A Study of the Application of Precepter Program with the Reducion of the Time of New Entry Nurse Orientation. (신입간호사 교육 기간 단축에 따른 Preceptor program 적용에 관한 연구)

  • The purpose of this study was to contribute the improvement of nursing staff development by implementing of effective and organized orientation program using the measure of effectiveness in the application of preceptor program with the reduction of the time of New entry nurse orientation. The subjects were comprised of 116 new entry nurses(59 new entry nurses were employed at June, 1996 and 57 at May & July, 1997) and 79 preceptors, who were in charge of new entry nurse orientation at 1997. These date were collected from June 18, 1996 to August 29, 1997. The characteristics of questionairs applied in this study were general characteristics(7 questions), knowledge of nursing care(138 questions); attitude toward her profession(40 questions) and the degree of satisfaction for the program(5 questions). The questionnairs were developed by the researchers of Asan Medical Center. The Check-list for basic nursing skill were used for the measurement of nursing skill which was developed by nursing department of AMC. The data were analyzed for the mean, standard deviation, difference of mean, ANOV A & t-test. The results of this study are as follows : 1) The changes in knowledge of nursing care : After three months of training the difference of mean between before and after the orientation was higher than the result of two month period training. However, the result was insignificant(p= .469). 2) Performance in nursing practice : Two month period orientation was more effective than three-month training period, however, the statistical significance can not be reached than 3 month period, which resulted in insignificant difference(p=.150). 3) The change in attitude toward her profession : After three-month of orientation, the difference of mean between before and after the orientation was higher than 2 month period orientation, which resulted in significant difference (p=.000). 4) The degree of satisfaction for the program with the time of new entry nurse orientation : After two-month of orientation, the mean of points was 2.57 where the score is higher than 3 month period with mean of 2.30. It resulted in significant difference(p=.000).

Compound K improves skin barrier function by increasing SPINK5 expression

  • Background: The skin acts as a barrier to protect organisms against harmful exogenous agents. Compound K (CK) is an active metabolite of ginsenoside Rb1, Rb2 and Rc, and researchers have focused on its skin protective efficacy. In this study, we hypothesized that increased expression of the serine protease inhibitor Kazal type-5 (SPINK5) may improve skin barrier function. Methods: We screened several ginsenosides to increase SPINK5 gene promoter activity using a transactivation assay and found that CK can increase SPINK5 expression. To investigate the protective effect of CK on the skin barrier, RT-PCR and Western blotting were performed to investigate the expression levels of SPINK5, kallikrein 5 (KLK5), KLK7 and PAR2 in UVB-irradiated HaCaT cells. Measurement of transepidermal water loss (TEWL) and histological changes associated with the skin barrier were performed in a UVB-irradiated mouse model and a 1-chloro-2,4-dinitrobenzene (DNCB)-induced atopic dermatitis-like model. Results: CK treatment increased the expression of SPINK5 and decreased the expression of its downstream genes, such as KLKs and PAR2. In the UVB-irradiated mouse model and the DNCB-induced atopic dermatitis model, CK restored increased TEWL and decreased hydration and epidermal hyperplasia. In addition, CK normalized the reduced SPINK5 expression caused by UVB or DNCB, thereby restoring the expression of the proteins involved in desquamation to a level similar to normal. Conclusions: Our data showed that CK contributes to improving skin-barrier function in UVB-irradiated and DNCB-induced atopic dermatitis-like models through SPINK5. These results suggest that therapeutic attempts with CK might be useful in treating barrier-disrupted diseases.

Comparison of the Measurement of the Injection Rate of Radioactive Drugs Using 99mTc in Nuclear Medicine (핵의학과에서 99mTc를 이용한 방사성의약품의 투여율 측정 비교)

