  • Objective : The aim of this study was to examine Differences of overall characteristics depending on Symptom Severity in Acute Stroke Patient Method : We studied hospitalized patients within 10days after their ictus who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2011. We compared the general characteristics of acute stroke patient according to Scandinavian Stroke Scale score Result : The patient who had mild severity show significant difference for FBS, PP2, Homocysteine, Exercise, Tongue color, Mouth dryness, Chest discomfort, Constipation, Stool hardness. Conclusion : The above result contribute to predict severity of stroke symtoms according to risk factor and general condition of patients. Also, After occurrence of stroke, We will can block worsening of symptoms progression. Further studies will be needed to observation of follow up studies about progression of stroke among acute stroke patients with a serious disability.

Effects of Extracts of Unripe Black Raspberry and Red Ginseng on Cholesterol Synthesis (복분자 미숙과와 홍삼 추출물의 콜레스테롤 개선 효과)

  • We investigated the effects of water extracts of unripe black raspberry (UBR) and red ginseng (RG) on cholesterol synthesis, 3-hydroxy-3-methylglutaryl (HMG)-CoA reductase activity, and expression of low-density lipoprotein (LDL) or high-density lipoprotein (HDL)-related genes in HepG2 and Caco-2 (human hepatoma and intestinal cell lines, respectively). Our results showed that cholesterol synthesis and HMG-CoA reductase activity in HepG2 cells were inhibited by UBR and RG. Further, co-treatment with UBR and RG had a greater effect than did treatment with either UBR or RG. In Caco-2 cells, treatment with UBR and RG increased the expression of LDL-regulated genes, such as LDL receptor and SREBP-2, and also upregulated the level of HDL-associated ABCA1. Moreover, co-treatment with UBR and RG appeared to be more effective than treatment with either UBR or RG. Taken together, our results indicate that UBR and RG regulate the level of HDL-associated ABCA1 via signaling pathway, thereby preventing cholesterol synthesis.

  • Students obtain most concepts through textbooks, and teaching-learning activities between teachers and students. Accordingly, if science teachers already have misconceptions they will inevitably affect students' scientific concept. This study found many problems in teachers' cognition on the concepts of nuclear radiation. Because 12th grade physics II is classified as an optional subject in the 7th curriculum, teachers have few chances to teach it and, more importantly, have difficulty in teaching it because of the need to prepare students for the university entrance examination. The concept of radiation must be taught correctly because of its emergence in the 'environment' unit of 10th grade Science. Finally, results from this study can help science teachers teach these difficult concepts more correctly. In addition, results can also be useful in in-service retraining programs.

Cholesterol Improvement Effects of Co-treatment with Black Raspberry and Red Ginseng Extracts in Mice Fed a High Cholesterol Diet (고콜레스테롤 식이 투여 마우스에서 복분자 미숙과 추출물과 홍삼농축액 복합 투여의 콜레스테롤 개선 효과)

  • We examined the effects of unripe black raspberry (UBR) and red ginseng (RG) extracts on cholesterol improvement in C57BL/6J mice fed a HCD (high cholesterol diet) for 12 weeks. Hepatic total lipid and total cholesterol contents were significantly induced in hyperlipidemic mice. However, supplementation with UBR, RG and simvastatin effectively reduced these lipid profiles. Further, UBR and co-treatment with UBR and RG increased expression of LDL receptor, SREBP2, and SR-B1 mRNA compared with HCD. The ApoB/ApoA1 ratio was reduced by co-treatment with UBR and RG compared to treatment with UBR. In addition, histopathologic evaluation showed that co-treatment with UBR and RG suppressed lipid accumulation as well as FAS and leptin expression in plasma. These results indicate that co-treatment with UBR and RG may be useful for the prevention of hypercholesterolemia.

