감염병소와 진단을 위해 여러 방사성성핵종 표지 사람비특이항체들이 임상이용되었으나, $^{123}I,\;^{99m}Tc,\;^{111}In$등 표지 핵종과 표지방법에 따른 체내동태의 차이에 대한 자료가 필요하며, 감염병소의 진단에 표준적으로 이용되어지던 $^{111}In$-oxine표지백혈구와 비교평가도 요구된다. 저자들은 $10^9$개의 포도상구균을 좌측 대퇴부에 주사하여 농양을 유발한 백서에서 $^{123}In$ 표지, iminothiolane을 이용한 $^{99m}Tc$ 표지, DTPA이용 $^{111}In$ 표지 사람비특이항체와 $^{111}In$-oxine 표지 백혈구의 체내동태 및 농양섭취율을 비교하였다. $^{123}In$-IgG는 갑상선 및 위의 방사능이 높아 체내 탈요드반응이 빠름이 시사되었으며, $^{99m}Tc$-iminothiolane IgG는 신장방사능이 높아 신장으로 IgG 또는 대사물이 배설됨을 알 수 있었다. $^{111}In$-oxine표지 백혈구는 간 및 비장의 방사능이 높았고, 혈액방사능 제거율이 가장 빨랐다. 주사 24시간 후의 농양섭취율은 $^{111}In$-DTPA IgG가 가장 높았고, 농양 대 혈액 방사능비는 $^{111}In$-oxine표지 백혈구가 가장 높았으며, $^{111}In$-DTPA IgG와 $^{99m}Tc$-iminothiolane IgG가 다음으로 비슷하였다. $^{111}In$-oxine표지 백혈구보다는 방사성핵종표지 IgG가 간편하게 이용될 수 있으며, $^{111}In$ 이 $^{99m}Tc$나 $^{123}In$보다 지연영상의 촬영에 유리함을 알 수 있었다.

The general use of a number of $^{99m}Tc-labelled$ compounds makes the need for good routine quality control procedures, especially the labelling efficiency measurements. The purpose of these study were to measure the labelling efficiency of $^{99m}Tc-labelled$ MDP, DTPA, Tin colloid, and Antimony sulfide colloid using miniaturized paper and instant thin layer chromatography. The chromatographic systems include whatman 3 MM paper and acetone, Gelman ITLC-SG and 0.9% sodium chloride. The chromatographic strips are miniaturized $(1\times10\;cm)$, marked and numbered. Labelling efficiencies of $^{99m}Tc-labelled$ DTPA and Tin colloid were above 98.0%. $^{99m}Tc-labelled$ Antimony sulfide colloid was 90.0%. And labelling efficiency of $^{99m}Tc-labelled$ MDP was 89.0% Coefficient variance of $^{99m}Tc-labelled$ MDP, DTPA, Tin colloid, and Antimony sulfide colloid were 5.14%, 2.06%, 1.82% and 4.90%, respectively. We found that this miniaturized chromatographic quality control was simple and reliable.

Objective: Pullulan derivatives (PD) can be used to make self-assembled hydrogel nanoparticles which are responsive to ionic strength. The aim of this study is to evaluate the potential of PD as a retaining carrier of radioisotope inside tumors. Materials and Methods: Four types of PD were evaluated which included pullulan acetate (PA), succinylated PA (SPA), PA-DTPA and SPA-DTPA conjugates. They were radiolabeled with Tc-99m. Labelling efficiencies were determined at 30 min, 1, 2, 4 and 12 hours after radiolabeling. CT-25 colon cancer cells were subcutaneously injected into Balb/c mice. After 2 weeks of subcutaneous injection, Tc-99m-labelled PD (Tc-99m-PD) were injected into the tumors. Whole body images of mice were obtained at 30 min, 1, 2, and 12 hr after intratumoral injection. All twenty mice were grouped into four groups by largest diameter; control A (largest diameter = 5 mm, n = 5), control B (largest diameter = 10 mm, n = 5), pullulan A (largest diameter = 5 mm, n = 5), pllulan B (largest diameter = 10 mm, n = 5). Dynamic images were obtained for 1 hour after intratumoral injection. Static images were obtained at 1 hr, 2 hr, 3 hr and 4 hr after intratumoral injection with Tc-99m pertechnetate and Tc-99m-PA. Target-to-background ratios and retention rates were calculated. Results: Labeling efficiencies of PA, SPA, PA-DTPA and SPA-DTPA were $94.5{\pm}5.9%,\;97.8{\pm}3.5%\;94.2{\pm}3.8%,\;and\;92.5{\pm}6.2%$, respectively (p>0.05). Percent retention rates (%RR) of PA and PA-DTPA were significantly higher than those of control, however, those of SP-DTPA and SPA became similar to control at 4 and 12 hr, respectively. %RR of pullulan A and pullulan B at 1, 4 and 8 hr is significantly higher than that of control (p < 0.05). However, %RR between pullulan A and pullulan B were similar. Conclusion: The lonic strength dependent PD-nanoparticles are retained in the tumor. No difference of %RR according to tumor size was noted. Therapeutic application of PD labelled with beta- or alpha- emitting radionuclides can be expected.

