Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration in the Diagnosis of Lymphoma

  • Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is highly accurate in diagnosing mediastinal lymphadenopathies of lung cancer and benign disorders. However, the utility of EBUS-TBNA in the diagnosis of mediastinal lymphomas is unclear. The aim of this study was to determine the diagnostic value of EBUS-TBNA in patients with suspected lymphoma. Materials and Methods: Sixty-eight patients with isolated mediastinal lymphadenopathy and suspected of lymphoma were included in the study. EBUS-TBNA was performed on outpatients under moderate sedation. The sensitivity, specificity, negative predictive value and diagnostic accuracy of EBUS-TBNA were calculated. Results: Sixty-four patients were diagnosed by EBUS-TBNA, but four patients with non-diagnostic EBUS-TBNA required surgical procedures. Thirty-five (51.5%) patients had sarcoidosis, six (8.8%) had reactive lymphadenopathy, nine (13.3%) had tuberculosis, one (1.5%) had squamous cell carcinoma, two (2.9%) had sarcoma and fifteen (22%) had lymphoma (follicular center cell, large B-cell primary, and Hodgkin lymphomas in three, two, and ten, respectively). Of the 15 lymphoma patients, thirteen were diagnosed by EBUS and two by thoracotomy and mediastinoscopy. The sensitivity, specificity, negative predictive value, and diagnostic accuracy of EBUS-TBNA for the diagnosis of lymphoma were calculated as 86.7%, 100%, 96.4%, and 97%, respectively. Conclusions: EBUS-TBNA can be employed in the diagnosis of mediastinal lymphoma, instead of more invasive surgical procedures.

  • 연구배경 : 폐암환자에서 완치 목적의 수술이나 방사선 치료에 명백한 금기증이 보이지 않으면 폐종격동에 대한 평가를 해서 해부학적 병기를 명확히 할 필요가 있는데, 기관지 내시경 검사 도중에 쉽게 시행할 수 있는 경기관지 침흡인 세포검사(Transbronchial Needle Aspiration cytology, TBNA)로 기관 분기부하 림프절 전이여부를 검색해서 양성인 경우는 외과적 병기 결정 방법을 피할 수 있는 보조적인 방법으로 사용할 수 있을 것으로 알려져 있다. 저자들은 TBNA의 임상적 가치를 평가하여 보기 위하여 본 연구를 하였다. 방법 : 폐암이 의심되는 환자에서 진단 목적의 기관지 내시경 검사중에 기관 분기부하 림프절의 TBNA를 동시에 시행하였으며, 이중 병리조직학적으로 폐암으로 확진된 42예에서 TBNA의 성적을 분석하였다. 결과: 1) 폐암으로 확진된 42예중 34예 (81.1%)에서 적절한 검체를 얻을 수 있었으며 이때 암세포 양성율은 14.7%이었으며, 암세포 형태에 따른 차이는 없었다. 2) 내시경상 기관 분기부위의 비후가 저명한 경우의 암세포 양성율은 33.3%로 그렇지 않은 경우 5.3%에 비해 유의하게 높았다(p<0.05). 3) 흉부 CT상 분기부하 림프절이 비대된 경우 암세포 양성율은 50.0%로 비대가 없는 경우의 암세포 양성율 4.8%에 비해 유의하게 높았다(p<0.01). 또한 CT상 기관 분기부하 림프절의 비후가 없었던 1예에서 TBNA상 암세포 양성이었으며, 수술에 의한 조직 검사상 양성으로 확인되었다. 4) TBNA를 시행한 후 특별한 합병증은 발생되지 않았다. 결론 : 폐암 환자에 있어서 기관 분기부하 림프절의 TBNA는 CT상 림프절이 커진 경우와 기관 분기부위 비후가 있는 경우에 양성율이 높은 것으로 나타났고, 따라서 이런 경우 TBNA에 의해 상당수가 외과적 병기 결정 방법을 피할 수 있을 것으로 기대되었다.