Prediction of Uniaxial Compressive Strength of Rock using Shield TBM Machine Data and Machine Learning Technique (쉴드 TBM 기계 데이터 및 머신러닝 기법을 이용한 암석의 일축압축강도 예측)

  • Uniaxial compressive strength (UCS) of rock is one of the important factors to determine the advance speed during shield TBM tunnel excavation. UCS can be obtained through the Geotechnical Data Report (GDR), and it is difficult to measure UCS for all tunneling alignment. Therefore, the purpose of this study is to predict UCS by utilizing TBM machine driving data and machine learning technique. Several machine learning techniques were compared to predict UCS, and it was confirmed the stacking model has the most successful prediction performance. TBM machine data and UCS used in the analysis were obtained from the excavation of rock strata with slurry shield TBMs. The data were divided into 8:2 for training and test and pre-processed including feature selection, scaling, and outlier removal. After completing the hyper-parameter tuning, the stacking model was evaluated with the root-mean-square error (RMSE) and the determination coefficient (R2), and it was found to be 5.556 and 0.943, respectively. Based on the results, the sacking models are considered useful in predicting rock strength with TBM excavation data.

Effects of maternal exercise on expression of GLUT-4, VAMP-2 in skeletal muscle and plasma insulin and leptin levels in pregnant rats (운동이 F344계 임신쥐에서 골격근의 VAMP-2 및 GLUT-4 단백질 발현과 혈중 인슐린, 렙틴 농도에 미치는 영향)

  • The main objective of this study was to investigate the effects of exercise on serum insulin and leptin levels and GLUT-4 and VAMP-2 in skeletal muscles from the pregnant rats. F344 rats were randomly divided into four groups (n = 15 in each group): control group (CG), pregnant group (PG), pregnant running group (PR), and pregnant swimming group (PS). From the 15th day of pregnancy, animals in the running group were forced to run on treadmill for 30 min with a light intensity, while those in the swimming group were forced to swim in swimming pool for 10 min once a day for 6 consecutive days. The present result demonstrated that in pregnant rat group, serum insulin levels significantly in-creased and leptin levels significantly decreased. Skeletal GLUT-4 and VAMP-2 protein expression was significantly decreased in pregnant rats compared to non-pregnant rats. However, matenal running during gestational period alleviated pregnancy-induced changes in plasma insulin and leptin levels, and it significantly enhanced skeletal GLUT-4 and VAMP-2 protein expression. From those results, it can be suggested that running exercise during gestational period may improve glycemic control by up-regulating GLUT-4 and VAMP-2 protein expression.

Serial Magnetic Resonance Imaging to Determine the Progression of Neglected Recalcitrant Rotator Cuff Tears: A Retrospective Multicenter Study

  • Background: To determine the natural progression of conservatively treated rotator cuff tears, we evaluated changes in radiologic and clinical parameters in patients whose recalcitrant tears were neglected after conservative treatment. Methods: A total of 73 patients with recalcitrant rotator cuff tears in spite of conservative treatment were included in this study. We measured changes in tear size and in the extent of fatty infiltration of the rotator cuff by comparing the initial and final follow-up magnetic resonance imagings (MRIs). To determine factors influencing the change in tear size, we collected the medical history of patients taken at the time of initial admission. Results: The average follow-up period was 20.1 months, and the average increase in tear size across this period was 6.2 mm. In terms of steroid injection, we found that the increases in tear size of the steroid injection group (p=0.049) and of the sub-group that had received more than three steroid injections (p=0.010) were significantly greater than that of the non-steroid injection group. Conclusions: We found that the increase in cuff tear size was on average 6.2 mm across the follow-up period, indicating that neglecting cuff tears may cause them to progress into more severe tears. We also observed that a history of steroid injection might be a possible risk factor for a worse prognosis of cuff tears. Therefore, we suggest that patients with rotator cuff tears and a history of steroid injection are recommended aggressive modes of treatment such as surgery.

