합곡혈(合谷穴)과 족삼리혈(足三里穴) 병용자극(倂用刺戟)이 TEL에 미치는 영향 (The analgesic effect of combined electroacupuncture at Hoku (LI4) and Zusanli (ST36) using TFL)

  • Objective : Zusanli(ST36) and Hoku(Li4) are analgesic acupuncture points frequently used for acupuncture in Oriental medicine. The present study was conducted to see the antinociceptive effects produced by electroacupuncture combined two frequencies(Low, High) and two different acupuncture points(LI4, ST36) in the rat tail flick test. Method : In this study the Rats (Sprague-Dawley, 250-300g) were partially anesthetized with thiopental sodium(40mg/kg, i.p.). The basal reaction time for the tail-flick was 3${\pm}$0.5 sec. Low frequency(3Hz, 5V, biphasic) and high frequency(100Hz, 5V, biphasic) were applied to the inserted needle for the period of insertion(twenty minutes). Experimental groups are divied as follow; a) electroacupuncture stimulation groups at Hoku with or high frequency(L-EA, H-EA), b) electroacupuncture stimulation groups at Zusanli with low or high frequency(1-EA, h-EA), c) low frequency at Hoku and Zusanli(LIEA), d) low frequency at Hoku and high frequency at Zusanli(LhEA), e) high frequency at Hoku and low frequency at Zusanli(HIEA), f) high frequency at Hoku and Zusanli(HhEA) Results : The individual stimulation at either Hoku or Zusanli with low frequency has stronger and longer analgesic effect than high frequency stimulation. In addition, the combined stimulation at Hoku and Zusanli with low frequency has superior effect to individual stimulation with low frequency. LhEA and LIEA have superior effect to other stimulation groups among the combined groups. In order to determine the involvement of opioid system on the different antinociceptive effects, Naloxone, an opioid antagonist, was used in the combined groups. LIEA is the most sensitive when naloxone was administrated among study groups. HhEA is the least sensitive in the administration of naloxone. Conclusion : From results, this study confirmed that the opioid system is involved in analgesic effect of low frequency stimulation of acupuncture point, and we also can suggest the stronger analgesic effect of combining stimulation points is due to the theory of spatial summation in the nervous system.

Capsaicin에 의한 척수 수준에서의 급성 진통효과와 Norepinephrine의 변화 (Capsaicin Induces Acute Spinal Analgesia and Changes in the Spinal Norepinephrine Level)

  • Capsaicin의 중추신경계에 대한 진통효과를 척수 수준에서 규명하고, 이에 대한 norepinephrine (NE)계의 역할을 규명하기 위하여 tail flick reflex (TFR) latency time의 증가와 척수 내 NE 양의 변화를 측정하였다. 웅성의 Sprague-Dawley 백서를 pentobarbital sodium으로 마취를 하여 femoral vein cannulation과 intrathecal catheterization을 하였고, 이를 통하여 시험약제를 투여하였다. TFR은 capsaicin 또는 용매 투여전, 투여후 10분, 30분, 60분, 때로는 2시간에 측정하였다. MK-801은 capsaicin 투여전 20분에 femoral vein을 통하여 주입하였다. $35{\sim}150{\mu}g$의 capsaicin을 intrathecal로 주었을 때, TFR latency가 최고치인 7초를 넘어 정상인 2.9초에 비해 100% 이상의 증가를 보였고, 척수 내 NE은 16 ng/mg protein에서 7 ng/mg protein으로 50% 이상 감소하였다. 이와 같은 TFR latency의 증가와 NE의 감소는 전 실험기간 (capsaicin 투여후 2시간)에 걸쳐 관찰되었다. 반면, 50mg/kg의 capsaicin을 피하로 전신투여 하였을 경우에 척수 NE은 같은 변화를 보였으나 TFR latency의 차이는 없었다. 또한, 0.5 mg/kg의 MK-801를 i.v.로 전처치 하였을 때, intrathecal capsaicin에 의한 TFR latency의 증가와 척수 내 NE의 감소가 모두 억제되었다. 이상의 결과는 capsain이 excitatory aminoacid (EAA)계 - NE계 - 척수 dorsal horn으로 이루어진 동통전달의 하행성 억제계 축의 활성을 항진시켜 진통효과를 가져옴을 보여준다. 또한, capsaicin의 피하 주사시에 진통효과를 볼 수 없음을 말초 동통전달신경의 흥분에 의한 중추 동통전달계 환성의 촉진이 진통 효과를 상쇄함을 암시한다.

