A Comparative Analysis on Korean Medical and Western Medical Service Usage Tendency of Rotator Cuff Surgery Patients - Using HIRA's Patients Sample Data

  • Objectives: To lay the foundation for future research into Korean Medicine treatment for Rotator Cuff repair surgery patients by analyzing Korean Medical and Western Medical service utilization and treatment duration. Methods: Data sampling was performed on 2015's HIRA patient data (confidence level of 97%) to analyze patients' Korean Medical and Western Medical service usage tendency. Sampled groups were divided into two groups: i) Patients who completed their treatment within five months after the rotator cuff surgery (termination group), ii) Patients who were treated for more than five months after the surgery (continuation group). Then the patients' Korean Medical and Western Medical service usage tendency was investigated and information of these patients are arranged. Results: Out of 1,453,486 patients who were gathered for sampling, 2,461 patients in total had gone through rotator cuff repair surgery. The termination group had 517 patients and the continuation group had 541 patients. The proportion of patients who visited a Korean Medicine clinic was lower in the termination group than the continuation group. Conclusion: The continuation group received more treatments (both in Western Medicine and Korean Medicine) and spent more on medical expenses compared to the termination group. Further research is highly recommended for more efficient Western Medicine and Korean Medicine treatments and reduced medical expenditure.

AN EXPERIMENTAL STUDY ON BOND STRENGTH OF COMPOSITE RESIN TO BLEACHED ENAMEL (표백된 법랑질에 대한 복합레진의 결합강도에 관한 연구)

  • The purpose of this study was to examine the shear bond strength of resin-enamel bond formed at specific time intervals after the termination ov vital bleaching. A total of 72 human extracted maxillary premolars were divided into nine groups : untreated control (group 1) ; enamel treated with 35% hydrogen peroxide(group 2, 3, 4, 5) ; and enamel reated with 15% carbamide peroxide gel (group 6, 7, 8, 9). After the treatment with 35% hydrogen peroxide for 2 hours and 15% carbamide peroxide for 24 hours, adhesion of a resin to bleached enamel was formed at 1 hour (group 2, 6) and 24 hours(group 3, 7) ; 3days(group 4, 8) and 7 days(group 5, 9) post-termination of bleaching treatment. A $3{\times}3mm$ mold was filled with Scotchbond Multi-Purpose and Z100. After 24 hours later, the specimens were shear-tested at crosshead speed 1mm/min and analyzed statistically. Fractured specimens from group 1,2, 6 were gold-coated with Eiko ion coater and observed under Scanning electron microscope at 25KV. The following results results were obtained : 1. Bonds formed at 1 hour post-termination of 35 % hydrogen peroxide(P<0.01) and 15 % carbamide peroxide bleaching treatment groups(P<0.05) showed significantly lower shear bond strength than untreated group. 2. Bonds formed at 24 hours, 3 days and 7 days post-termination of 35% hydrogen peroxide and 15 % carbamide peroxide bleaching treatment groups showed no significant differences in shear bond strength with untreated group(p>0.05). 3. SEM examinations of the untreated fracture specimen indicated cohesive fracture within enamel and exposed enamel prisms, but the bleached fracture specimens indicated adhesive fracture.

  • Parkinson's disease is a degenerative disease of a cranial nerve and has a main symptoms of irregular movement of muscle, stiffening, trembling which occurred by about 1% of population in the age of over 65. Moreover, the and prevalence rate and attack rate are soaring according to increase of elderly population. However, allopathy and surgery were done through dopamine and anticholinergic medicine for treatment but it developed a lot of complications due to medicine and progress since it makes slow progress or can't stop the treatment. Hereupon, I report that there is a case on one of the patients Young Jin Oriental Medical Clinic. The patient who is in state of treatment termination who doesn't need any further remedy and no worsening of symptoms after conduction of therapies of dialectic and 5 upright life cure regulations. 5 upright life cure regulations means five practive way for improvement of nature healing power. Upon undertaking the 5 upright life cure regulations, there were found significant results in such tests as Unified Parkinson Disease Rating Scale (UPDRS), Hoehn & Yahr Staging Scale, and Activity of Daily Living (ADL), and the ingestion of Benztropine 1mg and Requip 0.25mg was decreased from 3 times to 0 times. The study offers objective clinical data on Oriental Medicine treatment for Parkinson's disease which is one of representative neuro-degenerative diseases and thus broadens the application range of Oriental Medical treatment and presents the fundamental data on the clinical research on Parkinson's disease by adopting evidence-based medicine (EBM).