  • This study was conducted by SPECT test at the Department of Nuclear Medicine at Daegu P Hospital from June 1 to October 31, 2019. A 3-way injection material was mounted among inpatients, and a syringe that was administered with radiopharmaceuticals using a 99mTc labeled compound was secured. We tried to find a way to calculate the dose rate of each radiopharmaceutical and increase the dose rate. As a result of measuring the radioactivity of radio-pharmaceuticals using 99mTc, the average dose rate of 60 syringes of all 6 radiopharmaceuticals was 93.26±7.34%, and the average dose rate of 99mTc-DMSA was 77.72%, 15.54% lower than the total. As a way to increase the dosing rate, the average dose rate diluted twice with the remaining amount of syringe after administration using normal saline increased to 95.37±6.99%, and the average dose rate diluted three times increased to 96.32±6.86%. The corresponding sample t-test to compare the pre- and post-dose rates at 1 dilution and 2 and 3 dilutions. As a result of the dilution and 2 dilutions, the probability of significance was 0.013, which was significantly higher than the dilution(p<0.05). The probability of significance for dilution 1 and dilution 3 was 0.016, which was significantly higher than in one dilution(p<0.05). The sum of the average dose rate using the experimental 3-way line was the highest with 98.85±1.42% of 99mTc, 99mTc-ECD 98.82±1.26%, 99mTc-Mebrofenin 98.82 ± 1.16%, 99mTc-HDP 98.74 ± 1.91%, 99mTc -MIBI was 98.69 ± 1.48%, and 99mTc-DMSA was the lowest with 86.47 ± 4.74%. When the number of dilutions was 5 times using 0.5 cc of normal saline and when the number of dilutions was 5 times using 1 cc of normal saline, when the number of dilutions was 5 times using 0.5 cc of normal saline and 1 cc of nomal saline When the number of dilutions was 5 times and the syringe volume was 0.5 cc, there was a statistically significant difference (p<0.05). There was a statistically significant difference when the number of dilutions was 5 times using 1 cc of nomal saline and the number of dilutions was 5 times using 1 cc of normal saline, and the syringe volume was 0.5 cc (p<0.05).

COMPARISON OF CANAL SHAPING METHODS WITH GTTM ROTARY FILE AND CONDENSATION METHODS (GT rotary file을 이용한 근관성형법과 충전방법의 비교)

  • The purpose of this study was to compare the shaping time of two shaping methods and the leakage of three different obturation techniques. Ninty three canaled human molar teeth were used, which were randomly divided into two groups of forty teeth each and ten control teeth. After working length determination, the one group was prepared crown-down technique using rotary root canal instruments of GT rotary files .12/20, .10/20, .08/20 and .06/20 taper(Maillefer Instrument SA. Switzerland). The other group was instrumented with Gates Glidden burs(#1, #2, and #3) to coronal preparation and GT rotary files .08/20 and .06/30 taper to apical preparation. Shaping time was measured. After root canals were instrumented, they were divided to three subgroups and obturated as follows : Subgroup 1, obturated with single cone method Subgroup 2, obturated with lateral condensation : Subgroup 3, obturated with continuous wave technique. Three subgroups were obturated using non-standardized gutta-percha cone(Diadent, Korea, .06 or .08 taper) and AH-26(Dentsply DeTrey, Germany) as a root canal cement. Ten unobturated teeth served as positive and negative controls. After immersion in 2% methylene blue solution for 1 month, the teeth were washed during 24h. The teeth were demineralized in 10% nitric acid and dehydrated by immersion in 80, 90 and 100% ethyl alcohol. The teeth were finally cleared and stored in 100% methylsalicylate, and apical dye penetration was evaluated under stereomicroscope(Leica M420, LC, U.S.A)at $\times$8.75 magnification. Liner measurement of dye penetration was assessed with the use of digitalized image analysing system (analySIS, GmbH, Germany) The data were analysed statistically using independent T-test and Two-way ANOVA and Tukey test. The result were as follows 1. In canal prepared with GT$^{TM}$ rotary file, shaphing time taked more than the group of using Gates Glidden drill to coronal preparation without statistical significance (p>0.05) 2. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files showed significantly more apical leakage than those of lateral condensation and continuous wave technique regardless of shaping method (p<0.05). 3 The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed significantly more apical leakage than those of continuous wave technique regardless of shaping method (p<0.05). 4. Regardless of shaping method, The group of continuous wave obturation showed less apical leakage than those of lateral condensation without statistical significance (p>0.05). 5. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed more apical leakage than the group of lateral condensation using same shaping method with-out statistical significance (p>0.05).