  • The aim of this study was to investigate the effects of TENS and cold application on secondary thermal hyperalgesia in rats induced by muscle pain. Muscle pain was induced in male Sprague-Dowley rats by intra-muscular injection of gastrocnemius with $3\%$ carrageenan. The paw withdrawal latency(PWL) and tail flick test(TFT) to heat were used to detect secodary thermal hyperalgesia induced by the muscle pain. PWL and TFT were quantified before and 4, 10, and 24 h after induction of muscle pain and after application of TENS(100Hz, $100{\mu}s$, sensory intensity) and cold($4^{\circ}C$). TENS and cold significantly reduced the PWL and TFT to heat stimuli when compared with controls receiving no TENS and cold(p<.05). These results suggested that application of TENS and cold attributed to decrease secodary thermal hyperalgesia in rat induced by muscle pain.

  • Expression of c-fos, an immediate early gene, has accepted to be a marker of functional activity in neurons. This study was aimed to investigate the effects of cold therapy on the expression of spinal cord c-fos in rats of carrageenan-induced muscle pain. Muscle pain was induced in male Sprague-Dawley rats by intra-muscular injection of gastrocnemius with $2\%$ carrageenan. The paw withdrawal latency (PWL) and tail flick test (TFT) responses to heat stimuli were used to detect secondary hyperalgesia produced by the muscle pain and measured to assess the effects of cold. The expression of c-fos was determined in the lumbar regions of the spinal cord by reverse transcription-polymerase chain reaction (RT-PCR) and immunohistochemistry assays. The secondary hyperalgesia to heat simuli (PWL and TFT) were significantly reduced in cold therapy compared with that in the controls. In RT-PCR assays the expression of c-fos mRNA was down-regulated in the lumbar spinal cord in cold group. In addition, Fos immunoreactivity in the dorsal horn of the lumbar spinal cord was decreased in cold group. These results suggested that application of cold attributed to increase PWL and TFT responses and to decrease expression of the c-fos produced by muscle pain.

U-50,488H 진통성 내성형성에 대한 인삼 사포닌의 억제효과는 Serotonin 기전에 의존적이다 (Inhibitory Effect of Ginseng Total Saponins on the DEvelopement of Tolerance to U-50,488H-Induced Antinociception is Dependent on Serotonergic Mechanisms)

  • We have previously reported that the antagonism of U-50,488H-induced antinociception in mice pretreated with ginseng total saponins (GTS) Ivas abolished by pretreatment with a serotonin precursor, 5-hydroxytryptophan (5-HTP), but not by a noradrenaline precursor, L-dihydroxyphenylalanine (L-DOPA) in the tail flick test. In the present experiments, the effect of the same GTS on the development of tolerance to U-50,488H-induced antinociception was determined. GTS inhibited the development of tolerance to U-50,488H-induced antinociception. The inhibitory effect of GTS on the development of tolerance to U-50,488H-induced antinociception was reversed by 5-HTP, but not by L-DOPA. These findings suggest that the inhibitory effect of GTS on the development of tolerance to U-50,488H-induced antinociception is dependent on serotonergic mechanisms. 리도카인을 함유하는 마이크로에멀젼 겔의 피부침투성 및 in vivo 마취효과 (Skin Penetration and in Vivo Local Anesthetic Effect of Microemulsion-based Hydrogels Containing Lidocaine)

