여대생의 측두하악장애에 따른 T-scan III System을 이용한 교합분석 및 구강행동유형과의 관련성 (Relationship between occlusion analysis using the T-scan III system and oral behavior checklist according to temporomandibular joint disorder in female college students)

  • Objectives: This study aimed to evaluate the effects of occlusion patterns using the T-scan $III^{(R)}$ system and oral parafunctional behaviors on temporomandibular joint disorder (TMD) in female college students. Methods: A questionnaire survey assessed TMD symptoms and oral behavioral checklist (OBC) scores in 120 female college students in their early twenties. Occlusion was evaluated using the T-scan $III^{(R)}$ system. Logistic regression analysis was performed to investigate the relationship between occlusion and OBC scores based on patients' symptoms of TMD and to determine whether OBC scores were a predictor of TMD. Results: A total of 101 participants with an average age of 22 (${\pm}2.17$) years were included in the analysis. The reported symptoms of TMD were categorized as moderate (28.8%), slight (27.7%), severe (25.7%), and asymptomatic (17.8%). Occlusion was analyzed using the T-scan $III^{(R)}$ system, and the mean occlusion time was 0.42(${\pm}0.5$) seconds. The results of logistic regression analysis showed that the group that would 'clench or grind teeth' during sleep showed an odds ratio for TMD that was 8.9 times higher than that in the group without this behavior. The group that would 'hold, tighten, or tense muscles without clenching' while awake showed an odds ratio for TMD that was 21.3 times higher than that in the group without this behavior. Conclusions: We confirmed that oral parafunctional behavior affects TMD. Therefore, we would like to continue studying the diagnosis and treatment of TMD by evaluating occlusion patterns and related OBC scores.

T-scan III® system을 이용한 일부지역 노인의 교합양상과 인지기능의 관련성 (Evaluation of relationship between cognitive function and occlusal status in elderly individuals using the T-scan III® system)

  • Objectives: The aim of this study was to investigate the relationship between cognitive function and occlusal status in elderly individuals. Methods: A total of 162 individuals aged 65 years and older, who attended the senior citizen center in Daegu city, were included after consent for participation in the study was obtained. The Korean version of the Mini-Mental State Examination (MMSE-K) was used to evaluate the level of cognitive function. Occlusal status was determined using the T-scan $III^{(R)}$ system. All collected data were analyzed by ${\chi}^2$ test, t-test, one way ANOVA, and linear regression analysis using SPSS version 23.0 for Windows. Results: There was a significant relationship between the cognitive function and the use of dentures. Individuals with posterior occlusal status had higher MMSE-K scores than those with anterior occlusal status. There was a positive correlation between the cognitive function and posterior occlusal force. Conclusions: These results suggest that occlusal status was associated with cognitive function in the elderly individuals. Active national policies to improve occlusal condition in the elderly population are needed.

최대교두감합위 및 하악 전방운동 시의 교합접촉 및 교합유도 양상에 관한 분석 (Analysis of occlusal contact and guidance pattern during maximal intercuspal position and protrusive movement)

  • 연구 목적: 저작 및 하악 운동시 발생되는 치아접촉은 치열의 보존, 하악의 안정과 보철 수복과정에 있어서 중요하다. 이에 한국인의 20-30대 성인을 대상으로 최대교두감합위에서의 치아접촉점의 위치 및 교합유도양상과 교합유도치의 분포를 분석하여 알아보고자 한다. 연구 대상 및 방법: 29명의 성인을 대상으로 하악의 최대교두감합위에서의 치아접촉점의 위치와 분포 및 전방운동시 교합접촉양상을 shimstock foil (Whaledent, Langenau, Germany), T-Scan III (Tekscan Inc., Boston, MA, USA), polyvinylsiloxane registration material (Genie Bite, Sultan Healthcare, Hackensack, NJ, USA)을 이용하여 측정하였다. 측정시 자세는 직립위로 Frankfurt horizontal plane과 지면이 수평이 되도록 앉게 하였으며 접촉이 재현될 때까지 수 차례 반복한 후 3회씩 측정하였다. 최대교두감합위에서 세가지 방법 간의 통계적 유의성을 비교하기 Fisher's Exact Test (R-General Public License, ver. 2.14.1)를 이용하였고, 전방 운동시 Pearson's Test를 통해 통계 검증하였다(${\alpha}=.05$). 결과: 최대교두감합위에서의 치아 접촉 양상을 shimstock foil, T-Scan III, polyvinylsiloxane registration material로 측정시 전치부, 소구치부, 구치부 모두에서 접촉하는 경우가 대부분이었으며, shimstock 사용시 약51%의 최대교두감합위는 전치부 접촉에서 일어났다. Shimstock foil과 T-Scan III를 사용하여 전방운동 측정시 중절치의 접촉이 가장 많이 일어났다. 결론: 최대교두감합위에서 실제 모든 치아의 접촉이 이루어지지 않는 경우가 있었으며 따라서 구치부에서 전치부를 보호해 주고 있음을 확인할 수 있었다. 또한, 전방운동 시 전치부의 치아접촉은 과도한 구치부의 치아접촉을 방지해 전치부가 구치부를 보호해 주고 있었다. 따라서 교합 재구성 시에는 이러한 상호 보호 교합에 대한 고려가 필요하다.

구조방정식을 이용한 대도시 일부 노인들의 저작능력 및 구강건강관련 삶의 질과 인지기능 (Association between masticatory ability, oral health-related quality of life and cognitive function in the elderly population using structural equation modeling)

  • Objectives: The aim of this study was to investigate the association between masticatory ability, oral health-related quality of life, and cognitive function in an elderly population using structural equation modeling. Methods: A total of 308 subjects, aged 65 years and over, were recruited from the senior citizen center in Daegu, South Korea and provided consent for inclusion in the study. The Korean version of the Mini-Mental State Examination (K-MMSE) was used to assess the level of cognitive function. Masticatory ability was evaluated through chewing tests, patient-perceived masticatory function and, measurement of relative occlusal forces on molar teeth using the T-Scan $III^{(R)}$ system. All data were analyzed using ${\chi}^2$ tests, t-tests, and one-way ANOVA in SPSS version 23.0 for windows. Structural equation modeling was performed using SPSS AMOS version 22.0. Results: There was a significant association between cognitive function and molar teeth occlusal force. Ability of food chewing score and patient-perceived masticatory function could affect oral health-related quality of life (P>0.05). Higher occlusal forces on molar teeth, compared to anterior teeth, positively correlated with higher cognitive function. Conclusions: These results suggest that an individual's oral health condition could affect molar teeth occlusal forces and may be related to a risk of developing dementia. Therefore, there is a need for implementing nation-wide policies to improve oral health, such as masticatory function, in the elderly population.
