GI, 5 in GIA, and 10 in CA had leakage and discoloration. After 72 hours, among the 15 teeth of each group, 3 in MTA, 6 in GI, 5 in GIA, and 10 in CA showed leakage. The leakage in the CA group was greater than that in the MTA group at 48 and 72 hours based on Fisher's exact test (P=0.025), and the difference was statistically significant. Similarly, the leakage in the CA group was greater than that in the MTA group over time based on the Kaplan-Meier survival estimate (P=0.011), and the difference was statistically significant. Conclusion: Glass ionomer, glass ionomer after adhesive application, and MTA all showed leakage. Caviton showed greater leakage compared to MTA 48 and 72 hours after filling, and the difference was statistically significant; thus suggesting that Caviton is not appropriate as retrograde filling material considering its sealing ability.

Antimicrobial susceptibility and pathogenic genes of Staphylococcus aureus isolated from the oral cavity of patients with periodontitis

  • Purpose: The goal of this study was to characterize the patterns of antimicrobial resistance and virulence genes in samples of Staphylococcus aureus (S. aureus) isolated from periodontitis patients. Methods: From July 2015 to August 2015, oral saliva was collected from a total of 112 patients diagnosed with periodontitis, including 80 outpatients in dental hospitals and 32 patients in dental clinics located in Seoul and Cheonan. The samples were subjected to a susceptibility test to evaluate the prevalence of antimicrobial resistance, and the pathogenic factors and antimicrobial resistance factors in the DNA of S. aureus were analyzed using polymerase chain reaction. Results: A susceptibility test against 15 antimicrobial agents showed that 88% of cultures were resistant to ampicillin, 88% to penicillin, and 2% to oxacillin. Resistance to at least two drugs was observed in 90% of cultures, and the most common pattern of multidrug resistance was to ampicillin and penicillin. Enterotoxins were detected in 65.9% of samples. The cell hemolysin gene hld was detected in 100% of cultures and hla was detected in 97.6% of samples. All strains resistant to penicillin and ampicillin had the blaZ gene. The aph(3')IIIa gene, which encodes an aminoglycoside modifying enzyme, was detected in 46.3% of samples. Conclusions: In the treatment of oral S. aureus infections, it is important to identify the pathogenic genes and the extent of antimicrobial resistance. Furthermore, it is necessary to study patterns of antimicrobial resistance and cross-infection in the context of periodontological specialties in which antimicrobials are frequently used, such as maxillofacial surgery, where the frequency of antimicrobial use for minor procedures such as implant placement is increasing.

  • The purpose of this study was to examine the effects on the effects of science lessons using six thinking hats techniques on creativity and science academic achievement. To verify the research problem, the subject of this study was fourth-grade students selected from four classes of an elementary school located in Busan. For ten weeks, the experimental group of 40 students was taught using the six thinking hats techniques. The comparative group also of 40 students, was taught in normal classes which used a Text-book. Children were given a creativity test and academic achievement test in science to measure the effects on six thinking hats techniques. Likert scales were used to gather students feedbacks on creativity and science academic achievement. Through these procedures, the following results were obtained: First, the six thinking hats techniques had a more effect on creativity than the normal classes, where a text-book was used. Second, the six thinking hats techniques could be seen to be effective in improving the student's academic achievements in science than the normal classes which used a Text-book. Third, the result of the student recognition investigation, we could know that the students showed lots of interest in the science lessons using six thinking hats techniques, and they were able to understand the scientific theories. As a result, the elementary science class with six thinking hats techniques had the effects of developing creativity and science academic achievement. It means the science class with six thinking hats techniques has potential possibilities and value to develop creativity and science academic achievement.

A Comparative Study of the Nurse Licensure Exam Systems between the United States and South Korea (미국과 한국의 간호사 국가면허시험 문항개발과정 비교)

  • Purpose: This study was conducted to compare the nurse licensure examination system in Korea with that of the United States (US). Methods: Focus interviews with the administrative staff of the National Council of State Boards of Nursing (NCSBN) were held. two areas were explored in the interviews: the item development process and the general administrative operating procedures. Results: NCLEX-RN items are designed for entry-level practice. The items are subjected to a series of 12 steps including a item writing panel, a content and editorial review, a sensitivity panel, a pre-test, a Differential item functioning panel and a committee review. NCSBN is responsible for the test for registered nurses and one for practical nurses. In contrast the Korean National Health Personnel Licensing Board (KNHPLB) is responsible for nursing is and 21 other health related licensure. Another difference is that in Korea there is no consistent and specialized staff to develop question items. Items are developed by educators who are not active nurses (this last statemtne is not a finding but a point of view usually not given in findings.). Conclusion: Korean nurses form the largest group of health related job categories and the most direct to health care consumers. Therefore, the nursing licensure exam should be acknowledged as the most influential licensure exam in health care services. We recommend a nursing specialized licensing institution to be established with active nurse' participation in item development process to reflect clinical practice into licensure exam.

Development and Application of an Online Scoring System for Constructed Response Items (서답형 문항 온라인 채점 시스템의 개발과 적용)

  • In high-stakes tests for large groups, the efficiency with which students' responses are distributed to raters and how systematic scoring procedures are managed is important to the overall success of the testing program. In the scoring of constructed response items, it is important to understand whether the raters themselves are making consistent judgments on the responses, and whether these judgments are similar across all raters in order to establish measures of rater reliability. The purpose of this study was to design, develop and carry out a pilot test of an online scoring system for constructed response items administered in a paper-and-pencil test to large groups, and to verify the system's reliability. In this study, we show that this online system provided information on the scoring process of individual raters, including intra-rater and inter-rater consistency, compared to conventional scoring methods. We found this system to be especially effective for obtaining reliable and valid scores for constructed response items.