  • Several topical preparations containing lidocaine, a widely used local anesthetic agent, have been developed and marketed recently for the treatment of premature ejaculation. In this study, microemulsion(ME)-based hydrogels containing lidocaine were prepared by dispersing ME to hydrogel bases such as Carbopol, sod. alginate, and sod. carboxymethylcellulose. Lidocaine-containing ME was thermodynamically stable over 6 months and had a diameter ranging from 10 to 100 nm. In vitro skin penetration of lidocaine from ME-based hydrogels followed apparent zero-order kinetics. ME-based hydrogel showed higher drug penetration during fifteen minutes after application than alcoholic hydrogel, reference preparation. Tail flick test in rat was introduced to compare in vivo local anesthetic effects of different hydrogels, and the results showed that ME-based hydrogels are superior to other hydrogels. In optical microscopy, recrystallization of lidocaine was observed within 5 min after application of reference hydrogel, but there was no change in ME-based hydrogels even after 30 minnute. These results indicated that ME-based hydrogels had some advantages in skin penetration, anesthetic effect and physical stability compared with alcoholic hydrogels. Finally it is possible to conclude that ME-based hydrogels containing lidocaine is a good topical drug delivery system for the treatment of premature ejaculation.

급성통증 유발 흰쥐에 대한 위령선약침(威靈仙藥鍼)의 통증 억제효과 (Analgesic Effect of Clematidis Radix (CR) Herbal-acupuncture on the Test Rats with Induced Acute Pain)

  • Objectives : The objective of this study is to investigate Anti-nociceptive and anti-inflammatory effects of Clematidis Radix (CR) herbal-acupuncture on the test rats with induced acute pain. Methods : The effects of Clematidis Radix (CR)-distillates were investigated in three types of models with three different pain. Highly purified distillate of CR called CR herbal-acupuncture was injected to Zusanli ($ST_{36}$) acupoint. In the tail flick test, the CR herbal-acupuncture treatment did not show a significant effect of relieving acute pain. To investigate the anti-inflammatory effect of CR herbal-acupuncture, the second testing model'pain was induced by injecting formalin to its planter. For the last model, carrageenan was injected into tarsal joint. the medicinal effect of CR herbal-acupuncture was evaluated through the behavioral analyses such as licking time, weight distribution ratio and ankle circumference. Results : In the formalin test, the analgesic effect of CR herbal-acupuncture was more pronounced in the late phase (for 20 min after the early phase) than in the early phase (for the first 10 min post formalin injection). It was proven by weight distribution ratio testing and ankle edema testing that herbal-acupuncture of CR inhibited arthritis caused by the carrageenan. Conclusions : These results revealed that CR herbal-acupuncture was effective to alleviate the inflammatory pain and could be used as an analgesic treatment with an anti-inflammatory effect.

마우스에서 삼색제비꽃 추출물의 진통 효과와 매커니즘 (Antinociception Effect and Mechanisms of Viola tricolor L. Extract in Mouse)

  • In the present study, the antinociceptive profiles of Viola tricolor L. (V. tricolor L.) extract were examined in ICR mice. V. tricolor L. extract administered orally (200mg/kg) showed an antinociceptive effect as measured by the tail-flick and hot-plate tests. In addition, V. tricolor L. extract attenuated the writhing numbers in the acetic acid-induced writhing test. Furthermore, the cumulative nociceptive response time for intrathecal (i.t.) injection of substance P (0.7 ${\mu}g$) was diminished by V. tricolor L. extract. Intraperitoneal (i.p.) pretreatment with yohimbine (${\alpha}_2$-adrenergic receptor antagonist) attenuated antinociceptive effect induced by V. tricolor L. extract in the writhing test. However, naloxone (opioid receptor antagonist) or methysergide (5-HT serotonergic receptor antagonist) did not affect antinociception induced by V. tricolor L. extract in the writhing test. Our results suggest that V. tricolor L. extract shows an antinociceptive property in various pain models. Furthermore, this antinociceptive effect of V. tricolor L. extract may be mediated by ${\alpha}_2$-adrenergic receptor, but not opioidergic and serotonergic receptors.

Effect of $Agrimonia$ $pilosa$ $Ledeb$ Extract on the Antinociception and Mechanisms in Mouse
