This investigation was designed to determine the effectiveness of the posterior occlusal schemes on masticatory activity during mastication in complete denture. Twelve edentulous subjects were selected for this study. All subjects had no past history and no functional abnormality on masticatory system and TMjoint. And, they had residual ridge of favorable morphology, firm mucosa and Class I skeletal jaw relationship, Twelve experimental denture with interchangeable occlusions(0-degree teeth, 30-degree teeth, Levin teeth and S-A teeth) were constructed for this study. The masticatory performance was analyzed by means of standard sieve(10, 16, 20, 30sieve), and the electrical activity from selected muscles(Temporalis and Masseter muscle) was recorded simultaneously with electromyography (Bio-Pak system) as the subject masticated test foods (rice, peanut and gum) with four different occlusal schemes. Mandibular movement was, also, measured with Sirognathography(Bio-Pak system). These recordings were performed in immediately, after 1 week and after 2 weeks of insertion of complete denture. The results were as fellows; 1. The average masticatory performance of 0-degree artificial teeth was higher than any other artificial teeth. 2. Masticatory performance in denture wearer was affected preferentially by food and artificial occlusal schemes. 3. During chewing, there was a statistical difference of EMG activity between masseter and temporal muscle(p<0.01). Especially, EMG activity of working masticatory muscle was highly affected by food rather than by artificial occlusal schemes. 4. In denture wearer, the velocity of opening was not affected by food, whereas, the velocity of closing was faster in soft food chewing than in hard food chewing, and the amount of vertical displacement was grater in chewing of soft and large bolus than in chewing of hard and small bolus. However, the amount of lateral displacement showed conversely(p<0.05). 5. It was considered that masticatory performance in denture wearer is not affected by the condition of residual ridge. the history of denture wear, the preference, the adaptation to artificial teeth and the total mesiodistal length of artificial posterior teeth.

Objective: To evaluate the short-term changes in masticatory muscle activity and mandibular movement patterns after orthognathic surgery in skeletal Class III patients with facial asymmetry. Methods: Twenty-seven skeletal Class III adult patients were divided into two groups based on the degree of facial asymmetry: the experimental group (n = 17 [11 male and 6 female]; menton deviation ${\geq}4mm$) and control group (n = 10 [4 male and 6 female]; menton deviation < 1.6 mm). Cephalography, electromyography (EMG) for the anterior temporalis (TA) and masseter muscles (MM), and mandibular movement (range of motion [ROM] and average chewing pattern [ACP]) were evaluated before (T0) and 7 to 8 months (T1) after the surgery. Results: There were no significant postoperative changes in the EMG potentials of the TA and MM in both groups, except in the anterior cotton roll biting test, in which the masticatory muscle activity had changed into an MM-dominant pattern postoperatively in both groups. In the experimental group, the amount of maximum opening, protrusion, and lateral excursion to the non-deviated side were significantly decreased. The turning point tended to be shorter and significantly moved medially during chewing in the non-deviated side in the experimental group. Conclusions: In skeletal Class III patients with facial asymmetry, the EMG activity characteristics recovered to presurgical levels within 7 to 8 months after the surgery. Correction of the asymmetry caused limitation in jaw movement in terms of both ROM and ACP on the non-deviated side.
This study was undertaken to investigate the correlations bite force and the electromyographic activities of masticatory muscle in deepbite, using the T-Scan system and electromyograph. The subjects of this study consisted of two groups ; one of 20 individuals with normal occlusion, the other group of 30 with deepbite. The deepbite was composed of Class I deepbite(male 9, female 7) and Clas II div. 1 deepbite(male 8, female 6). The obtained results of this study were as follows : 1. The maximum bite force was 155.93 N in normal occlusion, 165.11 N in Class I deepbite group, 111.55 N in Class II div. 1 deepbite group. 2. The greater !he number of tooth contacts, the more the bite force increased in all groups. 3. During maximum clenching, masseter and ant. temporailsmuscle activity of normal and Class I deepbite group were significantly higher than that of Class II div. 1 deepbite group, and the activity of masseter muscle was higher than that of ant. temporalis muscle in all groups. 4. The greater the maximum bite force, the more the muscle activities increased in all groups.
This case report describes the symptoms and clinical course of a 35-year-old female patient who was diagnosed with a temporo-sphenoidal encephalocele. It is characterized by herniation of cerebral tissue of the temporal lobe through a defect of the skull base localized in the middle fossa. At the time of first presentation the patient complained about recurrent nasal discharge of clear fluid which had begun some weeks earlier. She also reported that three months earlier she had for the first time suffered from a generalized seizure. In a first therapeutic attempt an endoscopic endonasal approach to the sphenoid sinus was performed. An attempt to randomly seal the suspicious area failed. After frontotemporal craniotomy, it was possible to localize the encephalocele and the underlying bone defect. The herniated brain tissue was resected and the dural defect was closed with fascia of the temporalis muscle. In summary, the combination of recurrent rhinorrhea and a first-time seizure should alert specialists of otolaryngology, neurology and neurosurgery of a temporo-sphenoidal encephalocele as a possible cause. Treatment is likely to require a neurosurgical approach.