기관지주위 병변과 점막하 병변을 보이는 폐암의 진단에 있어서 경기관지 침흡인술의 유용성 (Transbronchial Needle Aspiration in the Diagnosis of Submucosal and Peribronchial Bronchogenic Carcinoma)

  • 목 적 : 폐암의 진단에 있어서 진단율이 낮은 기관지주위나 점막하 병변에서 TBNA의 진단율을 조직 생검과 비교하여 TBNA의 임상적인 유용성에 대해 조사하였다. 방 법 : 연세대학교 의과대학 세브란스병원에서 1994년 1월부터 2002년 12월까지 5,582예의 굴곡성 기관지내시경검사를 시행하였으며, 기관지주위나 점막하 병변을 보여 TBNA와 조직생검을 시행한 환자 중 폐암이 확진되었던 120예를 대상으로 후향적으로 조사하였다. 결 과 : 기관지주위나 점막하 병변을 가진 폐암에서 TBNA의 진단율은 75.8%로 생검(52.5%)에 비해 진단율이 높았으며(p=0.001), 생검 단독에 비해 TBNA와 생검을 병용하는 경우 진단율이 유의하게 높았다(88.3%, p=0.0001). 폐암의 세포형이나 내시경적 형태에 따른 TBNA의 진단율은 유의한 차이가 없었으나, 소세포암에서 좀 더 높았다. 결 론 : 기관지주위나 점막하 병변을 가진 폐암의 진단에 있어서 TBNA는 생검보다 진단율이 높았고 생검과 같이 시행함으로써 폐암의 진단율을 향상시킬 수 있었다.

Effect of Chlorhexidine Mouthrinse on Prevention of Microbial Contamination during EBUS-TBNA: A Study Protocol for a Randomized Controlled Trial

  • Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a standard diagnostic method for mediastinal and hilar lymphadenopathy. Although rare, fatal infectious complications can occur following EBUS-TBNA. However, to date, there is a lack of effective preventive strategies to reduce these complications. We started a trial to investigate the effect of chlorhexidine mouthrinse on the prevention of microbial contamination during EBUS-TBNA. Methods: This study is a single-center, parallel-group, assessor-blinded randomized controlled trial (RCT). We will enroll 112 adult participants undergoing EBUS-TBNA using a convex probe, and randomly assign them to two groups at a 1:1 ratio. The intervention group will gargle for 1 minute with 100 mL of 0.12% chlorhexidine gluconate before EBUS-TBNA, while the control group will have no mouthrinse before the procedure. Immediately after completion of EBUS-TBNA on all targeted lesions with an aspiration needle, a needle wash sample will be taken by instilling 5 mL of sterile saline into the used needle. The primary outcome is colony forming unit (CFU) counts in aerobic cultures of the needle wash samples. Secondary outcomes are CFU counts in anaerobic cultures, fever within 24 hours after EBUS-TBNA, and infectious complications within 4 weeks after EBUS-TBNA. Conclusion: This trial was designed as the first RCT to investigate the effect of chlorhexidine mouthrinse on the prevention of microbial contamination during EBUS-TBNA. Results from this trial can provide clinical evidence for a simple, safe, and cost-effective strategy to prevent infectious complications following EBUS-TBNA (ClinicalTrials.gov ID: NCT04718922, registered on 22 January 2021).

폐종양의 세포학적 진단에서 경기관지세침흡인검사의 유용성 (Effectiveness of Transbronchial Fine Needle Aspiration in Diagnosing Lung Cancers)

  • Transbronchial fine needle aspiration(TBNA) is one of the cytologic methods in diagnosing lung cancers. TBNA can be used in cases of hilar, mediastinal or lung masses adjacent to the bronchi. We analyzed and compaired the findings of 27 cases of TBNA and bronchial washing and brushing(BW/BB) in lung cancers confirmed by either biopsy or surgical resection between Jun, 1996 and May, 1997 in Asan Medical Center. They were 18 cases of non-small cell carcinomas(eight squamous cell carcinomas, nine adenocarcinomas, and one large cell undifferentiated carcinoma), eight cases of small cell carcinomas, and one case of metastatic hepatocellular carcinoma. The sensitivity of TBNA was 37%(10/27) and false negative was 63%(17/27). Although the sensitivity of BW/BB w3s 56%(15/27), it was not different statistically from that of TBNA(Chi square, p=0.38). Overall sensitivity of TBNA and BW/BB in this series was 70%(19/27). Forty-seven percent of false negative TBNA(8/17) were positive in BW/BB. The findings suggest that the addition of TBNA to the standard BW/BB increases diagnostic yield in cytologic diagnosis of lung cancer.

  • Endobronchial ultrasound guided transbronchial needle aspiration is a recently introduceddiagnostic method which enables real time aspiration of peritracheal and peribronchial lesions. The high sensitivity and the high diagnostic accuracy of EBUS-TBNA in the mediastinal staging of lung cancer have been reported. EBUS-TBNA also showed the high diagnostic yield in the diagnosis of lung parenchymal masses adjacent to the trachea or the large airways. EBUS-TBNA is a good diagnostic method for mediastinal diseases, such as sarcoidosis. Until now, no major complications of EBUS-TBNA have been reported. EBUS-TBNA should be considered for the mediastinal staging of lung cancer and the diagnosis of mediastinal lymphadenopathies.