COMPARISON OF LINEAR POLYMERIZATION SHRINKAGE IN COMPOSITES AND COMPOMER POLYMERIZED BY PLASMA ARC OR CONVENTIONAL VISIBLE LIGHT CURING (리노미터를 이용한 할로겐 가시광선 광조사기와 플라즈마 아크 광조사기의 복합레진 및 컴포머의 광중합 양상 비교)

  • The purpose of this study was to evaluate the effectiveness of plasma arc curing (PAC) unit for composite and compomer curing. To compare its effectiveness with conventional quartz tungsten halogen (QTH) light curing unit, the polymerization shrinkage rates and amounts of three composites (Z100, Z250, Synergy Duo Shade) and one compomer, that had been light cured by PAC unit or QTH unit, was compared using a custome made linometer. The measurement of polymerization shrinkage was peformed after polymerization with either QTH unit or PAC unit. In case of curing with the PAC unit, the composite was light cured with Apollo 95E for 6s, the power density of which was recorded as 1350 mW/$\textrm{cm}^2$ by Coltolux Light Meter. For light curing with QTH unit, the composite was light cured for 30s with the XL2500, the power density of which was recorded as 800 mW/$\textrm{cm}^2$ by Coltolux Light Meter. The amount of linear polymerization shrinkage was recorded in the computer every 0.5s for 60s. Ten measurements were made for each material. The amount of linear polymerization shrinkage for each material in 10s and 60s which were cured with PAC or QTH unit were compared with t test. The amount of polymerization shrinkage in the tested materials were compared with 1way ANOVA with Duncan's multiple range test. As for the amounts of polymerization shrinkage in 60s, there was no difference between PAC unit and QTH unit in Z250 and Synergy Duo Shade. In Z100 and Dyract AP, it was lower when it was cured with PAC unit than when it was cured with QTH unit (p<0.05). As for the amounts of polymerization shrinkage in 10s, there was no difference between PAC unit and QTH unit in Z100 and Dyract AP. The amounts of polymerization shrinkage was significantly higher when it was cured with PAC unit in Z250 and Synergy Duo Shade (p<0.05). The amounts of polymerization shrinkage in the tested materials when they were cured with QTH unit were Z250 (6.6um) < Z100 (9.3um), Dyract AP (9.7um) < Synergy Duo Shade (11.2um) (p<0.05). The amount of polymerization shrinkage when the materials were cured with PAC unit were Dyract AP (5.6um) < Z100 (8.1um), Z250(7.0um) < Synergy Duo Shade (11.2um) (p<0.05).

The Correlation between Korean Medical Treatment on Knee Joint Pain and BMI (슬관절 통증 환자의 한의학적 치료 호전도와 BMI 와의 상관성)

  • Objectives : This study was designed to assess the general distribution, clinical effectiveness of Korean medical treatment on knee pain and the correlation between Korean medical therapy on knee pain and BMI. Methods : This is an observational study. 65 patients admitted to Daejeon Jaseng Hospital of Korean Medicine with musculoskeletal disorders were observed from July, 2014 to July, 2015. They were analyzed according to sex, age, pain lesion, body mass index(BMI) and treatment efficacy. All patients received a combination of treatments during hospitalization, including acupuncture, pharmacopuncture, herbal medicines and physical therapy. A zero to ten numerating rating scale (NRS) assessing pain, Western Ontario and McMaster Universities Arthritis Index(WOMAC) index and Range of Motion(ROM) was used before and after treatments. Statistical correlations among assessment measurements were evaluated by examining the paired t-test and the Pearson's correlation coefficients. Results : The average BMI of all patients was $24.03{\pm}2.95kg/m^2$. According to clinical definition of obesity by Korean Society for the Study of Obesity, 33.8% of all patients was within normal weight, 1.5% was underweight, 33.8% was overweight, 24.6% was obesity and 6.2% was morbid obesity. For knee pain patients, NRS (Numeric Rating Scale) decreased from $6.28{\pm}1.63$ to $3.94{\pm}2.03$(p<0.001). WOMAC index decreased from $48.14{\pm}17.63$ to $40.37{\pm}18.28$(p<0.001). But, there were no significant correlations in statistics among BMI, knee pain index and knee pain index improvement. Conclusions : Korean medical combination treatment might be effective in reducing pain and improving functional disorders for patients with knee pain. This study further confirmed the efficacy of Korean medical treatment on knee pain. But, more studies on correlation between knee pain and obesity and development of assessment measurement are needed.