The purpose of this study was to evaluate the therapeutic effect of occlusal stabilization splint on the clinical symptoms and the condylar movement in patients with Temporomandibular Disorders(TMD). For the study, 15 TMD patients treated with stabilization splint and followed up were selected. The age of them was from 18 to 65 years and the mean period of treatment was 2.9 months. The author examined signs and symptoms of TMD according to Dr. Friction's evaluation from and recorded the condylar paths with Denar pantronic before and after splint therapy. The obtained results were as follows : 1. On the first visit, 11 patients(73.3%) showed muscle tenderness on palpation and the frequency was lateral pterygoid, masseter, medial pterygoid, temporalis, sternocleidomastoideus in the order named. 2. Occlusal stabilization splint was more effective in pain relief(100%) than in other dysfunction improvement(85.7%) 3. The amount of maximum opening increased from 37.1㎜ to 42.2㎜, but those of protrusion and laterotrusion changed little. 4. Pan. PRI scores decreased from 32.9 to 21.8, which meant improved reproducibility of mandibular border movements, and the group with sever dysfunction category showed more decrease in score than the group with moderate or slight dysfunction category.
Temporal hollowing is a common complication that occurs after coronal approach surgeries. However, temporal hollowing without previous nerve damage or trauma history is rare. Herein, we present a patient with cryptogenic temporal hollowing. A 22-year-old man without any history of craniofacial interventions or trauma presented with temporal hallowing. Magnetic resonance imaging revealed fatty degeneration of the left temporalis muscle. Electromyography and nerve conduction study showed no signs of neurologic abnormalities. The patient received autologous fat injection of 30 mL harvested from the left thigh using the modified Coleman technique. Temporal hollowing is commonly caused by atrophy of the superficial temporal fat pad. Its incidence is reported to be as high as 6% after coronal approach operation. Augmentation using porous hydroxyapatite or titanium mesh is a treatment option. Autologous fat graft can also be an option for mild to moderate temporal hollowing. In this case, a patient with no history of trauma, surgery, or myogenic disease developed temporal hollowing. Further study of the little-known cryptogenic form of temporal hollowing is warranted.
Maxillary cancer is usually detected late, and the majority of patients have advanced($T_3\;or\;T_4$) diseases at the first diagnosis. It invades outside the maxillary antrum, superiorly the orbit, ethmoid sinus and the anterior cranial base, anteriorly the facial skin. If the cancer extends through the posterior antral wall, the pterygoid plates, pterygoid muscles and infratemporal fossa are to be involved that make the conventional maxillectomy impossible to remove all the involved structures in infratemporal fossa completely. So, more extensive surgical apprdoach is necessary. We report surgical experience using infratemporal fossa approach(lateral facial approach) in four cases of maxillary cancer and one case of hard palate cancer which extends through the posterior antral wall and involving pterygoid muscles, pterygoid plates and temporalis muscle.

Background: Galeal or temporalis muscle flaps have been traditionally used to reconstruct skull base defects after tumor removal. Unfortunately, these flaps do not provide sufficient vascularized tissue for a dural seal in extensive defects. This study describes the successful coverage of large skull base defects using anterolateral thigh (ALT) free flaps. Methods: This retrospective study included five patients who underwent skull base surgery between June 2018 and June 2021. Reconstruction was performed using an ALT free flap to cover defects that included the intracranial space and extended to the frontal sinus and cribriform plate. Results: There were no major complications, such as ascending infections or cerebrospinal leakage. Postoperative magnetic resonance imaging showed that the flaps were well-maintained in all patients. Conclusion: Successful reconstruction was performed using ALT free flaps for large anterior skull base defects. In conclusion, the ALT free flap is an effective option for preventing communication between the nasal cavity and the intracranial space.
The purpose of this study was to investigate the peak electromyographic activity(EMG) and time intervals in firing sequence of Anterior temporalis(TA) and Masseter(MM) on clenching, the number of tender points in the head and neck muscles, the occlusal contact state on clenching, and their relationship. 78 patients with Temporomandibular Disorders(TMDs) participated and were classified into articular or muscular group by clinical signs and symptoms. BioEMG$\textregistered$ of integrated masticatory function analyzer, Biopak$\textregistered$ system(Bioresearch Inc., USA), was used to measure EMG and related items, and T- Scan$\textregistered$(Tekscan, USA), computerized occlusal analyser, was used to record occlusal contact state on maximum voluntary clenching. EMG and occlusal contact were synchronously recorded and analysed with SAS Statistical program. The results of this study were as follows : 1. In total subjects, EMG of TA was lower than that of MM in articular group but in muscular group, vice versa, As a result, the ratio TA to MM was significantly different between the two groups. no significant differece. 3. The number and force of occlusal contact were more in articular group, but there was no significant difference in Total Left-Right statistics(TLR) between the two groups. 4. In unilaterally affected subjects for muscle function, no significant difference was observed between the affected side and the contralateral side in articular group, but in muscular group, the item of the number of tender points showed significant difference. However, for occlusal contact items, contact force in articular group shows significant difference between the two sides. 5. Rate of coincidence of the first firing side of TA with affected side or preferred chewing side was higher and not different between the two groups, but no significant correlation was showed between the first firing side and the first occlusal contact side.