  • EBUS-TBNA는 성인에서 폐암의 진단 및 병기 결정에 중요한 역할을 하고 있는 시술로서 최근 흉곽 내 결핵 림프절염의 진단에 있어서의 역할에 대한 연구도 이루어지고 있는 중이나 아직 소아에 있어서 이 방법을 통한 진단은 보고된 바가 없는 상태이다. 저자들은 병력 및 피부 결핵 반응 검사, 흉부 CT 소견을 통해 결핵 림프절염이 의심되는 13세 여아에서 EBUS-TBNA를 통한 조직학적 진단 1례를 경험하였기에 보고하는 바이다. 환자는 3주간 지속된 발열 주소로 입원하여 진단적 검사 진행 중 피부 결핵반응 강양성 소견 보였고 흉부 CT상 종격동 및 폐문 주위의 커져 있는 림프절 소견 보여 이 부위에 대한 EBUS-TBNA 시행 후 결핵 림프절염 진단 후 총 6개월간의 항결핵제 치료를 완료하였다.

Incidence of Fever Following Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration

  • Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a minimally invasive diagnostic method for mediastinal and hilar lymphadenopathy. This study aimed to investigate the incidence of fever following EBUS-TBNA. Methods: A total of 684 patients who underwent EBUS-TBNA from May 2010 to July 2012 at Seoul National University Hospital were retrospectively reviewed. The patients were evaluated for fever by a physician every 6-8 hours during the first 24 hours following EBUS-TBNA. Fever was defined as an increase in axillary body temperature over $37.8^{\circ}C$. Results: Fever after EBUS-TBNA developed in 110 of 552 patients (20%). The median onset time and duration of fever was 7 hours (range, 0.5-32 hours) after EBUS-TBNA and 7 hours (range, 1-52 hours), respectively, and the median peak body temperature was $38.3^{\circ}C$ (range, $137.8-39.9^{\circ}C$). In most patients, fever subsided within 24 hours; however, six cases (1.1%) developed fever lasting longer than 24 hours. Infectious complications developed in three cases (0.54%) (pneumonia, 2; mediastinal abscess, 1), and all three patients had diabetes mellitus. The number or location of sampled lymph nodes and necrosis of lymph node were not associated with fever after EBUS-TBNA. Multiple logistic regression analysis did not reveal any risk factors for developing fever after EBUS-TBNA. Conclusion: Fever is relatively common after EBUS-TBNA, but is transient in most patients. However, clinicians should be aware of the possibility of infectious complications among patients with diabetes mellitus.

  • Background: In mediastinal lymph node sampling in non-small cell lung cancer (NSCLC) it is important to determine the appropriate treatment as well as to predict an outcome. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a recently developed, accurate, safe technique in patients with NSCLC for sampling mediastinal lymph nodes. We sought to determine the usefulness of EBUS-TBNA in mediastinal staging with NSCLC considered to be operable. Methods: We retrospectively reviewed the records of 142 patients who underwent EBUS-TBNA for mediastinal staging in the Asan Medical Center, Korea from July 2008 to July 2010. If patients were in an operable state, they underwent subsequent surgical staging. Diagnoses based on biopsy results were compared with those based on surgical results. Results: We performed EBUS-TBNA in 184 mediastinal lymph nodes in 142 NSCLC patients. Almost all of the EBUS-TBNA samples were from the lower paratracheal (112, 60.9%) and subcarinal (57, 31.0%) lymph nodes. In 142 patients, 51 patients (35.9%) were confirmed with malignant invasion of the mediastinal lymph node by EBUS-TBNA and 91 (64.1%) patients were not confirmed. Among the 91 patients, 64 patients (70.3%) underwent surgical staging. 3 patients (4.7%) who were misdiagnosed by the EBUS-TBNA were confirmed by surgery. After Diagnostic sensitivity of EBUS-TBNA, the prediction of mediastinal metastatsis was 94.4% and specificity was 100%. The procedures were performed safely and no serious complications were observed. Conclusion: We demonstrated the high diagnostic value of EBUS-TBNA for mediastinal staging.

  • Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is becoming a standard method for invasive mediastinal staging and for the diagnosis of paratracheal and peribronchial lesions. It is essential to understand the technical aspects of EBUS-TBNA to ensure safe and efficient procedures. In this review, we discuss the practical aspects to be considered during EBUS-TBNA, including anesthesia, manipulation of equipment, understanding mediastinal ultrasound images, target selection, number of aspirations needed per target, sample handling, and complications.