Type Analysis of Lower Trunk Body for the Slacks Pattern Design of Chinese Middle-Aged Men - Focused on Ningbo City, Zhejiang Province - (중국 중년 남성의 슬랙스 패턴설계를 위한 하반신 체간부 유형분석 - 절강성 영파 지역을 중심으로 -)

  • This study aims to classify lower trunk body types of Chinese men in their middle age (30s and 40s) and suggest the standard for them to design slacks pattern. Mollison's relation deviations were used to analyze the direct measurement items in this research compared to those of Size Korea 2004. Though Korean middle-aged men were higher in most items than Chinese counterparts, all items except mid-thigh circumference and side hip length were merely within the range of ${\pm}1\sigma$. According to the results of size classification by absolute values, factor analysis extracted to 2 factors(horizontal size of lower body and vertical size of lower body), and cluster analysis brought about 3 types(type1: tall and thin trunk (36.9%), type 2 : normal height and thick trunk(45.5%), and type 3 : short and normal trunk(17.6%) with some significant differences among them. Also, the results of shape classification by index values, they were extracted 4 factors (waist-hip flatness, waist-hip cross section, vertical waist-groin and hip-surface length) by factor analysis and revealed 3 types(type 1: different waist-hip width, thick, long waist, long upper hip, and short hip-surface length(27.8%), type 2 : different waist-hip width, flat, short upper hip, high hip and groin, and average hip-surface length(29.4%), and type 3: small waist-hip width, thick, average upper hip, and short hip-surface length (42.8%)) with significant differences among them by cluster analysis. The results of standard body types by shape-size combination, 19subjects(10.16%) under these values are regarded as standard body types. Significance was not seen in all items in the t-test results between the total group and the standard group. The latter had lower variation coefficients and smaller individual differences than the former. However, in-depth research is required for generalization since this research is limited to a small number of subjects in Ningbo of Zhejiang.

A Study on Cancer Patients' Quality of Life, Perceived Health Status and Susceptibility.Severity for Cancer Recurrence (암환자의 삶의 질, 지각된 건강상태 및 암재발 인지에 관한 연구)

  • Purpose: This study was designed to identify the relationship between quality of life, perceived health status, perceived susceptibility severity to recurrence of cancer and character of object in cancer patients to provide the basic data of effective nursing interventions. Method: The subject of this study was randomly chosen from the patients diagnosed of cancer and being hospitalised or receiving chemotherapy as outpatients, at a Division of Hemato-Oncology, Department of Internal Medicine of a university hospital located in Seoul. The tool used in this study was a measurement tool for quality of life (${\alpha}=.829$), perceived health status (${\alpha}=.903$), and perceived susceptibility and severity for cancer recurrence (${\alpha}=.860$). The collected data were analysed using SPSS PC 12.0 Programme for real number, percentage, average, standard deviation, Pearson's Correlation, t-test and ANOVA, according to the purpose of this study. Result: 1) The score of quality of life in cancer patients was 3.64 point, perceived health status was 2.62 and perceived susceptibility severity to recurrence of cancer was 2.41. 2) There was a significant difference in the quality of life according to general characteristics, such as level of education, occupation, level of activity, pain, medication period, diagnosis, disease stage, purpose of medication, and recurrence. There was a significant difference in perceived health status according to age, level of activity, pain, diagnosis, purpose of medication, and recurrence. There was a significant difference in perceived susceptibility severity to recurrence of cancer according to age, level of education, and pain. 3) The cancer patients' quality of life showed significant correlation with perceived health status, perceived susceptibility severity to recurrence of cancer, pain, job, treatment purpose, relapse. The perceived health status showed significant correlation with perceived susceptibility severity to recurrence of cancer, pain, treatment purpose, relapse. The perceived susceptibility severity to recurrence of cancer showed significant correlation with pain. The relapse showed significant correlation with treatment purpose. The cancer patients' quality of life, perceived health status, and perceived susceptibility and severity for cancer recurrence, as confirmed above, showed differences according to the related factors of each subject, and it was also confirmed that those factors were significantly related with general characteristics. Upon these results, I suggest further studies on the factors that affect the cancer patients' quality of life.