Characteristics of Initiation and Termination of Tibialis Anterior Contraction in Adults With Hemiplegia: A Preliminary Study

  • The purpose of this study was to investigate the relationship between delays in initiation and termination of tibialis anterior contraction through surface electromyographic (sEMG) analysis in adults with hemiplegia and healthy subjects and clinical assessment of lower-limb mobility. EMG activity of 6 long-term survivors of stroke and 5 healthy subjects was recorded during maximal isometric ankle dorsiflexion in 3 seconds beeper signals. It must be done as fast and forcefully as possible. Lower limb mobility was assessed with Modified Emory Functional Ambulation Profile (mEFAP). Delay in initiation and termination of muscle contraction was significantly prolonged in the affected lower limb relative to the unaffected limb. Termination of muscle contraction in the hemiplegic lower limb was significantly delayed than the initiation on the affected sides. Delay in initiation and termination of muscle contraction correlated significantly with a few range of mEFAP. Abnormally delayed initiation and termination of muscle contraction may contribute to hemiparetic lower limb mobility in hemiparetic patients. Consequently, this study showed that abnormal delay of initiation and termination of muscle contraction may contribute to hemiparetic lower limb mobility in adults with hemiplegia. Further studies are needed to demonstrate a treatment effect.

Effects of Spiral Taping in Motor Disturbance of the Neck Induced by Cervical Sprain -Randomized Clinical Controlled Trials-

  • Objectives : This study was conducted for objective verification of effects of spiral taping therapy for the patients with motor disturbance. Methods : To verify whether spiral taping therapy is effective for treating motor disturbance from cervical sprain, 28 patients were randomly allocated into the control and experiment groups. Then 23 subjects who fulfilled the experiment requirements were measured for lateral rotation angle using the goniometer. Changes in rotation were observed and compared. Control group received acupuncture and herbal acupuncture treatment, whileas the experiment group received spiral taping therapy in addition. Results : Differences in age and the degree of motor disturbance were disregarded in comparison of the groups prior to rendering treatments. For the control group, significant changes were observed after the second treatment until the termination of treatment. For the experiment group, significant changes were observed after the first treatment until the termination of treatment. Difference between the groups was insignificant but experiment group with spiral taping therapy showed better results. Conclusion : Spiral taping therapy can be an effective complementary treatment method for treating neck motor disturbance. Further studies in the subject should be conducted to yield more concrete verification.

  • The purpose of termination for high voltage tests of XLPE power cable is to prevent flashover during the breakdown test of specimen as well as to withstand the specified voltage between its conductor and screen without failure. For easier treatment and simpler construction of testing termination, resistivity grading method using semi-conductive material was employed. Based on the fundamental theory, its failure characteristics by changing the resistivity of semi-conductive material on the insulation surface was investigated. With two layers construction by difference resistivity on their surfaces, much improved result than that of one resistivity was obtained through the experiment for MV cable.

The effects of PPARβ/δ overexpression on PGC-1α mRNA and protein stability after accute endurance exercise in mice skeletal muscle (생쥐의 골격근에 PPARβ/δ 과발현이 1회 지구성 운동 후 안정시 PGC-1α mRNA와 단백질 안정성에 미치는 영향)

  • The purpose of this study is to identify the effects of PPARβ/δ over-expression on PGC-1α mRNA and protein stability after single bout of swimming exercise in mice skeletal muscle. Empty vector (EV) or PPARβ/δ was over-expressed in tibialis anterior(TA) using electroporation(EPO) technique to compare with non-treatment muscle(control; Con). TA muscles were dissected at 0h, 24h or 54h after termination of exercise. PGC-1α mRNA in Con, EV and PPARβ/δ over-expressed muscles were increased 6.8 fold (p<.001), 6.2 fold(p<.001) and 7.1 fold(p<.001), respectively, than sedentary(Sed) group at 0h after exercise and then reverted to Sed group levels at 24h and 54h after termination of exercise. PGC-1α and PGC-1α ubiquitination in EV treated muscles were increased 2.2 fold and 1.74 fold, respectively, than Sed group at 24h after termination of exercise, and then reverted to Sed group levels at 54h after termination of exercise. PGC-1α in PPARβ/δ over-expressed muscles at 24h and 54h after termination of exercise were increased 2.5 fold and 2.2 fold, respectively, than Sed group, but PGC-1α ubiquitination was not increased at 24h and 54h after termination of exercise. Our results indicate that PPARβ/δ over-expression does not increase PGC-1α mRNA stability, but increase PGC-1α protein stability through post-translation mechanism after termination of exercise.