  • A tuberculous pleural effusion may be a sequel to a primary infection or represent the reactivation of pulmonary tuberculosis. It is believed to result from a rupture of a subpleural caseous focus in the lung into the pleural space. It appears that delayed hypersensitivity plays a large role in the pathogenesis of a tuberculous pleural effusion. We encountered a 52 years old man with pleural effusion that developed several days after a CT guided percutaneous needle biopsy of a solitary pulmonary nodule. He was diagnosed with TB pleurisy. It is believed that his pleural effusion probably developed due to exposure of the parenchymal tuberculous focus into the pleural space during the percutaneous needle biopsy. This case might suggest one of the possible pathogeneses of tuberculous pleural effusion.

Dietary evaluation of a low-iodine diet in Korean thyroid cancer patients preparing for radioactive iodine therapy in an iodine-rich region

  • BACKGROUND/OBJECTIVES: Despite the importance of a low-iodine diet (LID) for thyroid cancer patients preparing for radioactive iodine (RAI) therapy, few studies have evaluated dietary intake during LID. This study evaluated the amount of dietary iodine intake and its major food sources during a typical diet and during LID periods for thyroid cancer patients preparing for RAI therapy, and examined how the type of nutrition education of LID affects iodine intake. SUBJECTS/METHODS: A total of 92 differentiated thyroid cancer patients with total thyroidectomy were enrolled from Seoul National University Hospital. All subjects completed three days of dietary records during usual and low-iodine diets before $^{131}I$ administration. RESULTS: The median iodine intake was $290{\mu}g/day$ on the usual diet and $63.2{\mu}g/day$ on the LID. The major food groups during the usual diet were seaweed, salted vegetables, fish, milk, and dairy products and the consumption of these foods decreased significantly during LID. The mean energy intake on the LID was 1,325 kcal, which was 446 kcal lower than on the usual diet (1,771 kcal). By avoiding iodine, the intake of most other nutrients, including sodium, was significantly reduced during LID (P < 0.005). Regarding nutritional education, intensive education was more effective than a simple education at reducing iodine intake. CONCLUSION: Iodine intake for thyroid cancer patients was significantly reduced during LID and was within the recommended amount. However, the intake of most other nutrients and calories was also reduced. Future studies are needed to develop a practical dietary protocol for a LID in Korean patients.

Temozolomide Salvage Chemotherapy for Recurrent Anaplastic Oligodendroglioma and Oligo-Astrocytoma

  • Objective : To evaluate the efficacy of temozolomide (TMZ) chemotherapy for recurrent anaplastic oligodendroglioma (AO) and anaplastic oligoastrocytoma (AOA). Methods : A multi-center retrospective trial enrolled seventy-two patients with histologically proven AO/AOA who underwent TMZ chemotherapy for their recurrent tumors from 2006 to 2010. TMZ was administered orally (150 to 200 $mg/m^2/day$) for 5 days per 28 days until unacceptable toxicity occurred or tumor progression was observed. Results : TMZ chemotherapy cycles administered was median 5.3 (range, 1-41). The objective response rate was 24% including 8 cases (11%) of complete response and another 23 patients (32%) were remained as stable disease. Severe side effects (${\geq}$grade 3) occurred only in 9 patients (13%). Progression-free survival (PFS) of all patients was a median 8.0 months (95% confidence interval, 6.0-10.0). The time to recurrence of a year or after was a favorable prognostic factor for PFS (p<0.05). Overall survival (OS) was apparently differed by the patient's histology, as AOA patients survived a median OS of 18.0 months while AO patients did not reach median OS at median follow-up of 11.5 months (range 2.7-65 months). Good performance status of Eastern Cooperative Oncology Group 0 and 1 showed prolonged OS (p<0.01). Conclusion : For recurrent AO/AOA after surgery followed by radiation therapy, TMZ could be recommended as a salvage therapy at the estimated efficacy equal to procarbazine, lomustine, and vincristine (PCV) chemotherapy at first relapse. For patients previously treated with PCV, TMZ is a favorable therapeutic option as 2nd line salvage chemotherapy with an acceptable toxicity rate.
