Current Status of Human Taeniasis in Lao People's Democratic Republic

  • Human taeniasis was investigated in Lao People's Democratic Republic (Lao PDR) between 2000 and 2011 as part of the nation's helminthiasis survey. A total of 55,038 inhabitants, including 29,846 school children, were examined using the Kato-Katz and scotch-tape anal swab method, and morphological observation of adult worms. Molecular identification of Taenia tapeworms was performed by multiplex PCR or DNA sequence analysis of the mitochondrial cox1 gene. Taenia eggs were present at a rate of 1.5% (845/55,038) in the subject population. Adult tapeworms were identified as T. solium or T. saginata by analyzing the collectable stool specimens (n=126). Three specimens identified as T. solium were found in Luang Prabang, while the remaining 123 specimens, which were T. saginata, were found in Bokeo, Bolikhamxay, Champasak, Houaphan, Khammouane, Luang Namta, Luang Prabang, Oudomxay, Phongsaly, Saysomboune, Saravane, Savannakhet, Xayaboury, Xekong, Xieng Khouang Province, and Vientiane Municipality.

  • Albendazole, a new broad spectrum anthelmintic drug, was evaluated for anthelmintic effects in 74 patients with single or mixed infections of Ascaris lumbricoides, Trichuris trichiura. Enterobius vermicularis, Metagonimus yokogawai, Clonorchis sinensis and Taenia spp. Albendazole was administered as a single dose of 400 mg. The cure rate for A. lumbricoides and E. vermicularis was 91.4% and 98.1% respectively at day 30 post-treatment: for T. trichiura it was 51.1% and for M. yakogawai and C. sinensis it was 33.3% and 13.3% respectively. The egg reduction rate at day 30 was 98.5% in A. lumbricoides, 76.5% for T. trichiura : 60.0% for M. yokogawai and 59.6% for C. sinensis However it was not cured in all 5 cases of taeniasis at three months follow-up examination. On the other hand, the various dosages of albendazole were given to the patients infected with C. sinensis and Taenia spp. respectively. The follow-up examinations were carried out at 30 days after treatment in clonorchiasis and at 3 months after treatment in taeniasis. In the results, the recommended dosage of albendazole for clonorchiasis is $3{\times}400mg$ for 3 consecutive days and for taeniasis $2{\times}400mg$ for 3 consecutive days. There were no side effects with the above dosages except only a mild abdominal pain and dizziness in a few patients. Albendazole appears to be more effective than the other available broad spectrum anthelmintic drugs.

Infection of Taenia asiatica in a Bai Person in Dali, China

  • We report here a human case of Taenia asiatica infection which was confirmed by genetic analyses in Dali, China. A patient was found to have symptoms of taeniasis with discharge of tapeworm proglottids. By sequencing of the mitochondrial cytochrome c oxidase subunit 1 (cox1) gene, we observed nucleotide sequence identity of 99% with T. asiatica and 96% with T. saginata. Using the cytochrome b (cytb) gene, 99% identity with T. asiatica and 96% identity with T. saginata were found. Our findings suggest that taeniasis of people in Dali, China may be mainly caused by T. asiatica.

  • This study was undertaken to evaluate the present status of parasitic infection and swine pen human latrine system in Cheju Do, from July to September 1982. The 663 stool specimens (male 323 and female 340) and 579 scotch tape anal swabs collected from 161 households of 2 areas in Cheju Do were examined. The methods employed were formalin-ether technique for the prevalence rate of various helminthic and protozoan infection, and scotch tape anal swab technique for the prevalence rates of Enterobius vermicularis. In addition to these, questionaire was used to evaluate the present status of swine pen human latrine system and prevalence rates of taeniasis in these areas. The results are as follows ; 1) Prevalence rates of parasitic infections of any kind was 33.9%. It was 35.7% in Cheju City in contrast to 32.1% in North Cheju Gun. The infection rates of Trichuris trichiura was 10.0% and it was the highest prevalence rate in this survey. The prevalence rates of the other parasites were as follows ; Ascaris lumbricoides 2,3%, Hookworm 0.2%, Clonorchis sinensis 0.5%, Hymenolepis nana 1.5%, Entamoeba coli 3.2%, and Giardia lamblia 0.5%. 2) The infection rates of Enterobius vermicularis in 579 peoples (male 285, female 294) by applying scotch tape anal swab technique was 13.1% through the survey. It was 16.9 in Cheju City and 8.5% in North Cheju Gun. 3) The Infection rate of Taenia species by applying the stool examination and making up a question was 19.2%(21.4% in Cheju City and 16.7% in North Cheju Gun). 4) Sexual distribution of the parasitic infections showed slightly higher rate in female than that of male. 5) The positive rates of parasitic infection by the stool examination and questionaire positive cases of taeniasis were higher in 0-9 and over 60 year old than any other age group. 7) The swine pen human latrine systems were used in 46 households (28.6%). 7) Relationship between swine pen human latrine system and taeniasis was not noted.

