악교정 골성형술후 약관절 장애의 변화 (CHANGES IN TEMPORO-MANDIBULAR DISORDERS FOLLOWING THE ORTHOGNATHIC SURGERIES)

  • Sixty four patients were followed-up for over 6 months following the orthognathic surgeries and were examined for the Temporo-mandibular joint dysfunction. The results are as follows: 1. Of 64 patients, 9(14.1%) had been suffered from TMD preoperatively : 4.7% in the prognathism and 46.2% in the laterognathism. 2. Following the orthognathic surgeries, 7 of 64 patients(10.9%) complained TMD, and 55.6% of the TMD patients resulted in significant improvement. 3. By the type of deformities, laterognathism with TMD could be expected to be improved in 30.8%, while 7.7% in the prognathism. 4. Non-rigid fixation resulted in higher incidence of TMD than the Rigid fixation after the SSRO ; 4 of 15(26.5%) and 2 of 25(8.0%) respectively.

  • Anatomical anomaly of temporo-madiblar joint and its dysfunction is becoming one of the important problem in dentistry because the number of these cases are increasing rapidly. Applying 'Lateral Transcranial Technic', 'Updegrave method' and 'Denar Accuard 100' to skull and adult with normal occlusion the author obtained following results: 1. Grewcock method combied with cephalostat as one of 'Lateral Transcranial Technic' revealed clear picture but the image of condyle head was tend to incline downward. 2. Direction of central radiation in 'Updegrave method' should be 2 recommended inch upward from auditorial mearus. 3. For functional analysis and correct diagnosis 'Denar Accuard 100' should be highly recommended.

  • Osteochondroma is a benign neoplasm, osseous projection surrounded with cartilage, 35.8% of benign osseous tumor, 8.5% of whole osseous tumor and usually arises from the skeletal bone. Osteochondroma is a cartilaginous derivation and relatively uncommon in the craniofacial bone. Osteochondroma of the mandible has slow growth rates which mainly affect women around forty years of age and it can appear through the coronoid process and mandibular condyle, especially in the medial half. Clinical finding associated with osteochondroma of condyle are primarily a palpable, painless temporomandibular area mass with facial asymmetry, malocclusion and midline deviations. Sometimes pain and dysfunction like trismus often accompany the anatomic derangement. Other features include malocclusion with open-bite on the affected side and cross-bite on the contralateral side. In this study, a 45-years old female patient exhibits pain on the left temporo-mandibular joint area and malocclusion due to loss of the molar region with osteochondroma on the top left of her mandibular condyle head. The patient is able to recover gradually from the symptom through treatments on manipulation, stabilization splint, arthroscopic lavage and surgical excision, thus, this is reported as a clinical case.

우리나라 성인에서 일부 질환과 연관된 건강관련 삶의 질 감소 (The Decline of Health-Related Quality of Life Associated with Some Diseases in Korean Adults)

  • Objectives: This study was conducted to measure the decline in the health-related quality of life (HRQoL) associated with some diseases in South Korean adults. Methods: The EQ-5D health states in the 2005 National Health and Nutrition Examination Survey (NHNES) and the Korean EQ-5D valuation set were used to obtain the EQ-5D indexes of the study subjects. Each disease group was defined when the subjects reported to the NHNES that they were diagnosed with the corresponding disease during the previous 1 year by physicians. Since the distributions of the EQ-5D indexes in each subgroup were negatively skewed, median regression analysis was used to estimate the effects of specific diseases on the HRQoL. Median regression analysis produced estimates that approximated the median of the EQ-5D indexes and there are more robust for analyzing data with many outliers. Results: A total of 16,692 subjects (6,667 patients and 10,025 people without any disease) were included in the analysis. As a result of the median regression analysis, stroke had the strongest impact on the HRQoL for both males and females, followed by osteoporosis, osteoarthritis, rheumatic arthritis, and herniation of an intervertebral disc. While asthma had a significant impact on the HRQoL only in men, cataract, temporo-mandibular dysfunction, and peptic ulcer significantly affected the HRQoL only in women. Conclusions: Stroke and musculoskeletal diseases were associated with the largest losses of the HRQoL in Korean adults.
