This study was designed to evaluate the diagnostic effect of the simulatneous multifilm individualized lateral tomography in the diagnosis of the temporomandibular disorders. The subjects consisted of 29 patients with symptoms of the temporomandibular disorders. The panoramic view, oblique lateral transcranial radiograph (OLTC) (Hirozontal angulation 0°, Vertical angulation 29°), submentovertex view, and simultaneous multifilm individualized lateral tomographs (SMFI) in centric occlusion (2.5㎜ thickness difference, 5 layers) were taken for the patients. This study compared the findings from each radiographs in the determining of mandibular condylar position with dual linear measurement of the subjective closest posterior and subjective closest anterior interarticular space and in the determining of bony changes on the studied 30 temporomandibular joints (TMJ) with symptoms of the temporomandibular disorders. The results were as follows: 1. The distribution of condylar position of OLTC and 5 layers of SMFI depended on the radiographs (p<0.05). The condylar position and the distribution of condylar position of OLTC were more similar to lateral sections of the SMFI than mesial sections, and in the distribution of the condylar position of SMFI, the more lateral sections of SMFI, the more concentric 2. positions. There were 10 cases in which all layers showed the same condylar position as that of OLTC. There were 3 cases in which no layers showed the same condylar position as that of OLTC. 3. In the SMFI of 30 Temporomandibular joints studied, there is 13 cases in which all five layers represented the same condylar position in the same TMJ and 11 cases in which 4 layers represented the same condylar position in the same TMJ and 6 cases in which 3 layers represented the same condylar position in the same TMJ. So at least 3 layers of SMFI represented same condylar position in the same TMJ. 4. The bony changes were not detected in conventional radiographs on the temporomandibular joint and the bony changes were not detected in simultaneous multifilm individualized lateral tomographs. SMFI provided a meams for a three dimensional visualization of the shape, the position and the extent of bony changes of TMJ.
Synovial chondromatosis is an uncommon disorder characterized by metaplastic formation of multiple cartilaginous and osteocartilaginous nodules within connective tissue of the synovial membrane of joints. Osteochondroma is a benign lesion of osseous and cartilagenous origin. It is frequently found in the general skeleton, but is rare in the mandibular condyle. We experienced 2 patients with abnormal appearance of temporomandibular joint. Histologic diagnoses were not obtained, because surgery was unwarranted in view of the lack of symptoms and the benign differential diagnosis. We describes 2 cases that show the characteristics of both disease simultaneously.
Authors studied on the T.M.J. dislocation of 5 cases who were visit Dental College Infirmary, S.N.U. The results were obtained as follows. 1) The main symptoms are pain, cracking sound, discomfort of temporomandibular joint region and swelling. 2) All cases except only one case, no specific etiological factors were detected in related to habitual custom and hoby. 3) The patients are all female aged from 19 to 24 years old.
Ankylosing spondylitis (AS) is a chronic inflammatory joint disorder characterized by sacroiliitis, spondylitis and enthesitis. Patients suffering from AS may also have extra-articular symptoms, such as uveitis, bowl disease, heart, lung, skin, bone and kidney involvement, but vary widely in severity and prevalence. Facial manifestation of AS include eye involvement and temporomandibular joint involvement. In this study, a case of an AS that mimicked dental pain was presented.
Arthrocentesis is a simple, less invasive, inexpensive, and highly efficient procedure for closed lock of the temporomandibular joint with regard to the lack of recurrence of symptoms during extended periods of follow-up and significiant improvement in pain and jaw function. It can be performed under local anesthesia. Arthrocentesis closed lock provide sustained normal joint function and marked pain relief. This study is the clinical outcome of arthrocentesis for closed lock of the temporomandibular joint. 160 patients(169 joints) who had experienced sudden-onset, persistent limited mouth opening were the subjects of this study. Arthrocentesis of the upper compartment of the affected TMJ was performed using normal saline. As results, at 3-60 months postarthrocentesis maximum mouth opening(MMO) had increased from mean of 27.7mm to 43.5mm, contralateral movements(CLM) from mean 5.74mm to 9.55mm, midline deviation during mouth opening from mean 3.04mm to 0.69mm. In 130 cases there was a history of joint noises, in 52 cases all noise had ceased after procedures.