THE EFFECTS OF BARIUM SULFATE AND IODIDE COMPOUND ON THE CHARACTERISTICS OF DENTAL ACRYLIC RESINS (치과용 아크릴릭 레진의 방사선 불투과도에 관한 연구 - 황산바륨과 요오드 화합물 첨가 -)

  • Aspirating or swallowing foreign bodies is a common occurrence. If they are wholly or partly radiopaque, their localization in and progress through the gastrointestinal tract can be more effective. Of the dental origin foreign materials swallowed, the most common things are fragments of anterior maxillary partial denture. But the radiopacity of denture base resins is not sufficient to determine the location of the objects. The purpose of this study was to develop a radiopaque dental acrylic resin, which has clinically detectible radiopacity with minimal change of mechanical properties and color. The radiopacity, color change(CIE 6..E) and microhardness of acrylic resins were determined after mixing barium sulfate or iodide compound. Thermocycling course was conducted to determine the change of characteristic of resins after using for a long time in the mouth. Five or ten percent of barium sulfate to total weight of cured material was mixed with heat curing dental acrylic resin or chemically curing orthodontic resin. In the case of iodide compound, the mixing ratio was two or three percent. After mixing the high radiopaque materials, resin was cured to 20×20×2 mm plate, polished with #600 sand paper and finally polished with Microcloth(Buehler). The specimens were thermocycled in 5 and 55 t distilled water for 2,000 times, and the measurement of radiopacity, color and Vickers hardness was repeated every 500 times thcrmocycling. The radiopacity of specimens on the X -ray films was measured with densitometer(X-rite). The color change was detennined with differential colorimeter(Model TC-6FX, Tokyo Denshoku), and the Vickers hardness number was measured with microhardness tester(Mitsuzawa). The following results were obtained : 1. All the three variables, the kinds of acrylic resins, the mixing or the kinds of high radiopaque materials and thermocycling, had combined effect on the radiopacity of the dental acrylic resins(p<0.0l). 2. The two variables, the mixing or the kinds of high radiopaque materials and thermocycling, influenced on the radiopacity of the dental acrylic resins(p<0.01). But the kinds of acrylic resins did not influence on the color change of mixed dental acrylic resins(p>0.05). 3. Each of the three variables, the kinds of acrylic resins, the mixing or the kinds of high radiopaque materials and thermocycling, influenced on the radiopacity of dental acrylic resins(p<0.0l). 4. The high radiopaque materials used in this study did not yield clinically usable radiopacity, and the color change was great after mixing those materials.

Anti-Inflammatory and Whitening Effect of the Lyophilized Powder of Oriental Plant Extracts Fermented with Streptococcus thermophilus (Streptococcus thermophilus로 발효한 한약재 발효분말의 항염증 및 미백 효과)

  • To identify new anti-inflammatory and whitening material, this study investigated the whitening and anti-inflammatory effects of the lyophilized powder from 6 oriental plant extracts (OPE; Citrus junos Tanaka, Mori cortex Radicis, Schisandra chinensis Baillon, Coix lachrymajobi var. mayuen, Angelica gigas NAKAI, and Sophora japonica L.) fermented with Streptococcus thermophilus by assessment of cytotoxicity on human dermal fibroblast, inhibitory effect of nitric oxide (NO) prodction, tyrosinase activity and melanin formation. The OPE was fermented with Streptococcus thermophilus at $37^{\circ}C$ for 2 days and the lyophilized powder was manufactured by freezing-dryer. OPE didn't show cytotoxicity at concentration of $500{\mu}g/mL$ using a cytopathic effect reduction method. OPE also exhibited inhibitory effect on nitric oxide (NO) prodction by Griess reagent system. Furthermore, OPE showed inhibitory effect on tyrosinase activity with dose dependent manner, and exhibited significant inhibition of melanin formation by measurement of melanin from culture media (p < 0.05). From these results, 6 OPE extracts showed anti-inflammatory and whitening effect and may be used as an active ingredient for cosmetics.