  • The present study was undertaken to evaluate the prevalence rate of Taenia species infection and distribution of taeniasis caused by Taenia solium among Koreans in Seoul and Cheju Do In Korea during the period from August to December 1984. A total of 4,256 stool specimens from the students of Mapo Ku and 360 stool specimens from the inhabitants of Guro-Ku in Seoul was collected and examined by cellophane thick smear technique. On the other hand 1,015 stool specimens from the students of Jocheon Myun and 265 specimens from the students of Jocheon Myun and 265 specimens from the inhabitants of Aeweol Myun and Gujwa Myun in Cheju Do were also examined. The results were summarized as follows ; Four (0.1%) out of 4.256 students were positive and 2 (0.6%) out of 360 inhabitants in Seoul were positive (Table 1, 2). Positive rates of taeniasis in Cheju Do were 1.7% (17) out of 1,015 students and 12.5% (33) out of 5 villagers (Table 3, 4). In order to observe the distribution of Taenia solium infection, the scolex or a part of Taenia spp. were collected from the stool of positive cases by anthelmintic treatment. For the species identification, expelled proglottides were examined microscopically by the number of branches of the uterus, presence of vaginal sphincter or the accessory ovarian lobe etc. Three cases were infected with Taenia solium among 6 egg positive cases in Seoul. But only 1 case was infected with Taenia solium out of 7 students taking anthelmintics voluntarily by recommendation of Korean Association for Parasite Eradication (KAFPE). On the other hand, among 32 cases of egg positive cases of this study and 26 cases of KAFPE in Cheju Do, 13 cases (22.4%) were infected with Taenia solium. But 13 cases who were not examined and complained expulsion of proglottides in their stool were infected with Taenia saginata (Table 5). Among 62 persons infected with Taenia saginata, only 4 cases did not know their infection of this worm. Rut in 17 cases with Taenia solium, 7 persons did not know their infection until stool examinations were performed (Table 6).

Neurocysticercosis: Clinical Characteristics and Changes from 26 Years of Experience in an University Hospital in Korea

  • The prevalence of human taeniasis has decreased in Korea. The stool egg positive proportion decreased from 1.9% in 1971 to 0% in 2004 in nationwide surveys. The neurocysticercosis (NCC) is also presumed to decrease. However, detailed information regarding the recent status of NCC in Korea is lacking. We retrospectively reviewed NCC cases from 1990 to 2016 at Asan Medical Center, a 2700-bed tertiary referral hospital in Korea. We identified patients based on clinical symptoms, brain imaging, pathology and serological assay. The cases were classified as parenchymal, extraparenchymal, and mixed NCC. Eighty-one patients were included in the analysis. The mean age was 54.5 years, and 79.0% were male. The number of NCC cases was highest from 1995 to 1999, and continuously decreased thereafter. Forty (49.4%) patients had parenchymal NCC, while 25 (30.9%) patients had extraparenchymal NCC, and 16 (19.8%) patients had mixed NCC. The seizure and headache were most common symptom of parenchymal NCC and extraparenchymal NCC respectively. Hydrocephalus was more common in extraparenchymal NCC, and patients with extraparenchymal NCC were more likely to require a ventriculoperitoneal shunt. Cases of NCC are decreasing accordingly with human taeniasis and lesion location was the most important determinant of clinical presentation and outcome of NCC in Korea.