The foramen of Huschke (FH) or foramen tympanicum is a persistent bony defect connecting the external auditory canal (EAC) to the temporomandibular joint (TMJ). It arises from an incomplete ossification of the tympanic part of the temporal bone that persists after the age of 5. If a herniation exists in the TMJ, otological symptoms may occur. An 80-year-old female patient complained of noise in her left TMJ and otorrhea in her left ear. On her cone beam computed tomography images, there were only degenerative joint disease signs on her left mandibular condyle. However, her computed tomography images revealed that the soft tissue of the TMJ herniate into the EAC. Additional examination was planned for the further evaluation. But the patient no longer visited the hospital due to her systemic health status, and symptoms disappeared spontaneously without any treatments. Usually this type of herniation is very rare, but years of mechanical stress from mastication may result in weakening and widening of the foramen with age. Therefore, although FH is usually congenital, sometimes it may be acquired in the elderly. The treatment plan should be determined in consideration of the patient's symptom level and the patient's general health status. If the clinical symptoms are not severe, no treatment is required.
The author obtained the transcranial-oblique lateral radiograms from 78 patients(26 male, 52 female) with temporomandibular dysfunction problem. And then, the author analyzed the dimensional changes of the TMJ space on centric occlusion, horizontal condylar movement and antero-posterior positional relationship of condyle to the articular eminence on 2.54㎝ mouth opening with clicking, TMJ pain and mouth opening limitation repectively, which were the symptoms of the temporomandibular joint pain dysfunction problem, and compared these data with control group. The results were as follows: 1. In centric occlusion, anterior and posterior TMJ space of experimental group was slightly lesser than those of the control group, also superior TMJ space of experimental group was significantly lesser than that of the control group. (p<0.01) 2. In 2.54㎝ mouth opening, the condylar horizontal movement and the antero-posterior positional relationship to the articular eminence were significantly lesser than those of the control group. (p<0.01) 3. Examined experimental group, the degree of condylar horizontal movement of affected ide was lesser than that of the normal side in 2.54㎝ mouth opening.
Clinicians are familiar with limitation of opening mouth caused by temporomandibular disorders. Sometimes, patients also complain of difficulty in closing mouth. Difficulty in closing mouth can be caused by anterior, posterior disc displacement, and subluxation of temporomandibular joint (TMJ). In this report, we presented a patient who had a difficulty in both opening and closing mouth. The patient complained of TMJ noises and intermittent limitation of opening mouth, and inability to get teeth back into maximal intercuspal position. Magnetic resonance images revealed that the left TMJ had an anterior disc displacement with relative posterior disc displacement. We reviewed the possible causes, signs and symptoms, and treatment for difficulty in closing mouth with related literatures.

Purpose: The aim of this study is to compare the short-term effectiveness of hard and soft stabilizing appliances (H-A and S-A) in treating patients with acute temporomandibular joint (TMJ) disc displacement without reduction (DDwoR). Methods: Eighty one patients diagnosed as acute TMJ DDwoR at oral medicine of Chosun University Dental Hospital from November 2016 to September 2017 were included in this study. The subjects were divided into three groups: The control group (34 patients treated without occlusal appliance), the H-A group (31 patients treated with H-A) and the S-A group (16 patients treated with S-A). The pain intensity using Visual Analog Scale and maximum mouth opening (MMO) were evaluated every week for 8 weeks. Results: The improvement of the pain intensity and MMO in the H-A group and the S-A group compared with the control group were noted (p<0.05). Conclusions: This study suggests that hard and S-As may be equally useful in treating patients with TMJ DDwoR. S-A could be recommended to reduce the symptoms of TMJ DDwoR in short period.

Headaches are one of the most common conditions associated with temporomandibular disorder (TMD). In the present paper, we evaluated the relationship between headache and TMD, determined whether headache influences the symptoms of TMD, and reported two cases of TMD accompanied by headache. Our practical experience and a review of the literature suggested that headache increases the frequency and intensity of pain parameters, thus complicating dysfunctional diseases in both diagnostic and treatment phases. Therefore, early and multidisciplinary treatment of TMD is necessary to avoid the overlap of painful events that could result in pain chronicity.