It is well known that Tc-99m HMPAO brain SPECT can reflect the functional lesions better than X-ray computerized tomography(CT) and magnetic resonance imaging(MRI) in the cerebral disorders. In order to evaluate the clinical utilities of Tc-99m HMPAO brain SPECT in patients with post-traumatic chronic organic mental disorder(OMD), we included 28 patients diagnosed as OMD in department of psychiatry after traumatic head injury. And we compared the results of Tc-99m HMPAO SPECT with those of MRI, EEG and MINI mental status ex amination(MMSE). The results were as follows 1) All patients diagnosed as OMD showed diffuse or focal decreased cerebral perfusion on Tc-99m HMPAO SPECT. 2) Most frequent lesion on brain Tc-99m HMPAO SPECT was decreased perfusion on both frontal lobe. And most frequent lesion on brain Tc-99m HMPAO SPECT showing normal brain MRI result was also decreased both frontal perfusion. 3) Eight of 28 patients showed focal brain MRI lesions(4 small frontal hygroma, 3 small cerebral infarction and 1 cerebellar encephalomalacia) which were not detected in brain Tc-99m HMPAO SPECT. 4) The patients showing less than 20 points on MMSE disclosed abnormal results of EEG more frequently than those disclosing more than 20 points. In conclusion, we think that Tc-99m HMPAO brain SPECT is sensitive method to detect functional lesions of the brains in patients with chronic post-traumatic organic mental disorder.

A 59-year-old man with thymic carcinoma underwent Tc-99m MIBI myocardial SPECT, and Tc-99m MIBI uptake in the mass was unexpectedly found in a planar image. SPECT images of the thorax showed an increased uptake area with central photon deficiency (a ring-like appearance), which reflected central tumor irreversible ischemia or progressing necrosis with peripheral viable tumor tissue.

Bone scintigraphy with $^{99m}Tc-methylene$ diphosphonate(MDP) performed for 20 weeks per two weeks interval respectively after fracture in seven(male 3 heads, female 4 heads) canine radius were analysed. All of bone scans performed 2 weeks to 20 weeks after fracture showed increase in generalized tracer uptake and showed localized increase in tracer uptake at the fracture site. Bone scans and ratio performed 6 weeks after fracture showed the most outstanding increase in generalized and localized tracer uptake. New bone formation had been observed from 2 weeks and they were incorporated completely on 18 weeks, they showed most activity during 6-10 weeks after fracture. It was recognized that the bone scan with $^{99m}Tc-methylene$ was quite sensitivity but low specificity on the fracture healing in canine radius.

Tc-99m HMPAO WBC scintigraphy is a highly sensitive and specific method for detection and localization of infection and abscess formation. Nevertheless, WBC accumulation in some neoplastic lesions has been reported. We experienced a case with unusual increased uptake of Tc-99m HMPAO WBC in chondrosarcoma. Although the exact cause ms unclear, the possible mechanism is discussed.

We present a case in which a patient with acute hemorrhagic gastritis demonstrated abnormal gastrointestinal accumulation of radiotracer during $^{99m}Tc$-methylene diphosphonate(MDP) skeletal scintigraphy. A hemorrhagic gastritis was subsequently demonstrated by endoscopy. The mechanism for the intestinal localization of $^{99m}Tc$-MDP in this patient is not clear, but we guess that the extravasated blood containing the radiopharmaceutical cannot recirculate and stays at the bleeding site, so we can see the intestinal activity.

Tc-99m methoxyisobutylisonitrile (MIBI) and Tl-201/technetium subtraction scintigraphy have been used for localization of abnormal parathyroid gland. The uptake mechanism of tracers has been postulated to be increased cellular density and vascularity, or dependent on the presence of mitochondria-rich cells. However, the uptake of these tracers was not specific for abnormal parathyroid gland. The author report a case of thymic carcinoma that would have been mistaken for carcinoma of parathyroid because of Tc-99m MIBI and Tl-201 uptake.