Four Cases of Taenia saginata Infection with an Analysis of COX1 Gene

  • Human taeniases had been not uncommon in the Republic of Korea (=Korea) until the 1980s. The prevalence decreased and a national survey in 2004 revealed no Taenia egg positive cases. However, a subsequent national survey in 2012 showed 0.04% (10 cases) prevalence of Taenia spp. eggs suggesting its resurgence in Korea. We recently encountered 4 cases of Taenia saginata infection who had symptoms of taeniasis that included discharge of proglottids. We obtained several proglottids from each case. Because the morphological features of T. saginata are almost indistinguishable from those of Taenia asiatica, molecular analyses using the PCR-RFLP and DNA sequencing of the cytochrome c oxidase subunit 1 (cox1) were performed to identify the species. The PCR-RFLP patterns of all of the 4 specimens were consistent with T. saginata, and the cox1 gene sequence showed 99.8-100% identity with that of T. saginata reported previously from Korea, Japan, China, and Cambodia. All of the 4 patients had the history of travel abroad but its relation with contracting taeniasis was unclear. Our findings may suggest resurgence of T. saginata infection among people in Korea.

Ten Cases of Taenia saginata Infection Confirmed by Analysis of the Internal Transcribed Spacer 1 rDNA Region in the Republic of Korea

  • From October 2015 to August 2018, tapeworm proglottids were obtained from 10 patients who were residents of Daegu and Gyeongbuk provinces and had a history of raw beef consumption. Most of them had no overseas travel experience. The gravid proglottids obtained from the 10 cases had 15-20 lateral uterine branches. A part of internal transcribed spacer 1 (ITS1) DNA of the 10 cases, amplified by polymerase chain reaction (PCR) and digested with AleI restriction enzyme, produced the same band pattern of Taenia saginata, which differentiated from T. asiatica and T. solium. Sequences of ITS1 and cytochrome c oxidase subunit 1 (cox1) showed higher homology to T. saginata than to T. asiatica and T. solium. Collectively, these 10 cases were identified as T. saginata human infections. As taeniasis is one of the important parasitic diseases in humans, it is necessary to maintain hygienic conditions during livestock farming to avoid public health concerns.

Prevalence and Risk Factors of Intestinal Helminthiasis in Remote Mountainous Villages of Northern Lao PDR: A Cross-Sectional Study

  • Helminth infections are prevalent in Lao People's Democratic Republic (Lao PDR). This study aimed at determining the prevalence and risk factors of intestinal helminthiasis in remote mountainous villages of northern Lao PDR. During the dry season in January 2017, a cross-sectional survey was conducted in 3 remote mountainous villages in Oudomxay province, Lao PDR. Villagers older than 18 years of age who agreed to submit stool samples or undergo an interview, were recruited. Stool samples from 198 individuals were examined by the Kato-Katz method, and a questionnaire surveyed 161 individuals among them. Univariable and multivariable logistic regression analyses were used to identify risk factors associated with the intestinal helminthiasis. An overall prevalence of intestinal helminthiasis was 75.8%. Hookworm infection was the most common (63.1%), followed by Opisthorchis viverrini/minute intestinal flukes (17.7%), Taenia spp. (15.2%), Trichuris trichiura (2.0%), Ascaris lumbricoides (1.5%), and Enterobius vermicularis (1.0%). Questionnaire analysis revealed sex (male) and absence of latrine to be significant risk factors for hookworm infection and consumption of raw meat for taeniasis. These results suggest that the mountainous area in northern Lao PDR has a different composition of helminth infections from other studies conducted in Lao PDR; a high prevalence of hookworm infection and taeniasis and low prevalence of T. trichiura and A. lumbricoides infections were observed. Also, liver flukes or intestinal flukes were similarly prevalent in the mountainous area.

Human Neurocysticercosis Case and an Endemic Focus of Taenia solium in Lao PDR

  • A male patient with neurocysticercosis was identified in Montai Village, Xay District, Oudomxay Province, Lao PDR in February 2004. He had a history of diagnosis for neurocysticercosis by a CT scan in Thailand after an onset of epileptic seizure in 1993. A pig in the same district was found to contain Taenia solium metacestodes (=cysticerci); the slaughtered pig body contained more than 2,000 cysticerci. In addition to morphological identification, molecular identification was also performed on the cysticerci by DNA sequencing analysis of the mitochondrial cox1 gene; they were confirmed as T. solium metacestodes. The patient is regarded as an indigenous case of neurocysticercosis infected in an endemic focus of T. solium taeniasis/cysticercosis in Oudomxay Province, Lao PDR.