Efficacy of Low Frequency Stimulator in Patients with Frozen Shoulder (오십견 환자의 치료에 있어서 저주파자극기의 효과)

  • Background: Frozen shoulder is not an uncommon disease, which is associated with chronic pain and joint movement limitation. However, there are numerous devices to assist in the treatment of shoulder pain, but their efficacy has not been proven and their use remains immensely controversial. Therefore, a randomized clinical study was conducted to determine the effectiveness of a low-frequency stimulator for the treatment of frozen shoulder. Methods: A randomized clinical trial was carried out on 40 patients with frozen shoulder, with 40 patients assigned to two groups; a control treatment group (group C, n = 20) and a low frequency stimulator application group (group T, n = 20). Both groups were given a routine treatment modality, such as trigger point injection, intramuscular stimulation or suprascapular nerve block etc. The level of the shoulder pain was evaluated using a 100mm VAS (visual analog scale) at each visit, with the limitation in the range of motion simultaneously evaluated. Results: All the subjects improved after treatment, with the VAS scores after termination of treatment showed a statistically significant reduction (P < 0.05). However, there was no significant difference between the two groups. One month after termination of 5 cycles of treatment, group T maintained their improved state, whereas the pain in some of those in group C reemerged, which also showed a statistically significant difference (P < 0.05). The limitation in the range of motion improved, with most subjects able to resume daily activity. Conclusions: Although the low frequency stimulation provided no more pain relief than routine treatment, the effect was significantly prolonged. From this result, low frequency stimulation can be considered to aide the therapeutic effect of classical frozen shoulder therapy.

Characteristics of Initiation and Termination of Muscle Contraction in Early Hemiparetic Wrists: Analysis of Median Frequency (초기 편마비 환자에서 손목 근수축 개시 및 종료의 특성: 중앙주파수 분석)

  • The purposes of this study were to investigate the median frequency (MDF) between initiation and termination of muscle contraction through surface electromyographic (sEMG) analysis and to propose the basis of clinical treatment for movement problems in early hemiparetic upper limbs. Thirteen patients who had stroke with onset less than 3 months prior to the study and seven control subjects participated in the study. The median frequency in initiation and termination of muscle contraction was recorded from wrist flexor and extensor muscles using the sEMG, with 3 second beeper signals, during maximal isometric wrist flexion and extension. Flexion and extension must be done as quickly and forcefully as possible. The results of the study were as follows: 1. The MDF of the onset and offset sections were significantly lower on the paretic than the nonparetic and control sides. 2. The MDF of the offset section significantly decreased on the paretic and nonparetic sides. Consequently, this study showed that the lowering of the MDF was due to the hemiparetic wrist motor impairment and muscle weakness. These results are also related to Fugl-Meyer motor assessment (FMA) scores in hemiparetic upper limbs. This study also suggests that since muscle weakness of early stroke patients affects the functional decrease of upper limbs, further studies must focus on the treatment to improve muscle agility and muscle fiber recruitment efficiency that can induce the functional recovery correlated to motor control.

Coronary Artery Fistula, associated with Patent Ductus Arteriosus (동맥관개존증을 동반한 관상동맥루 1례 치험)

  • Coronary artery fistula is an uncommon congenital heart defect that is readily amenable to surgical treatment. This fistula usually originates from the right coronary artery, but may arise from the left coronary artery, both coronary arteries, or single coronary artery. And the fistulous communication is most often to right ventricle, right atrium or pulmonary artery. Recently we experienced one case of congenital coronary artery fistula which was associated with patent ductus arteriosus. The fistulous communication, forming aneurysmal dilatation, was noted between the left anterior descending coronary artery and the right ventricular outflow tract. Cardiopulmonary bypass was employed in this case. After an arteriotomy was made on the aneurysmal coronary artery, both the proximal opening and the termination site of the fistulous tract were directly closed with partial aneurysmorrhaphy. The right ventricular chamber was also opened to evaluate the fistulous termination site. Postoperative hospital course of the patient was uneventful and she was discharged without problems.
